Purpose: The purpose of this study was to compare the maximum occlusal force implant prostheses to natural teeth. Material and Method: Fifty nine patients treated either with $Br{\aa}anemark$ implants and ITI implants during the recent ten years were involved in this study. The maximum occlusal force were measured with unilateral bite force recorder and dental prescale system. Results: 1. The maximum occlusal forces of the implant prostheses and natural teeth were not significantly different where measured with unilateral bite force recorder and dental prescale system. 2. The maximum occlusal forces were not significantly different between $Br{\aa}nemark$ implant and ITI implant prostheses. 3. The maximum occlusal forces of the implant prostheses had lower when compared with natural teeth during 1-6 months functional periods when measured with the unilareral bite force recorder(P<0.05) and 1-12 months functional periods when measured with the dental prescale system(P<0.05). After these periods there was not statistical significant difference between the implant prostheses and natural teeth. 4. The maximum occlusal forces of the wide diameter implant prostheses were higher than the maximum occlusal forces of the regular diameter implant prostheses when measured with dental prescale system(P<0.05), but there was no significant difference between the wide diameter and the regular diameter implant prostheses when measured with unilateral bite force recorder. 5. The maximum occlusal forces of the single implant prostheses were not significantly different with the splinting implants prostheses. 6. The maximum occlusal forces of the implant prostheses were not significantly different by age and sex. 7. There was significantly different between maximum occlusal forces measured with unilateral bite force recorder and dental prescale system(P<0.0001) but there was positive correlation(r=0.52. P<0.05). Conclusion: The maximum occlusal forces of the implant prostheses were not significantly different to natural teeth during clenching and unilateral maximum biting.
The severe atrophic edentulism with poor neuromuscular control militates against successful conventional denture therapy. To such situation, a prescribing of dental implant treatment has been considered with some probability. Implant materials used as a trial for dental implants includes metals, plastic polymers and ceramics. The purpose of this study was to observe histologic response in osseointegration process at titanium implant-tissue interface based on biocompatibility at specific period of sequential natures which were divided into a half month, one, month, two months, three months and immediate as a base line. In this study, unilateral lower left premolar and molar teeth were extraced in three dogs. After allowing to heal for 6 months, three kinds of osseointegrated implant, $Br{\aa}nemark$, Corevent and kimplant(a prototype of SNU implant study)were inserted in each dog respectively according to the above sequence from front to back. The specimens were taken from those dogs at the same time since implant were inserted quite reverse order of the specified periods, and decalcified and processed for histologic examination for the light microscopy and the electron microscopy. The microscopic histologic findings at the interface between titanium implants and tissue were interpretated as follows : A. Light microscopic findings : a. Immediate : Implant were surrounded by compact bone and spongy bone. Microcrak was observed in the superficial bone tissue. Osteocytes were disappeared and bone lacunae were observed as a vacant space in some parts. In the contacting with the spongy bone, bone trabeculae and bone marrow were in contact with the implant. b. A half Month : Osteoblasts exist as a monolayer in th inner bone trabeculae and do bone additiocn. Osteoblasts&inflammatory cells were observed in some parts. c. One Month : The presence of osteoclasts decreased. Osteoblasts did active bone fromation, and bone marrow was in contact with the implant in the many places. d. Two Months : Bone formation was advanced in comparison with the b and c. The presence of osteoclsts was not observed. e. Three Months : The superficial bone tissue contacted with the implants was entirely composed by the compact bone. B. Electron microscopic findings : a. A half month and one month group : In the parts of the active bone formation, osteoblasts with the well developed endoplasmic reticulum and Golgi apparatus were arranged in the monolayer. In the parts of the bone resorption, ruffled border was well developed and many osteoclasts with the well-developed golgi apparatus, mitochondria, vacuole, vesicle and lysosome were existed. b. Three months group : No osteoblasts were observed in the superficial bone tissue. Bone matrix with collaen fiber was observed. c. No significant dirrerence in the histologic findings was observed in $Br{\aa}nemark$, Core-vent and kimplant.
