임상간호사의 완벽주의 성향과 대처유형이 소진에 미치는 영향 (Influence of Perfectionism and Coping Styles on Burnout in Clinical Nurses)

  • Purpose: The purpose of this study was to identify the influencing factors on burnout after analyzing the relationship among clinical nurses' various perfectionism and coping styles. Methods: A descriptive, cross-sectional study was conducted with 200 clinical nurses who had six months or more years of clinical experiences from two general hospitals located in D city. The data were analyzed by t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise regression analysis using IBM SPSS statistics 23.0 program. Results: The burnout of clinical nurses was positively correlated with socially prescribed perfectionism and negatively correlated with active coping style. Socially prescribed perfectionism was positively correlated with active coping style and passive coping style. Influencing factors on the burnout of clinical nurses were socially prescribed perfectionism (${\beta}=.37$), satisfied work unit (${\beta}=-.64$) and neutral satisfaction of work unit (${\beta}=-.27$), over 40 years of age (${\beta}=-.21$), and active coping style (${\beta}=-.14$). The model consisting of these variables explained 42% of variance of burnout in clinical nurses. Conclusion: Based on the findings of this study developing intervention programs that consider influencing factors such as perfectionism and coping styles is needed to reduce the level of burnout of clinical nurses.

가미계작지모탕(加味桂芍知母湯) 주정 추출물이 류마티스 관절염 인자에 미치는 영향 (Effect of Gamikyejakjimo-tang(jiāwèiguìsháozhīm-tāng) ethanol extract on Rheumatoid Arthritis factors)

  • Objectives : The purpose of this study is to prove the effect of Gamikyejakjimo-tang (ji$\bar{a}$w$\grave{e}$igu$\grave{i}$sh$\acute{a}$ozh$\bar{i}$m-t$\bar{a}$ng, GK) on rheumatoid arthritis. Methods : We checked viability and measured production of IL-$1{\beta}$, IL-6, IL-17, TNF-${\alpha}$ in RAW 264.7 cell after treat by GK. Then we measured rheumatoid arthritis index score of DBA/1 mice with rheumatoid arthritis induced by CIA after GK oral administration, checked IL-$1{\beta}$, IL-6, IL-17, TNF-${\alpha}$ and hs-CRP tests in serum. Also we were observed mRNA expression of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ in spleen by RT-PCR. Results : GK showed cell viability of 100% or higher in all concentration in RAW 264.7 cells. GK inhibited LPS-induced productions of rheumatoid arthritis mediators cytokine in RAW 264.7cells. GK treated group showed improvement from rheumatoid arthritis at decreased the index score. Also, GK treated group decreased level in serum of IL-1b, IL-6, IL-17, TNF-a and hs-CRP tests by 31%, 35%, 20%, 57% and 58% respectively. Finally, GK treated group showed decrease expression of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ mRNA in spleen by 46%, 51%, 25% and 42% respectively. Conclusions : In this study, in-vitro and in-vivo results observed rheumatoid arthritis factors cytokine of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ decrease in RAW 264.7 cells, serum, mRNA expression. Also, GK showed decrease of inflammation figure in hs-CRP tests depending on effect of rheumatoid arthritis. Thus, these results can used as a effective drug of GK for rheumatoid arthritis.

소양호에서 세균군집구조의 계절적.수직적 변화 (Seasonal and Vertical Change of Bacterial Communities in Lake Soyang)

  • 소양호에서 fluorescent in situ hybridization 방법을 이용하여 세균군집의 계절과 수심에 따른 변화를 조사하였다. 이 방법에 사용된 oligonucleotide probe는 EUB338, ALF1b, BET42a, GAM42a 와 CF probe 였다. 조사기간중 Proteobacteria $\alpha$-group 은 0.7~32.9% 이었으며 $\beta$-group은 1.0~25.8%, $\gamma$-group은 2.4~37.1%, Cytophaga-Flavobacterium group은 4.7~23.6%의 분포를 나타내었다. 계절별로 살펴보면 규조류가 우점하는 봄펄에는 $\gamma$-group 이, 유기물의 농도가 낮고 수온이 높은 여름철에는 $\alpha$-group 이 우점하였으며, 남조류가 우점하는 가을철에 총세균수에 대한 Eubacterial group 의 비율이 크게 감소하여 특정 group의 우점현상은 나타나지 않았다. 이처럼 소양호는 계절과 수심에 따라 군집구조가 변화하였으며, 특히 세균의 군집구조는 식물플랑크톤의 천이와 밀접한 관계를 보이는 것으로 나타났다.

