Clinical Experience with Continuous Renal Replacement Therapy as a Method of Extracorporeal Elimination and as performed by Emergency Room Physicians for Patients with Poisoning (체외제거가 필요한 중독환자에서 응급의학과 의사에 의해 시행된 지속적신대체요법에 대한 임상적 고찰)

  • Purpose: Extracorporeal elimination of drugs is a critical part of managing poisonings, although the indications and optimal method remain a matter of debate. The aim of this study is to report our clinical experiences with continuous renal replacement therapy (CRRT), as performed by emergency room physicians, as method of extracorporeal drug elimination in patients with poisoning. Methods: This study was a retrospective study of the consecutive patients who underwent CRRT, as performed by an emergency room physician, for acute poisoning. The patient characteristics, the kinds of drugs and the method of extracorporeal elimination were analyzed by reviewing the patients' charts. Results: During eleven months, 26 patients with acute poisoning underwent extracorporeal elimination (2 patients; intermittent hemodialysis, 24 patients; CRRT). The mean time from the decision to performing extracorporeal elimination was $206.0{\pm}36.8$ minutes for intermittent hemodialysis, $62.9{\pm}8.5$ minutes for continuous venoveno-hemodiafiltration (CVVHDF) and $56.6{\pm}6.8$ minutes for charcoal hemoperfusion. For the patients with CRRT, CVVHDF was conducted in 10 patients (3 patients; valproic acid, 2 patients; Lithium, 1 patient; salicylates, 1 patient; methanol) and charcoal hemoperfusion by using CRRT was done in 14 patients (13 patients; paraquat, 1 patient; dapsone). For the 12 patients who required hemodialysis due to severe poisoning, 7 patients underwent CRRT because of their unstable vital signs. Conclusion: CRRT was an effective method of extracorporeal drug elimination in patients with acute poisoning, and especially for the cases with unstable vital sign and for those patients who required an early start of extracorporeal elimination according to the characteristics of the drug.   • Routh approximation method is the most computationally attractive. But this method may cause time-response error because this method does not match the time-response directly. In this paper a new mixed method for obtaining stable reduced-order models for high-order continuous-time systems is proposed. It makes use of the advantages of the Routh approximation method and the Minimization of Integral Squared Error(MISE) criterion approach. In this mixed method the characteristic polynomial of the reduced-order model is first obtained from that of original system by using the Auxiliary Denominator Polynomial(ADP). The numerator polynomial is then determined so as to minimize the intergral squared-error of unit step responses. The advantages of the propsed method are that the reduced models are always stable if the original system are stable and the frequency domain and time domain characteristic of the original system will be preserved in the reduced models.

  • A typical type-1 positioning servomechanism is theoretically analyzed as a multirate sampled-data system which contains two or more signals sampled at different rates by the Z-transform method. And also it is analyzed as a continuous system by using the Younsei 101 Electronic Analog Computer. Comparing the solution of the multirate sampled data system with that of the continuous system to a step input, it is concluded that the response time of the output of the multirate sampled-data system is reduced by a multirate controller, the ripple between samples is lessened and all the transients are diminished within one period.

  • The aim of this paper was to examine the continuous and discontinuous contact problems between the functionally graded (FG) layer pressed with a uniformly distributed load and homogeneous half plane using an analytical method and FEM. The FG layer is made of non-homogeneous material with an isotropic stress-strain law with exponentially varying properties. It is assumed that the contact at the FG layer-half plane interface is frictionless, and only the normal tractions can be transmitted along the contacted regions. The body force of the FG layer is considered in the study. The FG layer was positioned on the homogeneous half plane without any bonds. Thus, if the external load was smaller than a certain critical value, the contact between the FG layer and half plane would be continuous. However, when the external load exceeded the critical value, there was a separation between the FG layer and half plane on the finite region, as discontinuous contact. Therefore, there have been some steps taken in this study. Firstly, an analytical solution for continuous and discontinuous contact cases of the problem has been realized using the theory of elasticity and Fourier integral transform techniques. Then, the problem modeled and two-dimensional analysis was carried out by using ANSYS package program based on FEM. Numerical results for initial separation distance and contact stress distributions between the FG layer and homogeneous half plane for continuous contact case; the start and end points of separation and contact stress distributions between the FG layer and homogeneous half plane for discontinuous contact case were provided for various dimensionless quantities including material inhomogeneity, distributed load width, the shear module ratio and load factor for both methods. The results obtained using FEM were compared with the results found using analytical formulation. It was found that the results obtained from analytical formulation were in perfect agreement with the FEM study.

