Among U-healthcare services adapting the latest IT technique and medical technique, a body-injecting device technique providing medical service to a patient who has incurable disease. But the body-injecting device technique can be easily exposed during wireless section to the third person and it can be used illegally. This paper proposes certification protocol which certifies a patient and hospital staff using random number created by certification server and a patient with hospital staff by synchronization. Specially, the proposed protocol uses security information created by information registered in certification server previously by a patient and hospital staff so that in keeps from accessing of third person who didn't get approval. And it gives more stability.
This study is to survey the materials purchasing and inventory management status and the characteristics and opinions of the staff in charge of purchasing and inventory of the general hospitals in Busan area in order to contribute to the rationalization of its management through the grasp of actual situation and the presentation of desirable improvement plan for the materials purchasing and inventory management. The status of medical institute had been surveyed by the purchasing/ administration managers of total 26 general hospitals, and the purchasing/ management questionnaires had been commenced with 86 staff of the 26 hospitals. Its major survey results, after the analysis of 24 medical institute statuses (return rate of 92.30%) and 60 staff questionnaires (return rate of 69.76%), are as follows. First, post-purchasing evaluation system is not used actively, orders are being placed by phone or fax, and general merchandise is being purchased through free contracts in most of the hospitals participated in the survey. Second, as per the materials supplying methods, the requisition and delivery system is currently the most popular in the hospitals surveyed, however, both the requisition and delivery system and the par level transfer system are the most desired in the hospitals of more than 500 beds, and the par level transfer system is the most desired in the hospitals under 500 beds for the materials supplying system in the future. Third, as per the inventory management system that is desired the most in the future, the SPD and JIT types are preferred in the hospitals of more than 500 beds, the stockless strategy is preferred in the hospitals under 500 beds, the senior staff above section chief grade prefer the stockless strategy, and the junior staff prefer the ABC classification and SPD types. Fourth, The necessity of purchasing staff's training for the materials management is highly recognized but the effectiveness is not so much acknowledged, which is because such a training is thought to be so superficial and formal that it is not helpful much in the actual field. When summarizing the survey results as above, the materials purchasing and management system is differed for each group of hospitals according to the size of beds, and the more scientific management system is largely required by the general hospitals in Busan city. They also hope the introduction of joint purchasing system, materials management by the bar-code system, and positive execution of the market survey and training of the relevant staff for the management of purchasing affairs. So the more systematic purchasing and inventory management is regarded to be necessary through the introduction of scientific and specialized education of materials management, market survey, and post-purchasing evaluation system also through the computerization of materials purchasing and inventory management as soon as possible.
Objectives : This study was to determine how the perception and the satisfaction of outpatients who utilized clinics and hospitals are structurally related with their willingness to utilize the same institution in the future. Methods : Three hundred and ten responses (via convenient sampling) were collected from 5 hospitals and 20 clinics located in Seoul listed in the "Korea National Hospital Directory 2005". Service quality was utilized as the satisfaction measurement tool. For analysis, we used a structural equation modeling method. Results : The determining factors for general satisfaction with medical services are as follows: medical staff, reasonability of payment, comfort and accessibility. Such results may involve increased competition in the medical market and increased demands for quality medical services, which drive the patients to visit hospitals on their own on the basis of changed determining factors for satisfaction. Conclusions : The structural equation model showed that the satisfaction of outpatients with the quality of medical services is influenced by a few sub-dimensional satisfaction factors. Among these sub-dimensional satisfaction factors, the satisfaction with medical staff and payment were determined to exert a significant effect on overall satisfaction with the quality of medical services. The structural relationship in which overall satisfaction perceived by patients significantly influences their willingness to use the same institution in the future was also verified.

Objectives: This study aimed to find determinants of customers' use of and satisfaction with oriental medical services in Korea. Methods: A total of 33 articles which consist of 9 articles published in academic journals, 20 master's theses, and 4 doctoral dissertations were included in the systematic review. Among the articles used in the study, 29 used primary survey data, while 4 used secondary data produced by the Korean National Health and Nutrition Evaluation Survey. Results: First, it was found that people who prefer to use oriental medical services usually have one or more of the following diseases: musculoskeletal disorders, paralysis, or diseases or injuries without clear diagnosis. Other variables of gender, age, expectation of treatment outcome, and kindness of doctor and other staff were found to be significant determinants of using the oriental medical services. Second, the determinants of customers' satisfaction with oriental medical services were found to be age, gender, level of education, chronic disease with long length of stay, kindness of staff, medical cost, clinical environment, doctor's reputation, and public image of the institutions. Conclusion: The results of this study can be used to develop marketing strategies for oriental medical institutions in Korea.
Background: The present study investigated the radiation dose distribution of balloon kyphoplasty (BKP) among surgeons and medical staff, and this is the first research to observe such exposure in Japan. Materials and Methods: The study subjects were an orthopedic surgeon (n = 1) and surgical staff (n = 9) who intervened in BKP surgery performed at the National Hospital Organization Disaster Medical Center (Tokyo, Japan) between March 2019 and October 2019. Only disposable protective gloves (0.022 mmPb equivalent thickness or less) and trunk protectors were used, and no protective glasses or thyroid drapes were used. Results and Discussion: The surgery time per vertebral body was 36.2 minutes, and the fluoroscopic time was 6.8 minutes. The average exposure dose per vertebral body was 1.46 mSv for the finger (70 ㎛ dose equivalent), 0.24 mSv for the lens of the eye (3 mm dose equivalent), 0.11 mSv for the neck (10 mm dose equivalent), and 0.03 mSv for the chest (10 mm dose equivalent) under the protective suit.The estimated cumulative radiation exposure dose of 23 cases of BKP was calculated to be 50.37 mSv for the fingers, 8.27 mSv for the lens, 3.91 mSv for the neck, and 1.15 mSv for the chest. Conclusion: It is important to know the exposure dose of orthopedic surgeons, implement measures for exposure reduction, and verify the safety of daily use of radiation during surgery and examination.

