Effects of Delirium Prevention Interventions for Neurocritical Patients (신경계 중환자에게 적용한 섬망 예방중재의 효과)

  • Purpose: The purpose of this study was to investigate the effects of a delirium prevention intervention for patients in neurology and neurosurgery intensive care units (ICUs). Methods: This study was a quasi-experimental study. Participants were 87 patients. The experimental group was provided with nonpharmacologic and multicomponent delirium prevention interventions, consisting of regular delirium assessment, improvement in orientation, early therapeutic intervention, and environmental interventions. The control group was provided with routine intensive care. Data were analyzed using $x^2$ test, Fisher's exact test, and t-test. Results: Both groups were homogeneous. The incidence of delirium and length of hospitalization were significantly lower in the experimental group compared to the control group. ICU stay, mortality and unplanned extubation were lower in the experimental group compared to the control group, but there was no significant difference. Conclusion: Findings indicate that the delirium prevention intervention is effective in reducing incidence of delirium and length of hospitalization. Therefore, this intervention should be helpful in preventing delirium in neurology and neurosurgery ICUs and can be used as a guide in the prevention of delirium in neurological diseases's patients.

Measurement of Organic Solvent from Painting Work Inside the Steel Box Girder of Bridge (교량 스틸박스거더 도장공사의 유기용제 측정 실험)

  • This study is to find out how organic solvent will be propagated from painting inside the steel box girder of bridge. $2.9m{\times}3.0m$ of inside size of steel box girder is not suitable for painters to do his work comfortably and hygienically. Substance density(ppm)inside the space of the box unsatisfactory hygienical condition. Most of organic solvent(mainly toluene) came down to 0.5m in two minutes and 53sec. But personal protection for painter should be properly kept against flying this heavy organic solvent. longitudinally 27.1m in length is a cell unit of the whole length of bridge. Model XP-3l6A made in Japan is a main instrumentation adjusted to sense organic solvent especially toluene which can be measured up to 10,000ppm. Scenario analysis by computer program, safer release 2.0 has been performed first to estimate how the organic solvent will be propagated. And then actual test was done as a model. This has been measured for approximately five(5) minutes, with 30 sec interval. Actual measurement results showed much higher $10{\sim}20%$ than result analyzed by the computer program, meaning that this painting work can give worse effect to the worker who is painting inside the box girder of the bridge. The first meas urement level over the floor set up at 30cm height from the floor, because organic solvent was estimated to stay at the level. and then, they were measured at 1.0m, 1.5m level respectively, more.

The Numerical Analysis by the Change on the Length-Height Ratio of 2D Cavity in Supersonic Combustor (수치해석을 이용한 초음속 연소기 내의 2차원 Cavity의 종횡비 변화에 대한 혼합특성 비교연구)

  • The air velocity flowing in inner combustion chamber of Scramjet is supersonic and the time of its stay is very short as a few milliseconds. Within this short time, fuel injection, air-fuel mixing, and combustion process should be accomplished. Several methods are suggested for mixing enhancement. Among these, cavity is selected to study for enhancement of mixing. The numerical simulation is performed in the case of freestream Mach number of 2.5 and cavity located in front of fuel jet injection. 8 different sized cavities of length-height ratio were used in order to recognize the effect about cavity size. Also, the case without cavity was analyzed to find the effect of cavity. Used code compared with the result of experiment under identical conditions and it was verified. Through this comparison and verification, mixing enhancement by cavity could be confirmed.

The Distribution and Habitat of Bibari Snake (Sibynophis collaris Gray)in Jeiu Island, Korea (제주산 비바리뱀(Sibynophis collaris Gray)의 분포와 서식지)

  • This study was carried out to examine the distribution and habitat of Bibarl snake (Sibynophis Collaris) from March 1998 to June 2005 in Jeju island, Korea. 5 individuals of Bibari snake were observed on northern part of JeJu, 1 individual on southern area and 2 individuals on western area during research period. As a result, it was revealed that Bibari snake was found on the whole of Jeju island. Its observation was made from May to October, especially frequent from May to July. The body length of adult Bibari snakes were $406{\~}452mm$ and its tail length $29+{\~}202mm. Considering of number and shape of supralabials and number of temporals, the characters of Bibari snake were similar to those of S. chinensis. Therefore it seems need to reconstruct taxonomic position of Bibari snake. From a vertical viewpoint of Mt. Halla, Biabri snake prefers to stay on the grassland 600m below the sea. Consequently, it Is suggested that grassland be well-reserved for the protection of Bibari snakes.

