Subfertility Problems Leading to Disposal of Breeding Bulls

  • Subfertility problems are encountered frequently in the cattle and buffalo bulls commercially maintained for semen production in dairy farms and under field conditions for natural insemination. Reports are scarce on the incidence of subfertility in breeding bulls, especially in India. The objective of the present study was to assess the incidence of the male reproductive anomalies leading to disposal of bovine bulls at GADVASU dairy farm, Ludhiana, Punjab (India). Data on frequency of various subfertility and disposal pattern of bulls maintained at the dairy farm, GADVASU, were collected for 12 yrs (1999 to 2010) and compiled from different record registers. Percentage of bulls that produced freezable semen (out of reserved ones) was less in cattle (25.641%) as compared to that of buffalo (30.4%). Various subfertility traits like poor libido and unacceptable seminal profile were found to be the significant reasons (p<0.01) for culling of the breeding bulls. Inadequate sex drive and poor semen quality were the main contributing factors for bull disposal in cattle whereas poor semen freezability was most frequently observed in buffalo bulls. All the male reproductive traits were significantly different (p<0.05) for the periods of birth, except for semen volume, initial motility (IM), age at last semen collection (ALSC) and age at disposal. The ages at first and last semen collection as well as freezing (i.e. AFSC, ALSC and AFSF, ALSF, respectively) and age at disposal (AD) were higher in buffalo. The spermatological parameters and semen production period (SPP) were higher in cattle. The age at first semen donation and breeding period could be reduced by introducing the bulls to training at an early age. The results revealed an increasing trend in individual motility (IM) while semen volume, AFSC, AFSF, AD, FSPP, SPP, ALSC and ALSF showed a decreasing, however, not a definite trend, over the periods. The semen donation traits like, AFSF, of the cattle and buffalo bulls could be predicted from the AFSC, using prediction equation derived in the present study.

  • 울산광역시 동구 보건소 협력 사업으로 한 2014년도 한방 난임 치료 1증례를 포함한 6증례중에서 난임의 치료방법으로 4증례는 FCST의 편차수정, 음양균형장치를 포함한 PBT, 침 치료, 부항, 뜸, 한약 등의 복합치료로 전신 증상의 개선 및 감소에 현저한 치료효과가 나타남을 관찰하였다. 2증례는 음양균형장치와 PBT는 제외되었다. 또한 그 결과로 6증례 모두 자연 임신에 성공하여 출산하였고 출산한 아기들도 건강한 상태임을 확인하였다.

  • 울산광역시 ${\bigcirc}$구 보건소 협력 사업으로 한 2015년도 한방 난임 치료 2증례에서는 난임의 치료방법으로 FCST의 편차수정, 음양균형장치를 포함한 PBT, 침 치료, 부항, 뜸, 한약 등의 복합치료로 전신 증상의 개선 및 감소에 현저한 치료효과가 나타남을 관찰하였다. 또한 그 결과로 2증례 모두 자연 임신에 성공하여 출산하였고 출산한 아기들도 건강한 상태임을 확인하였다.

  • Objectives: The purpose of this study is to investigate the effect on both herbal medicine periodic therapy and western medicine for polycystic ovary syndrome subfertility. Methods: We searched 8 electronic databases and search keywords were 'PCOS' and 'periodic therapy' or 'cyclic treatment'. We included randomized controlled clinical trials (RCTs) using the periodic therapy combined western medicine for PCOS patients. Results: We selected 8 studies. In studies, there were three studies in which the menstrual cycle was divided into 2 periods, one study divided into 3 periods, and four study divided into 4 periods. The meta-analysis of the 7 trials indicated that pregnancy rates integrated clomiphene and periodic therapy were higher than clomiphene alone. Conclusions: The periodic therapy combined western medicine for PCOS subfertility patients seems to improve pregnancy rates from this research. However, this result should be taken cautiously by unclear risk of bias. It would be necessary to fulfill further clinical study with herbal medicine periodic therapy on PCOS in Korean medicine to establish standard evidence of them.

