Linoleic Acid from Bamboo (Phyllostachys Bambusoides) Displaying Potent α- Glucosidase Inhibition (대나무로부터 분리한 linoleic acid의 α-glucosidase 저해활성 연구)

  • Glycosidase inhibitors are major targets in the treatment of type II diabetes, cancer and viral infections. This study was carried out to investigate the glycosidase inhibitory substances from bamboo (Phyllostachys bambusoides). Bamboo was extracted with methanol and then further fractionated with n-hexane, chloroform, n-BuOH and aqueous to get an active fraction. All extracts were evaluated for ${\alpha}$-glucosidase inhibitory activities to identify the n-hexane fraction with 33.5 ${\mu}$g/ml of IC50 value. Active compound 1 in the n-hexane fraction was identified as linoleic acid, which exhibited inhibitory activity with 12.4 ${\mu}$M of IC50 value. Mechanistic analysis showed that linoleic acid exhibited noncompective inhibition. This is the first study in which bamboo is reported to show ${\alpha}$-glucosidase inhibitory activity.

Effects of high glucose concentration on IGF-I binding and glucose transporters in renal proximal tubule cells (신장 근위세뇨관세포에서 고포도당이 IGF-I 결합과 포도당운반계에 미치는 영향)

  • Diabetes mellitus is associated with a wide range of pathophysiological in the kidney. This study was designed to examine the effects of high glucose concentration on IGF-I binding and glucose transporters in renal proximal tubule cells. The results were as follows : The binding of $^{125}I-IGF-I$ reached the peak at the 30 minutes and gradually decreased by the time dependent manner. The binding of $^{125}I-IGF-I$ was inhibited by the unlabelled IGF-I($10^{-14}{\sim}10^{-8}M$) in a concentration dependent manner. The relative affinity of IGF-I receptor for IGF-I, IGF-II and insulin exhibited typical type 1 binding(IGF-I > insulin > IGF-II). However IGF-II did not compete for the cultured cell membrane $^{125}I-IGF-I$ binding site at $10^{-14}{\sim}10^{-8}M$. Under optimal conditions, IGF-I binding to the membranes from 5mM and 20mM glucose treated cells was analyzed. It was found that 20mM glucose treated cells exhibited higher binding activity for IGF-I. In order to further substantiate this increase in IGF-I binding sites, we performed affinity-labelling studies. The cross-linked cell membrane subjected to SDS-PAGE; labelled material was detected by autoradiography. 20mM glucose treated cells exhibited higher levels. The initial rate of $methyl-{\alpha}-D-glucopyranoside({\alpha}-MG)$ uptake was significantly lower($74.41{\pm}6.71%$) in monolayers treated with 20mM glucose than those of 5mM glucose. However, 3-O-methyl-D-glucose(3-O-MG) uptake was not affected by glucose concentration in culture media. IGF-I significantly increased ${\alpha}-MG$ uptake in both 5mM and 20mM glucose treated cells. However, 3-O-MG uptake was not affected by IGF-I in both conditions. In conclusion, 20mM glucose increased binding sites of $^{125}I-IGF-I$, inhibited Na/glucose cotransporter activity. But 20mM glucose did not change facilitated glucose transporter.