Characteristic of Aromatic Amino Acid Substitution at α96 of Hemoglobin

  • Replacement of valine by tryptophan or tyrosine at position $\alpha$96 of the $\alpha$ chain ($\alpha$96Val), located in the ${\alpha}_1{\beta}_2$ subunit interface of hemoglobin leads to low oxygen affinity hemoglobin, and has been suggested to be due to the extra stability introduced by an aromatic amino acid at the $\alpha$96 position. The characteristic of aromatic amino acid substitution at the $\alpha$96 of hemoglobin has been further investigated by producing double mutant r Hb ($\alpha$42Tyr$\rightarrow$ Phe, $\alpha$96Val$\rightarrow$Trp). r Hb ($\alpha$42Tyr$\rightarrow$Phe) is known to exhibit almost no cooperativity in binding oxygen, and possesses high oxygen affinity due to the disruption of the hydrogen bond between $\alpha$42Tyr and $\beta$99Asp in the ${\alpha}_1{\beta}_2$ subunit interface of deoxy Hb A. The second mutation, $\alpha$96Val$\rightarrow$Trp, may compensate the functional defects of r Hb ($\alpha$42Tyr$\rightarrow$Phe), if the stability due to the introduction of trypophan at the $\alpha$96 position is strong enough to overcome the defect of r Hb ($\alpha$42Tyr$\rightarrow$Phe). Double mutant r Hb ($\alpha$42Tyr$\rightarrow$Phe, $\alpha$96Val$\rightarrow$Trp) exhibited almost no cooperativity in binding oxygen and possessed high oxygen affinity, similarly to that of r Hb ($\alpha$42Tyr$\rightarrow$Phe). $^1$H NMR spectroscopic data of r Hb ($\alpha$42Tyr$\rightarrow$Phe, $\alpha$96Val$\rightarrow$Trp) also showed a very unstable deoxy-quaternary structure. The present investigation has demonstrated that the presence of the crucible hydrogen bond between $\alpha$42Tyr and $\beta$99Asp is essential for the novel oxygen binding properties of deoxy Hb ($\alpha$96Val$\rightarrow$Trp).

Diet and Physical Activity in Relation to Weight Change among Breast Cancer Patients

  • Background: This study aimed to provide an overview of lifestyle changes after breast cancer diagnosis and to examine the relationship between dietary and physical activity changes with weight changes in breast cancer patients. Women with breast carcinomas (n=368) were recruited from eight hospitals and four breast cancer support groups in peninsular Malaysia. Dietary and physical activity changes were measured from a year preceding breast cancer diagnosis to study entry. Mean duration since diagnosis was $4.86{\pm}3.46$ years. Dietary changes showed that majority of the respondents had decreased their intake of high fat foods (18.8-65.5%), added fat foods (28.3-48.9%), low fat foods (46.8-80.7%), red meat (39.7%), pork and poultry (20.1-39.7%) and high sugar foods (42.1-60.9%) but increased their intake of fish (42.7%), fruits and vegetables (62.8%) and whole grains (28.5%). Intake of other food groups remained unchanged. Only a small percentage of the women (22.6%) had increased their physical activity since diagnosis where most of them (16.0%) had increased recreational activities. Age at diagnosis (${\beta}$= -0.20, p= 0.001), and change in whole grain (${\beta}$= -0.15, p= 0.003) and fish intakes (${\beta}$= 0.13, p= 0.013) were associated with weight changes after breast cancer diagnosis. In summary, the majority of the women with breast cancer had changed their diets to a healthier one. However, many did not increase their physical activity levels which could improve their health and lower risk of breast cancer recurrence.

