  • Aim: Tumors of upper gastrointestinal tract are among the cancers that have a quite lethal course. Cytotoxic chemotherapy is the most efficient therapeutic modality for metastatic gastric cancer. In patients who do not respond to first-line treatment, the response rate to second-line therapies is generally low and the toxicity rates high. This study concerned the efficacy and the side effect profile of second-line therapy with irinotecan in the patients who were being followed-up with the diagnosis of metastatic gastric cancer in $\dot{I}$zmir, Turkey. Materials and Methods: We retrospectively evaluated the efficacy and toxicity in 31 patients with metastatic gastric adenocarcinoma who presented to the polyclinic of Medical Oncology of Izmir Ataturk Education and Research Hospital between May 2008 and July 2011. All received chemotherapy regimens containing cisplatin, fluoropyrimidine (5-FU) and docetaxel as the first-line therapy for late stage disease. Irinotecan as a single agent was given at a dose of 210 mg/$m^2$ on each 21 days. Irinotecan (180 mg/$m^2$ on day 1), 5-FU (500 mg/$m^2$ on days 1-2) and leucovorin (LV; 60 mg/$m^2$ on days 1-2) as a combined regimen were given over a 14 day period. Results: Median age was 54 (range, 31-70). Irinotecan was given as a combined regimen for median 6 cycles (range, 3-12) and as a single agent for median 3 cycles (range, 1-10). Metastases were detected in one site in six patients (19%), in two different sites in 17 patients (55%) and in three or more sites in eight patients (26%). Four patients (12.9%) showed partial response and six patients (19.3%) showed stable disease. Progression-free survival (PFS) was found to be 3.26 months (95% CI, 2.3-4.2). Median overall survival (OS) was found to be 8.76 months (95% CI, 4.5-12.9). The most commonly seen grade 3/4 side effect was neutropenia but the the therapy was generally well-tolerated. Conclusions: In this study, it was demonstrated that second-line therapy with irinotecan given following the first-line therapy with cisplatin, fluoropyrimidine (5-FU) and docetaxel was efficient and safe. Further studies are needed for confirmation.

  • The purpose of this study was to investigate current status of sedation training for the residents in pediatric dentistry training institutions and opinions about continuing education after the residency program. Surveys were sent to 18 pediatric dentistry training institutions by e-mail, and the responses were collected and analyzed. Most of the sedation education period for the residents were the 1st-year education (61.1%) and 1 - 3 years of integrated education (55.6%). In terms of an externship, 5 institutions (27.8%) sent their residents to the department of anesthesiology. Second half of the 1st year (50%) was the highest for a resident to use sedation for the first time. The period of supervisor participation varied from not participating at all to whole time throughout the residency program. The sedation training is conducted at all training institutions, but there were variations in the experience that a resident can gain. All training institutions agreed on the necessity of continuing education of the sedation, but there were various opinions regarding time, method, and the period of review course. Overall, this study suggested that continuing education should be consisted of 1 - 2 hours of didactic education every year and clinical skills and simulation training in every 2 - 3 years.

  • This study introduces lessons learned from investigation and analysis of major domestic and overseas cases of marine chemical incidents involving hazardous and noxious substances (HNS) during maritime transportation by chemical tankers carrying petrochemical products in bulk. The study then suggests plans to improve domestic marine HNS training curricula based on these lessons. Lessons learned from six incident cases are classified into the following six categories: 1) incident-related information, 2) safety, 3) pollution, 4) response, 5) salvage and 6) others. Based on these six categories, it is suggested that the curriculum provided by the Marine Environment Research & Training Institute for marine pollution prevention managers aboard noxious liquid substance carriers should be changed from the existing two-day training of eight subjects (16 h) to a three-day training of sixteen subjects (24 h). In addition, it is proposed that the marine chemical incident response course of the Korea Coast Guard Academy should be changed from the existing five-day training of fifteen subjects (35 h) to a six-day training of thirty-two subjects (48 h). These results are expected to contribute to sharing experiences and lessons learned about response to marine chemical incidents and to be used as basic data for improving the education and training courses for response personnel in preparedness for marine HNS incidents.

  • The vicious cycle of over-education has been made. That is, higher education enrollment rate is high, but university graduate employment rate is low. To eliminate this cycle and relieve youth unemployment and young people to enter the labor market early, dual education and training system is needed. This dual system can support working and learning in parallel. So, worker can get the opportunity pre-employment and post-learning and improve his/her job skills. Recent MOOC (Massive Open On-line Course), a new form of online education system, has emerged. MOOC combines education, entertainment and social networking, and emphasize the interaction between faculty and student and between students. The educational contents of MOOC are available free of charge. Using newly changed online education environments we can effectively provide knowledge and skills. In technology and engineering education hands-on training is necessary. In order to support work based learning dual system for worker to work and learn in parallel, we should build the multi-learning system to combine the online education and campus hands-on practice.
