Effect of Korean Medicine Treatments for Pain Reduction in Patients with Hip Fracture : A Retrospective Observational Study (고관절을 이루는 골격의 골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of this study was to observe the effectiveness of Korean Medicine treatments on pain reduction of patients who were diagnosed hip fracture - Acetabulum and Proximal Femur. We reviewed the 17 patients' records who had admitted to Daejeon Korean Medical hospital and diagnosed fracture of acetabulum or proximal femur bones from January 1, 2011 to April 30, 2020 during the admission retrospectively. Statistical analysis was done using the IBM SPSS statistics 25 program. We measured Numeric Rating Scale to evaluate the treatment's efficacy. Korean Medicine treatments reduced median of Numeric Rating Scale reduced statistically significantly from 6.00 to 3.00(p<0.05). During the admission, patients didn't experience the side effects of Korean Medicine treatments. In conclusion, these results implied that Korean medicine treatments showed a positive effect on pain reduction of hip fracture patients. Further research is required to confirm the effectiveness of Korean Medicine treatments.

Research Trends of Korean Medicine Treatments for Postoperative Sleep Improvement: Focusing on Herbal Medicine, Acupuncture, and Moxibustion (수술 후 환자의 수면 개선에 대한 한의치료 임상연구 동향 -한약, 침, 뜸 치료를 중심으로-)

  • Objectives: To review trends of clinical trials on Korean medicine treatments for postoperative sleep improvement. Methods: We searched randomized controlled trials (RCTs) on Korean medicine treatments for postoperative sleep improvement from ten domestic and foreign databases. Sample sizes, diseases, types of operation, diagnosis tools, pattern identification, interventions, outcome measurements, and main results of included studies were extracted and analyzed. Results: A total of 20 RCTs were selected. Most studies were published in China. The most common target disease was cancer, followed by cardiovascular disease. Most studies lacked detailed description regarding participants such as onset, duration of sleep disturbance, and preoperative sleep issues. Herbal medicine was the most frequently used in 12 studies. The most commonly used prescription was Suanzaoren decoction. The effectiveness of Korean medicine treatment on improving postoperative sleep was found to be significant in most studies. Conclusions: Korean medicine treatments might be effective in postoperative sleep improvement. However, the quality of included studies was low. Therefore, further well-designed research studies are needed to provide high quality clinical evidence on Korean medicine treatments for postoperative sleep improvement.

A Study on Satisfaction level with Herbal Public Health Services and its Improvement Plans (한방공중보건서비스 만족도와 개선방안)

  • Objective : In order to investigate and improve public Korean medical health service satisfaction level, this study was designed. Method : A questionnaire has been conducted on 212 patients who received treatments at six public health centers in the northern part of Gyeongbuk during 15 days between Sep. 24 and Oct. 8, 2011. Result : 1. An Investigation on the usage of herbal clinics in public health centers reveals that 63.7% have received three times or more medical treatments previously and 61.8% have had their illness treated at other medical institutions. In regard to illness 32.1% have had arthritis or muscle aches. 50.9% have taken insurance medication after having had treatments at the public health centers. 66% have assessed acupuncture and moxa cautery the most satisfying. 2. To a question regarding whether herbal health treatment costs higher than that of physician's, the highest response at 31.6% is 'No'. And to a question regarding whether herbal medicines administered at public health centers have more side effects than that of physician's, the highest response at 39.6% is 'No'. 3. To a question regarding whether herbal treatment of public health centers has little effect against acute disease, 48.1% of responses are 'Fair'. To a question regarding whether herbal treatments, when compared with physician's treatments, boost better recovery of patients, 48.1% of responses are 'Fair'. To a question regarding whether herbal medicine is unscientific, when compared with that of western medicine, 38.2% of responses are 'Fair', To a question regarding whether herbal medicine has faster effect on disease than western medicine, 41.0% of responses are 'Fair'. To a question regarding whether herbal medicine is more effective on disease prevention and promotion of health than disease treatment, 38.2% of responses are 'Fair'. And to a question regarding whether the lack of various types of physical therapy devices in herbal medicine, when compared with western medicine causes inconvenience in herbal treatment, 42.0% of responses are 'Fair'. Those responses take up highest portion at each questionnaire. 4. A comparative study between herbal treatments and physician's treatments has also been conducted. To questions regarding which one of the two considering types of disease is the better, responses are the latter accounted for 43.9% against 'Cancer', the latter accounted for 45.3% against 'Endocrine disorders', the former accounted for 30.7% against 'Psychiatric disorders', the latter accounted for 38.2% gainst 'Otolaryngological(ENT) disease', the former accounted for 47.6% against 'Post traumatic stress disorder', and the former accounted for 52.4% against 'Muscle-skeletal disease'. 5. An investigation on frequency of patients' visits via (p<0.05) of subjects show a statistically significant difference. 6. First, an investigation on frequency of reasons of medical treatments reveal that age, occupation, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of subjects taking insurance medicines after herbal health treatments reveal that monthly income (p<0.05) of subject shows a statistically significant difference. 7. First, an investigation on frequency of a claim that herbal treatments of public health center does not have great effect on acute disease reveals that age, education, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of analysis that herbal treatments has faster effect on disease compared with western treatments reveals that education level, religion, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. 8. When herbal clinics of public health centers and general herbal medicine institutions are compared, a survey on additional treatments that herbal clinics need the most reveals that education level, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of subjects who want various forms of herbal medicines reveals that occupation and insurance type (p<0.05) of subjects show a statistically significant difference. Conclusion : In order to improve efficiency of treatments and enhance patient's satisfaction level, this study suggests measures such as providing a differentiated acupuncture treatments as a whole, streamlining an reception procedure, adopting more elaborated computer system for a patient to get proper medical attention, standardizing a treatment duration in order for a maximum result, keeping regular office hours, and optimizing a consultation time for a patient.

