The Retrospective Study of Advanced Cancer Patients Receiving Integrative Cancer Treatments in single Comprehensive and Integrative Medicine Hospital

  • Objectives: Traditional Korean medicine (TKM) has been supplied as part of a national health care system in South Korea under a dual medical system, however it has been difficult to mix western medicine and TKM medical practices in existing hospitals. For the objective of innovative integration between them, Comprehensive and Integrative Medicine Hospital were founded in Daegu, South Korea. Here, we discussed the clinical outcomes of cancer patients who received integrative cancer treatment (ICT). Methods: A total of 678 patients previously diagnosed with cancer were retrospectively checked in our institution for 2 years. After applying inclusion/exclusion criteria, 573 patients were eligible for the final analysis. The overall survival (OS) of these patients in the aftercare period were determined. We looked at how clinical factors and treatments including chemotherapy, complementary and alternative medicine (CAM), and TKM affected the OS. Results: At the first visit, 212 subjects had no evidence of disease after tumor resection and 355 ones with advanced cancer, who receiving ICT including chemotherapy, CAM, and TKM showed better OS compared to the CAM including TKM or the best supportive care (median OS = 216, 78, and 46 days, respectively). The median OS was not reached in TKM only, even though the sample size was small (N=12). Even after adjusting for clinical factors associated to liver, renal, and hematologic manifestation, ICT still remained significant. Conclusions: We demonstrated that ICT might be beneficial for advanced cancer regardless of the performance status, liver and renal function, since it positively affected the OS.

  • Treatment method refers to a principle or method for treating diseases in Traditional Korean Medicine(TKM). As doctors determine the ideal treatment for a patient's disease or symptom, they are also able to prescribe effective treatment means for the diseases or symptom such as medicinal materials, prescription, acupuncture and moxibustion. Therefore, if significant symptom-treatment method combinations are found from literature or database, proper treatment means for the patient's diseases or symptom may be presented to TKM doctors and enhanced treatment accuracy and efficiency can be expected. This study aims to analyze the relation between symptom and treatment method by interpreting hypotheses through null hypotheses to find significant symptom-treatment method combinations. This combinations suggested in this study will be compared with TKM experts analysis result to find an objective analysis method and eventually apply the method to medical big data, e.g., a huge amount of literature or treatment records.

Current Issues in Evaluating Health Promotion Programme Using Traditional Korean Medicine in Korea

  • Purpose : The aim of this study is to identify current situation and issues of outcome measures to evaluate the public health programs using traditional medicine by public health centers in Korea. Method :This study analyse and review existing data and documents related to traditional Korean medicine and health policy using contents analysis method. To collect the information on outcome measures evaluating the programmes, this study reviewed annual reports for health promotion programmes using traditional Korean medicine(HP-TKM) of Hub public health centers, as pilot public health centers, which have implemented the health promotion programmes collectively. Additionally, the review included research articles, government documents and book chapters on the topics related assessments in health promotion. Results :HP-TKM are stroke prevention education, smoke free program, health promotion according to Four Constitutional Medicine, home visiting treatment, etc. However, there are only a few studies of traditional medicine focused health promotion evaluation strategies. The benefits of health promotion programs using TKM can be categorized as non-health benefit, physiological, psychological and physical effects. To manage and monitor the intervention programmes efficiently, attention should be given to developing relevant and valid outcome measures for evaluating the programmes by government and public health center. Conclusion :Up to now, considering number of researchers, research projects undertaken or published articles and reports, within traditional Korean medicine there is a lack of capacity in research. Thus, government should pay more attention to developing relevant and valid outcome measures for evaluating the programs.

