A Study on Urine Analyzer Using Color Images of Strip (스트립의 컬러영상을 이용한 요 분석기에 관한 연구)

  • This paper presents an urine analyzer using color images of strip. An image sensor has been used to acquire an color images. The analyzer has a characteristics which takes a pictures of independent each pad responding to urine items all at once, and outputs the measured urine values by analysing an acquired pad colors. This device measures and analyses 11 urine items, and has a speed-up operation, light weight, small size, and makes accurate observations by processing the measured colors using processor. The urine analyzer transmits the urine items to peripheral devices, personal computers, hospitals, and another places by using a telegraph and a wireless interfaces.

  • Color correction methods of chromaticity coordinates using Color Matching Function (CMF) were studied to develop a device-independent portable-type urine analyzer. The reflection spectra were measured for the degrees of 10 test items of the urine reagent strip (urine strip) to develop a portable-type urine analyzer. A computer simulation was performed to quantitatively distinguish the color reactions of the urine system, by using the spectral power distribution of Light Emitting Diode(LED), the reflection of a urine strip, and spectral sensitivity of a photodiode. To develop a device-independent system, chromaticity coordinates were modified to reduce the color deviations in the urine strip, by using the temperature compensation of LED and the color transformation by CMF. The experimental values obtained by developed urine system exhibited the accuracy above 95% for all color samples.

Comparison of WBCs and RBCs Concordance between the Cobas u 701 Analyzer and Three Manual Microscopy Methods in Urine Sediment (소변 침전물에서 Cobas u 701 Analyzer와 세 가지 수동 현미경검사법의 백혈구와 적혈구의 일치도 비교)

  • Urine sediments are performed by a microscopic examination of centrifuged urine by medical technologists. This study examined different urine sediment preparation procedures. The 107 fresh urine specimens that tested positive from white blood cells (WBCs) and red blood cells (RBCs) in the urine dipstick test and the cobas u 701 analyzer, respectively, were selected for manual microscopy. This study evaluated an automated urine sediment analyzer and three manual microscopy methods for WBCs and RBCs. The methods were performed according to the test guidelines. The coefficients of determination between the cobas u 701 analyzer and the Korean Association of Quality Assurance for Clinical Laboratory (KAQACL) for WBCs and RBCs were r2=0.977 and r2=0.970, respectively. The concordance rates between the cobas u 701 analyzer and KAQACL for WBCs and RBCs were 74.8% and 77.6%, respectively. A good correlation and concordance with the automatic analyzer were shown when the specimens were prepared and examined using the KAQACL method. Consequently, the differences in the urine sediment preparation procedures affected the sediment concentrations, influencing the cell number per high power field (HPF).

  • Background: Quantitation of urine protein is important in dogs with chronic kidney disease. Various analyzers are used to measure urine protein-to-creatinine ratios (UPCR). Objectives: This study aimed to compare the UPCR obtained by three types of analyzers (automated wet chemistry analyzer, in-house dry chemistry analyzer, and dipstick reading device) and investigate whether the differences could affect clinical decision process. Methods: Urine samples were collected from 115 dogs. UPCR values were obtained using three analyzers. Bland-Altman and Passing Bablok tests were used to analyze agreement between the UPCR values. Urine samples were classified as normal or proteinuria based on the UPCR values obtained by each analyzer and concordance in the classification evaluated with Cohen's kappa coefficient. Results: Passing and Bablok regression showed that there were proportional as well as constant difference between UPCR values obtained by a dipstick reading device and those obtained by the other analyzers. The concordance in the classification of proteinuria was very high (κ = 0.82) between the automated wet chemistry analyzer and in-house dry chemistry analyzer, while the dipstick reading device showed moderate concordance with the automated wet chemistry analyzer (κ = 0.52) and in-house dry chemistry analyzer (κ = 0.53). Conclusions: Although the urine dipstick test is simple and a widely used point-of-care test, our results indicate that UPCR values obtained by the dipstick test are not appropriate for clinical use. Inter-instrumental variability may affect clinical decision process based on UPCR values and should be emphasized in veterinary practice.

