목 적: 본원을 내원한 인공심장박동기를 이식한 유방암 환자에 대해 장비위치이전수술, 차폐 또는 빔 정형 등을 고려하여 방사선치료를 시행한 사례의 전반적인 과정을 고찰해보고자 한다. 대상 및 방법: 본원을 내원한 유방암환자 중 좌측 흉부에 인공심장박동기를 이식한 54세 여성 환자를 대상으로 방사선치료를 시행하였다. 환자의 방사선치료 시행이 결정된 후, 인공심장박동기로의 선량 유입을 최소화하기 위해 순환기내과와의 협의를 통해 환자 좌측 흉부에 이식되어있던 기기를 우측 흉부로 이동시키는 수술을 시행하였다. 총 선량 5,040 cGy, 일일선량 180 cGy, 28회, 치료 조건으로 광자선 에너지 10 MV, 조사야 크기 0/$9.5{\times}20$ cm를 사용하여 Half beam 대향이문조사치료를 시행하기 위한 방사선치료계획을 하였다. 방사선 치료계획 시 적합한 차폐체의 두께를 정하기 위하여 Solid water phantom($30{\times}30{\times}7$ cm)에 Farmer-type chamber (TN30013, PTW, Germany)를 이용해 차폐체(납, Pb $28{\times}27{\times}0.1$ cm)를 사용하였을 경우와 사용하지 않았을 경우 기기에 유입될 예상선량을 산출하였다. 전산화치료계획장비(Eclipse, Varian, USA)를 사용해 치료계획을 시행하고, 두께 2 mm의 차폐체를 사용하였을 경우와 사용하지 않았을 경우에 기기에 유입될 예상선량을 산출해내었다. 치료 첫 날, MOSFET Dose Verification System (TN-RD-70-W, Medical Canada Ltd., Canada)을 이용하여 인공심장박동기에 유입되는 선량을 측정하였다. 결 과: 차폐체 두께산정 실험에서 인공심장박동기가 위치한 B 지점에서 차폐체 2 mm일 경우, 105.265 cGy, 처방선량의 2.09%로 산출되어, 적합한 차폐체의 두께를 정할 수 있었다. 전산화치료계획장비에서 2 mm 차폐 시 총 치료기간 동안 11.5에서 38.2 cGy까지 기기에 유입될 수 있음이 나타났으며, DVH 최대값은 77.3 cGy로 나타났다. 환자의 첫 치료 시 MOSFET 측정 결과 4.3 cGy로 측정되어, 총 치료기간동안 120.4 cGy, 처방선량의 2.39%의 선량이 유입될 것으로 예상되었다. 환자는 치료기간동안 그리고 치료받은 후 어떠한 부작용도 일으키지 않았으며, 인공심장박동기는 치료시작 전과 후에 순환기내과에서 기능측정을 받은 결과 아무런 이상을 보이지 않았다. 결 론: 현재 인공심장박동기가 이식된 암 환자의 방사선 치료에 대한 공신력 있는 기관의 권고안이 노후화되어, 새로운 장비들에 대한 데이터가 부족한 상황에서 우리는 타과와 원활한 협업, 철저한 치료계획과 정밀한 QA, 지속적인 in-vivo dosimetry와 monitoring 등을 통하여 이러한 환자들의 방사선치료를 성공적으로 해낼 수 있을 것으로 사료된다.