Purpose: Developments in micro/macrostructures of implants and surgical techniques brought out stable outcomes of implant dentistry. The aim of this study was to evaluate the distributions of implant patients, the types of implanted sites, and the success or survival rates of various implant systems and to analyze the implant placement done at each specificintraoral site and situation. Materials and Methods: The data of dental implantations collected between 1992 and 2006 at the Department of Periodontology in 00000 University Hospital were analyzed. Results: 1. Largest part of the patients were at the age of 40s and 50s in bothgender who lost their teeth mostly by periodontaldiseases and caries at the posterior intraoral sites as major ones. Bone densities of type II(mandible) and III(maxilla) were likely to be seen with quantity of type B. Lengths of the implants between 10 and 15 mm and wide platform took the largest part. 2. Survival rates of $Implantium^{(R)}(98.8%)$, $Xive^{(R)}(100%)$ and ITI $TE^{(R)}(100%)$ were high when $Frialit-2^{(R)}$ showed 82%(poor bone density area) or 87.2%(combined with additional therapy). $IMZ^{(R)}$ had lowest cumulative survival(67.5%) and success rate(49.4%) amongst all. 3. Replacement with 2 wide or 3 regular platforms showed no significant differences in survival rate and marginal bone loss atmandibular posterior area. In single restoration of mandibular second molar, 5-year success rate of machined surface $Br{\aa}nemark^{(R)}(70.37%)$ was lower than that of rough surface $ITI^{(R)}$ SLA(100%). 4. Replacement of single tooth in anterior area showed high survival rate of 94.5%. 5. The success rates of $Br{\aa}nemark$ Ti-Unite and ITI SLA at posterior maxilla with poor bone density both showed stable outcomes. 6. 10-year cumulative survival rate of implants with maxillary sinus augmentation by lateral window approach appeared to be 96.60%. Low survival rate(75%) was shown when there were more than two complications combined. Height of grafted bone remained stable above the implant apex. Conclusions : Rough surfaced implants showed stable outcomes in most of the situation including poor bone density and additional therapy combined.

The aim of this investigation was to evaluate successfully functioning implants stability in the partially edentulous mandibular molar sites by resonance frequency measurements. Resonance frequency measurement is more objective and clinically non-invasive method than any other methods had been used. In this study, $Osstell^{TM}$ (Integration Diagnostics, Sweden) was used. 15 patients ( 7 males, 8 females ) were received each 2 implants in their mandibular unilaterally partial edentulous molar sites. Total 30 implants were installed, 28 implants were $Br{\aa}nemark^{(R)}$ self-tapping fixtures(MK II, Nobel Biocare, Sweden) and 2 were $3i^{(R)}$(USA) self-tapping fixtures. Minimum of functional loading durations was 12 months and there were no significant marginal bone resorptions and peri-implant problems. From this test, the following results were drawn: 1. Mean ISQ values of implants in the first and second mandibular molar area were $73.66{\pm}4.45$, $71.93{\pm}3.41$, respectively. There was no significant difference between two groups(p>0.05). Also mean ISQ value of total sum was 72.8. 2. Mean ISQ values of implants in males and females group were $71.64{\pm}4.06$, $73.81{\pm}3.76$, respectively. And there was no significant difference between two group(p>0.05). 3. Correlation between implant lengths and ISQ values was weak(r=0.128).
