The Effects of a Trauma Team Approach on the Management of Open Extremity Fractures in Polytrauma Patients: A Retrospective Comparative Study

  • Purpose: Open extremity fractures require prompt antibiotic medication and initial debridement surgery to reduce the infection rate and restore functional stabilization. We aimed to report the effects and positive outcomes of a trauma team approach on the management of open extremity fractures in polytrauma patients. Methods: This retrospective review included all polytrauma patients with open extremity fractures admitted between March 2009 and December 2019. Patients were divided into two groups according to whether they were treated before or after the implementation of the trauma team approach (March 2014). We analyzed the outcomes in each group with respect to the time interval until the doctor's arrival, total length of stay in the emergency department, the time interval until initial antibiotic treatment and operation, whether the initial operation was performed within 24 hours, and the rate of deep infections. Results: A total of 123 patients met the inclusion criteria. There were no statistically significant differences in demographic characteristics. The time interval until the doctor's arrival (64.12±49.2 minutes vs. 19.82±15.23 minutes; p=0.035) and initial antibiotic treatment (115.47±72.12 minutes vs. 48.78±30.12 minutes; p=0.023) significantly improved after implementing the trauma team approach. The union rate was not significantly different. However, the time interval until initial debridement, opportunity for initial debridement within 24 hours, and the rate of deep infections demonstrated better results. Conclusions: The reduced time interval until initial antibiotic treatment and debridement could be attributed to the positive effect of the trauma team approach on the management of open extremity fractures in polytrauma patients.

Effect of the Emergency Trauma Team's Management on the Treatment of Patients with Multiple Severe Trauma (다발성 중증 외상 환자들의 치료에 대한 응급 외상팀 운영의 효과)

  • Purpose: We performed this study to determine how the emergency trauma team affects the treatment of patients with multiple severe trauma and to discuss the effect and the direction of the emergency trauma team's management. Methods: We performed a retrospective analysis of 518 patients who visited our emergency department with severe trauma from August 2006 to July 2008. We divided the severe trauma patients into 2 groups : patients before and after trauma team management (Group 1 and Group 2). Then, we compared demographic characteristics, mechanisms of injury, and treatment outcomes (lengths of stay in the ED, admission ratio, and in-hospital mortality) between the 2 groups. In the same way, patients with multiple severe trauma were divided into 2 groups, that are patients before and after trauma team management (Group 3 and Group 4) and analyzed. Results: There was no significant difference, except mean age, between groups 1 and 2. In group 4 patients, compared to group 3 patients, the lengths of stay in the ED were lower (p value < 0.001), and the admission ratio were higher (p value = 0.017), but there was no significant difference in the in-hospital mortality between the groups 3 and 4. Conclusion: When patients with multiple severe trauma visit the ED, the emergency trauma team' management can decrease the lengths of stay in the ED and increase the admission ratio, but does not produce a decrease in the in-hospital mortality rate. Further investigations of emergency trauma team management are needed to improve treatment outcomes for patients with multiple severe trauma.

  • Si(Al)ON precursor was synthesized by formation of new Si-N bond using organoaluminum imine and liquid type poly(phenyl carbosilane). It was decomposed between $200-600^{\circ}C$, and the ceramic yield was 51% after pyrolysis. 150 - 200 nm in thickness of coating film was obtained by spin coating method. The precursor was easily oxidized during process because it was unstable in air. However the oxygen content was limited to 0.5 - 0.7 to silicon in heat treatment step. Even though the content of nitrogen was decreased by pyrolysis, Al-N and Si-N bonds were formed in ammonia atmosphere, and Si(Al)ON film was formed with 0.2 in content to silicon.

  • Effects of Zn and Zr additions on the microstructure and mechanical properties of Al-5%Mg-1%Mn alloys were investigated. As Zn content increased in the Al-Mg-Mn-Zn alloys, the tensile strength and ductility of as-cast alloys rather decreased while the tensile strength of the heat-treated alloys significantly increased mainly due to the precipitation of fine $MgZn_2$ phases. Small amount of Zr was added to the 3%Zn alloy to further enhance the mechanical properties, and it appeared to increase the strength and ductility, especially in as-cast state.