이스라엘 잉어의 근육 actomyosin의 열안정성과 그 첨가제에 의한 영향을 밝히기 위하여 배육골격근에서 추출한 actomyosin을 시료로 하여 온도의 변화가 actomyosin의 안정성에 미치는 영향을 ATP-감도, 유리 SH기 및 Ca-ATPase 활성도 등을 측정하여 분석하였다. 그리고 sucrose, sorbitol, Na-glutamate 및 L-cysteine등 첨가제의 영향에 대하여는 Ca-ATPase 활성도의 변화를 측정하여 단백질 변성속도당수($K_D$), 단백질변성보호효과(${\Delta}E/M$) 그밖에 열력학적 제상수를 계산 비교하였다. 실험결과를 요약하면 다음과 같다. 1. 배육골격근에서 추출한 이스라엘 잉어 actomyosin은 단백질농도 $4.12{\sim}4.68mg/ml$, 핵산의 함량 $2.63{\sim}2.9%$, actin과 myosin의 결합비 $1:2.20{\sim}2.63$, 지질의 함량 $4.33{\sim}5.26\%$, ATP-감도 109.78, Ca-ATPase 활성 $0.159{\sim}0.201\;{\mu}M-Pi/min/mg-protein$, 유리 SH 기 함량 $3.3{\sim}3.4M/10^5g-protein$ 이었다. 2. Ca-ATPase활성 및 ATP-감도는 온도가 상승 함에 따라 1차반응적으로 감소하였고, 유리 SH기는 $60{\circ}C$ 까지는 증가하다가 그 이후는 급격히 하강하였다. 3. 가열온도상승에 따른 Ca-ATPase 활성의 반감 시간은 $12^{\circ}C$ 일때 280분, $20^{\circ}C$ 일 때 125분, $30^{\circ}C$일때 55분, $40^{\circ}C$ 일때 13분이었으며 $20^{\circ}C$에서 활성화에너지는 5,395 cal/mole 활성화엔탈피는 4,814cal/mole, 활성화엔트로피는 -40.42 e.u, 자유에너지 값은 17,626cal/mole이었다. 4. 당 및 아미노산중에서 가열에 대하여 변성보호효과가 높은 것은 $3\%$ sorbitol이었으며, $8\%$ Na-glutamate, $1\%$ sucrose, $1\%$ L-cysteine의 순으로 낮아졌다. 5. 저온저장시 actomyosin이 가장 안정한 온도는 $-30^{\circ}C$이었으며, $0^{\circ}C,\;-20^{\circ}C$ 순으로 불안정하였다. 또한 $-20^{\circ}C$ 일 때의 첨가제에 의한 냉동변성보호효과는 $8\%$ Na-glutamate가 가장 좋았고 $3\%$ sorbitol, $1\%$ sucrose, $1\%$ L-cysteine의 순으로 효과가 떨어졌다.
One of the major obstructing factors against managing dairy farm in Korea has been a shortage of roughage supply, which resulted in excessive abuse of concentrate feed. In order to solve this problem, production of the wrap silage by the winter cereal forages raised in the fallow paddy field is strongly recommended in Korea. The main objective is to develop a tractor attached round bale wrapper which can process the silage by wrapping the round bales with thin plastic films. This is the first half of the study which is divided by two parts. In this first part, bale wrapping process was analyzed, and based on this results the followings were designed, developed and tested. 1. Bale wrapper which haying the maximum capacity of 1 ton bale with various functions such as loading, wrapping, discharging the round bales and supplying and cutting wrap films was designed. 2. An actuator and its hydraulic circuit of each process were developed and tested. 3. Also, the variations of hydraulic pressure and engine speed were investigated by operating bale wrapper developed. In this test, maximum pressure of the hydraulic circuit for the bale wrapping was 130 kg/㎠ when it raised the bale, which was quite below the relief pressure of 170 kg/㎠ of hydraulic circuit. In the engine speed test, speed drop was 20∼67 rpm, which meant that there was no over-load operation. Therefore, the experiment proved that developed hydraulic circuit and mechanism is stable in bale wrapping operation

A survival prediction model has recently been developed to evaluate the prognosis of resected nonmetastatic pancreatic ductal adenocarcinoma based on a Cox model using two nationwide databases: Surveillance, Epidemiology and End Results (SEER) and Korea Tumor Registry System-Biliary Pancreas (KOTUS-BP). In this study, we applied two machine learning methods-random survival forests (RSF) and support vector machines (SVM)-for survival analysis and compared their prediction performance using the SEER and KOTUS-BP datasets. Three schemes were used for model development and evaluation. First, we utilized data from SEER for model development and used data from KOTUS-BP for external evaluation. Second, these two datasets were swapped by taking data from KOTUS-BP for model development and data from SEER for external evaluation. Finally, we mixed these two datasets half and half and utilized the mixed datasets for model development and validation. We used 9,624 patients from SEER and 3,281 patients from KOTUS-BP to construct a prediction model with seven covariates: age, sex, histologic differentiation, adjuvant treatment, resection margin status, and the American Joint Committee on Cancer 8th edition T-stage and N-stage. Comparing the three schemes, the performance of the Cox model, RSF, and SVM was better when using the mixed datasets than when using the unmixed datasets. When using the mixed datasets, the C-index, 1-year, 2-year, and 3-year time-dependent areas under the curve for the Cox model were 0.644, 0.698, 0.680, and 0.687, respectively. The Cox model performed slightly better than RSF and SVM.
