Multiple Myeloma Similar to Traumatic Trigeminal Neuropathy after Dental Treatment: Case Report

  • Multiple myeloma is malignant neoplasm of plasma cells. Mandible lesions are rarely the first symptoms of multiple myeloma. While sensory dysfunction and pain are the main symptoms of traumatic trigeminal neuropathy, the same oral symptoms can appear in cases of multiple myeloma with developed mandible lesions. In addition, if the radiological osteoporosis or lytic lesion is seen in older patients, further examination is required to find the cause of the symptoms. In this paper, we present a case that was tentatively diagnosed as traumatic trigeminal neuropathy after left third molar extraction, but later confirmed as multiple myeloma.

유치 외상에 관한 연구 (A STUDY ON THE TRAUMATIC INJURIES TO PRIMARY TEETH)

  • 경희대학교 치과대학병원 소아치과에 2003년 1월 1일부터 2007년 12월 31일까지의 최근 5년간 치아외상을 주소로 내원한 생후 6개월에서 7세 미만(평균 2.8세)의 어린이 758명의 외상 받은 유치 1533개를 대상으로 하였다. 모든 대상자들의 진료기록 및 방사선 사진으로부터 내원한 어린이의 성별, 연령별 발생빈도, 원인, 장소, 외상 후 치료까지의 경과시간, 월별발생률, 발생부위, 치아손상의 개수, 외상의 양상, 초진 시 처치 등을 분석하여 다음과 같은 결과를 얻었다. 1. 유치의 외상은 남아가 여아보다 1.77배 많은 것으로 나타났으며, 남녀 모두 1-2세, 2-3세에 많이 발생하였다(p<0.001). 2. 외상의 주요 원인은 넘어짐, 부딪힘으로 나타났고, 주로 저연령층 어린이에서 두드러지게 나타났다(p<0.05). 외상의 발생장소는 집안, 집밖, 유치원순으로 나타났으며, 특히 기후가 온화한 5월, 9월, 10월에는 집밖의 비율이 높게 나타났다(p<0.001). 3. 외상 후 주로 24시간 이내(77.6%)에 내원하였으며, 월별 치아외상은 겨울을 제외한 3-9월에 고르게 나타났다(p<0.001). 4. 가장 손상을 많이 받은 치아는 상악 유중절치로 나타났으며, 치아손상은 단일치아 또는 두 개의 치아 손상이 주로 나타났다. 5. 치주조직 손상이 치아경조직 손상에 비해 2배정도 많이 발생하였으며, 외상의 양상은 아탈구, 측방탈구, 법랑질파절, 함입, 치근파절 순으로 나타났다.

어린이의 치아 외상에 대한 전라북도 초등학교 교사들의 인식도 조사 (A SURVEY ON THE KNOWLEDGE AND AWARENESS OF TRAUMATIC DENTAL INJURIES IN A GROUP OF JEON-BUK PROVINCE ELEMENTARY SCHOOL TEACHERS)

  • 본 연구는 전라북도 지역 초등학교 교사에게 적합한 치아외상 교육내용 및 효과적인 전달 방법이 마련될 수 있도록 치아외상에 대한 지식수준을 파악하고 기존 연구에서 자세히 다루지 않았던 교육현황을 조사하는 데 그 목적이 있다. 전북지역 초등학교 교사 231명에게서 설문지를 통하여 자료를 수집하였으며 설문 문항은 응답자의 일반 사항, 치아외상 교육현황 및 태도, 사례를 통한 지식수준, 치아 보관 매체에 대한 질문으로 구성하였다. 조사 결과 치아외상 대처방법에 교육을 받은 교사는 15.2%에 불과하였고, 교사들을 대상으로 치아외상 대처방법에 대한 교육이 필요한지에 대한 질문에서는 97%가 필요하다고 대답하여 교사 스스로도 치아외상 교육의 필요성에 대해 공감하고 있었으며 교육 방식으로는 전문가에 의한 직접 교육을 선호하였다. 사례를 통한 평가에서는 문항 간에 다소 차이는 있었으나 응급상황시 적절히 대처하기에는 지식수준이 대체로 불충분한 것으로 나타났다. 따라서 향후 전라북도 초등교사들에게 적합한 교육방식과 교육내용을 객관적으로 분석하는 연구 및 교사들에게 알맞은 교육 프로그램의 개발 등이 필요하리라 사료된다.

