ICT 기술이 발전함에 따라, 다양한 크기와 형태의 웨어러블 디바이스가 개발되어 지고 있다. 또한, 성능이나 스펙은 현재의 스마트폰과 연결 가능하도록 IOT 융합 제품으로 재탄생 되고 있다. 이것은 4차 산업혁명의 범용 기술 중 하나로, 우리의 삶의 질과 환경을 바꾸는 기술로 집중 조명을 받고 있다. 이와 함께 헬스케어 기술이 결합된 신기술 제품이 증가함에 따라, 이를 필요로 하는 사용자들에게 다양한 기능들이 제공되고 있다. 웨어러블 기술은 기술개발과 함께 지속적인 증가 추세에 있다. 또한 이를 스마트 와치 형태의 제품으로 개발하여 제품을 출시하고 있다. 또한 소형화를 위해 마이크로프로세서의 집적화 기술을 적용한 오픈소스 하드웨어들이 개발되고 있다. 그중 아두이노 관련 제품은 현재 다양하게 만들어져 있으며 용도에 따라 아두이노 프로세서를 개발 툴에 맞추어 사용하면 된다. 이에 본 연구에서는 오픈소스 하드웨어 기반의 타이니두이노를 활용하여 웨어러블 신체 생체 활동 모니터링 시스템을 개발하였다. 타이니두이노는 Atmega 프로세스를 기반으로 제작된 초소형 아두이노 호환 보드로 오픈소스 통합 개발 환경(스케치)에서 프로그램이 가능하다. 웨어러블 신체 생체 활동 모니터링 시스템은 일상적인 건강관리를 할 수 있는 스마트 u-Healthcare 시스템이라는 것이 큰 장점이라 할 수 있겠다.
The Microsoft Kinect is a motion sensing input device which is widely used for many motion recognition applications such as fitness, sports, and rehabilitation. Until now, most of remote rehabilitation systems with the Microsoft Kinect have allowed the user or patient to do rehabilitation or fitness by following the motion of a video screen. However in this paper we propose a smart remote rehabilitation system with the Microsoft Kinect motion sensor and a wearable ECG sensor which can allow patients to offer monitoring of the individual's performance and personalized feedback on rehabilitation exercises. The proposed noble smart remote rehabilitation is able to monitor and measure the state of the patient's condition during rehabilitation exercise, and transmits it to the prescriber. This system can give feedback to a prescriber, a doctor and a patient for improving and recovering motor performance. Thus, the efficient rehabilitation training service can be provided to patient in response to changes of patient's condition during exercise.
Development of portable device measuring the vital sign continuously with no limit of time and space is absolutely prerequisite for the U-health care that grafts the ubiquitous concept into medical system. Accordingly, it requires to develop a garment style apparatus for measuring vital-sign that is easy to wear on for a long time period. This study suggests a method to improve the insulation of electric cable and the skin adhesion of electrode by integrating the electric conductive material to garment, in order to develop a garment apparatus for measuring ECG for U-health care. Results of the research are as follows; In order to provide the adjacent conductive yarns with insulation, braid with narrow woven end was interlaced using polyester yarn. As a result, the direct contact between electric conductive yarns was restrained, which would be interposed into pin-tuck structured cable. Washable silicone gel applied around the electrode made of electric conductive fabric improved the adhesion, which prevents electrodes from dropping off from the skin surface during body movement. ECG signals on the human subject were tested using the garment apparatus developed by the above method. And the result was that the clear QRS wave formation in the typical form of ECG could be measured in both conditions of still and moving state as well. The result of this study is expected to contribute for the production of U-health care related medical apparatus by accelerating the practical uses of the garment measuring vital sign at a reasonable price.
U-Wellness Healthcare System(U-WHS)이란, 웰빙(wellbeing)과 피트니스(fitness)를 결합한 원격 건강 관리 시스템을 말한다. 이러한 시스템에서는 시간과 공간에 제약 없이 언제 어디서나 환자의 생체정보를 측정 및 관리 할 수 있는 것이다. 본 논문에서는 스마트모바일기기와 HIS(Hospital Information System)간 생체정보 전송시 암호화 모듈이 통신 평가에 끼치는 영향을 알아보기 위해 수행하였다. U-WHS 모델의 경우 클라이언트는 iOS Xcode환경의 Objective-c 개발 언어를 이용하여 SEED, HIGHT 암호화 모듈 적용을 하였다. HIS의 경우 클라이언트와 서버간 통신을 위하여 HTML5의 WebSocket API와 관계형 데이터베이스 관리 시스템인 MySQL를 적용하였다. 그리하여 WIFI 통신 환경에서 Wireshark를 사용, 분석하여 생체 정보의 데이터 전송율, 지연율, 손실율에 대한 평가를 확인하였다.
To test if the developed ubiquitous health care devices working well and vital information could be collected and monitored systematically through internet and to test if the devices and services could be used further. Kyungwon University, KT Co., Gil Medical Center, LIG Nex1 Co., and Sujeong Health Center conducted an ubiquitous health care demonstration project in Sujeong-Gu, Sungnam, Korea from Mar. 5 to May 16. We developed and applied several medical devices to monitor health of the elderly in their houses through internet. The devices were sphygmomanometer, glucometer, body fat scale, Health Pad, and activity sensor. We distributed the devices to 20 recipients of home care and 7 diabetes patients. After received the devices and were explained how to use them, they used the devices in their houses. The vital signs of the residents were monitored through internet. A nurse monitored and consulted their vital signs in the monitoring center in Kyungwon University during the demonstration period. The consultant called them and consulted on their blood pressure, blood sugar level, and body fat after a few seconds they used the devices as well as provision of recommended contents such as diets and activities through Health Pad. To investigate cognition and satisfaction of the participants for the devices, we surveyed the participants at the end of the demonstration period. For the change in blood pressure, blood sugar level, and activities, we conducted statistical test. After the demonstration period. cognition and satisfaction for the devices and change in blood pressure, blood sugar level, and activities were evaluated. Most of the participants were acknowledged how to use the device and satisfied with the use of the devices. The internet monitoring and services are considered to be promising because most of the participants were satisfied especially because somebody was monitoring their health status. However some weaknesses such as short battery life of the activity sensor, lack of connection of consultations with hospitals, and low understanding on usage of some of the devices need to be complemented.