Suggestion of experimental model on the innate kidney essence insufficiency syndrome for the development of humanitas traditional medicine

  • According to the traditional Korean medicine (TKM), kidney has been recognized as the roof of innate endowment because it plays an important role in the birth, growth, sexual reproduction, and aging. Kidney essence insufficiency syndrome (KEIS) is caused when kidney is impaired. KEIS is characterized by retarded development, decreased reproduction, tinnitus, loosening of teeth, and loss of hair and forgetfulness. In traditional Chinese medicine, KEIS mouse model was established by use of threatinjuring the kidney combined with over-fatigue. However, the TKM theory-based KEIS experiment model has not been described correctly. In the present study, we suggest a new KEIS experiment model including following cases; 1) weakness of father essence and mother blood, 2) life nurturing during pregnancy, 3) full-term gestation period.

자궁적출술 후에 갱년기 증후군으로 이행된 환자의 임상보고 (A Clinical Study of Climacteric Symptoms after Hysterectomy)

  • Purpose : The purpose of this study was to investigate the clinical application of Traditional Korean Medicine(TKM) treatment to climacteric symptoms after hysterectomy. Methods : The patient was a 51-years-old woman who suffered from hot flush, arthralgia, fatigue et al after hysterectomy. The patient was treated by TKM such as herb medication and acupuncture therapy. And her health status was evaluated by Kupperman's index and Medical Outcome Study Short Form 36( SF-36). Results : Climacteric symptoms after hysterectomy was reduced( Kupperman's index from 40 to 20). It got better that Physical Functioning(PF), General health(GH), Bodily pain(BP), Role limitation-Emotion(RE) and Mental Health(MH). So, it means that her health status was improved. Conclusion : TKM is expected to have positive effects on climacteric symptoms after hysterectomy, and can be improve woman's health status after hysterectomy.

"열하일기(熱河日記)" 소재(所載) "금료소초(金蓼小抄)" 번역(飜譯)에 관한 연구(硏究) (A Study on Translation of "Kumryosocho(金蓼小抄)")

  • Objective : This paper is aimed at suggesting further tasks by checking and rectifying the errors of the ancieut Chinese-vernacular Korean translations of Park Ji-won(朴趾源)'s "Kumryosocho". Method : In order to correct the wrongly transcribed "Kumryosocho" was contrasted with the original "Xiangzubiji(香祖筆記)", of which the part is "Kumryosocho". And then the errors and mistakes are discovered in published ancient Chinese-to-vernacular Korean translations. Result : In the course of checking the existing translations of "Kumryosocho", this paper identified the following types of errors. 1. Errors attributable to unfamiliar names of medicinal herbs 2. Errors due to the unfamiliarity with the names of diseases or symptoms in Traditional Koreau Medicine(TKM). 3. Errors committed in hand transcription. These types of errors were committed as well in translating jargons routinely used in TKM books. To the surprise, the errors above have been repeated even in the latest version of its translation. This means that the medicine-related materials by Silhak scholars, including "Kumryosocho", were placed at a dead zone of the research between Chinese classic scholars and TKM scholars. Conclusion : To minimize errors and mistakes, it is needed to activate the cooperative work of heterogeneous experts in two academic fields.

한의학계(韓醫學界)의 암연구동향(癌硏究動向)과 연구(硏究) 전략(戰略)에 대한 연구(硏究) (Study on trends of cancer study in TKM and its research strategy in future)

  • For the purpose of designing more successful cancer research, the strategy for cancer study in the field of Traditional Korean Medicine(TKM) during the 21th century was examined from the analysis of trends on cancer study in traditional korean medicine. The results were summarized as follows: 1. So far cancer research in TKM was chiefly done on cytotoxicity, side-effects by chemotherapy, tumor immunology, apoptosis, survival time with S-180 and pulmonary colonization assay and also clonogenic assay, cell adhesion assay, angiogenesis, cell-differentiation and side-effect by radiotherapy were partly performed. 2. It may be ideal that we should study synergistic effect between constituent drugs of prescriptions, tumor immunology, combined therapy between western and oriental medicines by reducing side-effect by radiotherapy and chemotherapy and antimetastasis according to the characteristics of oriental medicine chiefly and also supplement the studies on molecular biology, gene therapy, angiogensis and signal transduction. 3. We had better do specific-field research in cooperation between oriental medical colleges and Korea Institute of Oriental Medicine(KIOM) as well as study a target cancers such as hepatic cancer, pulmonary cancer and gastric cancer more intensively than all cancers domestically. 4. Our country must keep communication with China having many clinical data, Taiwan chiefly doing the combined tharapy between oriental and western medicines, Japan having done basic study actively on cancer.

