A Clinical Report on Treatment of Two Soyangin Cases of Complications of Post-hysterectomy

  • Hysterectomy can lead to various symptoms threatening decent quality of life. As the frequency of hysterectomy increases, there are many patients who want to take oriental medicine treatments for complications. Several clinical studies have reported the effectiveness of oriental medicine treatment for post-hysterectomy complications. Out of these symptoms, postoperative nausea and vomiting (PONV, hereafter) are indicated at about 20-40% of frequency within patients with general anesthesia, and they may occur not only during recovery from operation, but also after discharge. Although the incidence of PONV has decreased now thanks to the usage of antiemetics, PONV still causes some difficulties for patients returning to daily life. This study reports two cases of post-hysterectomy complications such as nausea, dizziness, vomiting, general weakness, and coldness of the limbs, which had good responses to herbal medication and acupuncture.

Diffuse alveolar hemorrhage and recombinant factor VIIa treatment in pediatric patients

  • Diffuse alveolar hemorrhage (DAH) is a life-threatening pulmonary complication in patients with hematologic malignancies or autoimmune disorders. The current treatment options, which include corticosteroids, transfusions, extracorporeal membrane oxygenation (ECMO), and immunosuppressants, have been limited and largely unsuccessful. Recombinant activated factor VII (rFVIIa) has been successfully administered, either systemically or bronchoscopically, to adults for the treatment of DAH, but there are few data on its use in pediatric patients. The current literature in the PubMed database was reviewed to evaluate the efficacy and risk of rFVIIa treatment for DAH in pediatric patients. This review discusses the diagnosis and treatment of DAH, as well as a new treatment paradigm that includes rFVIIa. Additionally, the risks and benefits of off-label use of rFVIIa in pediatric patients are discussed.

  • Numerous evidence have been reported that fine particulate matters can play an important role in threatening human health. Recently concerns on fine particle pollution from various engines may require re-examination of particulate emission standards. The particles emitted by most diesel engines are mainly divided into their size ranges such as Dp< 50 nm and 50 nm< Dp< 1,000 nm. In this work, the number concentration and the size distribution of fine particles emitted from an exhaust manifold of a railroad diesel engine were measured under load test conditions using a scanning mobility particle sizer (SMPS). The fine particles observed were within the range of 7 to 304 nm under different load conditions with two different dilution ratios. The fine particles exhibited unique patterns showing bimodal shapes in size distribution.

근년의 한국 양계 전개 양상 (Recent Developments of Poultry Industry in Korea)

  • The poultry industry in Korean has expanded considerably during the last 15 years. While the number of chicken farms of commercial scale has decreased steadily during the period average farm size increased rapidly. Though the gross national product per capita increased 4.5 folds during ten years period, consumption of chicken meat, like other meats, recorded only 2 folds increase during the period. When domestic market for chicken meat had been open for two years on the basis of minimum market access quotas, importation remained at levels below the quota during the first one and half years. Nevertheless, during the last six months and thereafter, chicken meat importation recorded a steep increase, threatening the domestic industry. Poor productivities of the commercial chicken farms might be the main cause for the inferior competitiveness. For the sustainability of the industry some suggestions were made.

Insect GPCRs and TRP Channels: Putative Targets for Insect Repellents

  • Many insects such as mosquitoes cause life-threatening diseases such as malaria, yellow fever and West Nile virus. Malaria alone infects 500 million people annually and causes 1-3 million death per year. Volatile insect repellents, which are detected through the sense of smell, have long been used to protect humans against insect pests. Antifeed-ants are non-volatile aversive compounds that are detected through the sense of taste and prevent insects from feeding on plants. The molecular targets and signaling path-ways required for sensing insect repellents and antifeedants are poorly understood. Transient Receptor Potential (TRP) Ca2+-permeable cation channels exist in organisms ranging from C. elegans to D. melanogaster and Homo sapiens. Drosophila has 13 family members, which mainly function in sensory physiology such as vision, thermotaxis and chemotaxis. G protein-coupled receptors (GPCRs) initiate olfactory signaling cascades in mammals and in nematodes C.elegans. However, the mechanisms of G protein signaling cascades in insect chemosensation are controversial. In this review, I will discuss the putative roles of G protein-coupled receptors (GPCRs) and Transient Receptor Potential (TRP) channels as targets for insect repellents.