Comparison of Competition Indices by Silvicultural Systems Before and After Treatments for Natural Deciduous Forests in Pyeongchang (평창지역 천연 활엽수림의 산림작업별 시업전후의 경쟁지수 비교)

  • The objective of this study was to estimate the changes of stand structure before and after applying the three different silvicultural treatments such as selection cutting system, two-storied system, and shelterwood system. This study has been conducted in the natural deciduous forests in Pyeongchang of Gangwon Province, Korea. Nine permanent sampling plots of 0.09 ha were established in the forests and each of the three silvicultural treatments was applied to three sampling points. Some tree variables were measured in each stand before and after the silvicultural treatments were applied. With these data, stand attributes were estimated in each stand before and after applying the silvicultural treatments. In this study, a competition index was used to analyze the differences among structures of stands managed by three different silvicultural treatments. Hegyi's distance-dependent competition index was estimated and compared to analyze the differences of stand structures among the stands before and after silvicultural treatments. A method using a height angle $50^{\circ}$ from the base of the subject tree was adopted as the selection method of competitor trees. Duncan's multiple range test and t-test were then employed to statistically analyze the difference of stand structure among the stands. The results revealed that competition status among trees in the stand seems to be improved after applying the silvicultural treatments. There are significant differences in the competition index between before and after silvicultural treatments for each stand. According to the evaluation of competition index, it was confirmed that spatial structure of the stands was improved by applying the silvicultural treatments.

Effects of Dietary $\omega$-Fatty Acids on Fatty Acids Composition and Storage Characteristics of Meats from Spent Hens (오메가 계열 지방산 급여가 산란노폐계육의 지방산 조성 및 저장성에 미치는 영향)

  • This study was conducted to investigate the influence of dietary $\omega$-fatty acids on fatty acids composition and storage characteristics in meat samples of spent hens. Spent hens were randomly assigned to one of the four dietary treatments : 1) Control (commercial feed), 2) T1 (commercial feed supplemented with 10% olive oil, 3) T2 (commercial feed with 10% canola oil), and 4) T3 (commercial feed with 10% sardine oil). They were fed one of the experimental diets for three weeks and slaughtered. The meat samples were stored at 4 ${\pm}$ 1 ˚C The storage characteristics and fatty acid contents were analyzed for meat samples stored over a period of 1, 3, 5, 7 and 9 days. The pH of all treatments significantly increased during the storage periods(P<0.05). The per-oxide values (POV) of all treatments were significantly increased up to 5 days ; after that the POV decreased significantly (P<0.05). The POV of treated groups were significantly higher than those of the Control in the thigh meat(P<0.05). The TBARS showed the highest POV among all treatments. Although the breast meat tended to he lower in POV than the thigh meat, no significant difference was detected between the two meats. The TBARS(thiobarbituric acid reactive substances) of all treatments were significantly increased as the storage period extended(P<0.05). After 3 days, the TB ARS of oil-treated groups were aignificantly higher than that of the Control (P<0.05). The T3 showed the highest TBARS among all treatments (P<0.05). The TBARS of the breast meat was lower than that of the thigh, but no statistical difference was found be- tween them. The unsaturated fatty acid content of the breast and thigh meats in all treatments were slightly decreased as the storage period extended. The oleic acid was higher in Ti, and the linoleic acid and linolenic acid were higher in T2 than the other treatments. The eicosapentaenoic acid and docosahexaenoic acid were higher in T3 than the other treatments. The unsaturated fatty acid contents of the breast meat were slightly lower than those of the thigh meat. The n-6 fatty acid contents of the breast and thigh meats were slightly increased as the storage periods ex-tended. The n-3 fatty acid content of T3 was the highest among all treatments. The n-6 fatty acid content of breast meat was lower than that of thigh meat. The n-6 fatty acid content of the breast meat was slightly lower than that of the thigh meat.

