Targeted Polymeric Gene Delivery for Anti-angiogenic Tumor Therapy

  • Gene therapy has become a promising strategy for the treatment of genetically based diseases, such as cancer, which are currently considered incurable. A major obstacle in the field of cancer gene therapy is the development of a safe and efficient delivery system for therapeutic gene transfer. Non-viral vectors have attracted great interest, as they are simple to prepare, stable, easy to modify and relatively safe compared to viral vectors. In this review, an insight into the strategies developed for polyethylenimine (PEI)-based non-viral vectors has been provide, including improvement of the polyplex properties by incorporating hydrophilic spacer, poly(ethylene glycol) (PEG). Moreover, this review will summarize the strategies for the tumor targeting. Specifically, a targeted polymeric gene delivery system, PEI-g-PEG-RGD, will be introduced as an efficient gene delivery vector for tumor therapy, including its functional analysis both in vitro and in vivo.

Optimized Image-Based Surrogate Endpoints in Targeted Therapies for Glioblastoma: A Systematic Review and Meta-Analysis of Phase III Randomized Controlled Trials

  • Objective: We aimed to determine the optimized image-based surrogate endpoints (IBSEs) in targeted therapies for glioblastoma through a systematic review and meta-analysis of phase III randomized controlled trials (RCTs). Materials and Methods: A systematic search of OVID-MEDLINE and EMBASE for phase III RCTs on glioblastoma was performed in December 2017. Data on overall survival (OS) and IBSEs, including progression-free survival (PFS), 6-month PFS (6moPFS), 12-month PFS (12moPFS), median PFS, and objective response rate (ORR) were extracted. Weighted linear regression analysis for the hazard ratio for OS and the hazard ratios or odds ratios for IBSEs was performed. The associations between IBSEs and OS were evaluated. Subgroup analyses according to disease stage (newly diagnosed glioblastoma versus recurrent glioblastoma), types of test treatment, and types of response assessment criteria were performed. Results: Twenty-three phase III RCTs published between 2000 and 2017, including 8387 patients, met the inclusion criteria. OS showed strong correlations with PFS (standardized β coefficient [R] = 0.719), 6moPFS (R = 0.647), and 12moPFS (R = 0.638). OS showed no correlations with median PFS and ORR. In subgroup analysis according to types of therapies, PFS showed the highest correlations with OS in targeted therapies for cell cycle pathways (R = 0.913) and growth factor receptors and their downstream pathways (R = 0.962). 12moPFS showed the highest correlation with OS in antiangiogenic therapy (R = 0.821). The response assessment in neuro-oncology criteria provided higher correlation coefficients between OS and IBSEs than the Macdonald criteria. Conclusion: Overall, PFS is an optimized IBSE in targeted therapies for glioblastoma; however, 12moPFS is optimal in antiangiogenic therapy.

Effects of Targeted Knee Flexion Angle on the Biomechanical Factors of Upward and Downward Phases during Forward Lunge

  • Objective: The aim of this study was to investigate the effect of targeted knee flexion angle on biomechanical factors of knee joint between upward and downward phases during the forward lunge. Method: Eight elderly subjects (age: $22.23{\pm}1.51years$, weight: $69{\pm}6.63kg$, height: $174.88{\pm}6.85cm$) participated in this study. All reflective marker data and ground reaction force during a forward lunge were collected. The knee joint movement and reaction force and joint moment at maximum knee flexion angle were compared by repeated measures one-way analysis of variance (ANOVA) (p<.05). The peak knee joint reaction force and joint moment between upward and downward phases were compared by repeated measures two-way ANOVA (p<.05). Results: The anterior and vertical knee joint movements, reaction force, and extensor moment of $80^{\circ}$ targeted knee flexion condition at maximum knee flexion angle was greater than both $90^{\circ}$ and $100^{\circ}$ conditions (p<.05). The $80^{\circ}$ knee flexed angle condition had greater peak joint reaction force and extensor moment compared with both $90^{\circ}$ and $100^{\circ}$ conditions between upward and downward phases during the forward lunge. Conclusion: As the targeted knee joint flexion angle increases, knee joint movement and kinetic variables become greater during the forward lunge exercise.

