국내 교대근무자의 수면연구에 관한 체계적 문헌고찰 (A Systematic Literature Review of Shift Workers' Sleep in Korea)

  • Purpose: The aim of this systematic literature review was to synthesize and investigate the effects of shift workers' sleep in Korea. Methods: A search was conducted through three electronic databases using keywords such as "shift work" or "rotation work" and "sleep", with sleep as the independent variable, and 17 papers were reviewed. Results: The design of those 17 studies was analyzed in a cross-sectional analysis. The most commonly measured characteristic was quality of sleep, whereas the others were sleep disturbance, sleep efficiency, and sleepiness. The study outcome variables were job-related factors, mental health, wellness, stress, fatigue, and metabolic syndrome. Conclusion: The results inform sleep and health-promoting behaviors and improvement in shift workers' working condition.

대학생의 건강증진해위 수행정도와 관련요인 (Health Promoting Behavior of University Students and Related Factors)

  • The purpose of this study was to examine the health promoting behavior of university students and its related factors. A self-administered questionnaire survey was carried out for 1,031(male, 477, female, 554) students from 7 universities in Taegu from March 10 to April 15 2000. Collected data were analyzed by the $X^2$-test, t-test, ANOV A and structural equation modeling analysis. The results of this study were as follows; Mean score of health promoting behavior of women was higher than mean score of men(p〈0.01). According to the subordinate fields, female students showed significantly higher scores in the fields of stress management, nutrition, personal relations and alcohol-cigarette(p〈0.05). And in male students, the only fields of exercise had meaningfully higher score(p〈0.01). The highest degree of performance was marked in alcohol-cigarette field in female, and interpersonal field in male. In Both male and female students, the lowest field was health responsibility. The mean score of health promoting behavior showed significant differences in intermediating factors such as age, health education and grade in male, and such as grade, pocket money for a month, taking health education, perceived body shape and gastrointestinal symptoms in female(p〈0.05). The mean score of health promoting behavior showed significant differences in cognitive-perceptual factors and cues to action such as perceived health status, perceived benefits, perceived barriers, and health concern of parents in both male and female(p〈0.05). By the results of structural equation modeling analysis in male students, the economic status, gastrointestinal symptoms and perceived body shape as lean had significant direct effects on cognitive-perceptual factors($\mid$T$\mid$〉2.0). Grade and health concern of parents had significant direct effects on health promotion behaviors($\mid$T$\mid$〉2.0). In female students, perceived body shape as lean and perceived body shape as fat had significant direct effects on cognitive-perceptual factors($\mid$T$\mid$〉2.0). Grade, pocket money and health concern of parents had significant direct effects on health promotion behaviors($\mid$T$\mid$〉2.0). On the consideration of above findings, it is recommended that perceived health status and perceived benefits should be raised and perceived barriers should be lowered to increase health promoting behaviors, by means such as institutional approach which can enhance the primary prevention for a disease. In addition, the health policy for improving parents' concern about health as well as university students, through various ways such as health education, consulting, and health program, should be employed.

임부 및 모유수유부와 가임기 여성의 건강행태 비교 (Comparison of Health-related Behaviors in Pregnant Women and Breast-feeding Mothers vs Non-pregnant Women)

  • Purpose: The aim of this study was to assess health-related behavior of pregnant women and breastfeeding mothers by investigating relevant risk factors. Methods: Data of 10,396 women (age 19 to 49 years) from the Korea National Health and Nutrition Examination Survey report from 2007 to 2012 was used to analyze factors associated with health-related behavior. The subjects were divided into pregnant women; breastfeeding mothers; and non-pregnant women. Bottle feeding mothers were excluded. Results: Current smoking rate including self-reported smoker and/or positive cotinine urine test were lower for pregnant or breast-feeding group than non-pregnant group. Heavy-drinking was not different among groups while monthly drinking rate was higher in non-pregnant group. Rate of stress recognition was lower in pregnant and breast-feeding group than non-pregnant group. Rate of experience for depressive symptoms and rate of suicidal ideation were not different among groups. Conclusion: Pregnant women and breast-feeding mothers maintain a good pattern of health- related behavior compared to non-pregnant women. However, substantial proportion of pregnant women and breast-feeding mothers continue to drink and smoke. This shows the need for a plan that will modify health-related behavior.