Since the concept of a direct contact between bone and implants, without interposed soft- tissue layers, was reported by Dr. $Br{\aa}nemark$, there has been increasing necessity for correct under-standing of bone-implant interface and surrounding tissue response. Beside quality of bone, surgical technique, load applied to implants, one must consider implant materials, design and surface characteristics to obtain osseointegration. In this study HA plasma-sprayed implants, TPS implants and $Al_2O_3$ implants were inserted into the alveolar bone of dog and tissue response was observed with radiograph, stereoscope, light microscope, and scanning electron microscope. Results were as follows : 1. There was rapid and active bone formation in the region adjacent to HA plasma-sprayed implants but in the deep supporting bone only slight bone formation was seen. 2. There was considerable lamella bone formation in the region adjacent to TPS implants and the deep supporting bone became more compact. 3. There was some gap and sclerosing bone formation in the adjacent region of $Al_2O_3$ implants, but there was irregular new bone formation in the deep supporting bone. Therefore, it seems that $Al_2O_3$ is not adequate for osseointegrated implants.
Statement of problem : Since the concept of osseointegrated dental implant by $Br{\aa}nemark$ et al was first applied to mandibular full edentulous patients. Recently it is considerated the first treatment option on missing teeth. A common problem associated with dental implant restorations is loosening of screws that retain the prosthesis to the abutment and the abutment to the implant fixture. Purpose : This study is to examine the influence on screw loosening of implant-abutment designs. Material and methods : External hex, cone screw, beveled hex, cam cylinder, cylinder hex by means of evaluating the loosening torques, with respect to a range of tightening torques after repeated loading. Result : 1. Cone screw, beveled hex groups are the highest initial tightening rate and cylinder hex, external hex groups are the lowest initial tightening rate (p < 0.05). 2. Cone screw groups are the highest after repeated loading tightening rate and cylinder hex groups are lowest after repeated loading tightening rate(p < 0.05). 3. Cone screw groups have the highest initial stability and anal stability. 4. All groups are decreased tightening rate after repeated loading.
During the previous several decades the osseointegrated implants have been considered as the most ideal therapy for the fully edentulous or partially edentulous patients. Although the research on the biocompatibility of osseointegration, the oral tactile function, and the histo-neurologic study had been performed, the change of the oral tactile sensibility and maximal occlusal force according to the postinsertion time of implant prostheses has not been studied. The purpose of this study was to compare the oral tactile sensibility and maximal occlusal force of implant pros-theses to natural teeth according to the specific postinsertion time intervals. The fifty seven patients treated with $Br{\aa}nemark$ implants during the recent seven years were involved in this study. The oral tactile sensibility and maximal occlusal force were measured according to the postinsertion time (${\leqq}6\;months,\;{\leqq}12\;{\leqq}\;18\;months\;{\leqq}\;24\;months,\;and\;{\geqq}20\;months$) 1. The passive tactile threshold of implants was higher than 6-7 times when comparing to natural teeth(P<0.05) 2. The absolute pressure threshold in horizontal and vertical direction for the passive tactile sensibility had decreased significantly in the post insertion time 13-18 months group and had increased significantly in the post insertion time over 25 months group (P<0.05) 3. 50% limit thickness and 100% absolute thickness for the active tactile sensibility had decreased significantly in the postinsertion time 7-12 months group ($AT_{50}:P<0.005,\;AT_{100}:P<0.05$) 4. The maximal occlusal force of the implants prostheses had decreased when comparing with natural teeth in the postinsertion time 1-6 months group(P<0.05) and after that there was no significant difference between the implant prostheses and natural teeth(P<0.05) 5. Maximal occlusal force according to the postinsertion time had increased significantly in the 13-18 months group (P<0.05) after that there was no significant difference. 6. There was positive correlation between age and oral tactile sensibility tested in horizontal and vertical direction ($P_{H:r}=0.39,\;P_{v:r}=0.41$) and there was no correlation between age and maximal occlusal force, age and active tactile sensibility 7. According to the results or the questionnaire, 76.6% of the patients were satisfied with the function and 96.7% of the patients were satisfied with the sensibility. In conclusion oral tactile sensibility and maximal occlusal force of implants have changed with the postinsertion time. Oral tactile function of implants was lower than that of the natural teeth, but for the maximal occlusal force there was no difference between implants and natural teeth.

