Background: It is axiomatic that average life span is prolonged in proportion to the advance of socio-economic improvements and medical science. Consequently, the number of elderly patients with skin diseases is increasing. Objective: This study was performed to examine the patterns of skin diseases in elderly patients that had visited Dongguk Oriental Medical hospital, Dept. of Dermatology. We also compared these patients to the young generational groups. Methods: We studied 67 elderly patients with complaints of skin diseases who had visited our hospital from March 2000 to February 2002. We analysed their skin diseases and compared them with the younger group's. Results: The results were as follows; 1. The common disease groups were eczema(32.8$\%$); pruritus(23.9$\%$); viral infections (13.4$\%$); psoriasis(9.0$\%$); fungal infections and urticaria(6.0$\%$). 2. The disorders prevalent in men were eczema(30.0$\%$); herpes zoster & pruritus(23.3$\%$); fungal infection(13.3$\%$); psoriasis (3.3$\%$); melasma(6.7$\%$). 3. The common diseases of women's group were observed in the following order; eczema(35.l$\%$); pruritus(24.3$\%$); psoriasis(13.5$\%$); urticaria(10.8$\%$); other dermatoses(8.1$\%$); herpes zoster(5.4$\%$). 4. The most frequent skin disease for those in their 50's was eczema(32.3$\%$); 60's, pruritus(36.8$\%$); 70's, eczema(50.0); and over eighties., eczema and pruritus(40.0$\%$). Conclusion: This study suggests that dermatologic problems in the elderly are very common, especially eczema, pruritus. It is a very different pattern from the younger group's. And we have to better consider more effective management and treatment for them; especially further dermatologic studies including substantial medical care. Usually Oriental medicine is good at treating chronic diseases and less harmful. So we expect it to be generally better for elderly patients than western medicine.

The recent phenomenon of job "Miss Match", which is inconsistent with knowledge in the demand of educational training institutes and industries, has spread to an increase in private education costs for reeducation and employment of new hires, resulting in weak individual job competency and poor employment capability, as well as economic and material waste at the national level. To compensate for these problems, the National Competency Standards(NCS), which are available immediately in practice and look for a standard point of national job competency with the aim of fostering human resources sought by industries, were developed, and even the NCS-based qualification system was launched in line with the stream of times. This study is intended to look into the importance and priority of competency units and competency unit elements at the NCS-based qualification L3 level in the skin care field for an overall check of the NCS-based qualification level at a time when educational institutes are organizing and operating the school curriculums according to the NCS and NCS-based qualification level. And it is attempted to provide basic data for the development of curriculum in fostering professional human resources required by industries. To analyze the needs for competency units and competency unit elements at the L3 level, a survey using AHP method was carried out to a group of field experts and a group of education experts. In addition, the SPSS(Statistical Package for Social Science) ver. 21.0 and Expert Choice 2000, an AHP-only solution was used to do statistical processing through the processes of data coding and data cleaning. The findings showed that there was a partial difference of opinion between a group of field experts and a group of education experts. This indicates that the inconsistencies between educational training institutes and industrial sites should be resolved at this time of change with the aim of fostering field customized human resources with professional skills. Consequently, the solution is to combine jobs at industrial sites and standardized educations of educational institutes with human resources required at industrial sites.

Essential oils have been used extensively in pharmacy, medicine, food, beverages, cosmetics, perfumery and aromatherapy. Although anti-bacteria, anti-virus, alleviation of fever operations and an anti-inflammatory properties have been reported, action mechanisms have not been fully discovered. In the present study, anti-inflammatory activities of thirty three essential oils have been evaluated in lipopolysaccharide (LPS)-treated macrophage RAW 264.7 cells by the evaluation of nitric oxide (NO) generation since NO generation is implicated in causal factor of inflammation. Among the tested 33 essential oil, Lemongrass oil showed the most inhibitory effect on LPS-induced NO generation in a dose dependent manner ($IC_{50}$ : $22 {\mu}g/mL$). In further study, it was found that Lemongrass oil inhibited the expression of inducible nitric oxide synthase. These results suggest that Lemongrass oil may be useful for improvements of the inflammatory disease such as pimple acne skin.