  • The thyroid hormones act on nearly every cell in the body. Moreover, the thyroid gland continuously interacts with the ovaries, and the thyroid hormones are involved in almost all phases of reproduction. Thyroid dysfunctions are relatively common among women of reproductive age, and can affect fertility in various ways, resulting in anovulatory cycles, high prolactin levels, and sex hormone imbalances. Undiagnosed and untreated thyroid disease can be a cause of subfertility. Subclinical hypothyroidism (SCH), also known as mild thyroid failure, is diagnosed when peripheral thyroid hormone levels are within the normal reference laboratory range, but serum thyroid-stimulating hormone levels are mildly elevated. Thyroid autoimmunity (TAI) is characterized by the presence of anti-thyroid antibodies, which include anti-thyroperoxidase and anti-thyroglobulin antibodies. SCH and TAI may remain latent, asymptomatic, or even undiagnosed for an extended period. It has also been demonstrated that controlled ovarian hyperstimulation has a significant impact on thyroid function, particularly in women with TAI. In the current review, we describe the interactions between thyroid dysfunctions and subfertility, as well as the proper work-up and management of thyroid dysfunctions in subfertile women.

가임기(可姙期) 여성(女性)의 임신경험(姙娠經驗)에 따른 양생수준(養生水準)과 난임변증(難姙辨證) (Yangsaeng Level and Pattern Identification of Subfertility in Childbearing Aged Women according to the Pregnancy Experience)

  • Objectives : The purpose of this study is to identify Yangsaeng level and pattern identification of subfertility in childbearing aged women according to the pregnancy experience. Methods : Data was collected by structured questionnaire distributed to 217 women(125 women experienced pregnancy, 92 subfertile women) in I city from Nov. to Dec. 2014. For data analysis, Chi-square test, Fisher's exact test, t-test and Pearson's correlation coefficients were performed using SPSS version 19.0. Results : Drinking rate and stress level of subfertile group were higher than pregnancy experience group. Subfertile women group have more disease related to reproductive system, irregularities of the menstrual cycle and dysmenorrhea than pregnancy experience group. Subfertile women showed lower scores for Yangsaeng level but higher scores for all types of pattern identification of subfertility more than those in the women experiencing pregnancy. Conclusions : The results of this study indicate that reproductive health program for the childbearing aged women is necessary before serviced by western or oriental medical treatment for subinfertile(infertile) women. These results can be used for a program based on oriental medicine in order to improve women's health.

2018년도 경기도 한의 난임 지원 사업 진료 결과 분석 연구 (A Study Assessing 2018 Gyenggi-do Korean medicine Support Project for Subfertility)

  • Objectives: This study aims to assess the Gyenggi-do support project of Korean medical treatment in subfertility in 2018. And we would like to propose a plan to improve the project by analyzing the results of the project. Methods: We got the medical records of 260 participants that personal information was deleted by the association of Korean Medicine in Gyenggi-do. We collected the participants' characteristics, treatment records, results of treatment, laboratory results and satisfaction survey results. We coded the data and analyzed them using SPSS 23 at 5% significance level. Results: After the treatment, 11.0% of participants became pregnant spontaneously. There was no significant change in blood test before and after the treatment and observation period, and there was no adverse event during the project. After the project, the satisfaction survey was conducted. 84.2% of participants answered that they were satisfied with the result of the treatment. Conclusions: We analyzed the results of Gyenggi-do support project of Korean medical treatment for infertile women in 2017 and 2018. This study suggests the effectiveness and safety of Korean medical treatment for infertile women.

2017년도 경기도 한방 난임 지원 사업 진료 결과 분석 연구 (A Study on the Subfertility Support Project of Korean Medicine in Gyeonggi-do)

  • Objectives: This study aims to assess the subfertility support project of Korean medicine in Gyeonggi-do. Methods: A total of 231 women ($36.84{\pm}3.79yrs$) out of 361 applicants completed the treatment in this study. The copies of the medical records and study-related documents were provided by the association of Korean Medicine in Gyeonggi-do, in which the personal information was discarded. Descriptive analysis and inferential statistics (e.g., Mann-Whitney U test, Paired t-test, Wilcoxon signed rank test, & logistic regression) were applied to examine the statistical differences between groups and before/after treatments utilizing SPSS 23. All Type I errors (${\alpha}$) for the statistical significance were set at .05 Results: After the treatment, 11.3% of participants became pregnant spontaneously. There was no significant change in blood tests before and after the treatment and the observation period. Also there was no adverse event during the project. After the project, the satisfaction survey was conducted, and 87.5% of participants were satisfied with the project. Conclusions: The findings of this study proved the efficacy of Korean medical treatment for infertile women.