Analysis of mechanical properties of agricultural products for development of a multipurpose vegetable cutting machine

  • The consumption of pre-treated vegetables (including fresh-cut vegetables) that are washed, peeled, and trimmed has been significantly increased because of their easy use for cooking. Vegetable cutting machines have been widely utilized for producing fresh-cut vegetables or agricultural products of different sizes; however, the design standard is not established for specific types of agricultural products. Therefore, this study was conducted to determine mechanical properties (compressive and shear forces) of targeted agricultural products (radish, carrot, squash, cucumber, shiitake mushroom, and sweet potato) for developing a multipurpose vegetable cutting machine. According to ASAE standard (s368.3), compressive and shear forces of targeted agricultural products were measured by using a custom built UTM (universal testing machine). Shape type of samples and speed ranges (5 - 15 mm/min) of loading rate on bioyield and shear points varied depending on the targeted agricultural product. The range of averaged bioyield points of targeted agricultural products were between 7.89 and 146.98 N. On the other hand, their averaged shear points ranged from 22.50 to 53.47 N. Results clearly showed that the bioyield and shear points of targeted agricultural products were thoroughly affected by their components. As measuring compressive and shear forces of a variety of agricultural products, it will be feasible to calculate blade cutting force for designing multipurpose vegetable cutting machine.

  • 법유전학에서 개인의 신원확인을 위한 STR 프로필 분석이 불가한 경우, DNA를 이용한 외형추정특성을 이용하여 개인에 대한 정보를 얻을 수 있다. 최근 눈동자, 머리카락, 피부 색과 같은 외형추정특성을 확인하는 방법들이 연구되고 있지만, 이러한 외형추정특성 정보만 가지고는 한국을 비롯한 동아시아 지역에서 적용하기에는 한계가 있다. 본 연구에서는 개인의 외형과 관련된 표현형을 수사정보로서 활용하기 위해 눈 모양, 머리카락 굵기, 피부 색 뿐만 아니라 탈모, 체형, 고도근시, 얼굴모양, 여드름, 행동습관과 관련된 SNP를 탐색하였다. 이들 표현형과 관련된 50개의 SNP를 선정하여 한 번에 증폭할 수 있는 targeted amplicon NGS 방식의 multiplex PCR 패널을 개발하였다. 실험 결과 14개 샘플에서 50개 SNP의 대립유전자 유형과 빈도를 확인할 수 있었다. 향후 본 패널을 가지고 더 많은 샘플을 이용하여 유전형과 표현형 간 연관성 확인 및 결과 해석 방법을 분석할 예정이다.

Cost-Effectiveness Analysis of HPV Vaccination: Comparing the General Population with Socially Vulnerable Individuals

  • Background: After the WHO recommended HPV vaccination of the general population in 2009, government support of HPV vaccination programs was increased in many countries. However, this policy was not implemented in Korea due to perceived low cost-effectiveness. Thus, the aim of this study was to analyze the cost-utility of HPV vaccination programs targeted to high risk populations as compared to vaccination programs for the general population. Materials and Methods: Each study population was set to 100,000 people in a simulation study to determine the incremental cost-utility ratio (ICUR), then standard prevalence rates, cost, vaccination rates, vaccine efficacy, and the Quality-Adjusted Life-Years (QALYs) were applied to the analysis. In addition, sensitivity analysis was performed by assuming discounted vaccination cost. Results: In the socially vulnerable population, QALYs gained through HPV vaccination were higher than that of the general population (General population: 1,019, Socially vulnerable population: 5,582). The results of ICUR showed that the cost of HPV vaccination was higher for the general population than the socially vulnerable population. (General population: 52,279,255 KRW, Socially vulnerable population: 9,547,347 KRW). Compared with 24 million KRW/QALYs as the social threshold, vaccination of the general population was not cost-effective. In contrast, vaccination of the socially vulnerable population was strongly cost-effective. Conclusions: The results suggest the importance and necessity of government support of HPV vaccination programs targeted to socially vulnerable populations because a targeted approach is much more cost-effective. The implementation of government support for such vaccination programs is a critical strategy for decreasing the burden of HPV infection in Korea.