Clinical Outcomes of Spontaneous Spinal Epidural Hematoma : A Comparative Study between Conservative and Surgical Treatment

  • Objective : The incidence of spontaneous spinal epidural hematoma (SSEH) is rare. Patients with SSEH, however, present disabling neurologic deficits. Clinical outcomes are variable among patients. To evaluate the adequate treatment method according to initial patients' neurological status and clinical outcome with comparison of variables affecting the clinical outcome. Methods : We included 15 patients suffered from SSEH. Patients were divided into two groups by treatment method. Initial neurological status and clinical outcomes were assessed by the American Spinal Injury Association (ASIA) impairment scale. Also sagittal hematoma location and length of involved segment was analyzed with magnetic resonance images. Other factors such as age, sex, premorbid medication and duration of hospital stay were reviewed with medical records. Nonparametric statistical analysis and subgroup analysis were performed to overcome small sample size. Results : Among fifteen patients, ten patients underwent decompressive surgery, and remaining five were treated with conservative therapy. Patients showed no different initial neurologic status between treatment groups. Initial neurologic status was strongly associated with neurological recovery (p=0.030). Factors that did not seem to affect clinical outcomes included : age, sex, length of the involved spinal segment, sagittal location of hematoma, premorbid medication of antiplatelets or anticoagulants, and treatment methods. Conclusion : For the management of SSEH, early decompressive surgery is usually recommended. However, conservative management can also be feasible in selective patients who present neurologic status as ASIA scale E or in whom early recovery of function has initiated with ASIA scale C or D.

IV Ketorolac Combined with Morphine PCA in Postoperative Pain Control after Lumbar Disc Surgery (요추 추간판절제술 후 Morphine PCA에 병용한 Ketorolac의 간헐적 정맥투여)

  • Background: This study was conducted to evaluate the efficacy of a parenteral nonsteroidal anti-inflammatory agent for management of post-surgical pain and its effect on hospital stay and long-term surgical outcome. Methods: Total of 40 patients undergoing lumbar discectomy were randomly assigned to two groups, receiving either 1) 30 mg intravenous ketorolac upon surgical closure, every 6 hours for 36 hours, and morphine IV PCA (intravenous patient controlled analgesia), or 2) only morphine PCA. A blinded investigator recorded; the visual analog pain scores, total postoperative narcotic consumption, complications by morphine PCA, length of hospitalization (from surgery to discharge), and long-term outcome at 6 weeks. Results: The patients who received IV ketorolac and morphine PCA reported significantly lower visual analog pain scores than patients receiving only morphine PCA. Cumulative morphine doses were significantly lower in the ketorolac group (P<0.001). There was no significant difference between groups in the frequency of side effects related to morphine PCA. Mean length of hospitalization was longer for patients receiving only morphine PCA, but there was no statistical significance. Six weeks after surgery, four (20.0%) patients who received only morphine PCA suffered persistent back pain. In contrary, all those patients who received ketorolac were free of back pain at follow-up (P<0.05). Conclusions: These results suggest that intermittent IV bolus ketorolac, when used with opioid IV PCA is more effective than opioid IV PCA alone for postoperative pain following lumbar disc surgery. However, this strategy did not contribute to early discharge from hospital after lumbar disc surgery. The effect to long-term surgical outcome was not conclusive.

Comparison of Food Neophobia Scale and Food Involvement Scale Between Koreans and East-South Asians (한국인과 동남아시아인의 푸드네오포비아와 음식관여도 차이 비교)

  • An individual's food-related personal traits play an important role in influencing personal food choice and habits. According to culture, their influence can manifest differently. To ascertain personal traits about food, FNS (food neophobia scale) and FIS (food involvement scale) were employed in recent studies. This study aimed to understand the food culture and food choices of East-South Asians who live or stay in Korea through comparison of FNS and FIS. Eighty Koreans and 233 East-south Asians (Indonesians, Filipinos, Malaysians, Vietnamese, Thai, Singaporeans, and Bangladeshi) completed a questionnaire to measure FNS (10 questions), FIS (12 questions), and sociodemographic conditions (9 questions). ANOVA was conducted to ascertain FNS and FIS between the groups, and regression analysis was carried out to determine which sociodemographic factors had an effect. The items were analyzed to determine the differences according to gender, age, marital status, nationality, religion, occupation, educational background, monthly income, and length of residence in Korea. FNS showed significant differences between the groups with regard to sociodemographic characteristics, except gender, age, and marital status, whereas FIS showed significant differences in gender, nationality, religion, occupation, educational background, monthly income, and length of residence in Korea. The results of the regression analysis suggest that nationality strongly affected FNS and FIS, and FIS was also affected by gender.

