발등 굽힘 제한이 있는 사람에게 장딴지근 신장운동과 발바닥 자가근막이완이 발등 굽힘 각도와 장딴지근 긴장도에 미치는 즉각적인 효과 비교 (A Comparative Study on the Immediate Effect of Performing Gastrocnemius Stretching with and without Myofascial Release of the Sole on Ankle Dorsiflexion Angles and Gastrocnemius Muscle Tone in Subjects with Limited Ankle Dorsiflexion)

  • PURPOSE: This study compared the effect of performing gastrocnemius stretching with and without the self-myofascial release of the sole on the active and passive ankle dorsiflexion angles and muscle tone of the gastrocnemius muscle in subjects with short gastrocnemius muscle. METHOD: A total of 23 subjects with short gastrocnemius muscles were included in this study. The study participants were divided into two experimental groups. Group A performed gastrocnemius muscle self-stretching exercises only, while group B performed self-myofascial release of the sole using a massage ball after the gastrocnemius muscle self-stretching exercises. For both groups, the active and passive ankle dorsiflexion angles were measured using a goniometer, and the tone of the gastrocnemius muscle was assessed using the MyotonPRO®. RESULTS: Within-group comparison showed that the participants in both groups A and B had significantly increased active and passive ankle dorsiflexion angles and decreased gastrocnemius muscle tone (p < .05) after performing their respective exercises. However, no significant differences in the said criteria were observed between groups A and B (p>.05). CONCLUSION: The results of this study showed that both methods were effective in increasing active and passive dorsiflexion angles and decreasing muscle tone. Thus, it is recommended to tailor gastrocnemius stretching exercises according to the patient's condition. If the patient does not experience discomfort in the plantar fasciae, it is recommended to perform the gastrocnemius stretching exercise only without myofascial release and use a massage ball afterward.

Immediate Effect of Intermittent Versus Continuous Hamstring Static Stretching on the Muscle Tone and Range of Motion

  • PURPOSE: This study examined which stretching exercise had the most positive effect on increasing the range of motion (ROM) of the knee joint in healthy people and whether there was a difference between continuous stretching and intermittent stretching. METHODS: This study included 30 healthy university students from OOO University. The subjects were asked to sit on a mat and perform hamstring-stretching exercise during which the ROM and muscle tone were measured with pre and post-tests. Each subject was assigned randomly to a continuous stretching group (stretching without relaxation time group, n=10, G1) or intermittent stretching group (stretching with 10s relaxing time group, n=10, G2; and stretching with 20s relaxing time group, n=10, G3). The participants conducted hamstring stretching exercises with a sit-and-reach box at three different rest times (0s, 10s, and 20s). Subsequently, they underwent passive knee extension (PKE) tests, in which the ROM of the knee joint was measured with a goniometer, and the muscle tone was evaluated using a MyotonPro. RESULTS: Significant differences in muscle tone, stiffness, and ROM were observed between pre-test and post-test in each groups (p<.05). Although the post hoc tests indicated no significant differences in muscle tone and ROM between the continuous stretching group and intermittent stretching group (p>.05), the rate of change of the ROM showed that the intermittent stretching group developed more effective maintenance of the hamstring flexibility. CONCLUSION: No significant differences in the muscle tone of the hamstring and ROM of the knee joint were observed according to the hamstring stretching exercises with three different rest times. On the other hand, the rate of change of the ROM showed that intermittent stretching maintained the hamstring flexibility more effectively.

The Effect of Stretching and Elastic Band Exercises Knee Space Distance and Plantar Pressure Distribution during Walking in Young Individuals with Genu Varum

  • PURPOSE: The purpose of this study was to investigate the effects of stretching and elastic band exercise on the knee space distance and plantar pressure distribution in people with genu varum. METHODS: The subjects of this study were students of a college who had genu varum of 14 subjects. The subjects were randomly divided into two groups as a stretching group (n=7, 4 males and 3 females, age: $20.14{\pm}2.54years$, height: $167.1{\pm}9.78cm$, weight: $58.6{\pm}10.13kg$) and a Thera-band group (n=7, 5 males and 2 females, age: $19.85{\pm}2.04years$, height: $166.5{\pm}5.82cm$, weight: $54.2{\pm}5.59kg$). The stretching and the There-band exercises were performed three times per a week, for four weeks. We measured changes in plantar pressure during walking, using a Gait Analyzer and distance of both knees at pre and post-intervention. RESULTS: These results suggest that the space distance of both knees showed differences before and after the intervention. The plantar pressure distribution was no changes in both groups before and after the intervention except for the left foot in a stretching group. CONCLUSION: As a result, the space distance of knees in both groups was significantly reduced. These result suggested that the Thera-band and stretching exercises were effective ways in alleviating genu varum.

