Background: Blood stasis (BS) is commonly used for pattern identification in traumatic injuries, including traffic accidents (TAs). Various studies have identified the efficacy of Korean medicine treatments for TA patients, but studies focusing on the BS-based herbal medicine (BSHM), including Tongdo-san (TDS), are rare. Methods: This was a single-center, prospective observational study, conducted from August 24th, 2018 to December 27th, 2018, which included 40 TA patients. Participants underwent routine Korean medicine treatments including acupuncture, electronic moxibustion, cupping, physical therapy, and herbal medicine. In the herbal medicine treatment, participants took BSHM with more than 3 days including taking TDS. The primary outcome measures were the scores from a 100 mm visual analogue scale (VAS) and numerical rating scale (NRS). Secondary outcome measures included scores from EuroQol-5 dimension (EQ-5D) and EQ-VAS questionnaires, the BS and cold/heat indices, and safety assessments. Results: There were significant improvements in the VAS, NRS, EQ-5D, EQ-VAS scores, and BS index after treatment. In the sub-analysis, VAS, NRS, EQ-5D, and EQ-VAS scores were higher in groups with a higher BS index. The moderate and severe BS index groups showed more improvement than the minor BS index group, and there was a significant difference in the EQ-5D scores. There was no significant differences observed in cold/heat index groups scores. Conclusion: BS is associated with TA-related symptoms. BSHM, including TDS, may significantly reduce BS, pain, and discomfort.

Objectives : This study aimed to understand the satisfaction degree of patients due to traffic accident(TA) in order to provide basic data which would give help not only to improve the quality of services, but also to strengthen the competitiveness of oriental medical hospitals on sequela of TA patients. Methods : The data were collected from inpatients and outpatients who answered a self-report questionnaire. Independent t-test, analysis of variance(ANOVA), $X^2$-test, correlation analysis were used for data analysis. Results : According to the research on medical satisfaction degree, the overall satisfaction degree was 4.141. On each field, satisfaction degree related to the doctor service recorded the highest degree among others(4.474). Satisfaction degree related to the appropriateness of service was the second(4.091). On the other hand, satisfaction degree of using procedure was the lowest degree(3.857). On the difference of degrees of satisfaction by general characteristics, patients who were unemployed houseworker group showed lower doctor service satisfaction degree than that of other groups. However, on the difference of degrees of satisfaction related to other general characteristics, there were no significant differences(p<0.05). Conclusions : The service distribution of oriental medical hospital was almost equal without concentrating on specified social classes.
Objectives : TA, a polyherbal extract, typically is used for pharmacopuncture therapy on patients with traffic accident-related injuries and musculoskeletal diseases. This study was performed to evaluate the safety of the TA extract, using a micronucleus test. Methods : The dose range and sampling time were first established. An in vivo micronucleus test was then performed to determine the induction of micronuclei in mouse bone marrow cells after a single intramuscular administration of TA to 7-week-old ICR mice (0.2 ml/animal, at 24 hours post-dosing). Results : The incidence of micro-nucleated polychromatic erythrocytes (PCEs) in PCEs in the TA group was similar to that in the negative-control group, while that in the positive-control group was significantly greater. The positive- and negative-control groups did not differ in the ratio of PCEs to total erythrocytes. Conclusions : Our toxicity study indicates that the TA extract does not induce micronucleus formation in mouse bone marrow cells.