  • Purpose: Whole-body PET using radiopharmaceutical is one of the imaging study methods for physiological changes of body. High specificity of the PET-CT examination is used to detect an early stages of cancer and metastatic cancer by imaging a physiological changes. During the imaging process, PET image has been characterized by a relatively low image quality due to its low sensitivity and the acquisition of random and scatter coincidences as well as patients figure. Therefore, the image quality as the changes of the acquisition times of patient weight was evaluated in this study. Materials and Methods: Thirty patients who presented to our hospital were enrolled. They were divided to normal, overweight, and obese group using BMI index, respectively. The patients with a liver disease and diabetes were excluded. $^{18}F-FDG$ was administered to the patients as 5.2 MBq per kg. After an hour from an injection, image acquisition was obtained as List mode in a part of liver in 1 bed. SNR (signal-to-noise ratio) of each groups acquisition times were confirmed from the calculated radiation counts and random fractions. The statistical significance of three groups was confirmed through one-way ANOVA test. On the basis of the counts of 2 minutes on normal group, the SNR of overweight group and obese group were compared. Results: The SNR were increased with loger aquisition time in 3 groups. In the condition of same acquisition time, the SNR had a statistical significance (P<0.05). The SNR were decreased to the normal, overweight, and obese, respectively. Liver activity had no significance difference on each group and RF had the significance differences (P<0.05). On the basis of the counts of 2 minutes on normal group, there were no statistical significance in a three minute acquisitions of overweight group and two minute acquisitions of obese group (P=0.150). Conclusion: In this study, the administrated amount of radiation dose did not adjust as the change of the patients weight. Increasing the acquisition time when the administration of the same amount of dose was able to get a good result of SNR. When the Based 2 minute on normal group, if overweight and obese case the increased acquisition time of 3 minute was able to obtain a similar SNR. On the basis of the normal group, the acquisition times of overweight and obese group were increased to 3 minutes per bed and the SNR were similar to the normal group.

The Risk Factors of Recurrent Ischemic Stroke (허혈성 뇌졸중의 재발과 연관된 위험인자)

  • To eveluate the risk factors which are related to recurrence of ischemic stroke, we selected subjects who were admitted to YNUH due to recurrent stroke and compared their risk factors with non-recurred group who suffered from single ischemic stroke. In the subjects, 55 of them are men and 22 were women and in the non-recurred groups, 84 of them were men and 40 were women, Subject's age ranged from 29 to 85 years(Mean 62, 5years), and non-recurred group's age ranged from 27 to 90 years(Mean 60, 7years), Peak incidence of ischemic stroke is in the 7th decade in both groups. Age and sex are not, statistically significant for recurrence of ischemic stroke. The patient's history of diabetes mellitus, myocardial infarction, atrial fibrillation, transient attack and type or site of ischemic stroke had no significant effect statistically on the recurrent ischemic stroke. However, when the patient had previous history of hypertension or systolic blood pressure more than 160mmHg and diastolic more than 95mmHg, there was substantial difference(P<0.05) between the two group in the recurrence of ischemic stroke. According to the above results, hypertension is most likely significant risk factor of the recurrence of ischemic stroke within 2years after initial one. Therefore, adequate treatment of the hypertension is important for the prevention of ischemic stroke. Further study is required for searching other risk factors.

Review of the Properties of the Laser and the Spectrum of Laser Instruments for Diabetic Ulcer (당뇨병성 궤양에 사용되는 레이저의 특성에 대한 연구)

  • Objectives : One of major complications of diabetes, diabetic ulcer is also one of the main reasons for amputation, and the prevalence rate is 4-10%. Laser therapy is widely used for leg ulcer and diabetic ulcer, and it is known to improve wound epithelialization, cellular content, and collagen deposition. The purpose of this study is to investigate the properties of the laser and the spectrum of laser instruments for diabetic ulcer. Methods : We performed literature search using the PubMed, Cochrane, CINAHL and Web of science for the data in English. In addition, other databases were checked for different languages such as OASIS and NDSL for the literature in Korean, CNKI in Chinese, and CiNii and J-STAGE written in Japanese. We excluded all review article and experimental studies, and only clinical studies using laser or light emitting diode (LED) for diabetic ulcer were selected. Results : A total twenty papers were selected. Different light sources were used as follows: LED, HeNe, InGaAlP, GaAlAs, GaAs, CO2, and KTP. The number of LED studies was 9, and HeNe laser was 7, and InGaAlP and GaAlAs laser was 2, GaAs, CO2, and KTP laser was 1 for each. Various energy density of the clinical study were reported. Conclusions : It is suggested that to select appropriate laser type and give the adequate output power to treat diabetic ulcer. Further evaluation and research for the condition of laser therapy to treat diabetic ulcers are warranted.

