  • Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation due to chronic airway inflammation and destruction of the alveolar structure from persistent exposure to oxidative stress. The body has various antioxidant mechanisms for efficiently coping with such oxidative stress. The nuclear factor erythroid 2-related factor 2 (Nrf2)-antioxidant response element (ARE) is a representative system. Dysregulation of the Nrf2-ARE pathway is responsible for the development and promotion of COPD. Furthermore, COPD severity is also closely related to this pathway. There has been a clinical impetus to use Nrf2 for diagnostic and therapeutic purposes. Therefore, in this work, we systematically reviewed the clinical significance of Nrf2 in COPD patients, and discuss the value of Nrf2 as a potential COPD biomarker.

Anti-Tumor Necrosis Factor Therapy in Intestinal Behçet's Disease

  • Intestinal Behçet's disease is a rare, immune-mediated chronic intestinal inflammatory disease; therefore, clinical trials to optimize the management and treatment of patients are scarce. Moreover, intestinal Behçet's disease is difficult to treat and often requires surgery because of the failure of conventional medical treatment. Administration of anti-tumor necrosis factor-${\alpha}$, a potential therapeutic strategy, is currently under active clinical investigation, and evidence of its effectiveness for both intestinal Behçet's disease and inflammatory bowel diseases has been accumulating. Here, we review updated data on current experiences and outcomes after the administration of anti-tumor necrosis factor-${\alpha}$ for the treatment of intestinal Behçet's disease. In addition to infliximab and adalimumab, which are the most commonly used agents, we describe agents such as golimumab, etanercept, and certolizumab pegol, which have recently been shown to be effective in refractory intestinal Behçet's disease. This review also discusses safety issues associated with anti-tumor necrosis factor-${\alpha}$, including vulnerability to infections and malignancy.

  • 혈관이 좁아지거나 막혀서 생기는 허혈은 심장, 뇌, 다리와 같은 인체의 여러 장기에 영향을 미친다. 최근 혈관신생 분야에서 많은 진전이 있어 기존 치료법으로 치료가 되지 않는 허혈성 환자들의 치료 가능성에 대한 기대가 많아졌다. 혈관형성은 여러 개의 인자들과 세포들이 관여하는 복잡한 과정이기 때문에, 한 개의 인자보다는 여러 인자들을 병합하는 요법이 점점 많이 시도되고 있는데, 이런 병합요법의 한 예로 전사인자를 전달하는 전략을 생각할 수 있다. 이에 본 연구에서는 cardiac ankyrin repeat protein (CARP)의 유전자를 대상으로 그 혈관신생 능력을 혈관내피세포에서 조사하였다. 아데노바이러스 벡터 내에 human CARP의 cDNA를 클로닝하여 재조합 아데노바이러스를 제조하였으며, 이를 이용한 유전자 전달실험 결과, CARP 유전자 전달 군에서 유의하게 혈관내피세포의 중식과 모세관 구조 형성, 그리고 vascular endothelial growth factor의 발현 등을 증가시킴을 확인하였다. 본 연구 결과는 CARP가 혈관신생 연구의 새로운 목표 유전자로서 그 기능에 대한 많은 연구가 필요함을 뒷받침해준다.

방사선 치료를 받는 유방암 환자의 가족지지와 성격이 삶의 질에 미치는 영향 (A Study on the relationship of family support and personality with quality of life in the Patients receiving radiotherapy for breast cancer)

  • This descriptive correlation study was undertaken in order to investigate the relationship of family support and personality with quality of life in patients receiving radiotherapy for breast cancer and to provide basic data to help them improve a better quality of life. This subjects for this study were 74 out-patients undergoing a radiation therapy at C hospital in Kwang-Ju. The data were obtained using a convenience sampling technique. The tool of this study were Ro's qualify of life scale, Kang's family support scale and Wallston & others health locus of control scale. The data were analyzed using a SAS program for percentage, mean, standard deviation, Pearson Correlation Coefficient, GLM. The results were as fellows: 1. The total average score of the quality of life of the subjects was 137.22 (minimum score 38-maximum score 227), item mean score(range 1-5) was 3.15. The total average score of the family support of the subjects was 40.38(minimum score 21-maximum score 47), item mean score(range 1-5) was 3.69. The total average score of health locus of control of the subjects was 42.47(minimum score 28-maximum score 59), item mean score(range 1-6 was 3.69. 2. The results of the analysis of the relationship between the quality of life scores and the health locus of control were as follows : the total average score of the qualify of life of internal locus of control scale was 133.50, he total average score of the quality of life of external locus of control scale was 138.41. 3. There was positive correlation between the health locus of control and the quality of life(r=0.0722, p=0.5413). 4. There were significant positive correlation between family support and quality of life(r=0.2328, p=0.0399). The results of the analysis of the relationship between the each factor in the quality of life scores and family support were as fellows : There were significant difference between the self esteem factor(r=0.2974, p=0.0124), relationship with family factor(r=0.2657, p=0.0241)