We investigated the morphological changes and TUNEL reaction of apoptotic cells in the liver of D-galactosamine (20 mg/mouse) and lipopolysaccharide (5 $\mu\textrm{g}$/mouse)-treated 30 mice (BALB/c), and in additioa also of apoptotic cells in kidney and spleen. The livers and other some organs of mice at 6, 12, 24, 48 and 72 hrs after treatment were collected and were fixed with 10% neutral formalin and paraffin sections were stained with hematoxylin-eosin or terminal deoxynucleotidly transferase-mediated dUTP nick end labeling (TUNEL) method. Morphological changes in apoptotic hepatocytes were chondensation of nuclei and density of cytoplasms, then the margination and pyknosis of chromatin, the formation of half-moon- or horse-shoe- or ship-like shapes of condensed chromatin mass, lastly formation of apoptotic bodies, disappearance of nuclear envelopes, decrease of stainability, then lysis and disappearance of apoptotic bodies. TUNEL positive reactions of hepatocytes were appeared first moderate in uncondensed hepatocytes, severe in condensed hepatocytes, moderate in chromatin-marginated hepatocytes. These reactions also were appeared moderate in hepatocytes with half-moon- or horse-shoe- or ship-like pyknotic chromatin mass or apoptotic bodies, and mild or negative in hepatocytes with lysed apoptotic bodies or with disappeared nuclear envelopes. Consequently these results suggested that TUNEL positive reactions of hepatocytes appeared at more early stages than appearance of chromatin condensation and disappeared at more early stage than disappearance of histological findings of apoptosis. We also confirmed that the differentiation of apoptotic cells from normal healthy cells of Kupffer cells and vascular endothelial cells in liver, reticular cells and lymphocytes in spleen and epithelial cells of tubules and ducts in kidney was impossible in H-E preparations but was possible in TUNEL preparations.
The solubility and physicochemical stability of caroverine hydrochloride (CRV), an antispasmodic, in buffered aqueous solutions were studied using a reverse phase high performance liquid chromatography. The solubilty of the drug at pH 2.76-5.40 was similar at the range 31.9-36.2 mg/ml $(34^{circ}C)$, but, at the pH higher than 6.0, markedly decreased. The use of polyethylene glycol 400 as a cosolvent did not increase the solubility at any compositions examined. Moreover. increasing molar concentration of aqueous phosphate buffer from 0 to 0.5 M remarkably decreased the solubility. The degradation of CRY followed the apparent first-order kinetics. The degradation was accelerated with decreasing pH and increasing storage temperature. The half-lives for the degradation of CRY (1.0 mg/ml) at pH 1.28. 4.01 and 5.93 $(45^{\circ}C)$ were 2.8, 31.4 and 124 hr. respectively. The pHs of incubated solutions were to some extent lowered perhaps due to the formation of acidic degradation products. The addition of disodium edetate (0.01%) to the CRY solution (pH 4.95) retarded 2.5 times the degradation rate at $45^{\circ}C$, but the use of sodium bisulfite (0.1%) accelerated 2.9 times the rate. The activation energy for the CRY solution (20 mg/ml. pH 5.4) containing 0.01% EDTA was calculated to be 5.98 kcal/mole. When the solution was stored under nitrogen displacement in ampoule, there was no significant degradation even after 3 months at $40^{\circ}C$, indicating that protection from oxidation by air (oxygen) is essential for the complete stabilization of CRY solution.