Effect of Breathing Exercises via Joint mobilization on the Lung Function and Spinal Alignment of Straight-necked Women

  • PURPOSE: This study examined the effects of breathing exercises via joint mobilization on the lung function and spinal alignment of 30 straight-necked women in their 20 s and 30 s. METHODS: The participants were divided into two groups: an experimental group who performed breathing exercises via joint mobilization, and a control group who performed general stretching exercises. The differences between the two groups were measured and compared at three points in time: during the pre-examination, after four weeks of intervention, and after a four-week maintenance period. The changes in each group were measured before and after the intervention and after a one-month maintenance period and compared. For data analysis, the SPSS 22 was used to obtain the mean and standard deviation with a significance level of α = .05. RESULTS: Eight weeks of breathing exercises via joint mobilization and general stretching exercises positively impacted the lung function and spinal alignment in straight-necked patients. In particular, the experimental group showed more significant differences in spinal alignment after four weeks period of intervention and four weeks of maintenance. CONCLUSION: Breathing exercises via joint mobilization can be considered an effective intervention that can improve the respiratory volume and spinal alignment for patients with straight necks.

수동신장운동과 탄력테이핑 동시적용이 위등세모근의 근긴장도에 미치는 영향 (The Effect of Combination of Passive Stretching Exercises and Elastic Taping on Upper Trapezius Muscle Tone)

  • PURPOSE: This study aimed to explore the effect of applying elastic taping and passive stretching exercises simultaneously on the muscle tone of the upper trapezius. METHODS: Thirty healthy adults were randomly divided into two groups: the 'passive stretching exercise' group (n = 15) and the 'passive stretching exercise with elastic taping' group (n = 15). Muscle tone was measured using the MyotonPRO®. The muscle tension was measured immediately after the stretching exercises and taping intervention, and again 5 minutes after the intervention. RESULTS: Within each group, there was a significant reduction in muscle tone in the upper trapezius after treatment (p < .05). However, there was no significant difference in the muscle tone reduction between the groups (p > .05). Both experimental and control groups showed a significant decrease in muscle tone in both the upper trapezius muscles over time, i.e., immediately after treatment and five minutes later (p < .05). The main effect of time was identified in the repeated measures analysis, while there was no main effect attributed to the treatment method (group) (p < .05). CONCLUSION: The simultaneous application of stretching exercises and taping as an intervention to reduce muscle tension in the upper trapezius is still considered challenging and not yet widely regarded as an essential intervention method.

Immediate Effect of Sustained Stretching Exercises with Far Infrared on the Ankle Range of Motion and Muscle Tone in Patients with Stroke

  • Purpose: The spasticity of stroke patients decreases the ankle range of motion and increases the gastrocnemius muscle tone. This study examined the effects of stretching exercise and far infrared irradiation on the ankle function in stroke patients with spasticity. Methods: This study was conducted on 20 stroke patients admitted to Jesaeng General Hospital, who were divided into a study group (stretching exercise with far infrared) and control group (stretching exercise only). The dorsiflexion range of motion was measured using a smartphone and the medial gastrocnemius muscle tone and stiffness were measured using a Myoton pro. Results: With the exception of the non-paretic gastrocnemius muscle tone in the control group, the medial gastrocnemius muscle tone and stiffness decreased significantly in both groups. In both groups, the dorsiflexion range of motion increased significantly. In addition, the experimental group had a significantly higher dorsiflexion range of motion than the control group. On the other hand, there was no significant difference between the two groups in terms of the medial gastrocnemius muscle tone and stiffness. Conclusion: For stroke patients with spasticity, stretching exercises increased the ankle's range of motion and decreased the gastrocnemius muscle tone. The addition of heat therapy further increased the ankle's range of motion. On the other hand, as the sample size was small, future studies should include more subjects.

자가 신장운동이 전방머리자세를 동반한 만성 목통증 환자의 자세개선에 미치는 영향 (Effect of Self-Stretching Exercises on Postural Improvement in Patients with Chronic Neck Pain Caused by Forward Head Posture)

  • PURPOSE: The purpose of this study was to provide clinical basic data to reduce pain and improve function by comparing neck muscle activity and neck alignment using self-stretching and passive stretching exercises for chronic neck pain caused by forward head posture. METHODS: The subjects were divided into 15 subjects assigned to perform self-stretching exercise and 15 subjects assigned to perform passive stretching exercise. The intervention was conducted for a total of 4 weeks. The muscle activity in the neck was measured by surface electromyography (EMG) before intervention, and craniovertebral and cranial rotation angles were measured by X-ray. The 4-week intervention was conducted and the above items re-measured in the same manner and analyzed. RESULTS: Muscle activity within both groups after intervention using self-stretching or passive stretching exercise was significantly different (p < .05)(p < .01). Neck alignment of both groups was significantly different (p < .001)(p < .01). Further, muscle activities of the upper trapezius and splenius capitis muscles showed significant differences (p<.05). Lastly, neck alignment showed statistically significant difference (p < .05). CONCLUSION: Self-stretching exercise activated motor nerves as a posture correction exercise, thereby improving inhibition of muscle activity, muscle contraction delay, and pathological conditions of the muscle. For future research, interventions of self-stretching exercise will be needed for patients with chronic back pain accompanied by forward head posture, and various clinical studies on postural improvement of forward head posture by maintaining a normal muscle tone state are needed.