  • The determination of the critical buckling load of multistory structures is important since this load is used in second order analysis. It is more realistic to determine the critical buckling load of multistory structures using the whole system instead of independent elements. In this study, a method is proposed for designating the system critical buckling load of torsion-free structures of which the load-bearing system consists of frames and shear walls. In the method presented, the multistory structure is modeled in accordance with the continuous system calculation model and the differential equation governing the stability case is solved using the differential transform method (DTM). At the end of the study, an example problem is solved to show the conformity of the presented method with the finite elements method (FEM).

  • This report proposes a novel method of identification of continuous time-delay systems from sampled input-output data. By the aid of a digital pre-filter, an approximated discrete-time estimation model is first derived, in which the system parameters remain in their original form and the time delay need not be an integral multiple of th sampling period. Then an identification method combining the common linear least squares(LS) method or the instrumental variable(IV) method with the genetic algorithm(GA) is proposed. That is, the time-delay is selected by the GA, and the system parameters are estimated by the LS or IV method. Furthermore, the proposed method is extended to the case of multi-input multi-output systems where the time-delays in the individual input channels may differ each other. Simulation resutls show that our method yields consistent estimates even in the presence of high measurement noises.

  • CWSII method was developed to overcome the problems of frequent occurrence in the application of existing downward construction methods, especially in the case of using slurry wall instead of SCW or CIP as a retaining wall. By the improvements in connecting steel beams with the wall, CWSII method is able to ensure the settlement of a steel beam and the diaphragm effect of a slab while reducing the degree of difficulty and the term of works and the cost of construction. As the desired results, CWS method can be applied as a practical downward construction method regardless of the type of retaining wall. In this paper, besides the concept and features of CWSII method, it can be seen that the method can provide reliable and economical performances by comparing with existing methods.

Comparison of N2O Emissions by Greenhouse Gas Emission Estimation Method (온실가스 배출량 산정 방법에 따른 N2O 배출량 비교)

  • In this study GC and PAS were used to calculate $N_2O$ concentration of exhaust gas from Wood Chip combustion system. Fuel supplied to the incinerator was collected and analyzed and then the analysis result was used to calculate $N_2O$ emissions. Tier 3 and Tier 4 Method were used to calculate the $N_2O$ emissions. Plant's Specific emission factor of $N_2O$ by Tier 3 Method was 0.35 kg/TJ, while default emission factor of Wood?Wood Waste proposed by 2006 IPCC G/L was 4 kg/TJ. So the $N_2O$ emission factor of this study was 3.65 kg/TJ lower compared to the IPCC G/L. The total emissions calculated by Plant's specific emission factor was 4.22 kg during the measuring period, but by Tier 4 Method it was 7.88 kg. This difference in emissions was caused by the difference of continuous measuring and intermittent sampling. It would be necessary to apply continuous measuring to calculate emissions of $Non-CO_2$ gas whose the density distribution is relatively high. However currently, according to the target management guideline of greenhouse gas and energy, the continuous measuring method to calculate greenhouse gas emission is applied only to $CO_2$. Therefore for reliable greenhouse gas emission calculation it would be necessary to apply continuous measuring to calculate $Non-CO_2$ gas emission.

Characteristics of Field Uplift Tests of Continuous Greenhouse using the Load Control Method (하중 제어법을 이용한 파이프 줄기초의 현장 인발저항 특성)

  • Institutional inertia anti-disaster standard was presented mainly on the upper surface, it is necessary to improve to the soil type standard and uplift the resistance standard greenhouse that are vulnerable to strong winds. In this study, we carried out a field test using the load control method in order to evaluate the uplift resistance of continuous foundation of greenhouse with different depths of the rafters. Institutional inertia anti-disaster standard of greenhouse foundation did not protect the greenhouse structure from the damages caused by strong winds and heavy snow. Therefore, field tests for behavior characteristics of continuous greenhouse foundation were carried out to ensure stable facility cultivation. The field test condition was evaluated using different embedded depth as follows: 30cm, 40cm, 50cm and spacing 50cm, 60cm, 70 cm. As a result of the uplift resistance field tests using the load control method, the minimum uplift resistance was found to be over 90kg and uplift resistance displacement was 9.4mm. Uplift resistance of the continuous greenhouse foundation was in the range of 90-180 kg according to embedded depth and spacing. Using the test condition, there was no constant trend in the uplift resistance.

  • The Linear system analysis for physical system is very powerful tool for system diagnostic utilizing relationship between the input signal and output signal. This method utilized generally to investigate physical properties of system and the nondestructive test by ultrasonic signals. This method can be explained by linear system theory. In this paper the Continuous Wavelets Transform is utilized to search the relation between the linear system and continuous wavelets transform.
