Background : Use of Ma Huang is controversial. Multiple reports of adverse effects led the U.S. Federal Drug Administration (FDA) to announce about prohibiting dietary supplements containing ephedrine alkaloids/Ma Huang. Objective : The objective of the study was to review the evidence from literature and rigorous clinical trials on the dosage, effectiveness, and adverse effects. Methods : Literature searches were conducted in U.S. FDA database after 2003 and digital Uibujeonok(醫部全錄). Hand searches of medical journal and bibliographies of identified articles were conducted. Results : Ma Huang has been used in Korean Oriental Medicine to treatment asthma and other ailments. Recently, Ephedrine/Ma Huang are used for weight loss or to enhance athletic performance in Europe and America, Korea. Ephedrine/Ma Huang are associated with increased risk of psychiatric, autonomic, or gastrointestinal symptoms, and heart palpitations. In addition to that, a risk of serious adverse events, including heart attack, stroke, epilepsy and psychiatric problem are also have been reported. Therefore, max dose of total alkaloids of ephedrine is studied in need of a specialist's part. Conclusion : In most reports, Ma Huang effectively promoted short-term weight loss and enhancement of athletic performance, But, safety with long-term use requires further investigation.

Objectives : This study was designed to collect the latest papers and to find more effective approach for childhood obesity. Methods : We searched 107 papers in English from Pubmed(www.ncbi.nlm.nih.gov) and selected 52 in favor of our study. In addition to that, we searched 29 papers in Chinese from www.cnki.net. Results are as follows. Results and Conclusions : There are not completely new treatments for childhood obesity. Instead of that, many studies aimed for prevention through family education, nutrition education, and exercises education. Specially, because some diseases including hypertension, diabetes and heart disease are connected with obese children, medication can be prescribed optionally. In that case, their mental health must be laid emphasis. And in China, TCM(Traditional Chinese Medicine) is very common treatment for childhood obesity. Acupuncture, herb medication and manual therapy could be used usefully. It is suggested that above latest treatments can be combined with Korean traditional treatments method in treating obese children.

Background : Mahuang (Ephedra sinica) has been used widely to treat respiratory disease in traditional medicine for over thousands years. Mahuang preparations contain approximately $0.75{\sim}1%$ ephedrine alkaloids. Recently in North America, the Eephedra alkaloids has aroused a controversy due to its adverse effects in those using whole extracts as "dietary supplements" for weight loss or athletic performance enhancement. Objective : To discuss the need for clinical practice guidelines on the use of Mahuang. Results and Conclusion : It is widely assumed that Mahuang is one of the useful medicines and is safe for human body. Most of scientific literatures support that the safety of Mahuang prescription in common dosage. However, it has been claimed that there were some warnings and apprehensions about the harmfulness of Mahuang as dietary supplements. This article does not negate the fact that it is necessary to pay more attention for use of Mahuang in clinical situations. We assert that the establishment of clinical practice guideline on the use of Mahuang, which is based on scientific materials and consensus, is required in traditional medicine.

Objective : Women's obesity brings prblems not only appearance but also health which men do not have. This study was conducted to investigate the different factors of obesity between men and women. Materials and Methods : We searched papers usin key words (women, gender, and obesity) on pubmed and obesity journal. Result : Women's obesity leads to amenorrhea, abnormal uterine bleeding, infertility, poly cystic ovarian syndrome, abortion, and luteal phase inadequacy. Obesity induces metabolic syndrome, type-2 diabetes, cardiovascular problems, hypertension, cancer, and psychophysiologic diseases. The difference in body morphology and in particular fat distribution between the sexes leads to gender-specific differences in prevalence of chronic diseases, and unique problems for each sex including infertility, problems during pregnancy, polycystic ovarian syndrome, and endometrial carcinoma in women, and prostate and testicular cancer in men. The influence of gender on obesity is had by genetic view, hormones, pregnancy, delivery, and menopause. Conclusion : Obese women have higher risk factors than men by the influence of gender.