Objectives: This study conducted a survey research to characterize traffic accident(TA) patients seeking Korean medicine treatment and to analyze the symptom improvement and satisfaction scores. Methods: A survey was conducted in 122 outpatients, who visited OO University Korean Hospital due to TA-associated symptoms from November, 2017 to May, 2018. The questionnaire included information on patient demographic characteristics, accident circumstance details, pain levels, reason for treatment selection, treatment methods, treatment purpose, symptom improvement and satisfaction. All statistical analyses were performed using Windows SPSS version 20.0 Results: The characteristics of traffic accident patients using Korean medical institution were usually accidents that occurred while driving slowly, and minor injuries like a sprain. The most important consideration in choosing Korean medical institution was its past experience. The biggest reason for switching treatment from Western medicine to Korean medicine was for diverse treatments. Satisfaction with Korean medicine was measured between very satisfaction and satisfaction. Patients showed the highest satisfaction with acupuncture, followed by pharmacopuncture and chuna manual therapy. Most treatments were measured between very satisfaction and satisfaction. After treatment, 95.90 percent of the patients said they would recommend Korean treatment. Conclusions: Although this study has limitations as research in survey format, we intended to analyze determining factors for the use of Korean medicine treatment through satisfaction, symptom improvement, and Numeric rating scale (NRS) change.

In South Korea, traffic accident patients can be treated under automobile insurance coverage. This study investigated the status of Korean medicine (KM) Automobile insurance system and usage status of main pharmacopuncture in KM clinic which reported that the largest number of automobile insurance fee claims among medical institutions. We surveyed 258 traffic accident patients who were treated at Namsangcheon KM clinic from 2014 to 2018 according to medical chart. The majority of the patients were male and thirties. In traffic accident situation, the highest distribution was car to car crash with 85.66% and 66.67% of the patients visited in the most acute phase. The most frequent treatment period was within 4 weeks and the number of treatments was 10 or less with 72.87%. In total results of treatment, the distribution was exellent with 10.08%, good with 46.90%, fair with 27.13% and poor with 15.89%, and the effective rate was 84.11%. The most frequent treatment period was within 4 weeks with 64.73% and the number of treatments was 10 or less with 72.87%. Of the 242 patients who received pharmacopunture, 91.5% were treated with HO, which was named after Honghwaja and TA, which was named after traffic accidents, and there were significant differences in the number of treatments and symptom improvement between the two groups. In this study, we confirmed the status of automobile insurance treatment and usage of main pharmacopuncture of single KM clinic with symptom improvement. This study can be regarded as one of the basis of KM treatment for the rapidly growing automobile insurance market.
This study analyzed the relative vehicle speed and vehicle deceleration time caused by traffic conflicts using vehicle speed data at expressway toll gates and traffic conflict occurrence data. According to the analysis, the greater the relative vehicle speed is at the toll gate, the more sudden vehicle deceleration occurs due to traffic conflicts. In particular, a comparison study of cases in similar operating conditions presents a finding that usage of lanes influences traffic conflict occurrences as well as relative vehicle speed. With this finding, the study further conducted a quantitative analysis of the accident rates in relation to the relative vehicle speed between vehicles using a "Hi-Pass" lane and a regular lane at the toll gate. It indicates that when the relative vehicle speed is greater, the accident rate is higher due to sudden vehicle deceleration and shorter deceleration time. Furthermore, when the expressway entrance/exit point is closely located to a toll gate and the relative vehicle speed is great, a analysis at a traffic conflict shows a low value.

Background: This study was conducted to verify the effectiveness and effectiveness-related factors of Korean medicine (KM) treatment for patients who have been involved in traffic accidents (TA). Methods: The medical records of 150 TA patients hospitalized in Dongguk University Bundang Oriental Medicine Hospital from September 2017 to May 2018 were reviewed. Medical records included information on patient demographic characteristics, TA situations, use of KM treatment, clinical characteristics, 4 scales [numeric rating scale (NRS), visual analogue scale (VAS), EuroQol-visual analogue scale (EQ-VAS) and neck disability index (NDI)]. Statistical analyses were performed to determine the effectiveness and effectiveness-related factors of treatment. Results: The 150 patients (almost 1:1 gender ratio) were typically in their 40's (26.67%), had rear impact to their vehicle (37.33%), were in the driver's seat (70.00%), and without head trauma and loss of consciousness (83.33%). The main symptoms were neck pain (60.66%) and lower back pain (54.00%). All 150 patients showed significant improvement after KM treatment. 4 scales significantly improved when the treatment started within 3 days from the accident and patients were hospitalized longer and took more diverse treatments. The patients experiencing a higher initial pain level (NRS 8-10, $VAS{\geq}70$) showed significant improvement of NRS and VAS scores with faster and longer inpatient treatments than the other patients. The patients with higher initial pain levels ($VAS{\geq}70$) also showed significant improvement in EQ-VAS scores. Conclusion: KM treatment was effective for TA patients in this study. Treatment initiation time, hospitalization period, treatment diversity and initial pain level are related to the degree of improvement of a patient's symptoms.

