An Exploratory Study of the Effect of Ongyeong-tang Administration on Insomnia Disorder in Patients with Neurological Diseases: A Retrospective Chart Review Study

  • Objectives: The aim of this study was to retrospectively evaluate the efficacy and safety of Ongyeong-tang (OGT) in the treatment of insomnia in patients afflicted with neurological diseases, according to the medical records of one medical institution. OGT is a herbal prescription that originates from the "Synopsis of Prescriptions of the Golden Chamber" and has been used for menopausal symptoms, uterine diseases, skin diseases, insomnia, and other neuropsychiatric symptoms. Methods: Medical records of patients who visited Kyung Hee University Korean Medicine Hospital from January 1, 2021 to June 30, 2022, received outpatient or inpatient treatment, and met the inclusion criteria were used in this study. Results: Twenty-four patients satisfied the inclusion criteria and were included. The Pittsburgh Sleep Quality Index-Korean version (PSQI-K) score was used to evaluate the outcomes before and after treatment. After an average of 14.42±9.2 days of OGT extract intake, 20 patients (83.3%) experienced improvement. The remaining four (16.7%) had no change in their symptoms, but none demonstrated worsening of symptoms. The total PSQI-K score showed a statistically significant decrease from 13.08±4.54 to 10.42±4.58 (p<0.001). Sleep quality, sleep duration, sleep disturbance, use of sleeping medications, and daytime dysfunction also showed statistically significant improvements. Conclusions: Administration of OGT extract may be a relatively safe and effective therapeutic option for insomnia in patients with neurological diseases.

A Clinical Study on 2 Cases of Premature Ovarian Failure by Serum Hormone Assay (호르몬 검사를 통하여 확인한 조기난소부전 환자 2례의 임상 경과 보고)

  • Objectives: The purpose of this study is to report the effects of oriental medicine on 2 patients with premature ovarian failure. Methods: The patients who had amenorrhea caused by premature ovarian failure were treated by oriental medicine such as herb medication, acupuncture and moxibustion. We evaluated the results of treatments by change of symptoms, serum hormone assay, basal body temperature and Kupperman's Index. Results: After oriental medicine, the patients could have their normal menstrual cycle. Also serum hormone assay, basal body temperature and Kupperman's Index were improved. Conclusions: This study shows that oriental medicine has effects on patients with premature ovarian failure. And serum hormone assay, basal body temperature and Kupperman's Index would be valid measurement to evaluate the effect of treatments for premature ovarian failure.

The Effects of Moxibustion on Cornell Medical Index in the Elderly (쑥뜸요법이 노인의 CMI에 미치는 효과)

  • Purpose: To improve health care of the elderly, we have used Moxibustion to examine the effectiveness of this product from 1st Feb. 2007 to 31 st May for 78 people who are older than 65 years old twice a week. Method: The tool of research was modified Cornell Medical Index(CMI) which Nam Ho Chang(1965) modified for the Korean and CMI which consists of 26 questions. Data were analyzed in frequency, paired t-test by SPSS. Result: Hypothesis: 'It was supported that CMI increases for those who have treated Moxinustion'. There were significant differences between before and after using Moxibustion when the symptom is one of those which are sore throat, tonsillar, edema, tightness of nose, common cold, cough, tightness of chest, palpitation, dyspnea, edema of leg, difficulty of urination, anorexia, heavy on stomach, nausea, vomiting, dyspepsia, fullness of abdomen, stomachache, diarrhea, constipation, incontinency of stool, arthralgia, shoulder or backache, lumbago, headache, burning or chillness, vertigo and dizziness. Conclusion: As a result, Moxibustion is effective for such symptoms so that it is considered to be using in everyday life this product for better health care.

The Clinical Study on the Effect of Sulguanjul-bang No.1 on Osteoarthritis of Knee Joint (퇴행성 슬관절염 환자에서 슬관절방(膝關節方) 1호(號)의 유효성 연구)

  • Objective : This study is designed to find out the effects of Sulguanjul-Bang No.1 on osteoarthritis in knee joint. Methods : 45 patients with symptomatic osteoarthritis of the knee joint were enrolled in a short term 8-week open clinical trial. The 45 patients were taken Sulguanjul-bang No.1 two times a day before meals for 8 week. After 8 week treatment period, the following parameters were analyzed ; Visual analogue Scale(VAS) as first effective parameter, Lysholm index score, patient's global assessment, passive movement range of the knee joint. Results : Sulguanjul-bang No.1 treatment led to significant improvement in the pain and symptoms of osteoarthritis as determined by all efficacy measures. After 8 weeks of therapy, there was significant improvement in VAS and Lysholm index; squat, sit down and up, crepitaion, swelling. The range of motion was improved in 6 of 9 cases. The improvement in patient's global assessment was seen in 84%. Conclusion : Sulguanjul-bang No.1 was very beneficial effect in pain relief and improvement functions on osteoarthritis.

Comparison of Prevention of TMD Between Elderly and Young Population (노인과 청년간의 측두하악장애증 발현양상 비교)

  • The author performed on epidemiological study of the TMD in 189 elderly people and 195 young people with Helkimo index. The clinical dysfunction index was based on data from clinical examination and the anamestic dysfunction index was based on data from the interview with the investigated person. The results were as follows : 1. In the elderly population, 27.5% reported that they had subjectively symptom of TMD but 43.4% had sign and symptoms of TMD in clinical examination. 2. The mean values for maximal opening differed significantly between elderly and young populations, 47.22mm and 51.44mm (p<0.001) and less than a 40mm opening was observed 7.9% in elderly population, 0.5% in young population (p<0.001). 3. The mean values for lateral movement to the right and left did not differ with age, which were 8.56mm and 8.47mm in elderly population, 8.90mm and 8.81mm in young population, but the mean value for protrusion differed significantly between elderly and young populations, 6.89mm and 7.64mm (p<0.01). 4. A higher incidence of TMJ noise was recorded in the elderly and young populations than young population, especially crepitus, but a higher incidence of clicking was recorded in young population (p<0.05).