The purpose of this study was to investigate the influence of the leadership of head nurses on the job satisfaction and job performance of staff nurses. The leadership styles of head nurses were classified to "authority-oriented" and "benevolence-oriented" based on korean traditional culture. The subjects of this study were 450 staff nurses working at medical-surgical units, intensive care units, emergency units of general hospitals in Kyungbuk area. Data were collected from March 13 to March 25 with several scales for the measurement of leadership styles of head nurses, job satisfaction and job performance of staff nurses. The Cronchbach alphas for the scales were. 79 in "authority-oriented" leadership style, 90 in "benevolence-oriented" leadership style, .91 in job satisfaction, .97 in job performance. Data were analyzed with SAS program using statistics of percentage, means, standard deviation, Pearson correlation, ANOVA and ANCOVA. The results were : 1. There were significant correlations between benevolence-oriented leadership style and job satisfaction, job performance of staff nurses(r=.24, p=.000 ; r=.12, p=.008), authority-oriented leadership style of head nurses and job satisfaction of staff nurses(r=.12 ; p=.024). 2. There were significant differences in job satisfaction of staff nurses by age and clinical career(F=10.36, p=.000 ; F=7.63, p=.000). Also there were significant differences in job performance of staff nurses by age, religion, marriage and clinical career(F=17.79, p=.000 ; F=2.89, p=.035 ; F=4.41, p=.036 ; F=14.33, p=.000). 3. There was significant differences in job satisfaction by the leadership style of head nurses. The job satisfaction of staff nurses with benevolence-oriented head nurses was higher than those with authority
Objectives : The purpose of this survey is to investigate the current status of violence that occurred in Korean medicine clinics targeting medical staff. Methods : First, questionnaires were developed to investigate the actual situation of violence against medical personnel occurring within Korean medicine clinics. The questions consist of five items on the characteristics of the respondent, ten items on identifying the actual state of violence against Korean medical workers, two items on relevance to COVID-19, two items on related legal issues, and three items on policies. The survey was conducted online from 16 November 2021 to 30 November 2021. Results : Total 519 Korean medical doctors responded to the questionnaire, and 314 (60.50%) responded that they had experienced violence demage while working. While the female staffs experience significantly more violence (𝜒2=18.398, p <0.001) and sexual violence (𝜒2=74.855, p <0.001) than the males, the males tend to be more exposed to physical assault than the female (𝜒2=3.900, p = 0.048). Conclusion : Through this study, we identified the situation of violence against medical staffs in Korean medicine clinics, and concluded that it should be necessary to establish strategies for the prevention and management of violence in a social and institutional way.
The purpose of this study is to analyze the change of hospitals that patients safety and quality improvement by accreditation process and to examine the impact or interrelation of leadership, organizational culture, hospital management activities and recognition of hospital management performances. The data were collected through a review of the literature, and selfadministered survey with a structured questionnaires to 714 subjects from several medical staff members, administration staff members, nursing staff members, medical technicians and other staff members working in 23 accredited hospitals in Korea. In this analysis hierarchical multiple regression and structural equation model were used. The conclusion of this study provides a theoretical model for understanding organizational changes brought about by accreditation system. Factor on improvement of efficiency and raise the morale, rather than increase of medical income and reduce of the cost factors, had a stronger influence on the accreditation process. In the future, the hospital's participation to induce the accreditation program voluntarily will come up with an alternative policy concern about financial perspective. Also, the hospitals which preparing accreditation program to achieve the goal efficiently, will make use of transformational leadership through enhancing individual consideration and intellectual development to leading members participation. Additionally, non-accredited hospitals should aim at professional culture by innovative and creative approaches, and inviting members to learning and growth in the organization.

It is reported that exposure for the patient and the medical staff from IVR is large. Direct measurement of patient exposure is difficult, since the measurement disturbs reading of images. The fluorescence glass-dosimeter system consisting of small-size glass chips is developed in recent years. Owing to its small size and physical characteristics, direct monitoring of surface dose may be feasible. The dose measurement for patient and medical staff during head interventional radiology (IVR) examinations was tried by using the fluorescence glass-dosimeter system. A dose response of the glass dosimeter is almost linear in large dose range but its energy dependency is high. About 20% variation of sensitivity was observed in the effective energy of 45-60keV which was used in IVR. In spite of this shortcoming, the fluorescence glass-dosimeter system is a convenient means for a dose monitoring during IVR performance.

It is believed that surgery on human immunodeficiency virus (HIV)-positive patients is dangerous and should be avoided due to the possibility of postoperative infection of the patients or HIV occupational transmission to the medical staff. We discuss here the preparations and measures needed to conduct surgery safely on HIV-positive patients, based on our experience. We performed sex reassignment surgery on two HIV-positive patients from January 2013 to January 2015. Both of them were receiving highly active antiretroviral therapy and were asymptomatic, with a normal CD4 count ($>500cells/{\mu}L$). The HIV-RNA was undetectable within the bloodstream. All the staff wore protective clothing, glasses, and three pairs of protective gloves in the operating room because of the possibility of transmission. Prophylactic antibiotics were administered to the patients, and antiviral therapy was performed during their perioperative course. Neither of the patients had postoperative complications, and none of the medical staff experienced accidental exposure. Both patients had satisfactory surgery outcomes without complications. HIV-positive patients can undergo surgery safely without increased risk of postoperative complications or HIV transmission to the staff through the proper use of antibiotics, active antiretroviral therapy, and supplemental protective measures with post-exposure prophylaxis for the staff in case of HIV exposure.
