Purpose: This study was conducted in order to identify factors that influence depression for low-income elderly who live at home from the International Classification of Functioning model (ICF). Methods: The subjects were 205 elderly people living at home in two public health centers located in metropolitan cities. Subjects were divided according to their depression scores, which were measured using the GDS-short form, including normal, risk, and depression groups. Each variable was consistent with factors of the ICF model, including health condition, individual factors, environmental factors, body function, activities, and participation. Data were collected using structured questionnaires. ANOVA, $x^2$, Pearson's correlation coefficient, and Multinomial logistic regression with IBM SPSS 21.0 were used for analysis of the data. Results: Statistically significant differences were observed among normal, risk, and depression groups regarding personal factors. Gender, education level, numbers of diseases, perceived health, life satisfaction, and social support were identified as the variables that had a significant impact on depression of low-income elderly living at home. Conclusion: Results of this study indicate that there is a need for construction and implementation of strategies that strengthen life satisfaction and social support in order to lower depression of low-income elderly.
Purpose: The aim of this study was to investigate gender differences in risk factors and sleep, depression, and mobility of Korean elderly with and without low back pain and sciatica. Methods: Data were derived from the 2011 Korean National Survey on Older Adults. Participants included 10,674 community-dwelling elderly. The group of elderly subjects with low back pain and sciatica and the group of elderly subjects without low back pain and sciatica were compared according to gender. Result: Age, monthly income, exercise, arthritis, osteoporosis, and providing caring support were significant risk factors for low back pain and sciatica in elderly male subjects. On the other hand, monthly income, living arrangement, arthritis, osteoporosis, providing caring support, and television watching time were significant risk factors in elderly female subjects. Significant differences in depression and mobility according to low back pain and sciatica were observed in both genders. Conclusion: Risk factors for low back pain and sciatica in elderly differed according to gender. Low back pain and sciatica showed an association with depression and mobility. These findings should be considered in planning for nursing intervention for low back pain and sciatica.
본 연구는 저소득 독거노인들을 대상으로 우울이 건강증진행위에 미치는 영향력에 있어서 사회적 지지의 매개효과를 검증하고자 하였다. 이를 위해 경기도 지역 거주 국민기초생활수급권을 가진 독거노인 294명의 자료를 활용하여 연구를 수행하였다. 경로분석을 통해 건강증진행위에 대한 우울과 사회적 지지의 직접적인 영향력과 우울이 사회적 지지를 매개하여 나타내는 간접적인 영향력을 검증하였다. 연구결과 저소득 독거노인의 우울과 사회적 지지는 건강증진행위에 직접적인 영향력을 갖고 있었고, 우울은 사회적 지지를 매개하는 간접적인 영향력도 갖고 있었다. 이를 통해 저소득 독거노인의 건강증진행위에 우울과 사회적 지지가 갖는 의미를 다시 확인할 수 있었고, 특히 건강증진행위에 있어서 우울과 사회적 지지가 갖는 관계에 대한 이해를 바탕으로 하여 개입방법을 모색해야 함을 확인할 수 있었다. 연구결과를 바탕으로 저소득 독거노인의 건강증진행위를 향상시키기 위한 제언들을 제시하였다.
Objectives: This study is the human factors and disease factors of the copayment system for the elderly (>65 years old) and to identify does the current status and characteristics of the applied elderly and conducted to provide basic data. Methods: Sample cohort data from the National Health Insurance Corporation database, from the years 2012-2015, were analyzed of 21,772 elderly people over the copayment ceiling. Results: The ratio of those who exceeded the copayment ceiling system rose sharply from progressive rates of 3.39% in 2012, 3.69% in 2013 and 5.03% in 2014, to rates of 37.13% from 2013. Factors identified that affect the instances of being over the copayment ceiling were: age, income group, region, severity, disability, sickness distribution, inpatient days, and outpatient days. Conclusions: The reorganization of the copayment ceiling system in 2014 favored low-income families of the elderly, but in 2015, the proportion of elderly was low (only 5.78%). The government's policies needs to change to allow for the amount of the deductible upper limit for low- and middle- income groups to be further subdivided in order for the elderly to receive more deductibles.
