A Case Report on Pregnancy-Related Carpal Tunnel Syndrome Worsened after Birth by Korean Medicine (임신 중 발생하여 출산 후 악화된 수근관 증후군 한방 치험 1례)

  • Objectives : This study is to report the effect of Korean medical treatment on a patient suffered from carpal tunnel syndrome (CTS) occurred in pregnancy and worsened after birth. Methods : We treated the patient diagnosed as carpal tunnel syndrome by Korean medical treatment such as acupuncture, pharmacopuncture and herbal medicine at ⃝⃝Hospital of Korean Medicine from July 31th 2015 to August 6th 2015. And we evaluated changes of symptoms by Visual Analog Scale (VAS), Tinel sign, Phalen’s test, and Digital Infrared Thermographic Imaging (DITI). Results : 1. Through Korean medical treatment, postpartum carpal tunnel syndrome was significantly improved. VAS was decreased from 4 to 2, Phalen’s test was manifested from 10 seconds to 26 seconds and Tinel sign was changed from positive to negative. 2. We compared the gap of temperature between palm and forearm before and after Korean medical treatment through DITI. The temperature on the right side was changed from 0.74℃ to 1.01℃ and the left one from 0.63℃ to 1.01℃. Conclusions : This case report shows that Korean medical treatment is useful to treat pregnancy-related carpal tunnel syndrome worsened after birth.

Clinical Study of Bell's Palsy with DITI and Nerve Conduction Test(EN0G and EMG) (DITI 및 전기신경전도검사(EN0G 및 EMG)를 이용한 구안와사(Bell's palsy) 환자에 대한 임상적 고찰)

  • The clinical data and thermographic imaging were analyzed on the 38 cases of Bell's palsy who were treated admission in the Oriental Medicine Hospital of Wonkwang University from January 2002 to May 2003. 38 patients with Bell's palsy were within one week after the onset of the paralysis, and thermal type in the DITI were hypo or hyper generally. Nerve conduction test(ENOG and EMG) examined in two weeks after onset. We studied interaction effect between thermal type and paralysis grade on admission day. We studied each main effect ; paralysis grade on admission day - nerve conduction test, nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, paralysis grade on admission day - paralysis grade after 4 weeks, sasang constitution - nerve conduction test. The following results were obtained that interaction effect between thermal type and paralysis grade on admission day showed no significance, each main effect ; paralysis grade on admission day - nerve conduction test, paralysis grade on admission day - paralysis grade after 4 weeks, showed significance, each main effect ; nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, sasang constitution - nerve conduction test, showed no significance.

Use of thermography in lameness caused by musculoskeletal disease in Thoroughbred mare (적외선 체열촬영술을 이용한 thoroughbred 말의 근골격계 파행진단 증례)

  • The mare had lameness and asymmetry edematous on its tarsal and metatarsal joints at the initial physical examination. The pain was elicited with a palpation along the metatarsal articulations. No significant abnormalities were detected in the screening test. However, thermographic images revealed a significant increase in the surface temperature at the joint of the hindlimb when compared to the reference range. At necropsy, an irregularity of the surface and excessive synovial fluid were observed on the right tarsal joint. No bacterial growth was shown in the cultures of synovial fluid. Staphylococcus aureus was detected in the subcutaneous discharge. Taken together, the thermography images were very useful in localizing the area of injury and were an effective diagnostic methodology for assessing lameness.

The Effects of Different Moxibustion Stimulation at Abdominal Acupoints ($CV_{12}$, $CV_6$, $CV_4$) on the Skin Temperature Changes (복부 혈위 뜸 자극 위치의 차이가 체표 온도 변화에 미치는 영향)

  • Objectives : This study aimed to investigate the difference of abdominal skin temperature responses following moxibustion comparing stimulation method. Methods : Moxibustion was applied on the acupuncture points of $CV_4$, $CV_6$, $CV_{12}$. Thirty healthy men were randomly divided into two groups, one receiving a single moxibustion stimulation in three locations '$CV_4{\cdot}CV_6{\cdot}CV_{12}$'(n=15) and the other receiving triple moxibustion stimulations in one location '$CV_{12}$'(n=15) for 30 min. To obtain the skin temperature on abdominal region, a thermograph was used. Three arbitrary frames(the upper abdominal, lower abdominal, whole abdominal regions) were made to analyse skin temperature. Thermographic images were obtained at before and after the procedure of indirect moxibustion and 5, 10, 15, 20, 25, 30 min afterwards. Results : An increase in skin temperature on the three abdominal regions was observed following both one point and three points moxibustion administrations. Significant increase in skin temperature of the whole abdominal region was observed at 30 min after the procedure of three points moxibustion compared with one point moxibustion stimulation. A tendency of skin temperature changes over time was observed. Conclusions : In this study, skin temperature of lower abdominal region does not increase after triple moxibustion stimulations on $CV_{12}$. Administration of single moxibustion on $CV_4$, $CV_6$, $CV_{12}$ makes greater changes in skin temperature on the whole abdominal region than triple moxibustion on $CV_{12}$.

