Purpose: For hypertrophic scars and keloids no universally effective treatment modality exists. Surgical revision, intralesional steroid injection, silicone gel sheeting, pressure, laser, and others have been used with variable success, but many treatments are associated with high recurrence rates. Although optimal treatment remains undefined, successful treatment can be obtained through a combined therapeutic approach. Methods: We used three therapeutic modalities in combination, which are intralesional injection of triamcinolone acetonide, silicone gel sheeting, and 585 nm flashlamp-pumped pulsed dye laser. Fifty-eight cases of hypertrophic or keloid scar were treated by combined therapeutic regimen for mean period of 18 months. The changes of thickness, color, and pliability of scars were evaluated with clinical photographs by grading scale. Results: As summing the grades and categorizing the result into three group, we obtained 28% good, 67% fair, and 5% poor results. There was a desirable improvement of scars with insignificant adverse effects. Conclusion: Combination of intralesional steroid injection, silicone gel sheeting, and pulsed dye laser can lead to successful treatment of hypertrophic scar and keloid.
This study was performed to assess that clinco-therapeutic effect of natural Italian honeybee (Apis mellifera) venom in adjuvant-induced arthritic rat. Ninety Sprague- Dawley rats of male were injected with complete Freund's adjuvant (CFA). Adjuvant arthritis was produced by a single subcutaneous injection of 1 mg Mycobacterium butyricum suspended in 0.1 ml paraffin oil into the right hindpaw. Righting reflex was uniformly lost and considered to be the point of arthritis development on day 14 after CFA injection. Experimental groups were divided into three groups. When arthritis was developed in the rat hind-paw, tested groups were administrated with prednisolone (10 mg/kg, p.o) and honeybee venom (one bee, s.c) at an interval of two days. Control group was subcutaneously injected with 0.1 ml of physiological saline solution in the rat at an interval of two days. Clinical findings, hematological values and histopathological findings were observed during or after the drugs administration. In tested groups, the development of inflammatory edema and polyarthritis on day 14 after treatment was suppressed. No significant differences of hindpaw edema volume and lameness score between prednisolone and honeybee venom groups were observed during or after therapeutic drugs treatment. WBC counts of prednisolone and honeybee venom treatment groups as compared with the control group were getting remarkably decreased during or after the therapeutic drugs administration(p<0.01). Erosions of articular cartilage and inflammatory cell infiltrations during or after the therapeutic drugs treatment was effectively suppressed in natural honey venom.
A needle-free injector is one of the new non-invasive players in impregnating the biological barriers. It is considered as the next phase in drug delivery and therapeutic applications. One of the major fields of application is in skin remodeling procedures. Although many studies were carried out in understanding the principle in the needle-free injection procedure, fewer studies were done with the aim of therapeutic applications. In the present study, we tried to identify key parameters that affect the jet divergence and peak stagnation pressure on the skin surface in a conventional needle-free injector for skin treatment. A summary of the working principle and effect of the key parameters are presented.
Genome engineering with clustered regularly interspaced short palindromic repeats (CRISPR) system can be used as a tool to correct pathological mutations or modulate gene expression levels associated with pathogenesis of human diseases. Owing to well-established local administration methods including intravitreal and subretinal injection, it is relatively easy to administer therapeutic genome engineering machinery to ocular tissues for treating retinal diseases. In this context, we have investigated the potential of in vivo genome engineering as a therapeutic approach in the form of ribonucleoprotein or CRISPR packaged in viral vectors. Major issues in therapeutic application of genome engineering include specificity and efficacy according to types of CRISPR system. In addition to previous platforms based on ribonucleoprotein and CRISPR-associated protein 9 derived from Campylobacter jejuni, we evaluated the therapeutic effects of a CRISPR RNA-guided endonuclease derived from Lachnospiraceae bacterium ND2006 (LbCpf1) in regulating pathological angiogenesis in an animal model of wet-type age-related macular degeneration. LbCpf1 targeting Vegfa or Hif1a effectively disrupted the expression of genes in ocular tissues, resulting in suppression of choroidal neovascularization. It was also notable that there were no significant off-target effects in vivo.
Neutropenia is a major dose-limiting side effect of cancer chemotherapy. The therapeutic effect of HM 10411 was examined on neutropenia caused by anticancer agents. Neutropenia in normal ICR mice was induced by a single combined intraperitoneal injection of 130 mg/kg of cyclophosphamide (CPA). 4.5 mg/kg of doxorubicin (DXR). and 1 mg/kg of vincristine (VCR) on day O. Neutropenia in tumor-bearing mice was made by a single intraperitoneal injection of 200 mg/kg of cyclophosphamide (CPA) into BALB/c mice bearing Colon 26 adenocarcinoma at 7 day after tumor implantation. HM 10411 or filgrastim (100 $\mu\textrm{g}$/kg/day) was subcutaneously administered for 5 consecutive days starting 1 day after injection of anticancer agents in order to stimulate neutrophil production. Injection of HM 10411 accelerated the recovery from these anticancer drug-induced neutropenia. In normal and tumor-bearing mice. neutrophil production efficacy of HM 10411 was similar than that of filgrastim. These results suggest that HM 10411 could be useful in the clinical treatment for neutropenia induced by anticancer agents.
