  • The Literatural Study on the classification of cause and the effect of Acupuncture and Moxibustion treatment for Youk Jeol Poung was studied from the viewpoint of therapeutic acupuncture and moxibustion effect. And the results were as follows: 1. Youk Jeol Poung is similar to Tong Bi, Ju Bi, Haeng Bi, Tong Poung. 2. The symptoms of Youk Jeol Poung are pain and weakness, difficulty of flexion, swelling and pain, severe pain etc., at night pain is more severe. 3. The cause, mechanism of Youk Jeol Poung is as follow, due to penetration of the wind, cold, moisture, uder situation of whole body is pain. 4. The main treatment is invigorate vital energy and blood, expel wind - evil, promote diuresis, eliminating phlegm, promote blood circulation, cold, heat - clearing. 5. The basal meridian of acupuncture and moxibustion treatment were the channels of Su Jok Sam Yang.

Job Performance of Advanced Practice Nurses, Perceived Difficulty and Importance, and Willingness to Legally Delegate Clinical Practices to Advanced Practice Nurses by Health Care Professionals (전문간호사의 업무 수행 빈도 및 의료인이 인식하는 업무 난이도와 중요도, 법제화 시 업무 위임 의향)

  • Purpose: To identify the clinical role of Advanced Practice Nurses (APN), and evaluate how other medical personnel perceive their work(difficulty, importance). Methods: A questionnaire survey was performed with 277 health care providers (APN 52, nurses 88, and medical doctors 137 [professors 51, fellows 44, & residents 42]) in a single, tertiary hospital. The questionnaire was categorized into 6 domains (total 40 tasks): 6 tasks on identifying health issues (A); 3 tasks on prescribing and conducting diagnostic tests (D); 18 tasks on disease treatment (T); 4 tasks regarding prescribing medicine (M); 3 tasks regarding medical collaboration (C); 6 tasks regarding patient education (E). The survey measured the frequency, difficulty, and importance of APN's clinical tasks, and evaluated the willingness of authorizing clinical tasks to APN. Results: The most frequent tasks for APN were A domain, lowest were T domain. The scores for perceived job difficulty were lower than those for job importance in all groups. The proportion of willingness to legally delegate clinical practices to APN was higher in A and E domains, but lower in D and T domains. However, professors, who spent the most time with APN, showed a higher willingness to legally delegate clinical practice. The participants favored medical doctors as substitutes for tasks which were not legislated for delegated job performance. Conclusion: In this study identified clinical roles that medical doctors considered possible for legal delegation to APN were identified. The results can be used as evidence for the legalization of the practice of APN.

A study on the childhood cancer survivors' perceived difficulties : Focus group interview (소아암 생존자가 인지한 어려움에 대한 탐색적 연구 : 포커스그룹 인터뷰를 중심으로)

  • This is a qualitative research that was carried out to analyze the childhood cancer survivors' perceived difficulties using the result of focus group interviews. This study conducted interviews with 22 childhood cancer survivors under follow-up management after cancer treatment at general hospitals in Korea. 22 persons were divided into 3 groups, and interviews were carried out with each group for 80-100 minutes. As a result of the research, four topic collections consisting of 'Physical limitation', 'Influence of lasting childhood cancer', 'Difficulty in forming relationships' and 'Lack of reliable information' and seven subjects, which consist of 'Lack of energy', 'Potential late-effects', 'Fears about cancer recurrence', 'Negative social awareness', 'Difficulty in interacting with', 'Difficulty in interacting with peers', and 'Difficulty in interacting with families' were derived. Based on the findings of this research, it is hoped that provides a basis for developing appropriate and effective services and programs for childhood cancer survivors.