Purpose: The purpose of this study was to examine cognitive functions, depression, and life satisfaction among the elderly receiving visiting nursing services. Methods: The subjects of this study consisted of 221 elderly individuals who were receiving visiting nursing services from a public health center located in Gyeonggi-do. The Mini-mental State Examination Korea version (MMSE-K), Geriatric Depression Scale-short form-Korea version (GDS-K), and Life Satisfaction Scale were utilized to determine cognitive functions, depression, and life satisfaction, Collected data was analyzed by the SPSS PC Win 12,0 program. Results: The prevalence of cognitive impairment (MMSE-K$\leq$24) was 86.4%, and the proportion of severe depression (GDS-K$\leq$10) was 22,6%, Older age, lower education, and low income were associated with cognitive impairment, and low education, no religion, and low income were associated with depression. Cognitive function negatively correlated with depression. Life satisfaction level was fairly low and was not associated with any variables. Conclusion: The high prevalence of cognitive impairment and low life satisfaction status were determined among low-income elderly who were registered at a visiting health care center. Thus, in the future at visiting health care services, efficient and various programs for the elderly should be tried, to improve cognitive functions and reduce depression. Additionally, consistent evaluation studies for those programs will be required.
Objectives: The purpose of this study was to investigate the factors influencing on the denture satisfaction in the low income elderly people. Methods: The subjects were 143 elderly people from 60 to 75 years old wearing dentures and receiving consistent follow-up in the public health center in Busan. A self-reported questionnaire was filled out by the elderly people from February 1 to March 1, 2013. The questionnaire consisted of denture satisfaction, social variables, and psychological variables. Data were analyzed by ANOVA and multiple regression analysis using SPSS 18.0. The sociodemographic characteristics included gender, age, and chronic diseases. The recognition of oral health included pronunciation, denture maintenance, mastication ability, and education for denture care. The questionnaire was measured by Likert 5 scale. Results: Gender, chronic disease, denture care instructions, and the self-preception of the oral health after denture treatment were closely correlated with denture satisfaction(p<0.001). Age and the number of repairs were very important factor to denture satisfaction(p<0.05). Elderly women were more satisfied with denture than men and those who had no chronic diseases tended to be more satisfied with denture. Those who received oral care instructions were more satisfied with the denture than those who did not. The younger age group and no repairing prosthetic group tended to be more satisfied with the denture. Conclusions: It is important to provide the denture management services to the low income elderly when they demand the services. The national dental health policy must be focused on connection of the elderly people denture services with the public health center.
Purpose: The purpose of this research is to identify factors affecting suicidal ideation of the low-income elderly. Methods: The study subjects were lowest-income(basic living allowance) and lower-income elders who were registered for home visiting services at five District Health Centers in Seoul. Interview data were collected from 280 elders and analyzed. Using SPSS/WIN 21, $x^2$-test and t-test were conducted to analyze the relationship between the subjects' characteristics and their suicidal ideation, and logistic regression was done to identify factors affecting suicidal ideation. Results: Of the subjects, 48.6% had suicidal ideation for the last one year. Self-evaluated psychological status, quality of sleep, depression, and loneliness were affecting factors to the subjects' suicidal ideation. Conclusion: Psychological and social factors are strongly related to the suicidal ideation of the elderly who are living alone in metropolitan areas. Therefore, home visiting services should supply physical, psychological and social cares continuously to the low-income elderly, especially those who live alone in metropolitan areas.
