Comparison of Health Status and Sleep Patterns between Shift Workers and Non-shift Workers in Manufacturing Plants (사업장 교대근무자와 비교대근무자의 수면문제와 건강상태 비교)

  • Purpose: This study was conducted to compare and estimate sleep patterns and health status of the shift and non-shift factory workers. Methods: The subjects of this study were 290 male workers in 13 companies, which manufacture automobile parts in Daegu, Korea. Survey data were collected from March 5th, 2007 to April 5th, 2007. PSQI was scored to check the quality of sleep, ESS for excessive daytime sleep disorder, ISI for insomnia, and SF-36 for health status. The data were analyzed by $X^2$ and t-test using SPSS 12.0 for windows. Results: There was a difference in quality of sleep patterns between shift and non-shift workers (p=.033). A difference in the pattern of excessive daytime sleep (P=.005) and in the degree of insomnia (p=.030) were observed between the two groups. However, there were no significant difference in health status between the two groups. Conclusion: Based on these results, shift workers are in need of careful and continuous monitoring regarding sleep disorders and we need to develop health promotion programs including sleep hygiene for shift workers.

Effects of a Full Body Massage on Uterine Contraction, Length of Labor, Type of Delivery, and Drug Intervention for Primipara during Labor (분만 중 전신마사지가 초산부의 자궁수축, 분만소요시간, 분만형태 및 약물사용에 미치는 효과)

  • This study was designed to test the effects of a full body massage on uterine contraction, length of labor, type of delivery, and drug intervention for primipara during labor. Data were collected using a quasi-experiment method (nonequivalent control group, pre-post test design) from November 1, 2001 to July 31, 2002. The subjects of this experiment consisted of 28 women in the experimental group and 29 in the control group, out of 57 primipara hospitalized at the U OB & GYN hospital in Inchon. The experimental group was given a 20 minute full body massage for each of the three delivery phases (latent, active, and transition). The control group was given conventional delivery care. Three (3) parameters were analyzed in this experiment. (1) The interval, duration, and strength of uterine contraction, using an electric tocodynamometer (2) The elapsed time for stage 1 and stage 2 labor. (3) The types of deliveries and drug interventions, using postpartum medical records The data collected were analyzed using the repeated measures analysis of variance (ANOVA), t-test, and $x^2$ test of the SPSS program. The results of the experiment are as follows: 1) Uterine contraction interval was significantly reduced (F=3.210, p=.050). Duration of uterine contraction showed significant increase only during the transition phase (t=-2.319, p=.023). Strength of uterine contraction showed no significant difference. 2) Total length of labor was significantly shortened (t=-5.245, p=.000). The length of 1st stage labor was significantly shortened (t=-5.164, p=.000), with latent phase showing (t=-4.709, p=.000), active phase (t=-2.973, p=.005), and transition phase (t=-2.031, p=.047). The length of 2nd stage labor showed no significant difference. 3) The number of natural deliveries were significantly increased ($x^2$=13.127, p=.004). 4) The number of drug interventions were significantly fewer ($x^2$= 4.493, p=.034). In conclusion, this study shows that a full body massage has a significantly positive effect on uterine contraction interval, length of labor, type of delivery, and drug intervention. Therefore, this study suggests that a full body massage be used clinically to help primipara during labor.

Level of Lead in Air and Blood Zinc Protoporphyrin of Workers in Lead Plants (연 취급 노동자의 연 폭로 수준 및 혈중 Zinc Protoporphyrin 농도)

  • For the purpose of estimating the working environment and the relationship between the airborne lead concentration and the ZPP level in the whole blood of the workers, the airborne lead concentrations and the ZPP level were measured at the 26 plants which deal with lead, from October 5 to November 5 in 1988. Analysis of the airborne lead concentration was performed by NIOSH Method 7082, and the ZPP level was measured by a hematofluorometer. The following results are concluded. 1. The average airborne lead concentration of the lead battery manufactures is 0.025mg/m$^{3}$ and that of the secondary lead smelters is 0.023mg/m$^{3}$. There were no significant differences between industry (p>0.1) 2. At the lead battery manufacture, the process of lead powder production showed the highest concentration of 0.034mg/m$^{3}$ but there were no significant differences among the processes (p>0.1). At the secondary lead smelter, the process of dismantling waste batteries showed the highest concentration 0.141mg/m$^{3}$, and there were very significant differences among the processes (p<0.005). 3. The ZPP level in the whole blood showed significant differences between industry (p<0.10). The average ZPP level of the lead battery manufactures is 133.0 + 106.3 $\mu$g/100ml and that of the secondary lead smelters is 149.6 + 110.9 $\mu$g/100ml. 4. The correlation coefficients between the airborne lead concantration and ZPP level were 0. 426 (p<0.001) for the lead battery manufactures and 0.484 (p<0.001) for the secondary lead smelters. The correlation coefficients between the work duration (in months) and the ZPP level were 0.238 (p<0.001) for the lead battery mannfactures and 0.075 (p>0.10) for the secondary lead smelters. 5. The linear regression equation, with the airborne lead concentration as an independent variable and the ZPP level as a dependent variable, is Y=96.84+1300.34X (r=0.448, p<0.001) for the 26 plants which deal with lead. The linear regression equation, with the work duration(in months) as an independent variable and the ZPP level as a dependent variable, is Y=127.28 +0.49X (r=0.162, p<0.05). 6. The correlation coefficients between the amount of inhaled lead and ZPP level were 0.349 (p < 0.001) for the lead battery manufactures and 0.318(p<0.001) for the secondary lead smeltes. The linear regression equation for the 26 plants surveyed, with the amount of inhaled lead as an independent variable and ZPP level as a dependent variable, is Y=123.63+18.82X (r=0. 335, p<0.001).