A study on illegal dental treatment (치과 무면허 불법 시술에 대한 연구)

  • Objectives : This study intended to figure out illegal treatment by unlicensed person in dental area and factors, which were considered as user's position on the basis of the local community investigation of health conditions in chungbuk provinces in 2008. Methods : This study used chi-square test and complex sample design of multi-variate logistic regression analysis to question 12,443 peoples who have experience on illegal dental treatment. Results : Multi-variate logistic regression analysis results showed that factors having related to illegal dental treatment are sex, age, education standards, subjective awareness of oral health condition, drinking experience of lifetime, unmet need of dental treatment, chewing difficulty, use of dentures, experience of scaling and the use of interdental care instrument. Conclusions : As a result of this study, in order to root out illegal dental treatment, we need to expand the breadth of health insurance coverage so that it can reduce the burden of dental expenses. Moreover, we need to set out health service of public oral health to inform the importances of maintaining good oral health and the problems of illegal dental treatment by unlicensed person.

A Study on the Levels of Subjective Oral Health Awareness in Local Residents from the City of Changwon and Haman County (창원시와 함안군 지역주민들의 주관적 구강건강인식수준 조사)

  • The purpose of this study was to examine the levels of the subjective oral health awareness of local residents in the city of Changwon and Haman county in an effort to lay the foundation for the development of customized local oral health education programs. The 2015 community health survey data were used, and the data from 864 residents in Changwon and from 719 dwellers in Haman county were analyzed. The Changwon residents brushed their teeth after lunch more, received more oral checkups over the past year and had more scaling experience. The Haman residents had more experience of not receiving dental treatment. The subjective health awareness, subjective oral health awareness, chewing difficulty and periodontal health of the Haman dwellers were all worse, and all the levels of awareness were lower among the respondents who were older and who were diagnosed with hypertension and/or diabetes. The respondents who made use of dentures and who didn't brush their teeth after lunch had experience of not receiving dental treatment, and all the levels of health-related awareness were lower among the respondents who didn't receive dental checkups nor had scaling experience. To remedy the situation, efforts should be made to prevent and manage chronic diseases, and the establishment of a system that guarantees regular dental checkups and scaling is required. And the kind of environments that everybody can receive dental treatment when they want should be prepared. In order to change negative awareness, necessary measures should be taken to provide oral health education that is not temporary but sustainable and tailored to local characteristics and different life stages.

A Qualitative Study of Physicians' Use of Clinical Information Resources and Barriers (임상의사의 진료목적 정보원 이용과 장애요인에 관한 질적 연구)

  • We analyzed the characteristics of the physicians preferred information sources and barriers through in-depth interviews. Information searches for patient treatment were subdivided into deciding patient treatment methods, understanding the latest treatment trends, and preparing presentation materials for conferences. The variables that affected the search behaviors were identified as being background knowledge on the topic, clinical experience, job title, search skills, user training, and familiarity with the library homepage. PubMed was the most preferred choice because of users' familiarity, reliability, and the vastness of information; Google was also used frequently for easy access and fast search result. The accuracy and the recentness of information were the most significant criteria. Easy interface and convenient access were also considered important due to physicians' time constraints. Searching obstacles were divided into difficulty of searching system, unfamiliar term, too vast resources, difficulty to get fulltext articles and complex advanced search features. The results of this study can be utilized as a basis for improving information service of library and curriculum development for physicians.

A study on the relationship between the symptom of Shanghanlon(傷寒論) and the defensive Gi(衛氣) (『傷寒論』 병증(病症)과 위기(衛氣)의 관계에 대한 연구(硏究))

  • Objectives : Shanghanlon is based on the Hwangjenaegyeong(黃帝內經)'s theory. Therefore, the contents of the Naegyeong can interpret Shanghanlon's provision, Chengwuji(成無己) followed the same way. Therefore, I studied the relationship between the symptom of Shanghanlon and the defensive Gi(衛氣), I try to prove that the Naegyeong is the theoretical basis of Shanghanlon. Methods : Naegyeong explains that defensive Gi protects the outer parts of the body, and runs the outer parts of the body. So I will explain the spontaneous sweating(自汗) aversion to cold(惡寒) pain(痛症) fever(發熱) difficulty of urination(小便不利) using the function of defensive Gi. Results & Conclusions : The defensive Gi and the nutrient Gi(營氣) run together, if the defensive Gi is weak, can not protect the nutrient Gi. If the defensive Gi does not perform the function of inducing astringency(固攝), the symptom of spontaneous sweating appears. If aversion to cold is caused by the weak of defensive Gi, we use the treatment of warming the Yang(溫陽). If aversion to cold is caused by the defensive Gi can not run, we use the treatment of activating the Yang(通陽). If the Gi and Blood(氣血) is not feeding properly, the pain occurs. In this case, we use the treatment of activating the Yang(通陽). The fever is caused by the stagnation of defensive Gi. If the defensive Gi is weak, the symptom of difficulty of urination appears.