BACKGROUND/OBJECTIVES: The study objectives were to examine the participation rate in food assistance programs and explore the factors that contribute to such participation among the Korean elderly population. SUBJECTS/METHODS: The study sample comprised 3,932 respondents aged 65 years or older who were selected from a secondary data set, the fourth Korean Welfare Panel Study (KoWePS). The factors, related to participation in programs were examined based on the predisposing, enabling and need factors of the help-seeking behavior model. Multiple logistic regression analysis was used to select the best contributors among the factors related to program participation. RESULTS: The predisposing rate in food assistance programs was 8.5% (7.1% for men and 10.4% for women). When all variables were included in the model, living without spouse, no formal education, low income, having social security benefits and food insecurity in elderly men, and age, low income, having social security benefits and feeling poor in elderly women were significantly related to a higher tendency to program participation. CONCLUSIONS: The predisposing and need factors, such as living without spouse, low education level, food insecurity and feeling poor were important for program participation, as well as enabling factors, such as household income and social security benefits. A comprehensive approach considering these factors to identify the target population for food assistance programs is needed to increase the effectiveness and target population penetration of these programs.
This study aimed to plan nutrition support programs for the elderly living alone whose nutrition status were seriously concerned, conducted seven stages nutrition intervention program on a trial basis, and evaluated the effectiveness of the program of the Elderly Nutrition Support Project. Subjects were selected for personalized nutrition management based on nutritional risk score and nutrition intervention were tailored to the problems occurred. The elderly nutrition support program targets were 44 senior citizens who lived alone with low income. The 33 (as Type 1) of the subjects with whom milk, tofu, seaweed, eggs, black beans have been supported, and also provide nutrition education, and the rest 11 persons (as Type 2) to whom food was not supported but provide nutrition education programs. As a result, all subjects showed that compared with pre and post program implementation, their daily exercise time and milk and protein consumption level were increased and some improvement was observed regular meals consumption and low-salt diets. Their nutrient intake level such as calories, protein, calcium, iron improved after implementation. In addition, NSL DETERMINE scores significantly improved from 13.21 to 7.24 in Type 1 and 11.27 to 9.91 in Type 2. As positive dietary behavioral changes were observed as in that they purchased more protein and calcium rich foods.
본 연구는 노인빈곤의 심화와 함께 노인 소득불평등 악화가 우려되는 상황에서 공적소득, 시장소득, 가족 소득 등 주요 노인소득원이 소득불평등에 미치는 영향을 확인하여 노인 간 소득불평등을 완화하기 위한 정책방안을 제안하는데 목적이 있다. 주요 분석결과는 다음과 같다. 첫째, 노인 소득원별 불평등 정도를 파악하기 위해 지니계수를 산출한 결과 전체소득 0.4809, 공적소득 0.4701, 시장소득 0.6735, 가족소득 0.1855로 나타났다. 한국 노인의 소득집단에 따른 소득원별 불평등 정도는 시장소득이 가장 크고 전체소득, 공적 소득도 매우 큰 것이다. 둘째, 노인 소득원이 소득불평등에 미치는 영향을 확인하기 위하여 전체소득에서 공적소득, 가족소득, 시장소득을 각각 제외한 소득의 지니계수를 산출한 결과 전체소득에서 공적소득을 제외한 지니계수 0.4864, 가족소득을 제외한 지니계수 0.5784, 시장소득을 제외한 지니계수 0.3609로 나타났다. 전체소득에서 제외한 소득의 지니계수가 전체소득 지니계수보다 약간 증가한 공적소득은 노인 소득불평등 완화효과가 매우 미미한 반면에 전체소득에서 제외한 소득의 지니계수가 전체소득 지니계수보다 매우 커진 가족소득은 노인 소득불평등 완화효과가 큰 것으로 나타났다. 반면에 제외한 소득의 지니계수가 전체 소득 지니계수보다 크게 낮아진 시장소득은 노인 소득불평등의 큰 원인이었다. 현재의 노인 소득불평등을 완화하기 위해서는 저소득층 노인을 대상으로 일자리를 제공하는 등 노인의 시장소득을 올릴 수 있는 기회를 적극적으로 제공해야 할 것이다.