Direct Intratumoral Injection of Ethanol in the Patients with Obstruction of Major Bronchus (주기관지 폐색환자에서 종양내 ETHANOL 주입치료 효과)

  • Background: For the relief of the occlusion of major bronchi, laser therapy, radiation therapy or combined therapy is generally used. But the effect of radiation therapy is very slow and not consistent and laser therapy requires expensive equipments and technical expertise with occasional serious side effects. Direct ethanol injection has been widely used for the control of bleeding in gastrointestinal lesion, esophageal varices or renal cell carcinoma with good results. So we tried direct injection of ethanol into the tumor to relieve the obstruction of major bronchus in 11 patients. Method: All procedures were done under the fiberoptic bronchoscopy with continuous oxygen supplement and aliquoted 0.5-1.0 ml of absolute ethanol directly into the tumor through the endobronchial aspiration needle. The tumor was endoscopically removed with a biopsy forceps immediately after ethanol injection. The whole procedure was repeated 3-4 days interval until the lumen opens. Usually after 2-3 trials of ethanol injection, the lumen opened up. Results: The immediate effect of ethanol injection was whitening of the mucosa and prompt cessation of bleeding. The late effect was necrosis of the tumor. The final results of this procedure were improvement of symptoms and reexpnasion of the lung in all patients. $FEV_{1.0}$ and FVC were improved and $PaO_2$ was increased from $68.1{\pm}9.2$ mmHg to $83.9{\pm}8.1$ mmHg, $SaO_2$: from $94{\pm}8.5%$ to $96.6{\pm}1.1%$, and $AaDO_2$ was reduced from $26.5{\pm}8.5$ mmHg to $10.9{\pm}9.1$ mmHg. Conclusion: Direct ethanol injection into the tumor tissue is a rapid, cheap and relatively safe method of relieving the complete occlusion of major bronchus.

Clinical Impact of Overexpression of FOXP3 and WT1 on Disease Outcome in Egyptian Acute Myeloid Leukemia Patients

  • Background: In the last decade, it has become clear that change of gene expression may alter the hematopoietic cell quiescent state and consequently play a major role in leukemogenesis. WT1 is known to be a player in acute myeloid leukemia (AML) and FOXP3 has a crucial role in regulating the immune response. Objectives: To evaluate the impact of overexpression of WT1and FOXP3 genes on clinical course in adult and pediatric AML patients in Egypt. Patients and methods: Bone marrow and peripheral blood samples were obtained from 97 de novo non M3 AML patients (63 adult and 34 pediatric). Real-time quantitative PCR was used to detect overexpression WT1 and FOXP3 genes. Patient follow up ranged from 0.2 to 39.0 months with a median of 5 months. Results: In the pediatric group; WT1 was significantly expressed with a high total leukocyte count median 50X109/L (p=0.018). In the adult group, WT1 had an adverse impact on complete remission induction, disease-free survival and overall survival (p=0.02, p=0.035, p=0.019 respectively). FOXP3 overexpression was associated with FAB subtypes AML M0 +M1 vs. M2, M4+M5 (p =0.039) and the presence of hepatomegaly (p=0.005). Conclusions: WT1 and FOXP3 overexpression has an adverse impact on clinical presentation, treatment response and survival of pediatric and adult Egyptian AML patients.

Analysis of Signal Properties in accordance with electrode area of x-ray conversion material (X선 검출 물질의 전극 면적에 따른 신호특성 분석)

  • In recent, a digital x-ray detector attracted worldwide attention and there are many studies to commercialize. There are two methods in digital x-ray detector. This method is an Indirect method and Direct method. This study is to see the differences between the digital x-ray detector based on a-Se used in the existing indirect conversion method and an x-ray conversion material that has better SNR(Signal-to-noise ratio) and property than the a-Se. To solve the problem that is difficult to make a large area film using Screen-Print method, we used a Screen-Print method. In this study, we used a polyclystal $HgI_2$ as x-ray conversion material and a sample thickness is $150{\mu}m$ and an area is $3cm{\times}3cm$. ITO(Indium-Tin-Oxide) electrode was used as top electrode using a Magnetron Sputtering System and each area is $3cm{\times}3cm$, $2cm{\times}2cm$ and $1cm{\times}1cm$ and then we evaluated darkcurrent, sensitivity and SNR of the $HgI_2$ film are measured, then we evaluated the electrical properties. And we used a current integration mode when I-V test. This experiment shows that the sensitivity increases in accordance with the area of the electrode but the SNR is decreased because of the high darkcurrent. Through fabricating of various thicknesses and optimal electrodes, we will optimize SNR in the future work.

