Negative Pressure Wound Therapy Applied to a Meshed Split-Thickness Skin Graft

  • Purpose: Skin grafting is used for the transfer of cutaneous tissue from one site of the body to another. To improve graft survival, close contact between the graft and the wound bed is essential for vessels to grow across the gap. Here, we introduce an easy and efficient dressing method to improve graft survival. Materials and Methods: A retrospective chart review was performed to identify patients who underwent split thickness skin graft and negative pressure wound therapy (NPWT) or conventional treatment between January 2007 and April 2015. Overall, 25 consecutive patients were included in the NPWT group and 49 were included in the conventional dressing group to compare the outcome of the procedure. The data were obtained from medical records, including age, sex, cause of the skin defect, size of graft, time for healing, wound preparation time, and complications. Results: Of the NPWT group, the average wound size was $147.04{\pm}146.74cm^2$ (range, $9{\sim}900cm^2$). With the exception of one patient, all wounds healed without the need for further procedure. The average duration of time required for the NPWT group, which was defined as removal of stitches (or staples) and no need for additional active dressing, was $6.4{\pm}1.97days$ (range, 5~15 days). The average time for the conventional dressing group was $10.78{\pm}2.38days$ (range, 5~15 days). Conclusion: NPWT can be used to cover regions in which wound healing does not occur fully or when neither tie-over nor compressive dressings are applicable. This treatment also reduced wound healing time and allowed earlier patient mobilization and hospital discharge.

웹 기반의 상처간호 전자교과서에 의한 자가학습의 학업성취도 평가 (Evaluation of Student Learning Achievement through Self Study Using a Web-based Wound Care E-book)

  • Purpose: The purpose of this study was to evaluate self-study learning achievement in undergraduate nursing students who used a web-based wound care e-book. Method: The web-based wound care e-book was applied to 80 nursing students at Y university. The students studied the wound care e-book for four weeks and practiced wound dressing by themselves in open laboratory. Learning achievement was evaluated according to achievement of unit objectives and performance of an actual wound dressing. Result: 1. The total mean score for achievement of unit objectives was 3.06 (${\pm}0.41$) and the total mean score on the performance of the wound dressing was 89.40 (${\pm}5.47$). 2. There was no difference between the scores in the performance test (F=1.012, p=.366) for students who used self-study and those who were given a lecture. 3. A positive correlation was found between achievement of unit objectives and performance of the wound dressing (r=0.306, p<0.05). Conclusion: The web-based wound care e-book was effective in facilitating self-study for nursing students, and there is a need to continuously develop and up-date web-based nursing education e-books to facilitate self-study.

Powdered Wound Dressing Materials Made from wild Silkworm Antheraea pernyi Silk Fibroin on Full-skin Thickness Burn Wounds on Rats

  • Purpose: This study evaluated powdered burn wound dressing materials from wild silkworm fibroin in an animal model. Methods: Fifteen rats were used in this experiment. Full-thickness $2{\times}2cm$ burn wounds were created on the back of rats under anesthesia. In the two experimental groups, the wounds were treated with two different dressing materials made from silkworm fibroin. In the Control Group, natural healing without any dressing material was set as control. The wound surface area was measured at five days, seven days and 14 days. Wound healing was evaluated by histologic analysis. Results: By gross observation, there were no infections or severe inflammations through 14 days post-injury. The differences among groups were statistically significant at seven days and 14 days, postoperatively (P<0.037 and 0.001, respectively). By post hoc test, the defect size was significantly smaller in experimental Group 1 compared with the Control Group and experimental Group 2 at seven days postoperatively (P=0.022 and 0.029, respectively). The difference between Group 1 and Group 2 was statistically significant at 14 days postoperatively (P<0.001). Group 1 and control also differed significantly (P=0.002). Group 1 showed a smaller residual scar than the Control Group and Group 2 at 14 days post-injury. Histologic analysis showed more re-epithelization in Groups 1 and 2 than in the Control Groups. Conclusion: Burn wound healing was accelerated with silk fibroin spun by wild silkworm Antheraea pernyi. There was no atypical inflammation with silk dressing materials. In conclusion, silk dressing materials can be used for treatment of burn wound.