대한민국 비당뇨 성인에서 만성신장질환과 인슐린저항성 및 베타세포기능의 관련성 (The Relationship between the Progression of Chronic Kidney Disease and Beta Cell Function in Non-Diabetic Korean Adults)

  • 본 연구의 목적은 대한민국 20세 이상(50.32±16.14세)의 당뇨질환이 없는 성인에서 만성신장질환과 인슐린저항성(homeostasis model assessment of insulin resistance, HOMA-IR) 및 베타세포기능(homeostasis model assessment of beta cell function, HOMA-B)의 관련성에 대하여 조사하였다. 본 연구는 2015년도 국민건강영양조사 자료를 이용하여 당뇨질환이 없는 20세 이상의 대한민국 성인 4,380명을 대상으로 하였다. 본 연구결과에서 중요한 결과는 다음과 같다. 첫째, 만성신장질환과 HOMA-IR와 관련하여, 관련변수를 보정한 후의 결과에서(Model 4), 그룹 1 (G1; estimated glomerular filtrationrate [eGFR], ≥90 mL/min/1.73 ㎡), 그룹 2 (G2; eGFR, 60~89 mL/min/1.73 ㎡), 그룹 3a (G3a; eGFR, 30~59 mL/min/1.73 ㎡), ≥그룹 3b (≥G3b; eGFR, <30 mL/min/1.73 ㎡)의 HOMA-IR 평균값(M±SE, 95% confidence interval [CI])은 각각 1.78±0.03 (1.73~1.83), 1.87±0.03 (1.81~1.93), 2.16±0.13 (1.91~2.42) 및 2.59±0.24 (2.12~3.06)으로 만성신장질환이 진행됨에 따라 HOMA-IR은 증가하였다(P<0.001). 둘째, 만성신장질환과 HOMA-B와 관련하여, 관련변수를 보정한 후의 결과에서(Model 4), G1, G2, G3a 및 ≥G3b의 HOMA-B 평균값(M±SE, 95% CI)은 각각 87.46±1.21 (85.08~89.84), 89.11±1.38(86.40~91.81), 104.82±5.91 (93.23~116.42) 및 123.97±10.87 (102.66~145.29)으로 만성신장질환이 진행됨에 따라 HOMA-B도 증가하였다(P<0.001). 대한민국 당뇨질환이 없는 성인에서 만성신장질환이 진행됨에 따라 인슐린 저항성이 증가하였고, 베타세포기능 또한 증가하였다.

Ultrastructural Abnormalities in APP/PSEN1 Transgenic Mouse Brain as the Alzheimer's Disease Model

  • Alzheimer's disease (AD) is a progressive neurodegenerative disorder. Neuropathological hallmarks of AD are amyloid plaques, dystrophic neurite, and alteration of subcellular organelles. However, the morpho-functional study of this degenerative process and ultimate neuronal death remains poorly elucidated. In this study, immunohistochemical and ultrastructural analyses were performed to clarify the abnormal morphological alterations caused by the progression of AD in APP/PSEN1 transgenic mice, express human amyloid precursor protein, as a model for AD. In transgenic AD mice brain, the accumulation of Amyloid ${\beta}$ plaques and well-developed dystrophic neurites containing anti-LC3 antibody-positive autophagosomes were detected in the hippocampus and cortex regions. We also found severe disruption of mitochondrial cristae using high-voltage electron microscopy and three-dimensional electron tomography (3D tomography). These results provide morpho-functional evidence on the alteration of subcellular organelles in AD and may help in the investigation of the pathogenesis of AD.

