Background: This study aims to identify the monthly average medical expenses of public pension recipients, and analyze the determinants of total health and medical expenses and Western and Oriental medicine expenses, medical service expenses, and medical supplies expenses. Methods: This study used the fifth year data of 2013 out of the raw data of the Korean Retirement and Income Study collected by the National Pension Research Institute. This study conducted t-test, analysis of variance, and linear regression to verify publicly the relevance between pension recipients' general characteristics and health and medical expenses status using IBM SPSS ver. 21.0 for data analysis. Results: It was analyzed that there is a difference in the spending of expenditure and health care costs according to public pension recipients. Medical expenses of the national pensioners was higher compared to the special corporate pensioners. The national pensioner is related expenditure size, education level, family members living together, residential areas, status of spouse, number of chronic illness, and status of limitation in daily life with psychological health status. Conclusion: Therefore, fairness does not occur fire to the medical use between the special corporate pensioners and national pensioners, aggressive of government such as health policy and financial support for the retiree pension policy that reflects the reality intervention would be required.
Objectives : This study aimed to investigate the use and patterns of health care resources of local patients and to analyze the health seeking behaviors focusing on shopping around phenomenon. Methods : Questionnaires on the use of health institution were developed and administered to the residents in Yangsan city, Korea. We selected patients who were suffering from any diseases two months and over. Patients' health seeking behaviors were categorized, and the factors for selecting health institutions in each visiting stages were analyzed. Results : 139 local patients participated in the survey, and 130 completed questionnaires were analyzed. We found that there were sixty eight different types of medical utilization paths. The most frequent path was 'western clinics ${\rightarrow}$ Korean medicine hospitals or clinics ${\rightarrow}$ western clinics'. Only 6 patients started at Korean medicine, however, 16 patients(12.3%) finalized their treatment at Korean medicine hospitals or clinics. In most health seeking stages, patients tended to select health resources by their own decisions, and the geographic factor was the key reason to select health resources. Conclusions : Patients had not any stable forms to treat with their diseases, and we observed diversified patterns in the health seeking behaviors of patients, a typical shopping around phenomenon.

Objectives : Among young generation, the prevalence of spinal disorders is known to be increasing. This study aimed to analyze the clinical data of spinal disorder in young ages in Korea. Methods : Number of patient, kinds of disorder, and medical cost were analyzed for patients(7~18 years) underwent spinal disorders using computerized database of Health Insurance Review and Assessment Service(HIRAS) from 2004 to 2008. We included dorsopathies as spinal disorder according to Korean Classification of Diseases(KCD) and excluded spinal disorder caused by trauma. We compared the data of traditional Korean medicine treatment with that of western medical care. Results : 4.8% of the children and adolescents had medical treatment with spinal disorder in 2008. The claim number and medical cost for both traditional Korean medicine and western medicine treatment are increasing 1.8 and 1.3 times respectively over 5 years. Total medical spent of western clinic was 2.1 times than those of traditional Korean clinic. The most common spinal disorder was dorsalgia(31.8%) and scoliosis(13.5%) in western clinics while back pain(29.0%) and neck pain(10.8%) in oriental clinic. Conclusions : We first reported the clinical tendency of spinal disorder in Korean children adolescents from 2004 to 2008. This study will support the development of a strategy for traditional Korean medicine-based prevention or treatment of spinal disorders in young generation.

The facial palsy is one of the most common illness in Western-Korean collaborative treatment (hereinafter "collaborative treatment"). The purpose of this study is to analyze facial palsy patients'collaborative treatment use patterns. By analysing the 2014 National Health Insurance Review and Assessment Patient Sample Data (NPS 2014) with the episode of care unit, we have found the following results. First, the collaborative treatment is preferred by patients over 50 years old and female. Second, western medicine mainly focuses on diagnosis and medical examination while korean medicine and collaborative treatment focus more on treatment activity. Third, western medicine showed the shortest treatment period, followed by korean medicine and collaborative treatment. However, the cost of medical treatment per day is highest in western medicine. The analysis of the use patterns of collaborative treatment shown in the study is expected to provide a direction for the development of clinical practice guidelines and the establishment of medical policies in the future.

Chyle leaks are uncommon complications after head and neck surgeries. Although uncommon, such a complication is noteworthy mention due to its perplexing diagnosis and management strategies. This scoping review aims to highlight and emphasize the diagnosis and management options proposed in the literature. A comprehensive search was performed in PubMed, Google Scholar, Cochrane Library, and Scopus databases and identified 617 articles that were reduced to 40 studies and reports after applying the eligibility criteria. Although numerous treatment options ranging from simple, conservative measures to invasive surgical procedures have been mentioned for low-output, high-output, and massive leaks, there is no concrete evidence on the best method. Thus, a combination of management options must be customized by case for optimum results.
Objectives : There has not been the comparison research of medication on the Insomnia as the complementary significance of the Oriental-Western Cooperative Therapeutic Viewpoint so far. The aim of this preliminary research is to study Eastern and Western Medication Model for the Insomnia through the comparison of Oriental Medication based on "Sang Han Lon" and "Dong yui bo gam". Methods : The author researched oriental-western viewpoints and the present treatment condition of Sleep and Sleeping Pills with reference to Korean articles issued since 1975, dissertations, journals registered or candidated on KCI, alternative medicine journals on Pubmed, and books concerning Neuropsychiatry. In the field of Oriental medical study, this study was demonstrated on "The Oriental Medical textbook of neuropsychiatry", "Sang han lon", "Dong yui bo gam" and the present research trend. Results : In Oriental Medicine, Insomnia is treated according to si-jie-bu-shui-zung (思結不睡證) type, rong-xie-bu-zu-zung (榮血不足證) type, yin-her-nae-re-zung (陰虛內熱證) type, xin-dan-her-qie-zung (心膽虛怯證) type, dam-xian-yu-jie-zung (痰涎鬱結證) type, and wei-zhong-bu-he-zung (胃中不和證) type etc. In Western Medicine, Insomnia has been treated by Barbiturate over 60's and BZ over 70's. According to recent treatment on Insomnia, Zolpidem or Zaleplon, which is short-lived and has less side effects, is used much, and tri-cyclic antidepressants are prescribed on a chronic psycho-physiological insomnia. Conclusions : Generally, Insomnia is being treated by controlling whole ways. The oriental medication is less effective for sleep induction. On the contrary, that of western medication is very effective for sleep induction, but there are many possibilities of side effects. If this two mutual therapy is conducted cooperatively, more effective medical care might be expected. Therefore, it is considered that not only the oriental medical study for sleep induction, but the study of Oriental-Western Cooperative Therapy should be preceded to treat Insomnia from now on.

