Implant overdenture using a locator bar system by drill and tapping technique in a mandible edentulous patient: a case report

  • Various options have been introduced for the selection of implant overdenture attachments. Attachment wear due to the repeated insertion and removal of dentures has caused problems such as decreased retention and the requirement for suprastructure remanufacturing. In these cases, a Locator bar system was applied using the drill and tapping technique to achieve total retrievability. In a 55-year-old female patient who showed three degrees of mobility in most of her teeth due to severe alveolar bone loss, a complete denture in the maxilla and an implant supported type overdenture in the mandible were planned after extracting all the remaining teeth. Six implants were placed from canine region to the distal molar region, and the locator was connected to the milled bar using the drill and tapping technique. For a 61-year-old female edentulous patient who complained of poor retention with old denture, a complete denture in the maxilla and an implant-tissue supported type overdenture in the mandible were planned. Four implants were placed in front of mental foramen, and the Locator was also connected to the Hader bar using the drill and tapping technique. With this technique, female parts can be easily replaced, and retention can be continuously maintained.

Effects of Dietary L-Carnitine and Protein Level on Plasma Carnitine, Energy and Carnitine Balance, and Carnitine Biosynthesis of 20 kg Pigs

  • Growing pigs (N=25; 18 kg) were used to study effects of L-carnitine and protein intake on plasma carnitine, energy and carnitine balance, and carnitine biosynthesis. Corn-soybean meal basal diets containing low or high protein (13.6% or 18%) were formulated so that protein accretion would be limited by metabolizable energy (ME). Each basal diet was supplemented with 0 or 500 mg/kg L-carnitine and limit fed to pigs for 10 d in a balance trial. Final carnitine concentration was compared with weight/age matched pigs measured on d 0 to calculate carnitine retention rates. Supplementation of carnitine increased (p<0.01) plasma free carnitine (by 250%), short-chain (by 160%) and long-chain acyl-carnitine concentrations (by 80%) irrespective of blood sampling time (p<0.01). The proportion of long-chain carnitine esters decreased by 40% (p<0.01) by carnitine supplementation; whereas, the proportion of short-chain acyl-carnitine concentration was not changed (p>0.10). All criteria of energy balance were unaffected by L-carnitine (p>0.10). Total body carnitine retention was increased by 450% over unsupplemented controls (p<0.01). Carnitine biosynthesis rates in pigs fed diets without L-carnitine were estimated at 6.71 and $10.63{\mu}mol{\cdot}kg^{-1}{\cdot}d^{-1}$ in low protein and high protein groups, respectively. In supplemented pigs, L-carnitine absorption and degradation in the intestinal tract was estimated at 30-40% and 60-70% of L-carnitine intake, respectively. High protein feeding effect did not affected plasma carnitine concentrations, carnitine biosynthesis or carnitine retention (p>0.10). We conclude that endogenous carnitine biosynthesis may be adequate to maintain sufficient tissue levels during growth, but that supplemental dietary carnitine (at 500 ppm) sufficiently increased plasma acyl-carnitine and total body carnitine.

치조제 흡수가 심한 환자에서 중립대를 이용한 총의치 수복 증례 (Neutral zone approach for rehabilitation of severely atrophic maxillary and mandibular ridges: A case report)

  • 심하게 흡수된 치조제를 지닌 환자에게 적절한 의치를 제공하기 위한 한 가지 방법으로써 중립대 인상 기법을 이용한 의치의 제작을 고려해 볼 수 있다. 중립대 내에 인공 치아를 배열하는 것은 다음의 두 가지 목적을 이루고자 함이다. 첫째, 치아가 정상적인 근육의 기능을 방해하지 않도록 하기 위함이며, 둘째, 의치에 저항하여 가해지는 근육 조직의 힘이 안정과 유지에 더 유리하게 작용할 수 있도록 하기 위함이다. 본 증례의 환자는 심한 잔존 치조제의 흡수를 보이는 78세 여자 환자로 오랜 기간 불안정한 상 하악 총의치를 사용하고 있었다. 의치의 안정과 유지를 향상시키기 위하여 중립대 인상 기법을 통한 새로운 의치의 제작을 계획하였다. Modeling compound를 이용하여 중립대를 인기하였으며 치아 배열 후 external impression을 시행함으로써 정확한 연마면을 형성할 수 있었다. 완성된 상 하악 총의치는 보다 향상된 안정과 유지를 보였으며 환자 역시 새로 제작된 의치에 만족감을 보였기에 이를 보고하고자 한다.

  • 총의치 환자의 치료 시 인상 채득 과정은 의치의 지지, 유지, 그리고 안정을 위해 매우 중요하며, 의치의 성공여부를 좌우한다. 총의치 인상 채득을 위한 여러 가지 재료와 방법들이 이용되어 왔으며, 그 중 조직 양화재 (tissue conditioner)를 이용한 동적 인상법 (dynamic impression method)은 보다 생리적이고 기능적인 방법이다. 본 증례들에서는 무치악 환자에서 조직 양화재인 Coe-comfort$^{(R)}$ (GC America, IL, USA)를 이용한 동적 인상법으로 변형된 연조직의 회복과 하께 가장 생리적인 상태로 인상을 채득하여 의치의 제작 및 첨상을 시행하였으며, 만족스런 결과를 얻었다.