  • The Pancreatic islet are the clusters of endocrine cells scattered through out the exocrine pancreas. Transplantation of a sufficient pancreatic islets can normalize blood glucose level so that may prevent devastating complications of type I diabetes(IDDM) and other side effects of the IDDM. Recently, there are several approaches to transplant sufficient pancreatic islet, and it was comprised in increase or regeneration of the endogenous $\beta$-cell mass from donor's pancreas, but relatively few studies have been devoted to the morphological characters of the isolated and 3 day cultured pancreatic islets. We investigated morphological pattern of intracellular structure of isolated and 3 day cultured pancreatic islets. The morphological characters of the pancreatic islets were observed by scanning electron microscope and transmission electron microscope, and insulin distribution of the each islets were observed by transmission electron microscope, and were labeled with insulin antibody. Intracellular structures including nuclei, mitochondria, RER, Golgi complex and many secretory granules were normally appeared in the isolated pancreatic islets which was extracted immediately dornor's pancreas, however, There is a significant morphological changes between the 3 day cultured pancreatic islets and isolated islets. 3 day cultured pancreatic islet's $\beta$-cells had normal nuclei but increased cytoplasm mass and RER and developed Golgi complex. Insulin secretory granules were decreased in numbers rather than isolated pancreatic islet. In this study, the pattern of intracellular structure variation was examined during pancreatic islet culture. Most distinct features are variation of the insulin secretory granules, and developed RER, and dilated golgi complex. Therefore, we suggested that the various change of the morphological characters on cultured pancreatic islets were responsible for the function(biosynthesis and secretion of insulin) and growth. These results were also cultured islets have greater ability to recover and maintain normoglycemia than isolated islet transplantation.

Salvage of Infected Breast Implants

  • Background Implant-based breast reconstruction is being performed more frequently, and implants are associated with an increased risk of infection. We reviewed the clinical features of cases of implant infection and investigated the risk factors for breast device salvage failure. Methods We retrospectively analyzed 771 patients who underwent implant-based breast reconstruction between January 2010 and December 2016. Age, body mass index, chemotherapy history, radiation exposure, and smoking history were assessed as potential risk factors for postoperative infection. We also evaluated the presence and onset of infection symptoms, wound culture pathogens, and other complications, including seroma, hematoma, and mastectomy skin necrosis. Additionally, we examined the mastectomy type, the use of acellular dermal matrix, the presence of an underlying disease such as hypertension or diabetes, and axillary node dissection. Results The total infection rate was 4.99% (58 of 1,163 cases) and the total salvage rate was 58.6% (34 of 58). The postoperative duration to closed suction drain removal was significantly different between the cellulitis and implant removal groups. Staphylococcus aureus infection was most frequently found, with methicillin resistance in 37.5% of the cases of explantation. Explantation after infection was performed more often in patients who had undergone 2-stage expander/implant reconstruction than in those who had undergone direct-to-implant reconstruction. Conclusions Preventing infection is essential in implant-based breast reconstruction. The high salvage rate argues against early implant removal. However, when infection is due to methicillin-resistant S. aureus and the patient's clinical symptoms do not improve, surgeons should consider implant removal.

Effect of 12 Weeks Combined Exercise and Nutrition Education on Body Composition, Liver Function, Serum Lipids and Insulin Resistance in Obese Middle-aged Woman (12주간 복합운동과 영양교육이 비만중년여성의 신체조성, 간기능, 혈중지질 및 인슐린 저항성에 미치는 영향)

  • This study investigated the effect of 12 weeks combined exercise and nutrition education program on body composition, liver function, serum lipids and insulin resistance in obese middle-aged women. The combined exercise program was performed for 55 minutes 3 times per week during which the intensity progressively increased from RPE 13 to 14, 60-70%HRR (1~5 weeks), RPE 15 to 17, 70-80% HRR (6~12 weeks) and nutrition education was conducted 20 minutes one day per week. The subjects were 21 obese middle-aged women were composed of the nutrition education group (A, n=7), combined exercise group (B, n=7), and the control group (C, n=7). The results of the study in the nutrition education group and combined exercise group were as follows; The A and B group had significantly lower body composition than did the control group. The A group had significantly lower liver function, TC, TG, HDL-C, and HOMA-IR than did the control group. Consequently, combined exercise and nutrition education program effective in preventing diabetes caused by obesity in obese middle-aged woman. On the basis of the results of this study, the following recommendations are made for further studies: additional studies are needed to confirm the results of this study and to compare the effects of combined exercise on these variables in obese and non-obese men and women by age or exercise type; they are also needed to develop effective nutrition education programs for different age groups or body types.