한방 단독치료로 전이 대장암의 반복적인 재발을 억제한 장기 생존 치험 1례 (A Case of Long-term Survival with Traditional Korean Medicine Alone for Recurrent Metastases of Colorectal Cancer)

  • Objective: This study was aimed at reporting on a patient with repeated recurrence of metastatic colorectal cancer treated with traditional Korean medicine (TKM) alone after operation and chemotherapy. Method: The patient with metastatic colorectal cancer received tumor resections four times and perioperative chemotherapy for repeated recurrence from February 2013 until March 2015. TKM, which included Gunchil-dan and Haeam-dan, was administered until June 2019 to prevent additional relapses. Results: After 49 months of TKM treatment, there was no tumor recurrence. A no evidence of disease (NED) state was maintained from March 2015 to June 2019. Recurrence-free survival (RFS) was shown for 49 months and overall survival (OS) for 78 months. Serious adverse events evaluated according to the Common Terminology Criteria for Adverse Events (CTCAE) Version 5.0 were not identified. This study suggested that TKM with herbal medicine including aRVS might prolong survival and inhibit recurrence of tumors in patients with metastatic colorectal cancer.

한의사 직능수행을 위한 대학교육에서 추구하는 한의사 인재상에 대한 논의 (Concept of Talent on the Doctor of Korean Medicine to Pursue University Education in Vocational Performed for Korean Medical Doctor)

  • Apparently, in the field of Traditional Korean Medicine(TKM), lots of papers have been published and they have helped TKM grow quantitatively. But from the inside, there has been a lot of problems like the gap of theory and practice, doubts about necessity of basic theory, and lack of internal communication. There could be many reasons for it, but it could be considered due to the lack of ideal talent that should be goal of teaching and performing TKM. In the field of TKM, the absence of ideal talent of Korean medical doctors(KMD) and inferior social position of KMD than Medical Doctors(MD) has beeb derived from the laws promulgated in Japanese Occupation. These days, KMD should have ability for diagnosis of diseases as primary care and capability for diagnosis and treatment based on TKM theory, therefore careful study of basic theory of TKM is necessary.

근위축성 측삭경화증 환자의 한방치료에 대한 국내 연구 문헌 고찰 (A Review of Korean Studies on Amyotrophic Lateral Sclerosis Treated with Traditional Korean Medicine)

  • ■ Objectives This study aimed to investigate the domestic research trend about Amyotrophic Lateral Sclerosis(ALS) treated with Traditional Korean Medicine(TKM). ■ Methods Searches of two major TKM databases were conducted for articles published up to July 2015. Searching keyword was "Lou Gehrig's disease" OR "amyotrophic lateral sclerosis". Studies dealt TKM for ALS were included. ■ Results 18 studies were met our inclusion criteria(two prospective studies, one retrospective study, one follow-up study, 14 case reports). Complex TKM intervention of herbal medication, acupuncture and pharmacopuncture was the most frequently used treatment. Individually, herbal medication was the most frequent followed by acupuncture. Short-term assessment showed improvement in ALS symptoms but long-term assessment showed progression of disease. Accessory symptoms including depression, anxiety and insomnia were improved in four studies. ■ Conclusion For more definite evidence, larger studies with long-term follow-up period are needed. Moreover, benefits that TKM can provide for ALS patients as a part of multi-disciplinary approach should be studied in depth.