Cost-Effectiveness Analysis in Comparing Alginate Silver Dressing with Silver Zinc Sulfadiazine Cream in the Treatment of Pressure Ulcers

  • Background The treatment of pressure ulcers is complicated, given the various wound dressing products available. The cost of different treatments varies and the cost-effectiveness of each product has not been thoroughly evaluated. We compare two wound dressing protocols-alginate silver dressing (AlSD) and silver zinc sulfadiazine cream (AgZnSD) with regard to wound healing and cost-effectiveness Methods Patients with grade III or IV sacral or trochanteric pressure ulcers were eligible for this prospective, randomized controlled trial. The patients were randomized to receive one of the two dressings for an eight-week period. The criteria of efficacy were based on the Pressure Ulcer Scale for Healing (PUSH) scoring tool. The cost of treatment was also assessed. Results Twenty patients (12 women and 8 men) were randomly assigned to receive either AlSD (n=10) or AgZnSD cream (n=10). The demographic data and wound characteristics were comparable in the two groups. The two groups showed no significant difference in the reduction of PUSH score, wound size, or volume of exudate. The tissue type score was significantly lower in the AlSD group ($3.15{\pm}0.68-1.85{\pm}0.68$ vs. $2.73{\pm}0.79-2.2{\pm}0.41$; P=0.015). The cost of treatment was significantly lower in the AlSD group (377.17 vs. 467.74 USD, respectively; P<0.0001). Conclusions Alginate silver dressing could be effectively used in the treatment of grade III and IV pressure ulcers. It can improve wound tissue characteristics and is cost-effective.

  • ${\beta}ig$-h3, is a TGF-${\beta}$-induced gene product, extracellular matrix protein with 68 kDa MW(683 amino acids) and has been known for its possible roles in cell adhesion, spreading, migration and proliferation. To minimize a proteolytic degradation of ${\beta}ig$-h3, ${\beta}ig$-h3 incorporated chitosan sponge was prepared and its effects on fibroblast adhesion and migration were investigated. And its wound healing efficacy was evaluated in deep 2nd degree burn rabbit ear wound model. ${\beta}ig$-h3 enhanced fibroblast adhesion and proliferation. In histological observation, a significant over-proliferation of epidermal regeneration was observed in ${\beta}ig$-h3/chitosan dressing applied wound while epidermal regeneration was not proceeded yet in chitosan only treated wound. ${\beta}ig$-h3/sponge dressing could enhance epidermal regeneration.

  • 창상 회복에 있어 적절한 산성의 환경은 세균의 증식을 막을 뿐만 아니라 세포의 활성도를 증가시키고 섬유아 세포의 증식을 촉진시키며 배열을 고르게 유지하여 결국 성숙된 상피화를 가져오게 된다. 본 연구에서는 펙틴과 CMC 의 다양한 조성으로 이루어진 드레싱제를 이용하여 펙틴과 CMC의 조성비와 이에 따른 pH의 변화가 신선창상, 감염 창상 및 화상에 있어 창상치유속도, 상피화 정도 및 콜라겐 침착 등에 미치는 영향과 이의 새로운 드레싱 재료로서의 가 능성을 알아보기 위해 동물실험을 시행하였다. 실험 결과로부터 pH 4.67인 펙틴/CMC=16/19 군이 급성창상의 창상 회복에 있어 창상의 수축률, 치유율 및 상피화율이 다른 군에 비해 높은 정도를 보여 가장 우수한 창상 치유 효과를 보임을 알 수 있었다.

Effect of Exogenous Collagen on Re-epithelialization of Skin Wound in Rabbits

  • This study was performed to assess the effects of exogenous collagen gel for the re-epithelialization of partial thickness skin wound healing in rabbits. Adult male rabbits (New Zealand White Rabbit) 1.5~2 kg, were used for experimental animals. Skin wounds (1.5$\times$2 cm length) were created bilaterally on the flank of 10 rabbits and then treated a periods of 9 days. Wounds on the experimental site were treated with exogenous collagen gel as well as fabric material gauze dressing. Control site wounds were covered with fabric material gauze dressing alone. Histological findings indicated that the epithelial migration of the experimental site of rabbits was far more rapid than that in the other control wound sites. Moreover, exogenous collagen gel provided a moist environment to keep wound clean, and facilitate keratinocyte proliferation. The wound dressed with exogenous collagen gel demonstrated a significant increase in the healing rate and re-epithelialization.