Assessment of Stand Diversity Change by Different Silvicultural Treatments for Natural Deciduous Forests in Mt. Gariwang (가리왕산 일대 천연 활엽수림의 산림작업별 시업전후의 임분다양성 변화 평가)

According to the evaluation of stand diversity indices, it was confirmed that spatial structure of the stands was improved by applying the silvicultural treatments.

Field Performance Evaluation of Micro-surfacing Method and Polymer Slurry Seal Method Used in National Highway (일반국도에 적용한 마이크로서페이싱공법과 폴리머슬러리실공법에 대한 현장 공용성 평가)

  • PURPOSES : Recently, crack, rutting, and stripping problems from the surface of asphalt pavements in National highway are observed and they affect the drivers to feel uncomfortable on the road. Surface treatments are recommended to use in distressed pavements due to cost-effective, and improvement of surface performance. The purpose of this study is to evaluate the performance of micro-surfacing and polymer slurry seal treatments for distressed asphalt pavements. METHODS : Surface conditions and friction resistance are evaluated for asphalt pavements treated with micro-surfacing and polymer slurry seal mixes in National highway 30 line and 34 line. Visual observation is conducted and surface performance is measured by PES (Performance Evaluation Surveyor) in terms of crack ratio, rutting and IRI(International Roughness Index). BPN(British Pendulum Number) is measured by BPT(British Pendulum Tester) to evaluate the friction resistance in the field. RESULTS : The surface evaluation results are presented for asphalt pavement treated with micro-surfacing and polymer slurry seal treatments in National highway 30 line and 34 line. Based on the visual observation, micro-surfacing and polymer slurry seal treatments show better improvements in terms of cracks and stripping. Based on the surface conditions measured by PES vehicle, the surface performance of micro-surfacing treatments improves from 53.3% to 54.2% and the surface performance of polymer slurry seal treatments improves from 21.6% to 59.7%. However, the friction resistance of both micro-surfacing and polymer slurry seal treatments decreases from 2.5% to 6.7%. Further, it should be verified to produce the surface exposed with aggregates during the construction process of both treatment methods in the field. CONCLUSIONS : Based on the performance evaluation results in the filed, the surface performance of asphalt pavement treated with micro-surfacing and polymer slurry seal treatments improves from 21.6% to 59.7%. While, the friction resistance of asphalt pavement treated with micro-surfacing and polymer slurry seal treatments does not improve. It can be concluded that current micro-surfacing and polymer slurry seal treatments would improve surface performance but would not improve the friction resistance.

Effect of Scavenging and Protein Supplement on the Feed Intake and Performance of Improved Pullets and Laying Hens in Northern Vietnam

  • Two feeding trials were conducted with 128 pullets from 4 to 20 weeks of age and 96 laying hens from 23 to 63 weeks of age to evaluate the effects of scavenging and type of protein supplement on the feed intake and performance of improved pullets and laying hens. The experiments had a completely randomized design with four dietary treatments and four replicates. Treatments were: Control (Cont), scavenging but with access to a balanced concentrate at night; confinement (CF) and given the control feed ad libitum; scavenging and supplemented at night with the control feed, but with soybean meal replaced by cassava leaf meal (CLM); scavenging and supplemented at night with the control feed, but with fishmeal replaced y soybean meal (SBM). The mean daily dry matter (DMI), metabolizable energy (MEI) and crude protein intakes (CPI) of the pullets and laying hens, respectively, were 28%, and 18% higher for the confinement treatment (CF) compared to the scavenging treatments (p<0.001). The DMI, MEI and CPI of the pullets were not significantly different among scavenging treatments (p>0.05), but for the layers DMI, MEI and CPI were significantly higher for the CLM and SBM treatments compared to the Cont treatment (p<0.001). In the growing period, the average daily weight gain (ADG), supplement feed conversion ratio (FCR) and supplement feed cost/kg eggs (FCS) were not significantly different for CF compared to Cont, and among scavenging treatments (p>0.05). In the laying period, the hen-day production was significantly lower, and supplement FCR and FCS significantly higher for the CF compared to the scavenging treatments (p<0.001). Egg weight, and yolk, albumen and shell percentage and shape index were not significantly different among the scavenging treatments (p>0.05). However, shell and yolk percentages were significantly lower for the CF compared to the Cont treatment (p<0.01). Mortality was significantly higher for the CF compared to the scavenging treatments for pullets, and was significantly lower for the CF compared to scavenging treatments for laying hens (p<0.001). It was concluded that scavenging pullets and layers were getting around 28% and 18%, respectively, of their nutrient requirements from scavenging activities, resulting in correspondingly lower supplement feed conversion ratios and feed costs. Daily gains of the pullets were not affected by scavenging or protein supplement, but egg production and mortality were lower for the confined hens.