Objectives: To investigate the characteristics of patients diagnosed with dementia or mild cognitive impairment who are treated by means of a blend between Western and Korean medicine. Methods: We searched for outpatients with dementia or mild cognitive impairment by means of a collaboration between Western and Korean medicine from August 1, 2015, to July 31, 2017, through electronic medical records in Wonkwang Hospital. The records were retrospectively analyzed according to the patients' demographic and clinical characteristics, pathway of medical care, diagnostic tests, treatment, and medical expenses. Results: Thirteen patients were included in the analysis. Among them, six patients were diagnosed with mild cognitive impairment, five with dementia, Alzheimer's type, one patient with frontotemporal dementia, and one patient with unspecified dementia. Twelve of the thirteen patients were over 60 years of age. The number of pathways from the Dept. of Neurology to the Dept. of Neuropsychiatry of Korean Medicine was almost the same as the opposite pathway. The most used diagnostic test in Korean medicine was a neuropsychological test such as SNSB, MMSE and GDS. In Western medicine, hematology and neuroimaging were frequently used for patients. Acupuncture in Korean medicine and medication in Western medicine were the most frequently used. In Korean medicine, uncovered service costs were much higher than covered service costs,. whereas, in Western medicine, covered service costs were higher than uncovered service costs. Conclusions: This study describes the basic characteristics of dementia and mild cognitive impairment patients treated by a collaboration between Western and Korean medicine. Based on these results, a clinical pathway of the collaborative practice system between Western and Korean medicine for dementia patients needs to be developed.
The royal medical officer system of the Joseon after the Gabo Reform can be roughly divided into the period of the Taeuiwon, the Jeonuisa, the Naeuiwon, and the Sijongwon period. This study shows: 1. The status of the royal medical office was related to the status of the royal family. 2. After Jeonuisa, traditional royal offices of the Joseon Dynasty were not used. 3. 'Jeonui' became synonymous with bureaucrats in charge of royal medical care after the Taeuiwon period. 4. The Minister of Jeonui was the highest in medical bureaucracy since the Joseon Dynasty. 5. The imperial medical service included Western medicine doctors after the Sijongwon period.
Objectives : This study was aimed to investigate attitude of western medicine(WM), Korean medicine (KM), and nursing school students toward the east-west collaborative medical practices(EWCMP). Methods : The participants were 185 WM students, 123 KM students, and 230 nursing students belonging to two universities (P and D) in Busan metropolitan city, Korea. Data were collected with self-administered questionnaires and analyzed with descriptive statistics, $X^2$ test, t-test and ANOVA with SPSS win 14.0. Results : Of 538 participants, overall 87.1% has heard EWCMP. Preferred type of EWCMP was significantly different by participants' backgrounds. WM students preferred (western) medical treatment with minor supportive Korean medical care(85.5%). However, KM students emphasized EWCMP with the same weight in both medical and Korean medical treatment(59.0%), and nursing students were in between two schools. Intention to recommend EWCMP for the consumer was 67.4%, and also showed very different between WM students and others, 37.3% of WM students, 89.4% of KM students, and 83.9% of nursing students. WM students showed more negative opinion on the EWCMP than KM and nursing students, Conclusions : The attitude of WM, KM, and nursing school students toward EWCMP was very similar to that of WM doctors, KM doctors, and nurses, respectively. WM students showed big difference in the overall attitude toward EWCMP from that of KM and nursing students. It is recommended to introduce the joint curriculum or exchange programs between WM and KM schools.

There are very many factors to onset cancer, for example the exposure to varius kinds of pollution materials by development of industry, excess intake of harmful food to one's health, spritual stress, as become old age society, increase of attack-rate of geriatric diseases etc, attack-rate of cancer comes to be high gradually. The author had study about to treatment cancer by TCM(traditional chinese medicine) and combining TCM & western medicine, in Guang An Men hospital in Beijing of China from February 2003 to January 2004. TCM papers are 105 and combining TCM & western medicine papers are 280 of total 385 papers. Clinical reserch papers are 166, review articles are 194, animal experimental papers are 25. The papers used development herbal medicine are 42(10.9%), the papers used undevelopment herbal medicineare are 228(59.2%), the papers used senior TCM doctor's therapeutic experience and theory are 17(4.4%), the papers used theory of TCM or theory of combining TCM & western medicine are 71 (18.4%), the papers used acupuncture, qigong, thermotherapy, etc, other methods are 5, the papers used TCM patient's care or dietary treatment are 12 and ststistical papers are 10. Cancer is complicated hard case disease in pathological process. Combining TCM & western medicine treatment system is more efficient than one of TCM or western medicine in diagnosis, treatment, reserch of cancer.