Piriformis muscle syndrome is a condition that causes direct muscle pain around piriformis muscle or sciatica from irritated sciatic nerve and the diagnosis remains debatable. The main treatment is symptomatic relief from conservative therapy such as medication and piriformis stretching exercise, and various therapeutic injections including local anesthetic, corticosteroid, botulinum toxin can be considered for diagnostic and therapeutic purposes. In this case, a 54-year-old male who had sciatica and gait disturbance showed piriformis muscle hypertrophy in the pelvis MRI. From imaging studies, electrodiagnostic study and physical examination, he was diagnosed with piriformis muscle syndrome. He underwent trigger point injection and botulinum toxin injection into the piriformis muscle, and pain and gait disturbance significantly improved. This case reports a case of piriformis muscle syndrome with clinical symptom of gait disturbance, which was improved by botulinum toxin injection.
The organ distribution study and the whole body scan were done in the albino rats at intervals of 5, 30, 60, 90 and 120 minutes after the intravenous injection of $^{99m}TC$-pertechnetate absorbed in Sephadex beads of $20{\sim}80$ micra in diameter. Effect of additional injection of physiological saline and saline absorbed in Sephadex beads of $20{\sim}80$ micra in diameter on the scan and organ distribution were also studied. The results were as follows. 1. Five minutes after the injection of $^{99m}TC$-pertechnetate absorbed in Sephadex beads of $20{\sim}80$ micra in diameter, Sephadex was well trapped in the lungs, with which the excellent lung scan was obtained. Two hours after the injection, kidneys were well visualized instead of lungs, which suggested that kidney acts as the excretory organ. Five minutes prior to scan, additional injection of physiological saline absorbed in Sephadex above mentioned was done. The bladder was also well visualized together with the kidneys. 2. In the distribution studies, most of radioactivity was detected in the lungs at 5 minutes and was gradually transferred chiefly to the kidneys and bladder and partly to the liver. 3. Additional injection of physiological saline resulted in a rapid transfer of $^{99m}TC$ trapped in the lung to both the kidneys and liver. 4. Additional injection of physiological saline absorbed in Sephadex beads of $20{\sim}80$ micra in diameter resulted in a rapid transfer of $^{99m}TC$ trapped in the lung to only the urinary system. 5. Results of these studies suggested that; a) Other nutrients and therapeutic compounds may be carried into the lungs along with Sephadex beads and then released in high concentration, which would exert greater therapeutic effect locally than that of the usual administration. b) Some radionuclides absorbed in Sephadex could be used as the lung scan agents, the flushing out of which by Sephadex-saline also give satisfactory renal and bladder scans. c) Other potent therapeutic radionuclides could be retained for some time by this method, which can be in the lungs easily flushed out within 2 hours.
Estrus was induced and the therapeutic effect was examined with 25mg of PGF2$\alpha$ intramuscular injection and 3mg of PGF2$\alpha$ intraovarian injection to Holstein and Simmental cows which were raised in the large size stockfarms and the dairy farms, and were diagnosed to anestrus. The results obtained were summarized as follows: 1. The estrus inducing effect observed in the cows treated by PGF2$\alpha$ was 83.3% with 25/30 heads in the intramuscular injection group, and 86.7% with 26/30 heads in the intraovarian injection group. 2. Average duration from PGF2$\alpha$ administration to the onest of estrus was 2.7 day in the intramuscular injection group, and 2.6 days in the intraovarian injection group. 3. In status of estrus in the cows treated by PGF2$\alpha$, vigorous estrus was 12.0%, normal estrus 60.0% and silent estrus was 28.0% in the intramuscular injection group, and vigorous estrus was 15.4%, normal estrus 61.5%, and silent estrus 23.1% in the intraovarian injection group. 4. Conception rate in the cows induced to estrus was 64.0% in the intramuscular injection group and 65.4% in the intraovarian injection group.
It has been demonstrated that the injection of naked DNA expressing vascular endothelial growth factor 165(VEGFl165) to the affected area can provide significant therapeutic effects on peripherol artery occlusive diseases. Success with this type of gene therapy highly depends on the quality of the vector delivering the therapeutic gene, especially in terms of the level and duration of gene expression in the localized area. We have recently developed a vector expressing VEGF165(pCK-VEGF) for the treatment of peripheral artery occulsive diseases and demonstrated high level expression of VEGF165 in mouse skeletal muscle. This study was designed to assess the acute toxicity of intramuscularly injected pCK-VEGF in BALB/c mice and Sprague-Dawely rats. There was no evidence of any changes in clinical signs, body weights, or gross pathological signs. We estimate LD50 values of pCK-VEGF higher than 50mg/kg in mice and 20 mg/kg in rats by intramuscular injection.
Rheumatoid arthritis (RA) is o chronic inflammatory disease of joints with proliferation of synovial epithelial tissue. Therapeutic approach of the RA consists of pharmacological and surgical interventions. Synovectomy is indicated in patients with progressive inflammatory signs and symptoms intractable to medical treatment including local intracavitary steroid injection. Recently, local injection of radionuclides which emit high energy beta rays are labeled with chemical compounds such as $^{90}Y,\;^{165}Dy-ferric$ hydroxide macroaggregate and have been introduced as an alternative therapeutic modality to surgical synovectomy. Holmium-166 is one of beta emitter and Ho-166-chitosan complex was developed for radiation synovectomy. Preclinical trial is on-going at our hospital using Ho-166-chitosan. The procedure and methods of preclinical trial are discussed.