미술치료에서 치료적 동맹과 전문적 역량에 관한 미술치료사의 인식 연구 (Study on the Perception of Art Therapists on the Therapeutic Alliance and Professional competencies in Art Therapy)

  • 본 연구는 미술치료사들의 실제 임상 경험을 통해 치료적 동맹을 촉진하는 미술치료사의 개입 과정 및 전문적 역량 요소들을 탐색하여 미술치료 교육과 미술치료사 수련에 도움이 되는 자료를 제공하려는 목적으로 실시되었다. 이를 위해 9명의 미술치료사를 대상으로 심층 면담을 실시하였고, 자료 분석은 합의적 질적 분석(CQR)을 사용하여 5개 영역과 41개 범주로 정리되었다. 연구 결과, 치료적 동맹 촉진을 위한 미술치료사의 개입 과정은 치료적 동맹을 위해 기본적으로 고려할 사항과 치료적 동맹을 구성하는 세 가지 차원을 토대로 안전한 환경으로 구조화하는 것을 의미하며, 미술치료사의 전문적인 역량은 치료적 동맹을 위한 미술치료사의 개입 요인들과 상호작용하여 치료적 동맹 효과에 직·간접적으로 영향을 미치는 것으로 나타났다. 따라서 미술치료사는 치료적 동맹을 촉진하기 위해 임상적인 문제를 가진 독특한 개별 사례를 개념화할 수 있는 능력, 풍부한 임상경험, 미술을 치료적으로 활용할 수 있는 능력, 치료적 대상으로 존재하기 위한 노력 등과 관련된 전문적인 역량 강화가 필요하다고 본다. 본 연구는 미술치료에서 치료적 동맹을 촉진시키기 위한 구조화와 미술치료사의 전문성 강화에 필요한 발달과제들을 도출하여 미술치료사의 교육과 훈련에 활용될 자료를 제시했다는데 의의가 있다.

Eupatilin treatment inhibits transforming growth factor beta-induced endometrial fibrosis in vitro

  • Objective: Endometrial fibrosis, the primary pathological feature of intrauterine adhesion, may lead to disruption of endometrial tissue structure, menstrual abnormalities, infertility, and recurrent pregnancy loss. At present, no ideal therapeutic strategy exists for this fibrotic disease. Eupatilin, a major pharmacologically active flavone from Artemisia, has been previously reported to act as a potent inducer of dedifferentiation of fibrotic tissue in the liver and lung. However, the effects of eupatilin on endometrial fibrosis have not yet been investigated. In this study, we present the first report on the impact of eupatilin treatment on transforming growth factor beta (TGF-β)-induced endometrial fibrosis. Methods: The efficacy of eupatilin on TGF-β-induced endometrial fibrosis was assessed by examining changes in morphology and the expression levels of fibrosis markers using immunofluorescence staining and quantitative real-time reverse-transcription polymerase chain reaction. Results: Eupatilin treatment significantly reduced the fibrotic activity of TGF-β-induced endometrial fibrosis in Ishikawa cells, which displayed more circular shapes and formed more colonies. Additionally, the effects of eupatilin on fibrotic markers including alpha-smooth muscle actin, hypoxia-inducible factor 1 alpha, collagen type I alpha 1 chain, and matrix metalloproteinase-2, were evaluated in TGF-β-induced endometrial fibrosis. The expression of these markers was highly upregulated by TGF-β pretreatment and recovered to the levels of control cells in response to eupatilin treatment. Conclusion: Our findings suggest that suppression of TGF-β-induced signaling by eupatilin might be an effective therapeutic strategy for the treatment of endometrial fibrosis.

Effect of PUVA on Nerve Growth Factor Expression in Cultured Keratinocytes

  • Nerve growth factor (NGF) is an important autocrine growth factor and also a survival factor for keratinocytes. NGF may act in the hyperproliferative condition, psoriasis. Clinically, the combination of psoralen and UVA (PUVA) has been used in the treatment of a wide variety of cutaneous disorders, such as psoriasis and vitiligo. However, the precise therapeutic mechanism of PUVA on the dermatologic diseases remains unclear. The purpose of this study was to examine whether the expression of NGF in cultured keratinocytes is influenced by PUVA. Thus, normal human keratinocytes were isolated from neonatal foreskin, and the third to fifth-passaged cells were used in this study. The cells were exposed to various doses of UVA (30, 60, 120 $mJ/cm^2)$ after adding 8-methoxypsoralen (8-MOP) to examine the expression of NGF mRNA. The RNA and protein of the cells were extracted at various time points (1, 8, 24 hours) after UVA irradiation to examine the expression of NGF mRNA and production of NGF protein. In keratinocytes, there were no differences in the expression of NGF mRNA between the different doses of UVA irradiation, however, the expression of NGF mRNA in UVA and PUVA groups tended to increase as the time increased. The expression of NGF mRNA was the highest in PUVA group, followed by UVA group and the lowest in 8-MOP group. The expressions of NGF protein at 1 and 8 hours after UVA irradiation were lower in the PUVA group than in the other groups. This study showed that the expression level of NGF protein in keratinocytes was relatively lower in the PUVA groups than in the other groups, suggesting that the therapeutic mechanism of PUVA in psoriasis is related to the decrease of NGF protein.