Changes in the Functionality of Cheonggukjang During Fermentation Supplemented with Angelica gigas, Rehmanniae Radix, and Red ginseng (당귀.지황.홍삼 첨가에 따른 발효 청국장의 기능성 변화 연구)

  • Cheonggukjang is one of the traditional fermented soy-based foods in Korean diets. Studies in cell cultures, humans have revealed anti-hypertension, anti-stress, anticancer, antioxidant, immune enhancing effects. Angelica gigas, Rehmanniae radix, and Red ginseng are popular medicinal plants and widely used for oriental medicine. In this study a strategy had been developed to mobilize beneficial phenolics from Angelica gigas, Rehmanniae radix, and Red ginseng combined with fermented soy by Cheonggukjang fermentation for antioxidant and Type II diabetes management. The quality and functional characteristics of Chenggukjang fermented with Angelica gigas, Rehmanniae radix and Red ginseng. Cheonggukjang (CKJ), Angelica gigas Cheonggukjang (CKJ-DD), Rehmanniae radix Cheonggukjang (CKJ-RG), Angelica gigas and Rehmanniae radix Cheonggukjang (CKJ-DD+RG) and Red ginseng Cheonggukjang (CKJ-RED) were evaluated. The mobilized phenolic profile was evaluated for antioxidant activity and the potential to inhibit ${\alpha}$-amylase linked to hyperglycaemia. This research has important implications for the development of functional soy-based-fermented foods enriched with Angelica gigas, Rehmanniae radix and Red ginseng phenolics for oxidative stress - induced diabetic complications. Furthermore, Hunter's color values of 5 types cheonggukjang, lightness (L-values), redness (a-values) and yellowness (b-values) were evaluated. Free amino acid content of CKJ-RED (0.993 mg/gd. w.) showed higher than that of CKJ (0.205 mg/g-d.w.).

Mitochondrial Ca2+ Uptake Relieves Palmitate-Induced Cytosolic Ca2+ Overload in MIN6 Cells

  • Saturated fatty acids contribute to β-cell dysfunction in the onset of type 2 diabetes mellitus. Cellular responses to lipotoxicity include oxidative stress, endoplasmic reticulum (ER) stress, and blockage of autophagy. Palmitate induces ER Ca2+ depletion followed by notable store-operated Ca2+ entry. Subsequent elevation of cytosolic Ca2+ can activate undesirable signaling pathways culminating in cell death. Mitochondrial Ca2+ uniporter (MCU) is the major route for Ca2+ uptake into the matrix and couples metabolism with insulin secretion. However, it has been unclear whether mitochondrial Ca2+ uptake plays a protective role or contributes to lipotoxicity. Here, we observed palmitate upregulated MCU protein expression in a mouse clonal β-cell, MIN6, under normal glucose, but not high glucose medium. Palmitate elevated baseline cytosolic Ca2+ concentration ([Ca2+]i) and reduced depolarization-triggered Ca2+ influx likely due to the inactivation of voltage-gated Ca2+ channels (VGCCs). Targeted reduction of MCU expression using RNA interference abolished mitochondrial superoxide production but exacerbated palmitate-induced [Ca2+]i overload. Consequently, MCU knockdown aggravated blockage of autophagic degradation. In contrast, co-treatment with verapamil, a VGCC inhibitor, prevented palmitate-induced basal [Ca2+]i elevation and defective [Ca2+]i transients. Extracellular Ca2+ chelation as well as VGCC inhibitors effectively rescued autophagy defects and cytotoxicity. These observations suggest enhanced mitochondrial Ca2+ uptake via MCU upregulation is a mechanism by which pancreatic β-cells are able to alleviate cytosolic Ca2+ overload and its detrimental consequences.