국산 유기가공식품 소비의향 분석 (Analysis of Consumption of Homemade Organically Processed Food)

  • The purpose of this study is to analyze consumption of homemade organically processed food (HOPF), and to derive directions for consumption promotion of HOPF. A survey was conducted for quantitative analysis regarding consumption. This study used an Ordinal Logistic Regression Model to derive more significant results in analyzing factors of consumption. The findings was that younger consumers with high income are more likely to purchase HOPF. And those consumers with high price and quality contentment are more likely to purchase HOPF. And contentment with certification institutions and improvement of health have a significant positive relationship with consumption. Consumers were found to pay 51 percent more for HOPF than for non-HOPF products. This level show that the current level of price premium for HOPF is 51 percent higher than their desired level. In order to reduce the price premium for HOPF, effective policy programs should be developed. A targeted market strategy to sell HOPF to younger consumers with high income is needed to boost consumption. A strict certification management system should be established to enhance consumer reliability in HOPF.

Screening for Patients with Non-small Cell Lung Cancer Who Could Survive Long Term Chemotherapy

  • Background: Lung cancer was one of the most common cancers in both men and women all over the world. In this study, we aimed to clarify who could survive after long term chemotherapy in patients with advanced non-small cell lung cancer (NSCLC). Methods: We enrolled 186 patients with stage IV NSCLC after long term chemotherapy from Jun 2006 to Nov 2014 diagnosed in Jiangsu Cancer Hospital. Multiple variables like age, gender, smoking, histology of adenocarcinoma and squamous-cell cancer, number of metastatic sites, metastatic sites (e.g. lung, brain, bone, liver and pleura), hemoglobin, lymphocyte rate (LYR), Change of LYR during multiple therapies, hypertension, diabetes, chronic bronchitis, treatments (e.g.radiotherapy and targeted therapy) were selected. For consideration of factors influencing survival and response for patients with advanced NSCLC, logistic regression analysis and Cox regression analysis were used in an attempt to develop a screening module for patients with elevated survival after long term chemotherapy become possible. Results: Of the total of 186 patients enrolled, 69 survived less than 1 year (short-term group), 45 one to two years, and 72 longer than 3 years (long-term group). For logistic regression analysis, the short-term group was taken as control group and the long-term group as the case group. We found that age, histology of adenocarcinoma, metastatic site (e.g. lung and liver), treatments (e.g. targeted therapy and radiotherapy), LYR, a decreasing tendency of LYR and chronic bronchitis were individually associated with overall survival by Cox regression analysis. A multivariable Cox regression model showed that metastatic site (e.g. lung and liver), histology of adenocarcinoma, treatments (e.g. targeted therapy and radiotherapy) and chronic bronchitis were associated with overall survival. Thus metastatic site (e.g. lung and liver) and chronic bronchitis may be important risk factors for patients with advanced NSCLC. Gender, metastatic site (e.g. lung and liver), LYR and the decreasing tendency of LYR were significantly associated with long-term survival in the individual-variable logistic regression model (P<0.05). On multivariate logistic regression analysis, gender, metastatic site (e.g. lung and liver) and the decreasing tendency of LYR associated with long-term survival. Conclusions: In conclusion, female patients with stage IV adenocarcinoma of NSCLC who had decreasing tendency of LYR during the course therapy and had accepted multiple therapies e.g. more than third-line chemotherapy, radiotherapy and/or targeted therapy might be expected to live longer.

  • Incompatible plant-pathogen interactions result in the rapid cell death response known as hypersensitive response (HR) and activation of host defense-related genes. To understand the molecular and cellular mechanism controlling defense response better, several approaches including isolation and characterization of novel genes, promoter analysis of those genes, protein-protein interaction analysis and reverse genetic approach etc. By using the yeast two-hybrid system a clone named Tsipl, Tsil -interacting protein 1, was isolated whose translation product apparently interacted with Tsil, an EREBP/AP2 type DNA binding protein. RNA gel blot analysis showed that the expression of Tsipl was increased by treatment with NaCl, ethylene, salicylic acid, or gibberellic acid. Transient expression analysis using a Tsipl::smGFP fusion gene in Arabidopsis protoplasts indicated that the Tsipl protein was targeted to the outer surface of chloroplasts. The targeted Tsipl::smGFP proteins were diffused to the cytoplasm of protoplasts in the presence of salicylic acid (SA) The PEG-mediated co-transfection analysis showed that Tsipl could interact with Tsil in the nucleus. These results suggest that Tsipl-Tsil interaction might serve to regulate defense-related gene expression. Basically the useful promoters are valuable tools for effective control of gene expression related to various developmental and environmental condition.(중략)