  • Influence of Ca addition on microstructure and room temperature mechanical properties has been studied for AZ31(Mg-3%Al-1%Zn-0.2%Mn)-(0~0.5)%Ca wrought alloys, based on experimental results from metallography, X-ray diffractometry and mechanical tests. Yield strength, ultimate tensile strength and hardness of the alloys increased remarkably with increasing Ca content, whereas elongation was deteriorated continuously. Microstructural examination revealed that Ca addition efficiently refined grains of ${\alpha}$(Mg) phase and that some of the Ca dissolved in ${\beta}(Mg_{17}Al_{12})$ precipitates. The former and the latter facts are thought to be responsible for improved strength and loss of ductility of the AZ31+Ca wrought alloys, respectively.

  • Low temperature plasma treatment with different gases and rf powers were performed to improve the adhesion strength between polytetrafluoroethylene(PTFE) and electroless deposited copper. According to the research, $H_2$ plasma having hydrogen radical was more effective in surface polarity modification than $O_2$ plasma due to the defluorination reaction. However, surface roughness of PTFE was more increased with $O_2$ than $H_2$ plasma. PTFE treated with $120W-O_2$ plasma and $250w-H_2$ plasma, consecutively showed rougher surface than single step $250w-H_2$ plasma treated one and more hydrophilic than single step $120W-O_2$ plasma treated one. And it showed 5B tape test grade, which is better adhesion property than 1B or 3B obtained by single step plasma treatment. In addition, adhesion strength between PTFE and Cu deposit is also deeply affected by residual water on its interface.

Diagnosis and management of the early defoliation of Korean flowering cherry in Gurye (구례 왕벚나무 가로수의 조기낙엽 진단 및 친환경적 관리 방법)

  • The Korean flowering cherry is a popular tree. However, the trees have started to defoliate early, including those in Gurye. Thus, it is necessary to identify the causes of the early defoliation and how to manage them. Therefore, the purpose of this study was threefold: 1) to investigate the early defoliation of flowering cherry trees, 2) to identify the differences in growth and flowering of the trees in response to treatment, and 3) to suggest an optimal treatment for the trees. The experiment was conducted in Gurye at a site 3km long with 102 flowering cherry trees along a street. There were three treatments: control, an environmentally friendly insecticide and a disinfectant(treatment 1), and an environmentally friendly insecticide, a disinfectant, and irrigation (treatment 2). The trees in Gurye were compared to trees on Jeju Island. The defoliation rates of the flowering cherry in Gurye were significantly higher than those on Jeju Island at each measurement time. Within Gurye, the defoliation rate was significantly higher in the trees of the control than in the two treatments and only the trees in the control from Gurye had shot-hole disease. Post-treatment, twigs collected from the control during April showed significantly poorer growth, lower numbers of flowering buds per twig, and lower numbers of foliar buds. The trees given treatment 1 showed significantly greater growth, number of total buds, flowering buds per twig, and density of buds than the trees given treatment 2. After analyzing the correlations, it was established that early defoliation led to poorer growth, affecting the flowering of the trees.

Characteristics of Algae Occurrence on Environmental Changes (환경변화에 따른 조류 발생 변화)

  • Pilot scale system was designed to identify the growth and movement of algae, depending on environmental changes(retention time, nutrient concentration, etc) in Gangjeong-Goryeong Weir of the Nakdong River. Considering the stability of algal culture and easy observation of algal growth, pilot scale system was made of transparent acrylic material(3 sets of flexible cylindrical water tanks with 1 m diameter and 4 m height). Auxiliary equipments include light intercepter, water inflow device for different water depth and storage of reclaimed water. The retention time was 2 days(before construction of weir; treatment 1), 8 days(after construction of weir, 2013; treatment 2) and 30 days(2014; treatment 3). According to the water temperature of treatment 1 were similar by depth, treatment 3 showed a difference between the surface(0 m) and bottom(4 m) more than $3^{\circ}C$. DO, pH showed relatively high in the surface than the bottom. Nutrients showed eutrophic condition in all experiments. The Chlrophyll-a concentration of the treatment 1 showed a relatively lower value than the Chlrophyll-a concentration of the treatment 2 and 3. Therefore, the retention time was considered to influence the growth of phytoplankton.