Purpose: This study was conducted to develop an education program for safe drug use in the rural elderly and to measure the effect of the program. Method: This study utilized a nonequivalent control group pretest-posttest design. The subjects of this study consisted of 40 older persons who were more than 65 years old and lived in G and C moon, Y gun, Gyeongsang-bukdo and visited the public health subcenter. Twenty were assigned to the experimental group and 20 to the control group. The education was provided for one and a half hours, once a week for 3 weeks. Data was collected before, right after, and one month after the program. Result: The first hypothesis was supported(F=79.24, p=0.000) showing that the knowledge scores of the drug use of the experimental group were significantly higher than those of the control group at post education and one month after education. The second hypothesis was supported(F=23.84, p=0.000) showing that the drug misuse and abuse prevention behavior scores in the experimental group were significantly higher than those of the control group at post education and one month after the education. Conclusion: This study suggests that the education for safe drug use is effective in promoting knowledge and behavior for safe drug use of the rural elderly.
The antidiabetic efficacy of Enicostemma littorale Blume (chhota chirayata) aqueous extract was examined in newly diagnosed non-insulin-dependent diabetes mellitus (NIDDM) patients taking only the extract and was administered as two divided doses, half an hour before meal as 5g of aqueous extract per single dose. Out of the 20 patients volunteered, 11 successfully completed the 2 month trial and a significant decrease in fasting & postprandial blood glucose and glycosylated haemoglobin levels were observed along with a significant improvement in the antioxidant parameters of the patients. There was also a significant increase in serum insulin levels in 7 patients after extract treatment as compared to levels before treatment. Serum total cholesterol and serum triglyceride levels were decreased significantly with a significant increase in serum HDLCholesterol levels. Other vital parameters remained stable and no side effects were observed. This is the first report showing the hypoglycemic, antioxidant and hypolipidemic properties of the aqueous extract of E. littorale Blume in non-insulin dependent diabetes mellitus patients.
Cytomegalovirus (CMV) is currently the most common agent of congenital infection and the leading infectious cause of brain damage and hearing loss in children. Symptomatic congenital CMV infections usually result from maternal primary infection during early pregnancy. One half of symptomatic infants have cytomegalic inclusion disease (CID), which is characterized by involvement of multiple organs, in particular, the reticuloendothelial and central nervous system (CNS). Moreover, such involvement may or may not include ocular and auditory damage. Approximately 90% of infants with congenital infection are asymptomatic at birth. Preterm infants with perinatal CMV infection can have symptomatic diseases such as pneumonia, hepatitis, and thrombocytopenia. Microcephaly and abnormal neuroradiologic imaging are associated with a poor prognosis. Hearing loss may occur in both symptomatic and asymptomatic infants with congenital infection and may progress through childhood. Congenital infection is defined by the isolation of CMV from infants within the first 3 weeks of life. Ganciclovir therapy can be considered for infants with symptomatic congenital CMV infection involving the CNS. Pregnant women of seronegative state should be counseled on the importance of good hand washing and other control measures to prevent CMV infection. Heat treatment of infected breast milk at $72{^{\circ}C}$ for 5 seconds can eliminate CMV completely.
Diltiazem inhibits calcium channels and Iεads to vascular smooth muscle rεlaxation and negative inotropic and chronotropic effects in the hεart. Diltiazem is almost completely absorbεd after oral administration, but its extent of absolute oral bioavailability is reduced because of considerable first-pass hepatic metabolism. Diltiazem is able to dilate renal vasculature and can increase the glomerular filtration rate and renal sodium excretion. The purpose of this study was to report the pharmacokinetic changes of diltiazem after oral administration of diltiazem, 20 mg/kg, in rabbits coadministered with cimetidine, 20 mg/kg and pretreated twice per day for 3 days at cimetidine dose of 20 mg/kg. The area under the plasma concentration-time curve (AUC) of diltiazem was significantly higher in rabbits pretreated with cimetidine than that in control rabbits (p<0.01), showing about 149% increased relative bioavailability. The peak plasma concentration $(C_{max})$ and elimination half-life of diltiazem were increased significantly (p<0.05) in rabbits pretreated with cimetidine compared with those in control rabbits. This findings could be due to significant reduction of elimination rate constant by pretreated with cimetidine. The effects of cimetidine on the pharmacokinetics of oral diltiazem were more considerable in rabbits pretreated with cimetidine compared with those in control rabbits. The results suggest that the dosage of diltiazem should be adjusted when the drug would be co-administerεd chronically with cimetidine in a clinical situation.

제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