Objectives : The purpose of this study was to investigate the effect of Bee-venom acupuncture on low back pain by traffic accidents. Methods : We included 34 patients with low back pain caused by TA for this study. The experimental group was treated by usual therapy plus Bee-venom acupuncture therapy, the control group was treated by usual therapy such as acupunture therapy and herbal medicine therapy. Oswestry Disability Index(ODI) and Visual Analog Scale(VAS) were evaluated at 4 week from baseline for the treatment effect in the both group. Results : After the subjects being treated by Bee-venom or general acupuncture therapy. It was detected that the scores of ODI and VAS in both groups decreased at the 4 week from baseline. There was significant difference between both groups in the VAS. However, there was no significant difference between both groups in the ODI. Conclusions : The results suggested that bee-venom acupuncture was effective on low back pain by traffic accidents.

Purpose: No previous study has assessed elderly pedestrian traffic accidents based on a nationwide database. This study aimed to help primary physicians who examine patients in emergency departments to determine and make prompt and accurate treatment decisions. Methods: This study used data from the Emergency Department-based Injury Indepth Surveillance from 2013 to 2017, managed by the Korea Centers for Disease Control and Prevention. Pedestrians aged ${\geq}65years$ were included, and using multivariate logistic regression multiple factors were analyzed to determine their relationship with injury severity. Results: Of 227,695 subjects, 6,498 were included, of whom 2,065 (31.8%) were severely injured. There were more female than male patients in all severity groups. Most accidents occurred in the afternoon and on general roads. In the multivariate analysis, the odds ratio (OR) of injury severity for male pedestrians was 1.165 (95% confidence interval: 1.034-1.313, p=0.012). Older age of patients and the use of ambulances were associated with greater injury severity. The accident time affected the degree of injury severity; i.e., compared to dawn, injury severity increased in the morning (OR: 1.246, p=0.047) and decreased at night (OR: 0.678, p<0.001). A significant difference was noted in the correlation between the type of vehicle causing the accident and the accident severity; i.e., motorcycle accidents had lower severity than bicycle accidents (OR: 0.582, p=0.047). Conclusions: Injury severity was correlated with sex, age, transportation to the ED, TA onset time, and type of vehicle. The study results suggest that injury severity may be positively reflected in initial assessments and overall integrated treatments by physicians and in the related policies.

This is a retrospective study on the nature and severity of zygomatic bone fractures. This study was based on a series of 358 patients with zygomatic bone fractures who treated as in-patient at Soonchunhyang Chonan Hospital during the period of Jan. 1993 through Dec. 1996. The results obtained are as follows : 1. The ratio of men to women was 4.41:1. The age frequency was highest in the second, third decade. The yearly distribution was the highest in 1993. The monthly distribution was highest in Feb. and Jun. 2. The most frequent cause was the traffic accident.(66.2%) 3. The most frequent type of fractures was the class I(undisplaced fx.).(25%) 4. The most frequent associated oral and maxillofacial injuries was soft tissue.(46.9%) 5. The most frequent associated systemic injuries was upper, lower extremity.(38%) 6. In A.I.S of oral and maxillofacial area, the mean score of A.I.S was $2.5{\sim}3$. In I.S.S, the highest mean score was in the ped TA.(19) 7. In respect of treatment, the most common method was open reduction with rigid fixation on fronto-zygomatic suture area.(47.8%) 8. The incidence of complication was 12% and the most common complication was malunion.(6.1%)