The Domestic Trends of Korean Medicine Treatments on Traffic Accident for Recent 5 Years (최근 5년간 교통사고 후유증의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives : The purpose of this study is to investigate the latest trends of korean Medicine treatments on traffic accident for recent 5 years. Methods : We investigated the studies on korean Medicine treatments for traffic accident via searching 6 Korean web databases. As a result, 65 research papers were found to be analyzed according to their published year, the titles of journals, the types of study, the number of cases, the sites of injury, the types of treatments the instruments for assessment and ethics approvals. Results : 65 articles were published since 2008. The studies on korean Medicine treatments about traffic accident were mainly published in Journal of Oriental Rehabilitation Medicine. 10 case studies, 14 case series, 11 non randomized controlled trials, 21 randomized controlled trials and 9 systemic reviews had been under research. The most frequently occurred symptoms were neck pain and low back pain. In most studies, various korean Medicine treatments were used to treat the symptoms. Visual analogue scale(VAS) and neck disability index(NDI) were used as primary assessments. In most clinical studies, there were no mention about approval of institutional review board(IRB). Conclusion : In this study, we analyzed the trends of korean Medicine treatments on traffic accident from 2008 to 2012. Reviewing the domestic trends of studies on korean Medicine treatments for traffic accident and examining the strong and weak points of those treatments are essential for the future studies. It is needed to adopt the qualitative methods of evaluation in order to develop a level of evidence.

Effects of Supplemental Lactobacillus on Broiler Performance (유산균의 급여가 육계의 성장능력에 미치는 영향)

  • This study was conducted to investigate the effects of feeding various Lactobacillus on performance, nutrients digestibility, intestinal microflora, and fecal noxious gas of broiler chicks. One thousand eighty one dat old male broiler chicks were fed into Lactobacillus crispatus avibrd 1(LCB), Lactobacillus reuteri avibro 2(LRB), Lactobacillus crispatus avihen 1(LCH), and Lactobacillus vaginalis avehen 2(LVH) at the level of $10^{4}$ and 10(sup)7cfu/g diet. There were four replicates with thirty chicks per pen. Diets contained ME 3,100, 3,100kcal/kg, and CP 22.0, 20.0% for starting and finishing period, respectively. Weight gain of checks fed Lactobacillus spp. tended to increase from the first week and was higher from 50 to 100g in Lactobacillus treatments than control. Feed intake and feed conversion were not statistically different of all treatments. Dry Matter(DM) digestibility of Lactobacillus treatments was tended to improve compared to that of control, but was not significantly different. Protein and Ca digestibility were also tended to improve in Lactobacillus treatments relative that of control. Lactobacillus treatments showed improved tendency in crude ash and fat compared to those of control, whereas phosphorus digestibility was not consistency. Nutrients digestibilities of bird fed LCH were superior to those of other treatments. It showed significantly higher in Ca and P digestibility than control(P〈0.05). Total Lactobacillus spp. of birds fed various lactobacillus was significantly higher in ileum for five weeks(P〈0.05), but was not different at cecum. Yeast was thought to be not completely attached to intestinal lumen for one week. However, total number of yeast was significantly increased in cecum and ileum from three weeks old chicks(P〈0.05). The number of anaerobes exhibited to tendency the increase in Lantobacillus treatments from one week old of age at both ileum and cecum. Moisture contents of birds fed Lactobacillus spp. was shown from 27 to 30% at five weeks old, whereas it was 38.2% in control. It decreased approximately 25% in Lactobacillus treatments relative to that of control and showed significance between them(P〈0.05). Fecal $NH_{3}$ gas emission was significantly decreased in Lactobacillus treatments and maintained $\frac{1}{3}$to $\frac{1}{2}$ of control(P〈0.05). LCH and LVH treatments were most effective to decrease fecal $NH_{3}$ gas and especiallt impreved ti significantly at the level of the level of 10(sup)7cfu/g diets covpared to that of $10^{4}$ cfu/g diets and control(P〈0.05). In conclusion, feeding Lactobacillus spp. increase performance and nutrients digestibilities of broiler chicks and improve environments of house. Proper supplemental level of Lactobacillus spp. is considered more than $10^{4}$ cfu/g diet.