전치부 외상성 손상시 치아고정술 후의 예후에 관한 연구 (A STUDY ON THE PROGNOSIS AFTER STABILIZATION OF THE TRAUMATIZED ANTERIOR TEETH)

  • This study was to evaluate the nature of traumatic injuries on anterior teeth of 530 patients and the prognosis of stabilized cases of 102 patients, including possible 18-follow up cases among them, treated at Dental Hospital, Yonsei Medical Center from Mar. 1983 to Feb. 1988. The obtained results were as follows. 1. The peak of monthly distribution of trauma to the anteriors was June and was the lowest in November. The ratio of male to female was 2.6 : 1. The most common incidence of the trauma to the anteriors was seen in the 3rd decades. The most chiefly involved teeth were central incisors in both upper and lower jaws. The causes of trauma were as followed in descending order by first blows, traffic accidents, falls, slip downs, and sports. The types of traumatic injuries of anterior teeth consisted of periodontal tissue injuries(55.9%), the injuries to the hard dental tissues and pulp(31.3%), and alveolar bone injuries(12.8%). 2. 35.1% stabilized anteriors were treated with root canal therapy. The success rate of stabilization was 95.3%, and 92.3% in replantation of avulsed teeth. 3. External root resorption, internal root resorption with calcification of pulp chamber or root canal, ankylosis of root, and other periapical pathosis were seen in follow-up radiographic findings. 4. Success rate of stabilization of the traumatized anteriors after 3 years showed 71.4% average.

J지역 운동선수의 악안면 외상 실태와 지식에 관한 비교조사연구 (A comparative study on J regional sport players' actual conditions of maxillofacial trauma and their knowledge about It)

  • Objectives : The purpose of this study was to investigate the maxillofacial injuries of the sports players' and analyzing the concerned factor. Methods : The subject in this study were 290 sports players' enrolled in Jeolla region. SPSS statistics program(Version 12.0) was used to analyzed the collectde data, Chi-square test and percentage were used as the analysis technique. Results : 1. It was found that 63.8% of all respondents ever experienced trauma in sport activities and 28.6% experienced maxillofacial trauma. 2. In terms of traumatic causality, it was found that most of our respondents attributed the biggest cause of maxillofacial trauma to 'sport activities.' 3. However, it was found that only 12.1% of all respondents gave correct answers to question items about good medical treatment for traumatic odontoptosis. 4. It was found that less than half of all respondents ever faced information on maxillofacial trauma. Conclusions : This finding suggests that it is required to build up current orientation programs about maxillofacial trauma for our sport players. The maxillofacial trauma of sport players' is a important factor that degenerate the quality of the oral health and bring about the change of life style.

An 18-year-old patient with Prader-Willi syndrome: a case report on dental management under sedation and general anesthesia

  • Prader-Willi syndrome (PWS) is a rare genetic disorder reported rarely in dentistry. Dental practitioners should know the features of PWS because affected patients have a variety of dental symptoms. The current report describes a case of PWS. An 18-year-old male patient presented with traumatic injuries. Initial emergency treatments were performed under sedation, and further treatments were conducted under general anesthesia. After adequate healing, periodic follow-up and dietary management according to the patient's age and nutritional phase were recommended. Dental management of PWS patients consists of active preventive measures in addition to dietary consultation according to age and nutritional phase.

Evaluation of dental status using a questionnaire before administration of general anesthesia for the prevention of dental injuries

  • Background: Dental evaluation and protection are important for preventing traumatic dental injuries when patients are under general anesthesia. The objective of the present study was to develop a questionnaire based on dentition-related risk factors that could serve as a valuable tool for dental evaluation and documentation. Methods: We developed a questionnaire for dental evaluation before administration of general anesthesia, investigated the association between patient-and-dentist responses and mouthguard fabrication, and assessed response agreement between 100 patients. Results: Protective mouthguards were fabricated for 27 patients who were identified as having a high risk of dental injury. There was a strong association between dentists' responses and mouthguard fabrication, depending on the general oral health status, use of ceramic prosthesis, presence of masticatory pain related to periodontal diseases, gingival edema, and implants (P < 0.05). Response agreement between patients and dentists for items related to dental pain, loss of dental pulp vitality, root canal therapy, dental trauma, aesthetic prosthesis, tooth mobility, and implant prosthesis was high (Cohen's kappa coefficient κ ≥ 0.6). Conclusions: A high agreement was observed between patient-dentist responses and a strong association with mouthguard fabrication for items pertaining to ceramic prosthesis, masticatory pain, and dental implants. Patients with a "yes" response to these items are recommended to undergo a dental evaluation and use a dental protective device while under general anesthesia.