Effect of Acupressure Massage on Temperatures of Acupoints, Severity of Facial Paralysis, Subjective Symptoms, and Depression in Bell's Palsy Patients

  • Purpose: Bell's palsy is a nerve paralysis disease that causes functional impairments and affects psychological and aesthetical parts. This study aimed to examine whether acupressure massage had positive effects on facial paralysis, subjective symptoms, and depression in Bell's palsy patients. Methods: This study was conducted by a nonequivalent control group pretest-posttest design. Participants were 60 patients with Bell's palsy. 30 patients were assigned to the experimental group and the remaining 30 patients were assigned to the control group. The period of the study was from October 1, 2008 to July 30, 2009. Acupressure massage was offered to the experimental group for 20 minutes per day for two weeks (a total of six times). A SPSS/Win 12.0 program was used for data analysis. Results: A difference in Digital Infrared Thermographic Imaging (DITI) between affected and unaffected sides was less in the experimental group having acupressure massage than in the control group and the score of the recovery of facial paralysis was also increased in the experimental group. The Facial Nerve Grade Systems by Brackmann score that is a more objective index showed a significant difference between two groups (F=26.81, p<.001). Subjective symptom and depression scores were more decreased in the acupressure massage group than in the control group. Conclusion: Based on the results, it is considered that acupressure massage can be applied to Bell's palsy patients as an alternative therapy. It can be used as an evidence-based East-West nursing intervention to improve patients' physical and mental functions.

Infrared thermographic imaging in patients with alopecia. (탈모 환자의 적외선 체열 진단상 안면부 체열 특성)

  • Objectives: To examine any potential relationships between the types of alopecia and the facial heat distribution in patients with alopecia Methods: 183 patients with alopecia participated in this study to provide facial heat distribution measured by the Infrared Thermography Scanner (ITS, Nec San-ei Instruments Ltd, Japan). The thermography scan was used in a light-and heat-protected room after 20 minutes’of resting period. 1.5m of distance maintained between the patients and ITS. Results: Specificity in the type of facial heat distribution was found as follow. 1. Types of facial heat distribution can be classified as T-type and diffused patterns. 2. There was a significant difference in the pattern of facial heat distribution among different types of alopecia (p=0.002): facial heat distribution appeared T-type in Androgenic Alopecia, Alopecia Areata, and Telogen Effluvium (71.3%, 85.7%, 70.4%), whereas diffused pattern was dominant in Seborrheic Alopecia(55.6%). 3. There was a significant difference in the pattern of facialheat distribution between men and women (p<0.001): While the T-type and diffused type were equally appeared in men (50.6%: 49.4%), T-type was dominant in women (88.0% vs. 12.0%). Conclusions: We conclude that the pattern of facial heat distribution differs depends on the types of alopecia and gender. These differences may provide useful information for the diagnosis and clinical therapy for this population.

Effect of Acupuncture and High Frequency Therapy Applied to the Region Branching to the External Carotid Artery on Reduction of Facial Edema in Patients with Sequelae of Peripheral Facial Palsy: A Case Report (말초성 안면마비 후유증 환자에서 침 치료와 바깥목동맥으로의 분지 영역에 시행한 고주파 병행 치료의 안면부종 감소 효과: 증례보고)

  • This study was performed to evaluate the effect of high frequency therapy applied to the region branching to the external carotid artery for peripheral facial paralysis sequelae. A patient suffering with facial edema due to facial paralysis sequelae had been treated with acupuncture, high frequency therapy on the branch area to the external carotid artery for 7 weeks. The evaluation of clinical outcome was done by degree of swelling by measuring the distance of the face and skin temperature of face through digital infrared thermographic imaging. After treatment, the patient's degree of swelling and the temperature difference between the affected side and normal side was decreased. In addition, the temperature was changed in the entire facial area as well as the treatment point of high frequency therapy. This result shows that acupuncture combined with high frequency therapy at the region branching to the external carotid artery could be an effective way to improve facial blood flow, although further clinical studies will be needed.

  • Objectives : The purpose of this study is to investigate the effect of acupuncture treatment on a patient suffering from CRPS Type 1. Methods : A 30-year-old female patient suffering from CRPS Type 1 with symptoms of intense burning pain, joint stiffness, restricted mobility, rapid hair and nail growth, allodynia, hyperalgesia and coldness of the right leg was treated with acupuncture on six acupuncture points on the leg from 13 July to 18 July of 2011. Improvement of the patient's symptoms was evaluated by Numerical Rating Scale(NRS), Digital Infrared Thermographic Image(DITI). Results : After 6 days of treatment, NRS score decreased significantly and the DITI results showed moderate improvement in temperature. Conclusions : The results suggest that acupuncture may be an effective in reducing the symptoms of CRPS. Further study is needed to evaluate the significancy of this report.

Changes in blood flow at the mandibular angle and Horner syndrome in a rat model of superior cervical ganglion block

  • Background: A stellate ganglion block (SGB) causes increased blood flow in the maxillofacial region, exhibiting the potential for regenerative effects in damaged tissue. The focus of this study was to understand the efficacy of SGB for regenerative effects against nerve damage. A rat model of the superior cervical ganglion block (SCGB) was created instead of SGB, and facial blood flow, as well as sympathetic nervous system function, were measured. Methods: A vertical incision was made on the left side of the neck of a Wistar rat, and a 5-mm resection of the superior cervical ganglion was performed at the back of the bifurcation of the internal and external branches of the left common carotid artery. Blood flow in the skin at the mandibular angle and mean facial temperature were measured using a laser-Doppler blood flow meter and a thermographic camera, respectively, over a 5-week period after the block. In addition, the degree of ptosis and miosis were assessed over a period of 6 months. Results: The SCGB rat showed significantly higher blood flow at the mandibular angle on the block side (P < 0.05) for 3 weeks, and significantly higher skin temperature (P < 0.05) for 1 week after the block. In the SCGB rat, ptosis and miosis occurred immediately after the block, and persisted even 6 months later. Conclusions: SCGB in rats can cause an increase in the blood flow that persists over 3 weeks.