망상과 관련되어 발생한 정신분열병 환자의 Refeeding Syndrome 1예 (Refeeding Syndrome Associated with Delusion in a Schizophrenic Patient)

  • Refeeding syndrome is a life-threatening but treatable condition occurring in any patients with starvation. Electrolyte imbalance, especially hypophosphatemia and fluid shift result in metabolic changes in various systems. The authors reported a case of refeeding syndrome associated with prolonged starvation due to delusion and hallucination in a 38-year-old schizophrenia patient. Hypophosphatemia, hypokalemia, edema and elevation of liver enzyme was occurred despite careful treatment. The symptoms were improved after consistent electrolyte supplement. Close observation and repetitive monitoring are important to prevent refeeding syndrome.

  • The chairbound, handicapped person often requires a cushion to distribute the supportive forces over the largest area possible in order to reduce the risk of the development of a pressure sore. Pressure sores are areas of damaged skin caused by staying in one position for too long and can cause serious infections, some of which are life-threatening. When sitting upright, the greatest proportion of body weight is centerd over the ischial tuberosities. So, it is important that comfortable seating and largest distribution of pressure to prevent pressure sores. Therefore, the objective of this study was to develop the physical pressure measurement device and orthodontic chair to prevent pressure sores.

역행성 뇌혈 관류를 이용한 상행대동맥류 수술 -4례 보고- (Retrograde Cerebral Perfusion in the Surgical Treatment of Ascending Aortic Aneurysm -Report of 4 Cases-)

  • Dissecting aortic aneurysm of ascending aorta is a life threatening condition which requires prompt surgical correction. With deep hypothermic circulatory arrest and retrograde cerebral perfusion via superior vena cava, we could replaced ascending aorta in 4 cases safely. All of 4 cases; femoral artery, right auricle were used as cannulation site. The duration of circulatory arrest were 28, 30, 45, 60 minute in each cases and rectal temperature was 2$0^{\circ}C$ at that time. At the time of retrograde cerebral perfusion, we maintained central venous pressure under 25mmHg. We resected all of dissecting portion and replaced it with Hemashield graft. There were no deaths but two of four reoperated because of bleeding.

폐국균증에 대한 외과적치료;11례 보고 (Surgical Treatment of Pulmonary Aspergillosis; 11 Cases Report)

  • Aspergillosis is not only the problem increasing the freguency for recent decades but also a serious one. Coexistent pulmonary diseases are frequent and the most common antecedent disease is Tuberculosis with healed cavitary lesion. Interest of the surgeons is aroused because of it`s predilection to sudden life-threatening, exsanguinating pulmonary hemorrhage. 11 patients with pulmonary aspergillosis evaluated at this hospital in a 7-year period, and the results were as follows: 1.male to female ratio was 1:1.75. 2.The most common symptom was gross or microscopic hemoptysis[72.7%]and other symptoms were chest pain[9.1%] and chronic productive coughing[9.1%]. 3.The right upper lobe was most common involving site[81.8%] 4.Surgical interventions were 8 cases of lobectomy, 1 case of lobectomy with wedge resection, 2 cases of wedge resection. 5.Post-operative complications developed in 5 patients and were 4 cases of wound infections, 2 cases of empyemas, 1 case of bleeding, 1 case of atelectasis.

A Novel Mediastinal Drainage Tube for Mediastinitis

  • Mediastinitis is a life-threatening disease, and effective drainage is needed to treat mediastinitis with abscess formation. We recommend an alternative drainage method using chest tube binding with a Silastic Penrose drainage tube. The use of a Silastic Penrose drainage tube may help to manage mediastinitis with abscess formation. This method facilitates effective draining and prevents tissue adhesion.