  • In this paper, we analyzed the spectroscopic properties of strip to analyzer urine qualitatively & quantitatively and make urine analyzer system stable by spectroscopy, and research the property of preamplifier unit. The analysis of spectroscopic properties of 10 pads in the strip is used for determine the wave length of light source of optic detector unit and used for basic materials which are necessary that we develop the algorithm analyzing the density grade of pad accurately. We make preamplifier unit by using the current to frequency method to measure the distribution of pad color. We implemented urine analyzer system. This system's hardware is composed of measuring unit for detect of distribution density of strip pad, main processing unit, communication unit, interface device, thermal printer, and indicator. The software consists of the program which manage the argument of test, proportion initial value of urine analyzer and calibrate analyzed result.

  • A computer simulation was performed to distinguish quantitatively a color reaction in a urine analysis systems by using the spectral power distribution of LEDs, the spectral reflectance of a urine strip, and the spectral sensitivity of photodiode. The CIE tristimulus values and CIE chromaticity coordinates ware modified to be conformable with real color reactions in a urine strips. Results on color simulation showed a of real color in comparison with those obtained by Colorimeter CM2C(Color Savvy).

  • This study was conducted to develop a electronic circuit of primary color reaction for urine analyzer for measuring color response of urine strip. A primary color reaction system is equipped with the computer, electronic circuit, tray, detecting assembly and software. The determination of coefficient($R^2$) between reagent and color sensor were 0.9801(R), 0.9868(G) and 0.9837(B). To evaluate the system verification, we measured the primary color reaction of erythrocytes, Bilirubin, Urobilinogen, Ketones and Protein. We concluded that it is possible to use the developed the primary color reaction system for urine analyzer using u-health.

Selection of Unnecessary Urine Culture Specimens Using Sysmex UF-5000 Urine Flow Cytometer (Sysmex UF-5000 소변 유세포분석기를 이용한 요배양 불필요 검체의 선별)

  • Background: Urine culture is one of the most frequently requested tests in microbiology. Automated urine analyzers yield much infection-related information. The Sysmex UF-5000 analyzer (Sysmex, Japan) is a new flow cytometry urine analyzer capable of quantifying urinary particles, including bacteria, WBCs, and yeast-like cells (YLCs) and can provide a Gram stainability flag. In this work, we evaluated how many unnecessary urine cultures could be screened out using the UF-5000. Methods: We compared the culture results of 126 urine samples among 453 requested urine cultures (from sources other than the Urology and Nephrology departments) with urinalysis results. Urine cultures were considered positive if bacterial or YLC growth was ${\geq}10^4CFUs/mL$. Results: We used urinalysis cut-off values of $50/{\mu}L$ and $100/{\mu}L$ for bacteria and YLC, respectively. Forty eight of the 126 (38.1%, or 10.6% of 453 requested) cultures were below these cut-off values and did not contain any culture-positive samples. Conclusion: Bacteria and YLC counts generated using the UF-5000 analyzer could be used to screen out negative cultures and reduce urine culture volume by ~10% without sacrificing detection of positive cultures.

Development of a multi-channel clinical chemistry analyzer (반사광을 이용한 다채널 임상화학분석기개발)

  • In this paper we report the device of a multi-channel clinical instrument developed for determi¬nation of the levels of the urinary urobilinogen, glucose, ketone, bilirubin, protein, ascorbic acid, nitrite, pH, occult-blood, specific gravity, and leukocytes semiquantitatively. The test parameters are expressed on the dry test strips as a range of color intensities by chemical reactions. The instrument measures the value of each substance by reading the reflectance light emanated from the test strips. We also designed the reagent strip cassette and loader in order to intercept the outside interference. The loader can be operated semi-automatically. The light source is consisted on light emitting diodes at three specific wavelengths (560 nm, 610 nm, 650 nm). Precision of the system was evaluated by testing a series of commercial control urine samples. Furthermore, the performance of the instrument was compared with two other test methods on the urine samples from 100 persons. Our results showed a good repeatability between tests and a satisfactory agreement between the readings by our instrument and visual evaluation.
