뇌졸중 환자의 이중재활 지원을 위한 XR 프로그램 연구: 향기 분출 기능과 데이터 시각화를 중심으로 (A Study on XR Program to Support Double Rehabilitation in Stroke Patients: Focusing on Fragrance Ejection Function and Data Visualization)

  • 본 연구에서 개발된 XR 게임은 이중 재활을 지원하는 핵심 도구로서 설계되었으며, 향기 분출 기능 또한 포함하고 있어 향기 치료까지 가능하도록 확장되었다. 이 프로그램은 사용자의 데이터를 세심하게 분석하여, 각 개개인의 진전을 지속적으로 추적할 수 있다. 더불어, 사용자는 과거와 현재의 데이터를 비교하고 분석하여, 지속적인 자기 개선을 위한 방향을 찾을 수 있다.

  • Purpose : The purpose of this study was to investigate the effects of a virtual reality-based occupational therapy program on the physical function and learning ability of intellectually disabled school-aged children. Methods : In this study, 20 intellectually disabled children of school age were randomly and evenly divided into experimental and control groups with children in the experimental group receiving a virtual reality-based occupational therapy intervention. The study adopted a pretest-posttest design. The intervention was conducted for a total of 12 sessions for six weeks, twice a week, and 50 minutes per session. As measurement tools, BOT-2 and grooved pegboard tests were used to compare physical function before and after the intervention program, and K-ABC was used to check changes in learning ability. Results : The occupational therapy program produced a significant improvement in both physical function and learning ability of the experimental group. A significant difference was observed between the experimental and control groups. Conclusion : This study confirmed the value and usefulness of virtual reality-based occupational therapy as a tool for enhancing the physical function and learning ability of intellectually disabled school-aged children. Based on the results, a variety of future studies are encouraged that would further test the effects of the occupational therapy program used here.

보상작용 억제와 피드백을 제공한 가상현실 치료가 만성 뇌졸중 편마비 환자의 상지기능에 미치는 영향 (The Effects of Virtual Reality Therapy With Compensation Inhibition and Feedback on Upper Extremity Function in Hemiplegic Patients With Chronic Stroke)

  • The purpose of this study was to investigate the effects of virtual reality (VR) therapy with compensation inhibition and feedback (CIF) on upper extremity function in chronic stroke patients. Seven chronic stroke patients participated in this study, which was a randomized controlled trial with a crossover design. Self upper extremity exercise, conservative VR therapy, and VR therapy with CIF were performed for one hour per session, 5 times per week, over a 3 week period. The main outcome measures involved range of motion (ROM) including shoulder, elbow, and wrist joints, a Manual Function Test (MFT), and a Motor Activity Log (MAL). Data were calculated as posttest and pretest changes in every session and were analyzed using Friedman and Wilcoxon signed-rank tests at p<.05. The results were as follows: 1) Statistically significant increase in ROM measurements of shoulder and elbow joints were seen with VR therapy with CIF compared to VR therapy and self upper extremity exercise (p<.05), whereas no significant increasing was noted for the wrist joint (p>.05). 2) Statistically significant increase in the MFT was seen with VR therapy with CIF compared with VR therapy and self upper extremity exercise (p<.05). 3) VR therapy with CIF also resulted in statistically significant increase in both activity of use (AOU) (p<.05) and quality of movement (QOM) (p<.05) on the MAL test when compared with VR therapy and self upper extremity exercise, respectively. In conclusion, VR therapy with CIF was more effective than conservative VR therapy and self upper extremity exercise in improving the upper extremity function in hemiplegic patients with chronic stroke.

  • Purpose : This study assessed the effects of transcranial direct current stimulation (tDCS) on balance, fall efficacy, and fall-related fitness in stroke patients, using a cohort of 30 stroke patients divided into two groups. Methods : The experimental group (was given transcranial direct current stimulation in a virtual reality program) and the control group was given false tDCS in virtual reality. there were 15 patients in each group, receiving appropriate treatment over 30 sessions (30 minutes per session per week over a six-week period). In order to assess the change in balance before and after the intervention, the Berg Balance scale was utilized. Fall efficacy was evaluated using the Korean Falls Efficacy Scale for the Elderly (FES-K), The following exercises were performed by patients to assess fall-related fitness : sitting down in a chair and standing up : walking a 244 cm round= trip, and standing on one foot. Results : After the intervention, the experimental group exhibited significantly increased fall efficacy and fall-related fitness, while the control group exhibited no change. These findings suggest that tDCS has positive effects on balance, fall efficacy, and fall-related fitness in stroke patients. Conclusion : Using tDCS as an intervention would bring positive effects on balance, fall efficacy, and fall-related fitness in stroke patients undergoing rehabilitation.