노인을 위한 건강증진프로그램 적용효과 (The Effects of a Health Promotion Program for Elderly)

  • Purpose: The purpose of this study was to develop and to evaluate a health promotion program for elderly. Method: Subjects were 63 elderly women (experimental group:33, control group:30). The study was a nonequivalant control group pretest-posttest design. The data was analyzed with an SPSS Window program, then the data was computed for the purpose of each study. Result: 1. In designing the program, the experimental group was given health education - 2 times per week, for 8 weeks - and they participated in recreation programs and stretching exercises five times a week. 2. Stretching exercises including health education, significantly affected the total Cholesterol, HDL-cholesterol, triglycerides, health behavior and self-efficacy. 3. The body fat weight was not significantly affected by the stretching exercises. Conclusion: This program was tested to promote the health of elderly and verified as an effective nursing intervention program, because the outcome of this program ascertains that this program enhances self efficacy of exercise, reduces Cholesterol and triglyceride levels' Increases HDL-cholesterol, and helps promote the understanding of heath behavior.

The Effect of Dynamic Neuromuscular Stabilization (DNS) on the Respiratory Function of Subjects with Forward Head Posture (FHP)

  • PURPOSE: The purpose of this study was to apply dynamic neuromuscular stabilization (DNS) to subjects with forward head posture (FHP) and to compare its effects on respiratory function as against the conventional neck stabilization exercise and neck stretching and extensor strengthening exercises. METHODS: The whole-body posture measurement system was used to measure the degree of FHP, and a spirometer and a respiratory gas analyzer were used to measure the respiratory function. After the intervention was completed, the changes over time were analyzed in the DNS group, the neck stabilization exercise group, and the neck stretching and extensor strengthening exercise group. The inter-group difference in the changes was also analyzed. A repeated ANOVA was performed to compare the respiratory function according to the period between the three groups, and the least significant difference (LSD) method was used for the post hoc test. RESULTS: After the 6-week exercise period, respiratory functions, such as forced vital capacity (FVC), forced expiratory volume for 1 second (FEV1), forced expiratory volume for 1 sec/forced vital capacity (FEV1/FVC), maximum oxygen intake (VO2max), and the volume of expired gas (VE), significantly improved according to the period (p < .05), but no inter-group differences were found. CONCLUSION: DNS is an effective training method, and can be applied along with neck stabilization exercise and neck stretching and extensor strengthening exercises, which are widely used in clinical practice, to people with FHP who cannot directly perform neck exercises to improve their respiratory function.

Comparative Effects of Novel Modified Sleeper and Cross-body Stretching on Scapular Anterior Tilting and Shoulder Internal Rotation in Subjects With Anterior Tilted Scapular and Shoulder Internal Rotation Deficits

  • Background: Posterior capsule tightness (PCT), commonly seen in overhead athletes, is a soft tissue adaptation that is also noted in non-throwers. PCT is associated with scapular and humeral kinematic alterations, significant restriction of shoulder internal rotation (IR) range of motion (ROM), and significant scapular anterior tilting. Sleeper and cross-body stretches (CBS) are suggested for PCT and IR deficits, and have been modified since introduction. A novel modified sleeper stretch (NMSS) was designed in this study to prevent the risk of anterior translation of the humeral head. Though the effects of posterior shoulder stretching exercise have been widely studies, to the best of our knowledge, no previous studies have investigated the effectiveness of posterior shoulder exercises in decreasing scapular anterior tilting. Objects: To compare the immediate effects of two posterior shoulder stretching exercises (NMSS and CBS) on scapular anterior tilting and shoulder IR ROM. Methods: Thirty-two subjects with anteriorly tilted scapula and IR deficits [mean age: 24.3 ± 2.5 years; 15 males and 17 females] participated in this study. Subjects were randomly assigned to either the NMSS or CBS groups. Scapular anterior tilting (at rest and at shoulder 60° active IR) and shoulder IR ROM were measured before and immediately after intervention. Results: Scapular anterior tilting significantly decreased, while the shoulder IR ROM significantly increased in both groups. However, there was no significant group-by-time interaction effect or significant difference between the groups. Conclusion: Both stretching exercises were effective in restoring shoulder IR ROM and decreasing scapular anterior tilting.