Weight Loss Effect of Wax Gourd (동아의 체중조절 효과)

  • This study attempted to evaluate the effectiveness of function of Wax Ground for obesity and the related disease of adult people can be developed by the combination of the 17 food materials screened in this study. The experiments using Sprague-Dawley male rats show that seventeen kinds of food material inhibited the increase of body weight gain, energy intake, fat pads weight, liver triglycerides content, atherogenic index, and blood glucose concentration induced by cafeteria diet. Oral administration of wax gourd decreased most body weight gain, energy intake, fat pads weight, and blood glucose concentration. Liver triglycerides content was the lowest in the group administered mulberry stem extract and the group administered pine leaves showed the lowest atherogenic index. The combined administration of wax gourd, mulberry stem extract and pine leaves improved more the obesity and related symptoms than the individual administrations.

Case Report of Efficacy and Safety of Long Term Medication of Oncheongeum-gamibang (온청음 가미방 3개월간 장기복용의 유효성과 안전성 보고)

  • This study is designed as pilot study to report the efficacy and safety of long term medication of Oncheongeum-gamibang on atopic dermatitis. The patient of atopic dermatitis was asked to take Oncheongeum-gamibang for 3 months. Every end of month, he had gotten hematological test and SCORAD Index examination. After the clinical trial was done, he had additional examination to see the changes and duration of medicinal effect. During the period of taking Onchung-um, estimation of symptoms by SCORAD Index examination showed little bit of improvement while taking medicine and after one month, the improvement was maintained. There were no significant changes in hematologic test and liver function test during and at the end of the clinical trial. We were able to find out that it is effective and safe to take long term medication of Oncheongeum-gamibang for an individual. However, due to deficiency of the cases and immunological values, there will be additional study to make up for better result.

Quality of Life of Women Undergoing Chemotherapy for a Gynaecological Oncological Disease in Turkey

  • Aim: Studies have shown effects of surgery, radiation and chemotherapy on quality of life in cases of gynaecological cancer. Very few studies are available examining the quality of life of individuals in Turkey who have been diagnosed with gynaecological cancer and undergoing treatment. Method: This study was performed to evaluate the quality of life of such patients using the EORTC-QLQ-C30 Quality of Life Index. Chi-square Yates, Mann-Whitney-U tests and variance analysis used for statistical analizing. Results: The EORTC-QLQ-C30 Quality of Life Index mean points for "general well-being and quality of life" of the patients were found to be $60.5{\pm}25.0$. In the sub-groups of the Quality of Life Index determined fatigue ($60.1{\pm}24.8$), economic difficulties ($46.9{\pm}33.3$), pain and loss of appetite ($42.9{\pm}27.8$; $42.9{\pm}34.0$) and insomnia ($40.1{\pm}34.0$) were the symptoms most reported to have a negative effect on quality of life. Statistical significance was noted for marital status and income status (p<0.05) but not educational level. Conclusion:Determination of quality of life of women with a diagnosis of gynaecological oncological disease who are undergoing chemotherapy enables provision of a more comprehensive and higher quality of care.

A Case Report of an Insomnia Patient Treated with Gamiguibi-tang (加味歸脾湯으로 호전된 불면 환자 치험 1례)

  • Objective: To report the effect of Gamiguibi-tang (加味歸脾湯) as a treatment for insomnia. Methods: An insomnia patient who scored 27 points for her insomnia severity index was enrolled in this study. She received oriental treatment with herbal medication (Gamiguibi-tang), acupuncture, and moxibustion for 1 month. Results: After treatment, sleep duration and sleep quality were improved. After the patient had taken Gamiguibi-tang, her score for the Pittsburgh Sleep Quality Index (PSQI) decreased from 21 to 9, and associated symptoms were also improved without taking sleeping pills. Conclusions: Gamiguibi-tang is effective for the treatment of insomnia. More clinical data and studies are required in patients suffering from insomnia.

Cephalometric Characterstics and their Relationship to Respiratory Disturbance Indices in Snorer and Patients with Obstructive Sleep Apnea (코골이와 폐쇄성 수면무호흡증 환자의 두부규격방사선사진 상의 특징 및 호흡장애지수와의 상관관계)

  • Objectives : The purposes of the study was to evaluate cephalometric characteristics in snorers and patients with obstructive sleep apnea (OSA) and to see any relationships between the cephalometric measurements and respiratory disturbance indices (RDI). Materials and Methods : Twelve snoring patients, 11 patients with OSA, and 10 normal subjects were included for the study. After taking a screening sleep study for a night to obtain RDI, $SaO_2$, and snoring index, a detailed cephalometric analysis was conducted to obtain SNA, SNB, SN-MP, IAS, MAS, SPAS, SAAS, Mn-H, and PNS-P. All the data were compared between groups. For a group including 12 snoring patients and 11 OSA patients, correlation coefficients were calculated between respiratory disturbance index and each item of cephalometric measurements. Results : The mandible and the maxilla are retropositioned. The oropharyngeal and hypopharyngeal airway is reduced in area and is a factor that could produce or enhance OSA symptoms. The hyoid bone is displaced inferiorly. The soft palate is significantly elongated. Conclusions : These data suggest that cephalometric evaluation could be useful to evaluate snoring and OSA patients, and to assist with the planning treatment for improvement of upper airway patency.