Staple과 수종의 봉합물의 봉합부위 창상치유 조직반응 (TISSUE HEALING RESPONSE OF INCISED WOUND SUTURED BY STAPLES AND VARIOUS SUTURE MATERIALS)

  • The purpose of this study was to observe the tissue response in applying staples and various suture materials to both scalp and buccal mucosa in rabbits. 18 rabbits were divided into 6 groups. The incised wounds of both scalp and buccal mucosa were sutured with staples, polyglactin 910, chromic catgut, mer silk and nylon. The experimental animals were sacrificed after 1, 3, 5, 7, 10, 14 days posto peratively 3 animals at one time. The tissue was stained with Hematoxylin and eosin, and Masson's Trichrom. In light microscopic examinations, the sutured sites were examined histologi cally according to 6 degrees about inflammation and collagen deposit. The results were obtained as follows, 1. The chromic catgut, an absorbable suture material, was absorbed by 7 days, whereas polyglactin 910 and mersilk began to get absorbed after 7 days. 2. Mersilk manifested a broad range of inflammation in the scalp, and both staple and nylon showed a severe inflammatory reaction in the buccal mucosa. 3. With polyglactin 910, both tissue samples showed only minor foreign body reaction, however in the scalp, the process of fibrosis took place compara tively slowly, whereas in the buccal mucosa, it occurred promptly and manifested active fibrosis by 7 days. 4. Mersilk showed widespread a matrix formation in both scalp and buccal mucosa, and showed the most severe inflammatory reaction by 3 days, which did not seem to decrease even after 7 days. 5. Both staple and nylon showed relatively a severe inflammatory reaction, however fibrosis took place rather promptly compared to the other groups. 6. Generally, in the buccal mucosa fibrosis occurred more promptly than in the scalp in both control and experimental groups. 7. Retention of the suture material and stability of the knot were the best with the staple, and better stability was manifested by the multi-stranded poly glactin 910 and mersilk than singlestranded chromic catgut and nylon. From above results, in the buccal mucosa absorbable suture materials especially polyglactin 910 showed better response in the aspect of inflammatory reaction, while in the scalp monofilament suture materials such as staple and nylon manifested a early fibrosis and collagen formation.

Factors influencing the bio-impedance data in tissue segments along the three arm meridians: a pilot study

  • Bioelectric impedance measurements have been reported to show significant variation between individuals. Different physiological conditions like thickened skin, obesity, and fluid retention can affect the impedance measurement. Therefore, it is important to learn what other factors can affect the measurements of impedance even in healthy individuals. Such information is a prerequisite for understanding the changes in impedance associated with acupuncture treatment. This study investigated the bio-impedance properties of tissue segments in the arms of a number of healthy subjects, so as to define the factors that might influence the variation of the bio-impedance data in acupuncture meridians studies. 51 healthy subjects were recruited through Liverpool Hospital, Sydney. Demographic data was collected from each subject including the age, sex, BMI, and time since most recent meal. Electrodes were applied to the forearms of each test subject. Measurements were done by a purpose-built Bio-Impedance Research Device (BIRD-I) which allowed the determination of core resistance (Rc) and core reactance (Xc) of each of the three meridian tissue segments on the anterior surface of the forearm. No significant difference was found in the core resistance attributable to age group, gender, BMI or meal intake. However, a statistically significant trend in increasing resistance from the radial to ulnar aspect of the forearm (p < 0.001) was found. No significant difference was found in the core resistance of test tissue segments among the 51 healthy subjects measured in this study. However, the trend of increasing core resistance from the radial to ulnar aspects of the arm deserves further investigation.

Effects of Energy Intake on Performance, Mobilization and Retention of Body Tissue, and Metabolic Parameters in Dairy Cows with Special Regard to Effects of Pre-partum Nutrition on Lactation - A Review -

  • The present review focuses on the effects of energy intake on performance, changes in body tissue during lactation, and metabolic parameters in dairy cows. Especially, pre-partum nutrition and its influence on lactation are emphasized. In recent decades the increase in genetic potential of dairy cows has increased milk yield. This fact sharpens the problem of a negative energy balance in early lactation because the amount of energy required for maintenance and milk production exceeds the amount of energy cows can consume. Around parturition, reduced feed intake reinforces the situation. Continuing negative energy balance causes decreasing milk yield, fertility problems, and incidence of metabolic diseases. Hence, the cow has to rely on body reserves that were stored in late lactation and the dry period. It is evident that the nutritional status pre-partum acts as the key factor for milk yield and fertility parameters in the following lactation. Cows overfed during the foregoing gestation and which have gained large quantities of body fat have lower dry matter intake along with the need to mobilize larger quantities of body reserves in lactation. The milk yield in the following lactation is lower than in cows fed according to their requirements. Cows restrictively fed in late gestation have a higher feed intake in lactation and a lower mobilization of body reserves. The effect of energy intake post-partum plays only a minor role for performance parameters in lactation. Lipid mobilized from body reserves makes a substantial contribution to the energetic cost of milk production in early lactation and adipose tissue undergoes specific metabolic alterations. Adipose tissue is degraded to free fatty acids, which are used in liver for energy purposes. High lipid mobilisation promotes the development of a fatty liver and therefore a reduced gluconeogenesis.