남자 간호대학생의 진로결정자기효능감, 병역의무계획이 진로준비행동에 미치는 영향 (The Effect of Career Decision Making Self-efficacy and Military Duty Planning on Career Preparation Behavior among Male Nursing Students)

  • Purpose: This study aimed to investigate factors influencing career preparation behavior among male nursing students in South Korea. Methods: Data were collected from May 25 to June 17, 2018 and a total of 158 male nursing students participated in this study. Career decision making self-efficacy, military duty planning, and career preparation behavior were measured using a structured questionnaire. Collected data were analyzed with SPSS 25.0 for windows. Results: There was a statistically significant difference in career preparation behavior of participants according to entrance motivation and desired career after graduation. There were statistically significant correlations between career decision making self-efficacy and career preparation behavior (r=.65, p<.001), and between military duty planning and career preparation behavior (r=.45, p<.001). The influential factors for career preparation behavior were career decision making self-efficacy (${\beta}=.64$, p<.001), and military duty planning (${\beta}=.42$, p<.001). These factors accounted for 42% of the variance in career preparation behavior. Conclusion: The results of this study indicate that to improve career preparation behavior for male nursing students, it is necessary to enhance career decision making self-efficacy and military duty planning.

흰쥐 뇌 소교세포에서 진세노사이드 Rg3의 Type A Macrophage Scavenger Receptor 발현 증진효과 (Enhancement of Type A Macrophage Scavenger Receptor Expression by Ginsenoside Rg3 in Rat Microglia)

  • Macrophage scavenger receptors (MSRs) induce microglial interaction with ${\beta}$-amyloid fibrils (fA${\beta}$) that are associated with Alzheimer's disease (AD). Although microglia are know n to have a dual effect on formation of plaque and clearance of fA${\beta}$ in the AD brain, receptor-mediated phagocytosis is a very important tool for preventing amyloid plaque via activated microglia in the early stage of AD. In the study, we examined whether ginsonoside Rg3 enhances the microglial Phagocytosis of A${\beta}$1-42 through Phagocytosis assay, gene expression (RT-PCR) and protein assay (western blots) for the cell responsiveness presented between Rg3-treated and non-treated groups. Fluro-labeled Ac-LDL and E.coli particles were used as control proteins for phagocytosis. In previous studies, this was a particularly interesting property of Rg3 in the stimulation and phagocytosis of macrophages in the periphery. We report here that ginsenoside Rg3 increased the expression of type-A MSR (MSR-A) in microglia and thus accelerated the phagocytosis with an effective degradation of engulfed fA${\beta}$. This result suggests that Rg3 may play an important role in removing fA${\beta}$ by enhancing the receptor-mediated phagocytosis. In addition, Rg3 could be a potential candidate for balancing the rate of production of fA${\beta}$ in AD brain.

Eliminations from (E)-2,4-Dinitrobenzaldehyde O-Aryloximes Promoted by R3N/R3NH+ in 70 mol% MeCN(aq). Effects of Leaving Group and Base-Solvent on the Nitrile-Forming Transition-State

  • Elimination reactions of $(E)-2,4-(NO_2)_2C_6H_2CH=NOC_6H_3-2-X-4-NO_2$ (1a-e) promoted by $R_3N/R_3NH^+$ in 70 mol % MeCN(aq) have been studied kinetically. The reactions are second-order and exhibit Br$\ddot{o}$nsted ${\beta}$ = 0.80-0.84 and ${\mid}{\beta}_{lg}{\mid}$ = 0.39-0.42, respectively. For all leaving groups and bases employed in this study, the ${\beta}$ and ${\mid}{\beta}_{lg}{\mid}$ values remained almost the same. The results can be described by a negligible $p_{xy}$ interaction coefficient, $p_{xy}={\partial}{\beta}/pK_{lg}={\partial}{\beta}_{lg}/pK_{BH}{\approx}0$, which describes the interaction between the base catalyst and the leaving group. The negligible pxy interaction coefficient is consistent with the $(E1cb)_{irr}$ mechanism. Change of the base-solvent system from $R_3N$/MeCN to $R_3N/R_3NH^+$-70 mol % MeCN(aq) changed the reaction mechanism from E2 to $(E1cb)_{irr}$. Noteworthy was the relative insensitivity of the transition state structure to the reaction mechanism change.