걷기 프로그램 '걷쥬' 참여자의 여가만족이 건강증진행위 및 삶의 질에 미치는 영향 (Effects of Leisure Satisfaction on Health Promotion Behavior and Quality of Life of Participants In the Walking Program "Geod-Jyu")

  • Purpose : This study aimed to investigate the leisure satisfaction of participants in the Geod-Jyu walking program and to examine the relationship between health promotion behaviors and quality of life. In addition, we investigated the impact of leisure satisfaction with walking among program participants on health promotion behaviors and quality of life. By clarifying these relationships, we aimed to identify factors of leisure satisfaction with walking that could enhance participants' health promotion behaviors and quality of life. Methods : A survey was conducted among 301 participants enrolled in the Geod-Jyu walking program in Chungcheongnam-do, with 288 responses analyzed. Data analysis included Pearson's correlation, confirmatory factor analysis, and multiple regression analysis using SPSS 27.0. Result : Leisure satisfaction exhibited a positive correlation with health promotion behavior (r=.544, p<.01) and quality of life (r=.478, p<.01). Furthermore, health promotion behavior showed a positive correlation with quality of life (r=.636, p<.01). Leisure satisfaction positively influenced quality of life, with physiological satisfaction (β=.16, t=2.32) and relaxation satisfaction (β=.15, t=2.04) emerging as notable contributors. In addition, leisure satisfaction had a significant positive impact on health promotion behaviors, with psychological satisfaction (β=.24, t=3.09) and educational satisfaction (β=.20, t=3.09) playing key roles. Health promotion behavior had a positive impact on quality of life, with all sub variables exhibiting significant positive effects in the following order: self-actualization (β=.24, t=4.16), stress management (β=.22, t=3.97), exercise (β=.22, t=4.05), and health responsibility (β=.12, t=2.14). Conclusion : The findings indicate that factors related to physical and rest satisfaction, as well as health-promoting behaviors facilitated by walking, significantly affected the quality of life among Geod-Jyu participants. Thus, promoting leisure satisfaction and engaging in health-promoting activities through walking can enhance overall well-being. Encouraging participation in the Geod-Jyu walking program is critical for physical and psychological benefits, and for promoting healthier behaviors. Further development of Geod-Jyu is expected to enhance walking satisfaction, promote self-care skills for health management, and positively influence quality of life.

한국문화에서 주관안녕에 영향을 미치는 사회심리 요인들 (Correlates of Subjective Well-being in Korean Culture)

  • 필자와 공동연구자들(2002)이 선행연구에서 개발한 주관안녕척도를 사용하여 한국문화에서 주관안녕에 영향을 미치는 변인들을 알아낸 연구 결과들을 개관하고, 국내외 연구들과 비교하여 시사점을 논의하고, 장래 연구할 과제들도 제안하였다. 먼저 주관안녕에 영향을 미치는 선행요인들로 ① 개인차와 인구통계 변인들, ② 개인과정 요인들, ③ 대인과정 요인들 및 ④ 한국문화의 요인으로 사회규범에 따른 행동을 다룬 연구 결과들을 개관했다. 또한 노인을 대상으로 주관안녕이 동시점에서 신체건강의 예측에 기여하는 수준과 아울러 1년 이상이 경과한 시점에서 종단적으로 신체건강이나 생사에 어떤 영향을 미치는지도 알아냈다. 본 논문은 한국문화에서 필자와 공동연구자들이 수행한 실증연구의 결과들과 연결시켜서 주관안녕을 연구하는데 따르는 이론, 방법 및 과제들을 구체적으로 논의함으로써 장차 문화비교 연구와 아울러 국내 연구에 시사점들을 제시한데 의의가 있다.

119 구급대원의 직무만족도와 그의 관련요인 (Job Satisfaction and Its Related Factors among 119 Rescue Workers)

  • 본 연구는 119구급대원의 인구사회학적특성, 건강관련행위특성, 직업관련 특성, 폭력경험, 소진 및 직무스트레스에 따른 직무만족도 수준을 알아보고, 폭력경험, 소진 및 직무스트레스와 직무만족도와의 관련성을 규명하고자 시도하였다. 조사대상은 전국 14개 소방서에 근무하고 있는 119구급대원 1,240명으로 하였으며, 조사는 2016년 3월 1일부터 4월 30일까지의 기간 동안에 구조화된 무기명 자기기입식 설문조사에 의하였다. 연구결과, 조사대상자의 인구사회학적 및 건강관련행위특성에 따른 직무만족도는 연령이 낮을수록, 기혼군보다 미혼군에서, 규칙적인 운동을 한다는 군보다 하지 않는다는 군에서, 여가시간이 있다는 군보다 없다는 군에서, 주관적인 수면의 질이 좋다는 군보다 좋지 않다는 군에서, 주관적인 건강상태가 좋다는 군보다 좋지 않다는 군에서 유의하게 낮았다. 직업관련특성에 따른 직무만족도는 직위가 낮을수록, 근무경력이 짧을수록, 월수입이 낮을수록, 업무의 신체적 부담정도가 적당하다는 군보다 힘들다는 군에서, 업무에 만족한다는 군보다 만족하지 못한다는 군에서, 업무에 대한 적성이 맞는다는 군보다 맞지 않는다는 군에서, 직업전환의사가 없다는 군보다 있다는 군에서 유의하게 낮았다. 또한, 조사대상자의 직무만족도 점수는 폭력경험, 직무스트레스 및 소진이 높은 군일수록 유의하게 낮았으며, 직무만족도가 낮은 군에 속할 위험비는 폭력경험, 직무스트레스 및 소진이 낮은 군에 비해 높은 군으로 갈수록 유의하게 증가하였다. 위와 같은 연구결과는 119구급대원의 직무만족도는 인구사회학적 및 건강관련행위 특성이나 직업관련 특성뿐만 아니라 직무스트레스, 폭력경험 및 소진과도 유의한 관련성이 있음을 시사하고 있다.