Two patients with partial edentulous maxilla were scheduled to undergo installation of implant fixtures using a tooth-supported surgical template based on computer assisted treatment planning. After 3-dimensional (3D) computed tomographic scanning was transferred to the OnDemand3D (Cybermed Co., Seoul, Korea) software program for virtual planning, fixtures of MK III Groovy RP implant of the Br${\aa}$nemark System (Nobel Biocare AB Co., G$\ddot{o}$teborg, Sweden) was installed using the In2Guide (CyberMed Co., Seoul, Korea) tooth-supported surgical template with a Quick Guide Kit (Osstem Implant Co., Seoul, Korea) system in the posterior maxilla of each patient. Sinus floor elevation with a xenogenic bone graft procedure was also performed simultaneously in one patient. Fixture installations were completed successfully without complications, such as sinus mucosa perforation, bony bleedings, fenestrations, or others. During the last two-year follow-up period after prosthetics delivery, each implant was found to be fine with no other minor complications. The entire procedures are reported and the literatures on use of tooth-supported surgical template was reviewed.

The successful use of osseointegrated implants to replace missing teeth has been demonstrated for both the completely and the partially edentulous patients. Many studies have confirmed an excellent long-term prognosis. The successful outcome of any implant procedure is surely dependent on the interrelationship of the various components that includes the following: biocompatibility of the implant material, macroscopic and microscopic nature of the implant surface, the status of the implant bed in both a health(noninfected) and a morphologic(bone quality) context, the surgical technique, the undisturbed healing phase, the subsequent prosthetic design, and long-term loading phase. Periodontally compromised patients have poor status of the implant bed and periodontal pathogen. No longitudinal data are available whether these factors affect the prognosis of implants. In this study, 102 machined $Br{{\aa}}nemark$ implants are inserted to analyze the success rate of 1-4 years and marginal bone loss in 49 chronic periodontitis patients. The following conclusions could be drawn from this study. 1. The cumulative success rate of implants at the 4-year of loading was 95.10%. 2. 5 failed implants have been removed. One implant have been removed due to infection, two implants were removed due to failure of osseointegration. and other two implants were removed due to mechanical failure caused by over-loading. 3. Mean marginal bone loss from the time of loading was 0.94mm at first year, 1.12mm at second year, 1.25mm at third year. These results suggest that implant therapy is good treatment modality in chronic periodontitis patients, and periodontal treatment including oral hygiene program is completed prior to insertion of implants.

The aim of this retrospective study was to compare the amount of marginal bone loss between upper anterior area and upper posterior area with 71 upper single-tooth restorations on 2 stage machined $Br{{\aa}}nemark$ implants since Jan 1995. The second aim was to compare the bone defect group which had dehiscence and fenetration and the others in the upper anterior region. The results were as follows. 1. The most frequent reason of missing tooth in the upper anterior region was trauma by 61%. While upper posterior region showed various reasons such as congenital missing, advanced periodontitis, trauma. 2. Peri-implantitis with fistula occurred 1 of 41 implants in the upper anterior group in 1 year after loading and 2 of 32 implants in the upper posterior group failed before loading. The 1 year success rate of upper anterior group was 97.56 %, and 93.75 % for upper posterior group. 3. The mean marginal bone loss in the upper anterior group was 0.44${\pm}$0.25 mm, while 0.57${\pm}$0.32 mm in the upper posterior group. There was statistically significant difference in the amount of mean marginal bone loss (P${\pm}$0.10 mm at one year, and 0.48${\pm}$0.26 mm for the control group. No statistically significant difference of mean marginal bone loss was showen between bone defect group and the others at implantation. According to the results, the upper anterior region showed less marginal bone loss than the upper posterior region. In case of missing single upper tooth, careful consideration on recipient residual ridge to determine proper implant diameter and length, sufficient healing time, proper loading would lead to implant success. Single tooth implants in the maxilla seemed to be an alternative to fixed partial dentures without damage to adjacent teeth.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