Effects of Korean Medicine on Postural Instability and Gait Difficulty in Patient with Parkinsonism: Retrospective Study (파킨슨 증후군 환자의 자세 불안정과 보행장애에 대한 한의치료의 효과: 후향적 연구)

  • Objectives: The purpose of this study was to investigate the effects of Korean Medicine on postural instability and gait difficulty(PIGD) in patient with parkinsonism. Methods: We reviewed the charts of patients with parkinsonism who were admitted to the department of Korean internal medicine, stroke and neurological disorders center, Kyung Hee University Hospital at Gangdong from October 2009 to May 2017. We analyzed the data of UPDRS (Unified Parkinson's disease rating scale) and calculated PIGD related UPDRS before and after the admission. Results: The average of PIGD score for the 23 patients of Parkinsonism significantly decreased from $8.35{\pm}5.13$ to $4.52{\pm}3.68$ after treatment. The average of PIGD score for the 16 patients of idiopathic parkinson's disease and 7 patients of atypical Parkinsonism decreased from $9.13{\pm}4.46$ to $4.63{\pm}4.15$ and from $6.57{\pm}6.45$ to $4.29{\pm}2.56$ after treatment, respectively. Conclusions: These results provide that Korean Medicine has an effect on the motor function of patients who suffer from Parkinsonism with Postural Instability and Gait Difficulty. Furthermore it could be effective for improving activities of daily life.

Electroacupuncture Treatment for Idiopathic Trigeminal nerve Paralysis in a Dog (개에서 특발성 삼차신경 마비의 전침 치료)

  • A 3 years-old male Tosa dog was referred to Seoul National University Veterinary Medical Teaching Hospital with a history of difficulty in mastication. Clinical signs of dropped jaw, drooling, mild depression and dehydration were observed. According to history taking, physical examination, neurologic examination, complete blood count (CBC), serum chemical profile and radiography, the dog was diagnosed as idiopathic trigeminal nerve paralysis. Electroacupuncture treatment was applied to the dog on local and distal point at an interval of 7 days. Local points were GB-1 (Tong Zi Liao) of gall bladder meridian and ST-7 (Xia Guan) of stomach meridian. Distal points were PC-4 (Xi Men), PC-6 (Nei Guan) of pericardium meridian. Electrical stimulus was performed for 20 minutes at the frequency of 3 Hz, 3 Volts on ST-7. Ten days after the initial electroacupuncture treatment, clinical signs related to trigeminal nerve paralysis were almost disappeared.

편측성 악관절 골성강직의 외과적 처치의 치험례

  • Unilateral bony ankylosis of temporomandibular joint is not extremely rare. Treatment in all cases should be surgical. Considerable difficulty has been encountered by surgeons in the judgment of the planning of the treatment for anklosis; either by gap arthroplasty or by interposition arthroplasty. A recent review of the literautre reveals that many authors reported more of the interposition arthroplasty treatment because of no recurrence and the excellent results obtained. This 22-year-old Korean female, who had suffered from unilateral temporomandibular joint recurrent anklosis, was received an operation of ramus osteotomy followed by interpositional arthroplasty with using of thin gold plate. She formerly was received condylectomy twice at the age of 8 and 9, under the diagnosis of unilateral bony ankylosis due to trauma. My experience with alloplasty in unilaterally recurrent ankylosed T-M joint is presented here for her general and oral functional condition proved to be satisfactory after 4 years follow-up.