전자선의 선량분포에 있어서 Field Shaping의 효과에 관한 연구 (A Study on the Effect of Field Shaping on Dose Distribution of Electron Beams)

  • 전자선 치료시에 납판이나 저융점 합금이 조사면의 형태의 변형을 위하여 사용되고 있다. 콜리메이팅 장치와 마찬가지로 조사면 변형을 위한 물질도 전자선의 출력에 영향을 미친다. 저자들은 폴리스티렌 팬톰에 삽입된 Farmer형 전리함을 이용하여 Clinac-18의 전자선의 변형된 조사면에 대한 출력율을 측정하고 출력율에 영향을 미치는 요인들에 관해 분석했다. 전자선의 출력율은 전자선의 초기 에너지, 콜리메이팅 장치뿐만 아니라 조사면 크기에도 좌우되었다. 변형된 조사면에 대하여 잔자선의 에너지에 관계없이 X-선 콜리메이터와 전자선 어플리케이터의 조합이 고정되면 조사면의 크기 대 출력율은 A/P로 표시된 등가조사면의 크기에 따라 변하지만 조사면의 형태에 대해서는 무시할 수 있었다. 그러나 개조사면에 대한 출력율은 변형조사면의 출력율로부터 예상될 수 없고, 그것들만의 독립적인 관계를 가지고 있었다. 어플리케이터와 콜리메이터의 조합이 고정된 경우에 한해서 판자선의 변형조사면에 대한 출력율은 A/P로서 표시된 등가 조사면 방법에 의하여 구할 수 있다.

플라스틱 세포치료제 보관용기의 극저온 환경 노출 시 구조적 변화에 대한 해석적 비교 (Analytical comparison of structural changes of plastic cell-based therapeutic drug storage containers when exposed to cryogenic environments)

  • Recently, research and commercialization related to the field of cell-based therapeutic drug development has been actively conducted. In order to maintain cell viability and prevent contamination, refrigeration preservation devices, such as CRF (controlled rate freezer) or vapor type LN2 tanks have been developed. On the other hand, the storage container for liquid nitrogen tanks currently on sale minimizes the flow structure to prevent structural defects when stored in a liquid nitrogen tank having a high thermal conductivity than vapor nitrogen. If the cell-based treatment drug is stored in the gaseous LN2 tank as it is, the cell survival after thawing is greatly reduced. It was estimated that the existing storage container structure was a factor that prevented the rapid entry and circulation of gaseous nitrogen into the container. Therefore, this study intends to propose a new supercellular storage container model that can maintain the mechanical strength while maximizing the fluid flow structure. To this end, we estimated that the structural change of the storage container effects on the equivalent stress formed around the through-holes of them when exposed to a cryogenic environment using thermal-structural coupled field analysis. As a result of storage experiments in the gas phase tank of the cell-based therapeutic agent using the developed storage container, it was confirmed that the cell growth rate was improved from 66% to 77%, which satisfied the transportation standards of the FDA(Food and Drug Administration) cell-based therapeutic agent.

불안장애 환자에서의 치료약제 중복사용에 대한 후향적 의약품사용평가 연구 (Retrospective Drug Utilization Review Study on the Therapeutic Duplication in Patients with Anxiety Disorders)

  • The present study was aimed to examine the occurrence and influencing factors therapeutic duplication (TD) of medications for anxiety disorders by analyzing the relevant prescription data. In this study, the prescription data issued on March 19, 2008 in domestic medical institutes were utilized. TD was defined as more than two medications under the same therapeutic classification per prescription based on the Anatomical Therapeutic Classification (ATC) code. The assessment of TD was performed based on the number of cases and on the ratio determined. To identify the influencing factor of TD, the variables related to the differences in the TD ratio were analyzed based on the results of the Chi-Square test conducted with the variables; patients, medical institutes, diseases, and treatments. The number and ratio of TD were determined to be 1,333 out of the total of 19,219 anxiety disorder cases, and 6.94%, most cases involving benzodiazepine derivatives, respectively. The TD ratio was found to be higher in relation to males than to females. Patients with national health insurance benefits have a higher TD ratio compared to the medical-aid beneficiaries. The TD ratios were highest in clinics, psychiatry divisions, and Gyeongsang district. The TD ratio of the cases with more than two anxiety disorders was found to be higher than that of the cases with only one anxiety disorder. As the number of medications per prescription increased, the TD ratio was shown to have become gradually higher. In conclusion, in order to prevent TD, the concurrent DUR system should be implemented. The prescribers and pharmacists must be educated regarding duplicated medications to promote the safe and effective use of medicines, without unnecessary TD.