지역사회 보건사회지표를 이용한 시군구 지역 간 건강수준 비교 및 관련 요인 상관관계 분석 (A Comparison of Community Health Status by Region and an Investigation of related Factors using Community Health Indicators)

  • Purpose: The purpose of this paper was to compare community health status by region and to investigate related factors using community health and social indicators. Methods: Data were collected from statistics of local districts that were provided by KNSO and KCDC. ANOVA and correlation were analyzed using PASW 18.0. Results: The standardized cancer mortality rate was higher in metropolitan areas than in other areas. On the contrary, the mortality of respiratory disease, traffic accident, and suicide were higher in rural areas. Small cities and county districts showed higher prevalence in obesity prevalence than metropolitan areas. Metropolitan areas presented higher prevalence in alcohol drinking during the previous month, perceived stress, and seat belt use. The age-adjusted standardized mortality rate was correlated with higher prevalence of smoking, obesity, percentage of the elderly, number of beds, number of social welfare facilities, number of registered cars, lower percentage of financial independence, number of doctors, and percentage of water supply service & sewage. Conclusion: Since significant differences in mortality rate and prevalence of health risk behaviors exist between regional areas and the mortality rate was correlated with other social indicators and health indicators, health policies and social policies considering these differences should be develop and implemented to the communities.

과음하는 성인남자 53 명의 음주패턴과 간 장애에 대한 분석 연구 (Analytic Study for Alcohol Consumption-related Parameters in 53 Heavy Drinkers)

  • Objectives : To investigate the correlations among alcohol consumption, alcoholic liver disorders, physical symptoms, and behaviors in heavy drinkers. Methods : 53 males who self-realized their severe alcohol consumption were enrolled in this study. 10 answers for a questionnaire, serum parameter, sonographic finding and body mass index were attained. The correlations between them were analyzed using Pearson's correlation and Student's t-test. Results : The average consumption of alcohol in these subjects was 2.5-fold over social drinkers. The incidence of alcoholic hepatitis was around 30%, while fatty liver 73%, and abnormal GGT 77%, respectively. No specific correlation between average volume of daily alcoholic consumption and alcohol-related hepatic parameters was shown in this study, but correlative tendency between fatty liver and body mass index was exhibited. Conclusions : This study may indicate that alcoholic liver injuries are caused by not just volume of alcohol consumed but more mixed factors including inherited genetic components, body fat mass, foods and other physical or emotional stress.

성인의 건강행위 변화단계와 건강관련 삶의 질에 대한 연구 (Stages of Health Behavior Change and Health Related Quality of Life among Korean Adults)

  • Purpose: The Transtheoretical Model (TTM) is a theoretical construct explaining stages of health behavior change. The purpose of this study was to describe and analyze the stages of health behavior change and health related quality of life (HRQL), Method: A descriptive survey design was utilized, and, using a questionnaire, which included a series of 5 questions designed to assess stages of health behavior change and HRQL instrument, data were collected from 292 adults. Means, standard deviation, t-test, ANOVA, and SNK test were used to analyze the collected data. Results: Health risk behaviors were lack of stress management(44%), alcohol use (42.5%), poor nutrition (37%), smoking (36.3%), a lack of spirituality (28.8%), poor communication skills (21.9%), Lack of safety(21.6%), lack of fittness (20.1%), violent behavior(12.3%), and drug use(6.8%). There was a significant difference in HRQL according to stage of health behavior change (P=0.001). Conclusion: The results of the study identified the need for individualized nursing interventions that based on the stage of health behavior change. Nursing interventions that focus on the stage of health behavior change would be effective for health promotion for Koreans.
