Thoracolumbar Spine Injury (흉요추부 손상)

  • Method of management of the spine injury should be determined, based on the status of neurological injury as well as on the presence of traumatic instability. At the thoracic and lumbar spine, patterns of neurological injury are different from the cervical spine due to their neuro-anatomical characteristics. Especially, at the thoracolumbar junction, neurological injury patterns with their respective prognosis vary from the complete cord injury or conus medullaris syndrome to the cauda equina syndrome according to the injury level. The concept of Holdsworth's instability based on the posterior ligament complex theory has evolved into the current 3-column theory of Denis. Flexion-rotation injury and fracture-dislocation are well known to be unstable that surgical fixation is frequently needed for these injuries. However, there have been some controversies for the stability of burst fractures and their treatment, such as indirect or direct decompression and anterior or posterior approach. In this article, current concepts and management of traumatic instabilities at the thoracic and lumbar spine have been reviewed and summarized.

Bioaccumulation Patterns and Ecophysiological Responses of Monochoria korsakowi Exposed to Cadmium

  • We have examined the bioaccumulation patterns and the ecophysiological responses (photosynthetic pigment and total antioxidative capacity) of Monochoria korsakowi exposed to various cadmium concentrations, one of major environmental pollutants. Cadmium ion contents in M. korsakowi increased significantly with higher cadmium concentration, and most of the accumulated cadmium was found in the root parts. Biomass of each part decreased with higher cadmium concentration. As cadmium treatment concentration was increased, chlorophyll a content was decreased, whereas chlorophyll b content was increased. However, the variations of total chlorophyll and carotenoid contents were not evident. Total antioxidative capacity in the leaves of cadmium treated M. korsakowi increased greatly with higher cadmium concentration. We considered these results as indicative of the ability of M. Korsakowi plants to take up cadmium from wetlands.

Utilization Patterns of National Health Insurance and Medical Aid Inpatients in Tertiary Hospitals (건강보험환자와 의료급여환자의 상급종합병원 입원이용 비교)

  • The objective of this study is to analyze the utilization patterns of national health insurance and medical aid inpatients in tertiary hospitals. For the analysis, the study utilize the nationwide data on '2010 Survey of Patients' of Ministry of Health & Welfare. The statistical methodology used in the study is the logistic regression model. This study has three major findings. First, utilization rate of national health insurance inpatients was higher than medical aid inpatients in tertiary hospitals. Second, the significant affecting demographic factors in utilizing tertiary hospitals were sex, age, surgery case, treatment result, inpatients residence region and short length of stay. Third, compared to other disease groups, the inpatients on 'congenital malformation, deformity and chromosomal abnormalities', 'factors influencing health status and contact with health services' and 'neoplasm' groups are more likely to utilize tertiary hospitals. Finally, according to residence region, Seoul residence inpatients in both of national health insurance and medical aid more likely to utilize tertiary hospitals than other region inpatients.

The Relationships Between Obese Index and Major Risk Factors in Patients with Cardiovascular Disease (심혈관계 질환 환자의 비만도와 주요 위험인자와의 관계)

  • This study was performed to investigate the relationships between obese index and major risk factors of cardiovascular disease in CVD patients. Anthropometry, food intake patterns, blood pressure, serum lipids and hematological values were analyzed in 550 patients from June, 1991 to August, 1992. The results are as follows: Total food intakes, especially total vegetable food and alcohol intakes were significantly related with obese index, so that the heavier patients tended to consume more carbohydrate and less fat. The blood pressure were observed to have positive correlation with obese index in man but not in women. However, both in men and women, blood levels of TG, TC, and VLDL tended to be related to obese index positively, while HDL and HDL-C were shown to be oppositive. So, it seems that as the degree of obesity is higher the blood pressure and blood lipids patterns worse. Therefore, these results suggest that obesity is one of the major risk factors of CVD, and for prevention & treatment of the disease, controlling body weight by nutritionally adequate diet along with alcohol restriction is necessary.

Various injury patterns due to combustion (typical but unfamiliar to physicians and easy to miss) in Korea: a case report

  • Patients transported from fire sites may exhibit various injury patterns. Major trauma, skin burn, inhalation burn, and carbon monoxide poisoning are typical injuries. However, most physicians may be unfamiliar that cyanide poisoning can frequently occur due to combustion. Cyanide poisoning is highly significant owing to high mortality and the existence of antidotes. I present a 35-year-old man who was transported from a burning building and suffered severe metabolic acidosis despite no major bleeding as well as mild carbon monoxide poisoning. I suspected cyanide poisoning and administered the antidote; subsequently, the patient showed improvement. The next day, sudden airway obstruction developed, and emergency endotracheal intubation was performed. The inhalation damage was detected only in the lower airway tract and not in the upper airway. Physicians should be aware of cyanide poisoning and inhalation burn to avoid missing treatment opportunities.

  • Facial esthetics is one of the most important goal of the orthodontic treatment and main concern of many patients. Facial esthetics should be considered in orthodontic diagnosis and treatment planning. Prediction of soft tissue profile changes after orthodontic tooth movement should be considered as well. The purpose of this study was to find out the effect of orthodontic treatment on lip profile in adult patient. The pre and post treatment cephalometric roentgenograms of 87 female adult with bimaxillary protrusion were used to analyze lip profile change. All subjects were treated with four bicuspids extraction. Obtained results were as follows . 1. Lip thickness changes after incisor retraction showed different patterns according to areas of the lip. The thickness of the red lip area showed 2.78 mm increase in average. In contrast the thickness of the cutaneous area showed 0.65 - 0.7 mm decrease according to the different cutaneous areas. 2. The length of the red lip area decreased(1.3mm) after incisor retraction. 3. The length of the cutaneous lip area increased(2.9mm) after incisor retraction.