Identification of Irradiated Crabs by ESR Spectrometry

  • Electron spin resonance (ESR) spectroscopy was used to investigate the effect of irradiation dose on the ESR signal intensity of irradiated crabs and the stability of these radicals under 9 weeks of storage. Swimming and small crabs were irradiated with doses of 0, 1, 3, 5 and 7 kGy using a Co-60 irradiator at ambient temperature. A claw, a walking leg and a cars- pace of the crab pieced and dried were placed in a resonant quart tube within an EPR X-band spectrometer. The irradiated crabs presented an asymmetric absorption in shape at g$_1$=2.002 $\pm$ 0.003 and g$_2$=1.998$\pm$0.005, and were different from the non-irradiated ones. The intensity of the ESR signals was greatest in the claw, intermediate in the carapace and lowest in the walking leg. Samples given low and high doses of irradiation could also be distinguished. The ESR signal after irradiation was stable, even after a 9-week storage.

The Effects of Lifestyle Factors on Metabolic Syndrome among Korean Adults (한국인의 대사증후군에 대한 생활습관 요인의 영향)

  • Purpose: The purpose of this study was to estimate the effects of lifestyle factors on metabolic syndrome (MS) among Korean adults ($age{\geq}20$). Methods: A total of 7,798 subjects (weighted subjects=37,215,961) were recruited from the 2009 Fourth Korea National Health and Nutrition Examination Survey (KNHANES IV-3). Data were analyzed by t-test, ${\chi}^2$-test, and logistic regression in consideration of strata, cluster and weight as national data using the SAS 9.1 program. Results: The prevalence of MS by definition of AHA/NHLBI and waist circumference cutoff points for Koreans was 22.4%. The mean clinical MS score for MS patients was 3.4, but the mean score for the non-MS group was 1.2 out of 5.0. Among the lifestyle factors, smoking (OR=1.024), stress ($0.546{\leq}OR{\leq}0.587$) and drinking (OR=1.005) had significant influence on the MS risk and MS scores, but exercise did not. Conclusion: The results of this study indicate that further research is necessary on the effect of lifestyle factors on MS risk and nurses should focus on effective programs about smoking, stress and drinking for the prevention and reduction of MS risk.

The Association between Changes in Food and Nutrient Intakes and Changes in Bone Metabolic Indicators in Postmenopausal Women with Osteopenia after a 12-week Intervention of Nutrition Education and Aerobic Exercise (폐경 후 골감소증 여성에 대한 12주간의 영양교육과 운동 중재 전.후 식품 및 영양소 섭취량 변화와 골밀도 지표 변화와의 관계)

  • Few studies investigated the effects of nutrition education and exercises in women with osteopenia. This study examined the relationship between changes in dietary intakes and changes in indicators related to bone health in postmenopausal women with osteopenia (-2.5 ${\leq}$ T-score ${\leq}$ 1) after a 12-week intervention. Thirty-one postmenopausal women aged > 50 years residing in Seoul were recruited and participated in nutritional education regarding bone health and general nutrition practices and aerobic exercises (three times a week; 60 min per session). Twenty-five subjects completed the study and were eligible for the analysis. Bone mineral density (BMD) at femoral neck was measured by dual energy x-ray absorptiometry. Serum calcium, osteocalcin, and intact parathyroid hormone (PTH) were also measured. Dietary intake was estimated by using a one-day 24 recall by a clinical dietitian. After 12 weeks, meat consumption increased (P = 0.028) but vegetable intake decreased (P = 0.005). Intakes of animal protein (P = 0.024), vitamin B1 (P = 0.012) and vitamin $B_2$ (P = 0.047) increased, and sodium intake decreased (P = 0.033). Intact PTH (P = 0.002) decreased and osteocalcin (P = 0.000) increased, however, BMD decreased (P = 0.000). Changes in mushroom consumption were positively correlated with femoral neck BMD (r = 0.673, P = 0.003). Changes in animal iron intake were negatively correlated with intact PTH (r = -0.488, P = 0.013) but were positively correlated with osteocalcin (r = 0.541, P = 0.005). These results suggested that the association between animal iron intake and biochemical markers of bone turnover may play an important role in bone metabolism. Further studies are needed to shed light on complicated mechanisms of diet, hormonal levels of bone metabolism, and bone density.

  • We have investigated the superconducting properties and surface morphology of $Sm_xBa_yCu_3O_{6+z}$ thin films deposited on LMO/IBAD-MgO/Hastelloy which prepared with different composition ratio by co-evaporation method(EDDC, Evaporation using Drum in Dual Chambers). We observed the composition ratio of SmBCO thin films by EDS analysis. We fabricated SmBCO thin film with critical current density of $1.5{\times}10^6A/cm^2$ at composition ratio of SM:Ba:Cu=1.10:2.01:3(at 77 K self-field). And, we confirmed that substitution of Sm-Ba did not occur at Cu rich phase by EDS analysis.