만성 창상의 치료에서 이동식 음압요법의 신고안 (The Management of Chronic Wound with Modified Portable Negative Pressure Therapy)

  • Purpose: V.A.C. is a new concept which is closed dressing with negative pressure to promote wound healing. It has been widely used as the treatment of chronic and acute wounds such as pressure sores, burns, stasis ulcers, and other complicated wounds. However It has disadvantages such as high cost and the need of specific equipment. In this article, we described new method to overcome these disadvantages. Methods: We made newly innovated equipment with 50 cc syringe and spring to create negative pressure. From May 2006 to May 2007, we applied it to two patients with chronic wound. Results: The treatment period was 5 weeks for one case and 3 weeks for the other case. Both patients were healed completely without admission and wound healing was accelerated. During follow-ups, there were no complications. The mean cost for single dressing was 9,590 won. Conclusion: Modified portable negative therapy using newly innovated equipment could accelerate wound healing better than conventional dressing. It lowers the number of dressings, saves cost of treatment, and enables treatment as outpatient basis.

알로인이 함유된 창상치료제용 PVA/CMC 하이드로젤의 개발 (Development of Aloin Loaded PVA/CMC Hydrogel for Wound Healing)

  • 알로인이 첨가된 하이드로젤을 제조하여 창상치유의 효과를 관찰하였다. Poly(vinyl alcohol)(PVA)과 carboxyl methyl cellulose(CMC)를 동결-융해 방법으로 결합시킨 하이드로젤을 제조하였다. 젤의 특성과 약물의 방출, in vivo 창상치유 실험 및 병리학적 평가를 진행하였다. 쥐 등에 만든 상처의 치유 실험은 알로인이 첨가된 PVA/CMC(5:5) 하이드로젤 그룹과 대조군 그룹의 알로인이 함유된 카보머934 젤 혹은 순수 카보머934 젤로 처치한 그룹보다 CMC의 수분과 알로인의 피토케미칼 활성 때문에 빠른 치유력을 확인할 수 있었다. 결론적으로 알로인이 첨가된 창상용 드레싱으로 5% PVA, 5% CMC 그리고 0.125% 알로인의 조성은 상처치료에 효과적이며 창상용 드레싱으로 활용을 기대할 수 있다.

  • 개의 전층피부 창상에서 삼출물의 정도와 창상수축, 상피화, 그리고 치유에 대해 hydrocollodi(Duoderm/$\circledR$, HC)과 hydrogel(Nu-Gel$\circledR$, HG) occlusive dressing materials 의 효과를 비교하였다. 3쌍의 정사각형 전층피부 창상을 2${\times}$2cm 의 크기로 12마리 개의 몸통 등외측 면에(6개의 창상/한 마리) 만들었다. HC와 HG, 생리 식염수(대조창)를 무작위로 배분한 2개의 창상에 각각 적용하였다. 창상의 육안적 소견과 병리조직학적 소견을 4주 동안 평가하였다. 창상유발 7일 후, 비교 그룹 간의 평균 창상수축 정도, 평균 상피화율, 평균 창상 치유율에 유의성있는 변화가 관찰되지 않았다. 창상 유발 14일(P<0.01)과 21일(P<0.05) 후, HG 처치창의 평균 상피화율이 HC 처치창의 평균 상피화율과 대조창의 상피화율보다 유의적으로 더 높게 나타났다. 창상 유발 21일 후(P<0.05), HG 처치창의 창상 수축율이 HC 처치창의 평균 창상 수축율과 대조창의 평균 창상 수축율보다 유의적으로 더 높게 나타났다. 창상 유발 21일 후(P<0.02), HG 처치창의 창상 치유율이 HC 처치창의 평균 창상 치유율과 대조창의 평균 창상 치유율보다 유의적으로 더 높게 나타났다. 창상 유발 1, 4, 7일 후, HC 처치창과 대조창의 피하에 다형핵 백혈구의 심한 침착이 관찰되었고, HG 처치창의 피하에 중등도의 다형핵 백혈구의 침착이 관찰되었으나, HG 처치창, HC 처치창 간의 어떠한 유의적인 변화도 발견되지 않았다. 창상 유발 14일 후, HG 처치창에서 상피성 세포가 유의적으로 표면 위로 자라났고, 부종은 창상 밑 조직까지 감소하였으며, 육아조직은 교원질 섬유로 대체되었다. 창상 유발 21일 후, 다른 창상에 비해, HG 처치창은 유의적으로 창상 표면이 거의 재생성 상피로 덮였으며, 육아조직은 창상 유발 14일 후와 비교해서 유의적으로 교원질 섬유로 대체되었다. 위의 모든 결과에서 보듯이, 개에서 전층피부 창상의 처치 시 HG의 사용은 HC와 생리 식염수에 비해 창상치유 복구기의 치유 속도를 촉진하는 것으로 사료된다.