The Correlation between Ammonia Emissions and Bedding Materials in a Cow House

  • Because ammonia from livestock production may substantially contribute to environmental pollution, emissions from all possible sources (housing systems, manure storage, manure application, outside grazing) should be reduced. The purpose of this study was to investigate the effect of different bedding materials on ammonia emissions in a cow house. By applying a combination of four treatment types: treatment $1-T_1$ (sawdust (50%)+sawdust pellets (50%)), treatment $2-T_2$ (sawdust (50%)+corn stalk pellets (50%)), treatment $3-T_3$ (sawdust (100%)), and treatment $4-T_4$ (sawdust (50%)+palm kernel meal pellets(50%)) as bedding materials in a cow house, the effects of such treatments on ammonia flux were assessed in approximately one month. The magnitude of ammonia emissions (mg $m^{-2}\;min^{-1}$) varied in the following order: $T_1$(2.226), $T_4$(2.052), $T_2$(1.845), and $T_3$(1.712). The patterns of pH had a decreasing trend for all bedding treatments during the experiment, and there was no significant relationship with ammonia flux. The results reveal that the most important factor influencing ammonia emissions is the physical structure of the bedding types.

Evaluation of monthly environmental loads from municipal wastewater treatment plants operation using life cycle assessment

  • Life cycle assessment (LCA) methodology can be used to assess impacts on the environment that might be generated during treatment of wastewater and sludge treatment. In this work, LCA methodology was suggested to evaluate monthly environmental impact of wastewater treatment plants (WWTPs). Two field scale WWTPs, A2/O process and conventional activated sludge process (CAS), were selected as target plants and the operational data were collected from those plants. As the function units, the unit volume of treated wastewater of $1m^3$ and 1 kg T-N eq. removed were selected. The environmental effect of target WWTPs operation were assessed as impact categories such as global warming potential, eutrophication potential, and so on. From monthly profiles of each index, it was shown that the environmental impact of WWTPs has seasonal patterns influenced by the influent flow rate variation causing higher impacts in winter than summer. This is due to the fact that there were no significant increase in the electricity consumption and chemical usage during the summer while the treated volume of wastewater was increased.

Quality Changes of Cucumber Kimchi Prepared with Different Minor Ingredients during Fermentation (부재료 첨가량을 달리한 오이 김치의 저장 기간에 따른 품질 변화)

  • This study was carried out to investigate the properties of cucumber kimchi prepared with different minor ingredients(potato, puchu). Acidity, pH, color value, hardness, and lactic acid bacteria were measured under the condition of $10^{\circ}C$ for 25 days. Five conditions of making cucumber kimchi included: cucumber kimchi with puchu 300g(treatment 1, control), cucumber kimchi with potato 90g, puchu 210 g(treatment 2, 5-1), cucumber kimchi with potato 150g, puchu 150 g(treatment 3, S-2), cucumber kimchi with potato 210g, puchu 90 g(treatment 4, S-3), cucumber kimchi with potato 270g, puchu 30g(treatment 5, S-4). Hardness of cucumber kimchi appeared higher values as the potatoe's volume increased(S-1, S-2, S-3, S-4), during all fermentation days. The results showed very significant values in pH(p<0.001), acidity(p<0.001), 'L' of lightness(p<0.05), hardness(p<0.001), lactic acid bacteria(p<0.001) according to fermentation. And the results showed very significant values in 'a' of redness(p<0.01), hardness(p<0.001) according to cucumber kimchi samples. These results showed that fermentation patterns of cucumber kimchi were influenced by the different minor ingredients used.

A Clinical Analysis of Out-Patient with Skin Disease (한방안이비인후피부과에 내원한 피부질환 환자의 임상적 고찰)

  • Objective : This study was performed to examine the patterns of skin disease in patients that had visited Dept. of Dermatology Hospital of Oriental Medicine, Dongguk University. We also compared on disease, age and treatment. Methods : We analysed statistic study in 505 patients, who had visited to our hospital from July, 2005 to February, 2007. Results : The results were as follows; 1. Distribution of dermatology classification was 40.6%, males were 41 % and females were 59%. 2. Distribution of aging was 18.2% in 0-9 years old and 18.8% in 10-19 years old and 17.4% in 20-29 years old. 3. Common disease group were Atopic dermatitis (24.55%); Acne (10.89%); Eczema (10.89%); Urticaria (10.30%). 4. Distribution of treatment according to diagnosis were external preparation in atopic dermatitis, acne, eczema, dermatitis seborrheica ; herbal-medication in urticaria ; herbal-medication and external preparation in facial redness. 5. Distribution of treatment according to aging were external preparation in all group except 50-59 years old; acupuncture treatment in 20-29 years old; herbal-medication in 10-19 years old; consultation in 70-79 years old. Conclusion : This study suggests that oriental medical treatments is popular in dermatologic diseases, especially atopic dermatitis and acne. We have to take steps for effective management and treatment for special diseases and ages.