Objectives : This study has been done to know which herbs and herbal formulae are frequently used for the treatment of obesity in Korea and overseas since 2000. Methods : We conducted a computerized literature search in KISS (Korean studies Information Service System), Pubmed, Journal of Oriental Association for Study of Obesity ,and Journal of Oriental Rehabilitation Medicine(2000 -Apr. 2006) using search key words obesity, obese, weight loss, weight control combined with herb, herbal formula. Result and conclusion : Seventy-two studies have been identified(50 Korean papers and 22 pubmed papers) and 81 herbs have been found. In USA it was mainly used as single herb for food, but in Korea it was mainly used as herbal formula. For single herb, ephedra(麻黃) was the most studied single herb both in Korea and the USA. For herbal formulae, Sasang Constitutional Medicine(SCM, 19times) was the most frequently used herbal formula, in which the herbs for Tae-eum type person like Taeyeumjowui-tang(太陰調胃湯, 7times) and Choweseungcheng-tang(調胃升淸湯, 5times) were mainly prescribed. The next was Chegameuiin-tang(體減薏苡仁湯, 11times) and Bangpungtongsungsan( 防風通聖散, 3times). Calculating each herb consisted in each herbal formula, Coicis Semen(薏苡仁), Angelicae gigantis Radix(當歸), and Glycyrrhizae Radix(甘草), Raspberry Ketone, Cnidii Rhizoma(川芎), and Ephedra Herba(麻黃) turned out to be the most frequently used herb.

Objectives : The aim of the study was to investigate the principal symptoms and a syndrome differentiation in the obesity using surveys from Oriental medical doctors and obese patients. Methods : Seventy three Oriental medical doctors who participated in the 2006 autumn annual conference of Korean Oriental Association for Study of Obesity and 243 obese patients responded to the survey. Results : Twenty nine percent of Oriental medical doctors replied that the syndrome differentiation is the most important diagnosis index, and 21 percent of them replied they use Sasang Constitution classification during diagnostic process. The syndrome differentiations used were mainly phlegm-fluid, blood stasis, spleen vacuity, food accumulation, damp phlegm, and Gi deficiency order. In the response of doctors and patients about principle symptoms of 6 syndrom differentiation belong inside 5 place except phlegm fluid and liver stasis Conclusions : We should develop syndrome differentiation questionnaire about obese symptoms.

Objectives : This study was performed to identify the efficacy and safety of ephedra and green tea mesotherapy on localized fat. Methods : Twenty-five healthy $20{\sim}35$ year overweight women (body mass $index{\geq}23kg/m^2$) were recruited and randomly assigned to two groups : experimental(mahuang and green tea) group (n=13), placebo(Normal saline) group (n=12). Total 8 times of herbarl mesotherapy treatment were performed once per week, 4 times were performed on right thigh (1st period), after wash-out for 1 week, the other 4 times were performed on left thigh (2nd period) without any modification of diet or exercise. Body composition, thigh circumferences were measured at each period before and after the intervention. Subcutaneous fat was measured by CT scan at before and after the 1st period of intervention and free fatty acid(FFA) was measured immediately before 1st intervention, 1 hour later, 24 hours later, and 7 days later. Satisfaction score and adverse events were evaluated. Results : In the subcutaneous fat, there were no significances, but in the circumferences. Experimental group had significant change when compared with placebo group (p<0.05). In FFA, experimental group has significant elevation at after 1 hour (p<0.05) evaluation. There were no serious adverse events in both groups. Conclusions : Experimental group had some significant findings in circumference, FFA, and had no serious adverse events comparing with placebo group, and no serious adverse events have been reported in both group. Mesotherapy with herb have a possibility of treating localized fat

Objectives : This study was performed to evaluate the effect of oriental medical treatment on aged female obese patient. Methods : We experienced an aged female obese patient. We applied herbal medication(Choweseuncheng-tang), electrolipolysis, auricular acupuncture to her from 26th, March. 2007 to 25th, May. 2007. And we examined the changes of her weight, body composition, body size, skin resistance variability, visual analogue scale of knee pain. Results : 1. Her weight decreased from 73.5kg to 71.1kg and body mass index(BMI) decreased from $29.46kg/m^2$ to $28.11kg/m^2$. 2. Body fat mass(BFM) decreased from 25.7kg to 21.6kg but body muscle mass(BMM) increased from 45.1kg to 46.7kg. 3. Waist-hip ratio(WHR) decreased from 0.96 to 0.92. 4. Basal metabolic rate(BMR) increased from 1,273kcal to 1,314kcal. 5. The circumference of upper extremity decreased 1.03cm and lower extremity decreased 1.30cm but the circumference of the muscle of upper extremity increased 0.41cm and lower extremity increased 0.63cm. 6. The circumference of chest and hip are decreased. In addition to the circumference and fat of abdomen are decreased. 7. The visual analogue scale of knee pain is improved. Conclusions : According to above results, oriental medical treatment could be used safety to aged obese patients without loss of fat free mass. The study is more required to aged obese patient from now on.