유치열기의 치아 외상에 관한 연구 (A STUDY OF THE TRAUMATIC INJURIES IN THE PRIMARY DENTITION)

  • 본 연구는 경희대학교 치과대학병원 소아치과에 치아외상을 주소로 내원한 411명의 어린이를 대상으로 치아외상의 발생 연령과 남녀간의 성차, 발생의 양상, 부위, 원인, 장소 및 외상 후 내원 시기와 계절적인 발생빈도 등을 조사하여 외상의 예방, 적절한 진단과 치료 그리고 보호자의 계도 및 외상에 관한 홍보에 도움이 되고자 시행하였다. 1998년부터 2000년 사이에 치아외상을 주소로 내원한 환자 중 생후 5개월에서 6세 사이 남아 261명, 여아 150명의 외상치아 745개를 대상으로 다음과 같은 결과를 얻었다. 1.치아 외상은 남아가 여아보다 1.74배 많은 것으로 나타났으며, 남녀 모두 $1{\sim}2$세(34.1%) 및 $2{\sim}3$세(24.3%) 사이에 많이 발생하였다. 2. 치아 외상 양태는 진탕 32.2% 함입 11.9%, 측방변위 10.1%의 순으로 나타났다. 3. 상악 유중절치가 외상 치아의 75.3%로 가장 많이 발생하였다. 4. 치아 외상의 원인은 '넘어짐' 46.0%, '부딪힘' 40.3%, '떨어짐' 6.0% 순으로, 장소는 집안 48.0%, 집밖 37.7%, 유치원 9.2% 순으로 발생하였다. 5. 치아 외상 후 24시간 내에 내원하는 경우가 46.0%, 1주일내 29.5%, 1시간내가 7.5%를 보였다. 6. 월별 치아 외상은 10월(12.7%), 6월(12.2%), 5월(11.7%) 순으로 발생하였다.

Apexogenesis and revascularization treatment procedures for two traumatized immature permanent maxillary incisors: a case report

  • Traumatic injuries to an immature permanent tooth may result in cessation of dentin deposition and root maturation. Endodontic treatment is often complicated in premature tooth with an uncertain prognosis. This article describes successful treatment of two traumatized maxillary central incisors with complicated crown fracture three months after trauma. The radiographic examination showed immature roots in maxillary central incisors of a 9-year-old boy with a radiolucent lesion adjacent to the right central incisor. Apexogenesis was performed for the left central incisor and revascularization treatment was considered for the right one. In 18-month clinical and radiographic follow-up both teeth were asymptomatic, roots continued to develop, and periapical radiolucency of the right central incisor healed. Considering the root development of these contralateral teeth it can be concluded that revascularization is an appropriate treatment method in immature necrotic teeth.

  • In an attempt to regain function and aesthetics in the craniofacial region, different biomaterials, including titanium, hydroxyapatite, biodegradable polymers and composites, have been widely used as a result of the loss of craniofacial bone. Although these materials presented favorable success rates, osseointegration and antibacterial properties are often hard to achieve. Although bone-implant interactions are highly dependent on the implant's surface characteristics, infections following traumatic craniofacial injuries are common. As such, poor osseointegration and infections are two of the many causes of implant failure. Further, as increasingly complex dental repairs are attempted, the likelihood of infection in these implants has also been on the rise. For these reasons, the treatment of craniofacial bone defects and dental repairs for long-term success remains a challenge. Various approaches to reduce the rate of infection and improve osseointegration have been investigated. Furthermore, recent and planned tissue engineering developments are aimed at improving the implants' physical and biological properties by improving their surfaces in order to develop craniofacial bone substitutes that will restore, maintain and improve tissue function. In this review, the commonly used biomaterials for craniofacial bone restoration and dental repair, as well as surface modification techniques, antibacterial surfaces and coatings are discussed.