Intensive Care Experience of Critical Care Patients and Its Related Factors : A Secondary Analysis Study (중환자실 환자의 집중치료 경험 및 관련 요인: 이차분석 연구)

  • Purpose : This study investigates the experiences of patients in intensive care units (ICUs), and the factors influencing these experiences. Method : We used a cross-sectional survey design to analyze previously collected cohort data from 891 patients who spent over 24 hours in 19 ICUs across four university hospitals in Busan, South Korea between June 2019 and July 2020. Within a week of ICU discharge, participants completed the Korean version of the Intensive Care Experience Questionnaire, covering four areas: "awareness of surroundings," "frightening experiences," "satisfaction with care," and "recall of experiences." We used multiple linear regression to identify factors associated with the ICU experience. Results : Low income (𝛽 = -.08, p = .016), unplanned hospitalization (𝛽 = -.09, p = .006), sedation (𝛽 = -.16, p < .001), and delirium (𝛽 = -.15, p < .001) reduce patients' awareness of their surroundings. Frightening experiences are associated with being female (𝛽 = -.07, p = .027), experiencing delirium(𝛽= -.15, p<.001), and longer stays in the ICU (𝛽= -.14, p <.001). Using sedatives decreases satisfaction with care (𝛽 = -.08, p = .048). Living alone (𝛽 = -.08, p = .013) and using painkillers (𝛽 = -.08, p = .020) reduces recall of experiences. Conclusion : Negative ICU experiences are significantly associated with being female, living alone, lower income, unplanned admission, using sedatives and painkillers, delirium, and longer stays in the ICU. Thus, improving ICU experiences requires interventions that address modifiable factors, such as delirium, medication, and length of ICU stays.

A Study on the Effective Management for the International Sea-borne Container (국제 해상 컨테이너의 운용방안에 관한 연구)

  • In the process of containerization, the problem of regional maldistribution of container management plan arises seriously due to several factors like a number of unbalances of containers between loading and discharging ports. This study focus on the minimizing cost. This study is composed of two models which in effective management decision making show decision of the number of containers and transfer of empty containers. One is decision of the number of containers which carriers should possess by appropriate forecasting and the other is effective management decision making which includes the transfer of empty containers on calling ports. This study has suggested as follows, First, the Time Series analysis method, especially the "Exponential Smooting with Trend Adjustment" was used to forecast the trade volumes for the designated traffic route. Second, the Time Series analysis method in deciding the optimal number of owned container at the unbalances trade situation between East Bound and West Bound service, most important variables were found such as total traffic volume, the calling interval at a port, the number of days of voyage and the length of stay on shore of container for the optimal number of owned container. Third, effective management decision making model, which makes it possible to analyze the impacts of change in important matters such as lease and positioning policy, and actually influence decision making.on making.

Development of Case Management using Critical Pathway of Posterolateral Fusion for Lumbar Spinal Stenosis (사례관리를 위한 Critical Pathway 개발 : 요추협착증 환자의 융합술)

  • It is well recognized that case management is required to survive in the rapidly changing medical environment. One of the case management is the critical pathway(CP) which is assumed to increase the quality of care and at the same time to decrease the length of stay in hospital. The purpose of the study was to develop a CP for the management of patients with postero-lateral fusion for lumbar spinal stenosis. Through review of literature and medical records of patients with spinal stenosis, a pilot CP was designed, including 8 different care components such as medication, laboratory tests, assessment etc., from one day before surgery to 6 days of postoperative care. Every item of the pilot CP was evaluated by a panel of experts to test the content validity. The items not agreed on by more than 4 out of 6 experts were deleted or modified to be integrated in the CP. To apply the modified CP to a clinical environment, the items reflecting treatment, medication and lab work were entered into an order communication system(OCS), and doctors and nurses were taught to use the CP. Finally, the development of CP for the patients with posterolateral lumbar fusion was completed after the application and variance analysis of the CP.