가상현실을 이용한 흡기근 저항운동이 흉곽 움직임 제한이 있는 여성환자의 가로막 움직임과 호흡기능에 미치는 영향 (Effect of Virtual Reality Inspiratory Muscle Training on Diaphragm Movement and Respiratory Function in Female Patients with Thoracic Restriction)

  • PURPOSE: This study was conducted to analyze the effects of virtual reality inspiratory muscle training and conventional inspiratory muscle training on diaphragm movement and pulmonary function in patients with thoracic restriction. METHODS: This study measured diaphragm movement, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), peak expiratory flow (PEF), and thoracic mobility (upper, middle, and lower trunk) under two different conditions. Forty young women between 19 and 24 years of age who had no history of orthopedic symptoms for the last 6 months were divided into experimental and control groups. The experimental group performed virtual reality inspiratory muscle training and diaphragm breathing, and the control group performed conventional inspiratory muscle training and diaphragm breathing. RESULTS: The control group showed a significant increase in all dependent variables except for lower trunk mobility and PEF. The experimental group showed a significant increase in all dependent variables except for lower trunk mobility. Particularly, the experimental group showed significant increases in diaphragm movement (p<.05), FVC (p<.05), FEV1 (p<.05), and PEF (p<.05) relative to the control group. CONCLUSION: We recommend inspiratory muscle training with a virtual reality program over conventional training to improve diaphragm movement and pulmonary function in patients with thoracic restriction.

열자극을 병행한 가상현실훈련이 만성 뇌졸중 환자의 위팔 능동가동범위와 기능에 미치는 영향 (Effects of Virtual Reality Training Combined with Thermal Stimulation on Upper Extremity AROM and Function in Chronic Stroke Patients)

  • Background: This study was performed to evaluate the effects of Thermal stimulation combined virtual reality training (TV) on improvement of upper extremity AROM and function in patient with chronic stroke. Design: Two groups pre-post randomized controlled design. Methods: A single-blind, randomized controlled trial was conducted with 30 chronic stroke patients. They were randomly allocated two groups; the TV group (n=15) and Virtual Reality training group (VT) (n=15). The TV group received treatment for 30 min - 15 min of Thermal stimulation, and 15 min of VR training. The VT group received 15 min of VR training. Each group performed 30 minutes a day 3 times a week for 8 weeks. The primary outcome upper extremity AROM and function were measured by a active range of motion test, Manual Function Test (MFT) and Jebsen-Taylor hand function Test (JTT). The upper extremity active range of motion was evaluated using a digital dual inclinometer. MFT and JTT were used to evaluate the hand function. The measurement were performed before and after the 8 weeks intervention period. Results: Both groups demonstrated significant improvement of outcome in muscle strength and upper extremity function during intervention period. TV group revealed significant differences in AROM and upper extremity function as compared to the VT groups (p<.05). Our results showed that TV was more effective on upper extremity AROM and function in patients with chronic stroke. Conclusion: Both groups demonstrated significant improvement of outcome in muscle strength and upper extremity function during intervention period. 코로나 19팬데믹 상황에서 의료관광 산업의 발전 방안에 관한 탐색적 연구 (An exploratory study on the development plan of the medical tourism industry in the context of the COVID-19 pandemic)

  • 본 연구는 코로나19 팬대믹 이후 외국인 환자의 급감으로 심각하게 타격을 입은 국내 의료관광 산업 회복을 위한 방안을 모색하고자 연구하게 되었다. 전문가 패널 에게 순위별 키워드 자문을 구하여 Kendall의 W검증을 사용하였다. 연구의 결론은 외국인 환자를 유치하는 기관은 중증 외국인 환자 치료 확대로 위기상황을 기회로 바꾸는 계기가 필요하겠다. 또한 가상, 증강현실을 통해 해외 방문이 어려운 상황에서 병원에 대한 친숙함과 신뢰를 얻을 수 있고 언택트 시대에 감염의 위험을 막고 환자를 보호 할 수 있는 원격지료 서비스를 시행함으로써 국내 입국을 하지 않고도 현지에서 의료의 혜택을 누릴 수 있다고 하겠다. 그리고 블록체인은 환자의 정보 보완을 유지하며 안전하게 공유가 가능하고 가상화폐 등을 활용한 지불 수단을 이용해 고객의 불편함을 최소화 할 수 있고 마지막으로 스마트 헬스 케어는 장소와 관계없이 환자를 관리하고 정보를 제공 할 수 있다.