ICSI시대에서의 남성불임 (Male Infertility in the Era of ICSI)

  • As a result of the technological advance provided by intracytoplasmic sperm injection (ICSI) in 1992, the evaluation and treatment of the infertile male has changed significantly. Many men who were previously thought to be irreversibly infertile have the potential to initiate their own biologic pregnancy. However, not all men having impaired semen parameter are ideal candidates for ICSI for numerous reasons including a lack of addressing the underlying problem causing the male infertility, unknown genetic consequences, and cost-effectiveness issues. In this era of ICSI, the fundamental approach to the male with suspected subfertility is unchanged and is based on a history, physical examination, and focused laboratory testing. The urologist should approach the patient with an intent to identify remediable causes of subfertility given the specific clinical situation. For instance, should a gentleman have his varicocele repaired or vasectomy reversed, or should he proceed directly with ICSI? If no factors can be improved in a timely manner, then ICSI should be considered using the available sperm. Examples of recent advances include the diagnosis and treatment of ejaculatory duct obstruction, indications and techniques for performing testis biopsy, and technique for sperm harvesting. In addition, potential genetic causes of male subfertility should be diagnosed and discussed with the patient. Cystic fibrosis gene mutation, karyotype abnormallities, and Y-chromosome microdeletions all have recently been identified as causative for male infertility in otherwise phenotypically normal men. With recently evolved diagnostic and therapeutic techniques now available for the infertile couple, even the most severe male factor problems in patients previously considered irreversibly infertile are now potentially treatable. The physician should be aware of the availability and limitations of these new and exciting reproductive technologies because they will allow him to provide timely and more effective therapy for the infertile couple. An understanding of these advances by all physicians is important as we progress into the $21^{st}$ century

Subfertility in Males: An Important Cause of Bull Disposal in Bovines

  • The study had two objectives, namely, to estimate the andrological disorders leading to disposal of Karan Fries (KF), Sahiwal cattle and Murrah buffalo bulls and to study the effect of various factors (species/breeds, season of birth and period of birth) on male reproductive parameters. Records on occurrence of subfertility problems and disposal pattern of bulls maintained at the National Dairy Research Institute herd were collected for 15 years (1991 to 2005). Percentage of bulls producing freezable semen was less in the crossbred cattle (58.46%) as compared to Sahiwal (81.69%) and Murrah bulls (81.05%). Various subfertility traits like poor libido and unacceptable seminal profile were found to be the significant reasons (p<0.01) for culling of the breeding bulls. Inadequate sex drive was the main contributing factor for bull disposal in Sahiwal (22.55%) and Murrah bulls (15.12%) whereas poor semen quality and freezability were most frequently observed in KF bulls (24.29 and 7.29 percent, respectively). Least squares analyses of different male reproductive parameters showed that species/breeds had significant effect (p<0.05) on all traits except for frozen semen production periods (FSPP). Periods of birth were significantly different (p<0.05) for all traits except for semen volume. Age at first semen collection (AFSC), age at first semen freezing (AFSF) and age at disposal (AD) were highest in Murrah, while frozen semen production period (FSPP) and semen production period (SPP) were highest in KF and lowest in Sahiwal. The age at first semen donation and breeding period could be reduced by introducing the bulls to training at an early age. These results revealed a declining trend in AFSC, AFSF, FSPP, SPP and AD, thereby indicating an improvement in reproductive performance over the years. The age at first semen donation in bovines can be reduced by introducing the young male calves to training at an early age, which could increase the dosage of semen obtained from each male.