Three melanogenesis inhibitors were isolated from the roots and rhizomes of Veratrum nigrum L. and were identified as (3S,20S,25S)-22,26-iminocholesta-5,22(N)-dien-3-ol (verazine), (3S,2OR,25S)-22,26-iminocholesta-5,22(N)-dien-3-ol (epi-verazine) and (3R,23R)-14,15,16,17- tetradehydroveratraman-3,23-diol (veratramine) on the basis of their spectroscopic data. It was turned out that these compounds did not directly inhibit tyrosinase activity, the key enzyme responsible for the formation of melanin pigment while these compounds showed strong inhibition on the melanogenesis in B16 F1 mouse melanoma $(IC_{50}<1\;{\mu}g/ml)$. Due to the strong inhibitory activity and safety compared to current whitening agents such as arbutin, kojic acid and AHA, the compound can be a good candidate for new skin whitening agents.

The aim of this study was to investigate the anti-inflammatory and ultraviolet (UV)-protective effect of Pleurotus eryngii extract (PEE) in activated RAW 264.7 cells and UV-induced mouse skin damage. The results showed that PEE strongly inhibited the production of inflammatory mediators such as nitric oxide, interleukin $(IL)-1{\beta}$, and IL-6 at high concentrations in LPS-stimulated RAW 264.7 macrophages. In addition, PEE treatment suppressed erythema, melanin index, and epidermal thickness to a greater degree than ascorbic acid (AA) treatment in UV-irradiated mice. Finally, PEE treatment inhibited the infiltration of mast cell to a similar degree of AA treatment. Therefore, these results indicate that PEE could improve inflammation and skin damage in immune cells and UV-irradiated mice. This study may provide positive insights into PEE as a functional food and cosmetic ingredient for treatment of inflammation and skin damage.
In this study, four types of retinol, retinyl palmitate, adenocin, and polytoxylate-dretinamide, which are the ingredients of the Ministry of Food and Drug Safety Notice, included in the study. also we looked at trends in research on Sciadopitys verticillata, Prunella vulgaris, Celosia cristata L., Brazilin, Persicaria hydropiper, Astragalus membranaceus Bunge, Forsythiae Fructus, Lithospermum root, Rheum undulatum L. and Cistanche deserticola Y. C. Ma a natural material that has the efficacy of antioxidant aging. The anti-aging study so far has been found to be centered mainly on collagen production and elastase synthesis inhibition mechanisms. However, given that the aging process of the skin is caused by various ageing processes, it is believed that anti-aging studies using safe and effective natural materials that can help the skin age with various mechanisms should be conducted.

