  • The versions of the Oswestry disability questionnaire (ODQ) is regarded as one of the most extensively used condition-specific instruments measuring disability resulting from low back pain. It has been shown to have adequate psychometrics, reliability, validity, and responsiveness as a whole, yet the summated total score of the instrument often provide little information to rehabilitation clinicians. A keyform analysis based on Rasch measurement model is an innovative way of illustrating the specific test items that an individual may or may not perform. By applying the keyform of the Rasch measurement model to the ODQ, rehabilitation clinicians may able to select more challenging ODQ items matching an individual's ability and document them as attainable treatment goals. The results demonstrated how a keyform analysis assist to setting possible goals for the treatment of individuals with low back pain. Forty-two individuals with low back pain were recruited from rehabilitation clinics in Gainesville, Florida. A series of Rasch analyses on the 10 items of the ODQ were performed using Winsteps software. The performance of two individuals on those 10 items was illustrated on the keyform. The keyform analysis of the Rasch measurement model may be translated into a useful tool for making clinical judgements.

Methods: Study participants included 58 pediatric physical therapists. They were recruited from the internet communities for physical therapists. The data was collected through a web-based questionnaire using Google Documents tool. Questionnaire was focused on the importance (10-point scale) of each factor influencing the motor development in children with cerebral palsy. The mean scale of each factor was compared by itself and according to the participants' career characteristics. Results: The personality characteristics received the least importance rating (6.77) in comparison with other domains (healthcare service: 7.70, family ecology: 7.53, secondary impairment: 7.53, primary impairment: 7.45). In the detailed items, home exercise (8.33) received the highest mean importance rating, while sociability (6.03) received the lowest mean importance rating. Comparing the importance of factors by the type of work institution, subjects working in the welfare institutions perceived the contextual factors (personality characteristics, family ecology, healthcare service) as more important than those working in the medical institutions. Conclusion: The authorsuggests that pediatric physical therapists should try to have an identical approach by reaching a consensus. Also, the support for family-centered service to children with cerebral palsy should be strengthened.

  • Purpose: The purpose of this study was to compare the change in pain, Neck Disability Index score, and the craniovertebral angle by performing scapular correction exercise or general neck exercise for five weeks in participants with mechanical neck pain. Methods: A total of 31 participants were randomly assigned between the scapular correction exercise and the general neck exercise groups, and all participants performed intervention for 40 minutes each, three times a week for five weeks The effects were evaluated by measuring the Visual Analog Scale score, the Neck Disability Index score, and the craniovertebral angle, before and after the intervention. Independent t-tests were used to compare differences between two groups, and to compare differences between pre- and post-intervention, paired t-tests were used. Results: As measured before and after the intervention, the scapular correction exercise group showed significant improvement in all variables (p<0.05), while the general neck exercise group improved only in the neck disability index score. The differences between the two groups revealed further improvement in the scapular correction exercise group compared to the general neck exercise group (p<0.05). Conclusion: We found that five weeks of the scapular correction exercise to modify the position and movements of the scapula is clinically an important treatment tool for recovery from chronic mechanical neck pain symptoms and restoration of proper neck function.

  • Background: Policy network theory was proved to be an appropriate analytic tool for the current social welfare policy making process. This study aimed to analyze policy making process related to the nurse expansion and policy output while focusing on the interactions and activities among various policy actors. Methods: In this study, we used reports related to the need for expansion of nursing personnel journals, dissertations, newspaper articles, for hearings and debate policies for securing nurse data, and interviews. We examined three components of policy network, that is, policy actors, interactions, linkage of interest. Results: For that to expand the nurse before the 2000s in expanding the supply of medical supplies have been conducted without much disagreement among policy actors under the government's initiative. However, there was lacked a close relationship between the expanding supply of nurses and inaccurate analysis of supply and demand. As the policy is applied between the various policy actors' needs and claims, conflict was intensified and many policy options had been developed. Government only took a role as a coordinator among policy actors in the 2000's. Also, it was difficult to find sufficient and clear evidence that policy-making process based on fair judgment. Conclusion: Therefore, it is urgently required to determine the policy through a social consensus to address the appropriate policy means and the process by correct analysis of the policy issues.