AR(증강현실)/VR(가상현실) 활용한 군 교육훈련 사례 연구 (Case study of military education and training using AR (Augmented Reality)/VR (Virtual Reality))

  • AR/VR 기반의 교육훈련 체계는 전장 환경과 유사한 실전적 훈련 효과뿐 아니라 사고 예방 및 예산 절감에도 큰 기여를 할 것으로 평가되고 있다. AR/VR을 학습에 활용하기 위한 연구는 꾸준히 진행되고 있으며, 교육과 훈련에 활용할 경우 실세계에서 발생할 수 있는 실패의 경험 없이도 기술을 향상시킬 수 있다. 국방 주요선진국들은 이러한 AR/VR 기술의 이점을 일찍부터 인식하고, 개인의 무기체계 운용 숙달에서부터 종합 전투훈련 체계, 전쟁사 교육, 외상 후 스트레스 치료 등 다양한 분야에서 AR/VR을 개발하고 적용하는 노력을 하고 있다. 한국군도 최근 AR/VR 활용 교육훈련 적용방안을 적극적으로 검토하고 있다. 이에 본 연구의 목적은 국방 선진국의 AR/VR 적용 교육훈련 사례들을 살펴보고 한국군에 적용할 시사점을 도출하는 것이다.

치아 스케일링 훈련을 위한 햅틱 시뮬레이션 알고리즘 (Haptic Simulation Algorithm for Tooth Scaling Training)

  • 치아 스케일링은 치아에 단단하게 결착된 치석을 제거하는 치과 시술로서 치아 우식 및 잇몸염증을 예방하는 중요한 시술이다. 특히 요즘에는 치료시간의 단축을 통한 효율성 증대의 목적으로 전기적 에너지를 미세한 진동에너지로 바꾸는 원리를 활용한 초음파 스케일링 기법이 많이 행해지고 있다. 하지만 치아 및 치석 확보에 따른 어려움으로 인해 스케일링 시술을 충분히 훈련하기란 쉽지 않다. 본 논문에서는 사용자가 가상현실을 통해 시각 및 촉각 피드백을 받으며 초음파 스케일링 시술을 훈련할 수 있는 치아 스케일링 시뮬레이션을 위한 알고리즘을 제안한다. 치아, 치석 및 잇몸의 볼륨모델과 스케이러 팁을 구성하는 각 부문의 관통깊이를 이용한 햅틱 랜더링 기법을 적용하여 스케일러의 모양에 따른 햅틱 피드백을 생성하였다. 그리고 치아의 손상을 줄이기 위해 스케일러의 팁 부문이 치아 표면에 되도록 평형을 이루어야 한다는 점에 입각하여, 치석을 구성하는 복셀들의 치아 디스턴스필드 값 비교를 통해 치석과 치아 사이의 접착면을 추출하고 스케일러의 팁 부분과 충돌하는 추출된 집착면의 각도를 고려한 스케일링 알고리즘을 구현하였다. 또한 수동 스케일링과는 달리 초음파 스케일링은 초음파의 진동에너지에 의해 점진적으로 치아와 치석 사이의 결속력이 감소된다는 점에 착안하여 치아와 치석의 접착면을 구성하는 지점 사이의 거리에 따른 결속력 감쇠 모델을 고안하였다.

가상현실을 이용한 재활 운동이 뇌졸중 환자의 정적 균형 및 보행에 미치는 효과 (Effects of Virtual Reality-based Exercise on Static Balance and Gait Abilities in Chronic Stroke)

  • Purpose: This study examined the effects of a virtual reality-based exercise program on the functional recovery of balance and gait in chronic stroke subjects. Methods: A total of 42 chronic stroke patients were enrolled in this study. The participants were allocated randomly to 2 groups: a VR (n=22) and control group (n=20). Both groups received treadmill training for 3 sessions (10 minutes each), 30 minutes per week over a 6 week period. The VR group practiced additional virtual reality programs consisting of 3 programs for 10 minutes each. The data was analyzed using a paired t-test and independent t-test to determine the statistical significance. Results: The virtual reality-based exercise group showed significant increases in gait velocity, cadence and stride length compared to the control group (p<0.05). However, there were no significant differences in static balance. Conclusion: These results support the perceived benefits of exercise programs that incorporate virtual reality to augment the balance and ambulation of stroke patients. Therefore, virtual reality is feasible and suitable for stroke patients