In this study, the potential anti-aging properties of Prunella vulgaris extract were investigated. According to our results, Prunella vulgaris extract increased collagen synthesis(74.7% at 250 ${\mu}g/mL$) and decreased on MMP-1 synthesis(90.2% at 200 ${\mu}g/mL$) and elastase activity(43.7% at 2.0%). Furthermore, it also showed free radical scavenging activity(76.9% at 2.0%) and reduced $H_2O_2$-induced cytoxicity(49.9% at 2.0%). A double-blind clinical study to investigate the effect of Prunella vulgaris extract on the skin's surface was conducted with 22 healthy volunteers, aged 34 to 48 years. The volunteers applied a cream formula with 4.0% of Prunella vulgaris extract, or placebo cream, on each crow's feet twice a day for 12 weeks. Skin wrinkles were evaluated with the naked eye and instrumental image analysis of silicone replicas, followed by statistical analysis. Twelve weeks after application of cream formula with 4.0% of Prunella vulgaris extract, we found significant improvement of facial wrinkle. Moreover, silicone replica analysis confirmed notable improvement in average of R2 and R3 at 12 weeks(p<0.05). These results demonstrate that Prunella vulgaris provides a remarkable and significant tensor and anti-wrinkle effect on the skin, which could be of great use in anti-aging skin care products.
This research was to investigate the inhibitory effects of tyrosinase, elastase and collagenase using rapeseed meal extract for the functional cosmetics. Gamma-tocopherol and alpha-tocopherol contents were 304.9 and 212.2 mg/kg, respectively. In the case of delta-tocopherol and plastochromanol-8, they were 12.1 and 35.7 mg/kg, respectively. The total phenol content of methanol extract was the highest (49.6 mg/g) which was about 4.96 fold higher than that of water extract. The maximum nitrite scavenging activities of methanol and acetone extract at pH 1.2 were 85.2 and 80.1%, respectively, at 8.0 mg/mL. When the extract concentration of rapeseed meal increased upto 2.0 mg/mL, cell viabilities did not appear to have any significant cytotoxic effect, irrespective of extracts. Tyrosinase and elastase inhibition activities increased from 25.2 to 42.5% and 25.3 to 48.0%, respectively, as methanol extract concentration increased from 0.5 to 1.0 mg/mL. The collagenase inhibition activities of methanol and acetone extracts at 1.0 mg/mL were 67.2 and 68.0%, respectively. These results showed that the methanol and acetone extract of rapeseed meal can be used as a new source of functional cosmetic agent.
Noni has been used for medicinal purposes for more than 2,000 years in South Pacific Polynesia, China and India, and has been heavily ingested as an extract for its excellent antioxidant and anti-inflammatory effects. However, a recent study found that the noni extract causes digestive disorders, kidney problems, and liver diseases, which made it necessary to use it for other purposes than as an extract. In this study, we want to evaluate the potential of noni as an anti-oxidant, anti-inflammatory and anti-wrinkling agent. Methods: Noni was freeze-dried, extracted in water, and concentrated. Skin cells were treated with the noni extract for 24 hrs and then were exposed to UVB (55 mJ/cm2). After 48 hrs of incubation, pro-inflammatory cytokine, elastase, MMP-1 and type-1 procollagen levels were measured by ELISA. Results: To find out the antioxidant effect of the noni extract, the DPPH and ABTS radical scavenging activity experiments were conducted and the noni extract showed 97.0 % and 92.0 % antioxidant efficacy at 200 ㎍/mL respectively. The noni extract (50 and 100 ㎍/mL) decreased IL-6 and TNF-α in RAW 264.7 cells induced by LPS in a concentration-dependent manner. In the RT-PCR experiment involving NO production, the noni extract (50 and 100 ㎍/mL) inhibited NO production by strongly inhibiting iNOS mRNA expression, and also inhibited the elevation of MMP-1 and elastases caused by UVB irradiation by 25.0 % and 7.0 % respectively. In addition, type-1 procollagen was elevated by 20.0 % by the noni extract treatment in HaCaT cells. Conclusion: The noni extract has photoprotective ability by reducing proinflammatory mediators, elastase and MMP-1 production, and elevation of collagen synthesis. Our findings suggest that the noni extract might be a good natural substance to protect against UVB-induced premature skin aging.
The purpose of this study were to analyze the home care services and to evaluate the client's satisfaction with the home care services provided by home care service center in the C hospital. The data were collected by reviewing charts of 128 home care clients who were receiving home care services at C hospital from October 1997 to September 2000. The subjects for satisfaction of home care service were 20 clients from July 10 to September 30, 2000. The tool for measurement of present condition of home care service was developed by the researchers. The satisfactions of the home care services were measured by using the instrument developed by Im(997). The data were analyzed by using the SPSS/PC+. The results of this study were as followings : 1. Majority of the subjects was female(61.7%). The average of age was 63.5 years. The service has been used mostly by the elderly 60 years of age or older(71.1%). The economic level of most of subjects was in middle class(94.5%). 2. Majority of the subject had a cancer(55.4%), following stroke(25.0%). The average duration of disease for the subjects was 31 months. The average time of hospitalization for the subjects was 3.3 times. The duration of hospitalization was 10$\sim$30 days(26.6%), 30$\sim$60 days(23.4%) and above of the 210 days(9.4%). 3. Most of the subjects used his/her doctor (47.7%), as a consultant, following his/her nurse (28.1%), other patients or their family (21.9%). Most of reasons for a consultation were supportive management(Infusion or medication, 60.94%), following tube management(L-tube or T-tube, 25%), Foley catheter management (15.63%) etc. 4. 28 types of nursing diagnoses were used by the home care service. The nursing diagnosis altered nutrition: less than body requirement were used mostly by the home care service, following risk for infection, impaired skin integrity, impaired swallowing, ineffective airway clearance altered comfort: pain, impaired physical mobility. By the human-response pattern, exchanging(63.2%), moving(7.5%), feeling(10.4%), knowing(5.2%), communicating (2.6%), relating(0.5%) perceiving(0.4%) and choosing(0.3%). There were 42 nursing intervention types were performed by the home care service. By the NIC(nursing intervention classification. McCloskey. Bulech. 1996). physiologic: complex (30.3%) was the most, safety(28.3%), behavioral(20.0%), physiologic: basic(10.8%) and health system(1.7%). Observation or assessment was the most nursing intervention performed by the home care service. following IV infusion. vital sign observation. infusion management and fluid-electrolyte balance management. 5. The level of client's satisfaction with provided home care services showed considerably high(2.67/ 3).