The U-healthcare is increasingly important as there is a growing tendency for elderly people to acquire chronic disease such as heart problems and high blood pressure. In this paper, we implemented an oximeter which can interact and communicate with the smart devices such as smartphones through which users can manage their measuring data. The designed pulse oximeter is capable of detecting hemoglobin saturation levels in the range of 70-100% and the heart beat rate(BPM) in the range of 30-240 BPM.
Optical coherence tomography (OCT) is being developed to guide various ophthalmic surgical procedures. However, the high cost of the intraoperative OCT system limits its availability mostly to the largest hospitals and healthcare systems. In this paper, we present a design and evaluation of a low-cost intraoperative common-path free-hand scanning OCT system. The lensed fiber imaging probe is designed and fabricated for intraocular use and the free-hand scanning algorithm that could operate at a low scanning speed was developed. Since the system operates at low frequencies, the cost of the overall system is significantly lower than other commercial intraoperative OCT systems. The assembled system is characterized and shows that it meets the design specifications. The handheld OCT imaging probe is tested on multilayer tape phantom and ex-vivo porcine eyes. The results show that the system could be used as an intraoperative intraocular OCT imaging device.
개인 휴대단말기로부터 측정된 생체정보 데이터가 PHR에 데이터베이스화되기 위해서는 응용 계층 표준 프로토콜인 HL7 CDA 표준에 따라야 한다. 하지만 국제 표준 HL7 CDA는 병원 시스템 간 의료정보 전달을 위한 프로토콜이기 때문에 개인 건강 기기로부터 측정된 생체 정보가 IEEE 11073 PHD 표준에 준하여 전달하더라도 HL7 CDA 프로토콜과 연동될 수 없다. 즉, 모바일 단말기 기반의 u-헬스케어 모니터링 서비스를 위해서는 IEEE 11073 PHD와 HL7 CDA 간 메시지 변환 게이트웨이가 필요하다. 이에 본 논문에서는 개인 휴대단말기를 통해 원격지에 있는 의료진이 사용자의 생체데이터를 실시간 관리하기 위해 ISO/IEEE 11073과 HL7 CDA 간 메시지 변환 게이트웨이 및 HL7 기반 진단지원시스템을 설계하였다.
인구의 고령화로 만성질환자가 증가함에 따라 개인건강관리를 위한 U 헬스 서비스에 관심이 증대되고 있다. U 헬스 서비스를 활용하기 위해서는 다양한 생체정보를 간편하게 측정하는 개인건강기기(PHD, Personal Health Device)가 필요하다. 하지만 혈압, 심전도, 혈당, 산소포화도 등을 측정하는 개인건강기기는 제작 회사마다 서로 다른 프로토콜과 포맷으로 개발되어 기기간의 상호 운용성(Interoperability)이 보장되지 않는다. 또한 각기 다른 포맷으로 측정된 데이터를 병원정보시스템(HIS, Hospital Information System)간 공유하고 관리하는데 어려움이 존재한다. 이에 따라, 본 논문에서는 개인건강기기 국제 표준인 IEEE 11073 PHD 를 기반으로 다양한 개인건강기기와 실시간으로 통신하며, 측정된 생체정보를 관리하고 전송하는 모바일 헬스케어 매니저 설계를 제시한다. 제시한 매니저는 다양한 모바일 기기 중 스마트폰 기반으로 설계하였고, 각기 다른 포맷으로 측정된 데이터를 국제표준 메시지 교환방식인 HL7 (Health Level Seven)메시지로 변환하여 전달하는 게이트웨이 역할을 담당한다.
1. Objectives Our goal is to observe the feasibility of constitution discrimination from computing quantitative roughness index from dynamic friction coefficients and their gradients with the measurement device of skin friction with 3-Axis load cell sensor. 2. Methods In the traditional Korean medicine, skin diagnosis is one of the examination methods to discriminate Sasang constitution since it was known that Tae-eumin has rough skin, and Soyangin has smooth one. It is based on the skin roughness on the back of one's hand for the discrimination. The measurement device of skin friction with 3-axis load cell sensor has been developed in order to provide quantitative skin roughness through dynamic friction coefficients. The effective interval of the coefficients is obtained from the automatic sampling algorithm to use their curvature and slope. Then, Fisher's discriminant function of them makes the discrimination. 3. Results The success rate of extracting the effective interval was about 90% and the discriminant accuracy between Tae-eumin and Soyangin was 70% and 68% for men and women, respectively. The entire methods showed the possibility to distinguish between Tae-eumin and Soyangin by using stochastic properties of roughness index, which can make the entire system to include the measurement, the computation of the roughness index and the discrimination of constitution automatical. 4. Conclusions The measurement device, the automatic sampling algorithm of dynamic friction coefficients and the constitution discrimination algorithm were developed, respectively, and their combination can become the serial and automatic procedure for quantitative and objective skin diagnosis, which mimics the movement of the Oriental medical doctors' skin diagnosis. It can be applied to healthcare as well as the diagnosis of constitution in a u-Health system soon.