Effects of Dietary Selenium Supplementation on Growth Performance, Selenium Retention in Tissues and Nutrient Digestibility in Growing-finishing Pigs

  • This experiment was conducted to investigate the effects of selenium (Se) sources and levels on growth performance, nutrient digestibility and Se retention in growing-finishing pigs. A total of 56 crossbred pigs ([$Landrace{\times}Yorkshire$]${\times}$Large White) with average $28.5{\pm}0.2kg$ BW were allotted to 7 treatments on the basis of sex and weight in two replicates and four pigs per pen. A $2{\times}3$ factorial arrangement of treatments was used in a randomized complete block (RCB) design. Two sources of Se (selenite Se or Se-enriched yeast) were added at 0.1, 0.3 and 0.5 mg/kg to each treatment diet. A basal diet without Se supplementation was the seventh treatment group. Three pigs per treatment were randomly selected and samples of loin, liver, pancreas and a kidney were collected, frozen and later analyzed for Se. The digestibility trial was conducted to evaluate the apparent absorption and retention of Se and availability of other nutrients. Growth performance was not affected by dietary sources and levels of Se. No growth retardation was observed in the 0.5 mg/kg dietary Se treatment group regardless of Se sources. The Se concentration of serum in Se supplemented groups was increased compared with the control group (p<0.01). During the growing and finishing phase, Se in serum was clearly increased when organic Se was provided (p<0.01). Interaction of Se source ${\times}$ Se level was observed in Se concentration of loin, liver and pancreas of the pigs at the end of experiment. Selenium retention in the liver, kidney, pancreas and loin of pigs was increased as dietary Se level increased and was higher when pigs were fed organic Se resulting in an interaction response (p<0.01). Nutrient digestibilities were not affected by dietary Se sources or levels. No dietary Se source ${\times}$ Se level interaction was observed in nutrient digestibility. The results from this experiment indicated that dietary Se sources and levels affected the distribution of Se in the body of growing-finishing pigs. Organic source of Se, such as Se-enriched yeast resulted in higher serum and tissue Se concentration compared to inorganic form, while no beneficial effects on nutrient digestibility were observed from dietary Se supplementation in growing-finishing pigs.

Factors affecting smile esthetics in adults with different types of anterior overjet malocclusion

  • Objective: This study aimed to quantitatively assess the relationship of smile esthetic variables with various types of malocclusion, and identify the cephalometric factors affecting smile measurements. Methods: This retrospective study included 106 patients who were treated with retention at the orthodontic department of Taipei Medical University Hospital. Hard-tissue variables were measured using lateral cephalographic tracings, and nine smile esthetic variables were measured using facial photographs. The patients were divided into three groups according to their overjet (< 0, 0-4, and > 4 mm). An analysis of variance was conducted to compare the pretreatment cephalometric variables and smile esthetic variables among the three groups. Multiple linear regression analysis was performed to identify the cephalometric factors affecting the smile measurements in each group. Results: Except the upper midline and buccal corridor ratio, all of the smile measurements differed significantly among the three groups before orthodontic treatment. Some of the smile characteristics were correlated with the cephalometric measurements in different types of malocclusion. The overjet was the major factor influencing the smile pattern in all three types of malocclusion. Conclusions: Smile characteristics differ between different types of malocclusion; the smile may be influenced by skeletal pattern, dental procumbency, or facial type. These findings indicate that establishment of an optimal horizontal anterior teeth relationship is the key to improving the smile characteristics in different types of malocclusion.

흰쥐 육종 180에서 $^{169}Yb-Citrate$의 종양 친화성 (Accumulation of $^{l69}Yb$ Citrate in Tumor Bearing Mice)

  • Tumor localizing properties of $^{67}Ga$ citrate and $^{169}Yb$ citrate were studies with sarcoma 180 bearing mice. To compare precisely the accumulation of $^{67}Ga$ citrate and $^{169}Yb$ citrate in tumor tissue itself, $^{67}Ga$ and l63yb were administered simultaneously as a mixture of same chemical form of citrate. The yield of $^{169}Yb$ labeled citrated and the of radiopharmaceuticals purity was identified more than 90% by chromatography on silica gel plates. $^{67}Ga$ and $^{169}Yb$ were injected into mice and the mice were killed at 3, 24, and 48 hours following intraperitoneal injection of the radiopharmaceuticals. The retention value of $^{67}Ga$ and $^{169}Yb$ in tumor was similar but they differ from each other markedly in normal tissue distribution. $^{169}Yb$ citrate is cleared up from blood more rapidly than $^{67}Ga$ citrate, and a high tumor to blood ratio is achieved shortly after injection.
