Personality Characteristics of Patients with Obstructive Sleep Apnea and Simple Snoring : A Preliminary Study (2014.21.1.21폐쇄성수면무호흡증과 단순코골이 환자의 성격 특성 : 예비연구)

  • Objective: Few studies have evaluated the personality characteristics of patients with obstructive sleep apnea (OSA) and simple snoring. We investigated the personality characteristics of OSA and simple snoring patients and compared differences in personality between the two groups using the Eysenck Personality Questionnaire (EPQ). Materials and Methods: Two hundred and thirty-seven patients who were suspected to have OSA or simple snoring participated in this study. A self-questionnaire which included the EPQ was administered to all participants. All subjects underwent polysomnography in a sleep laboratory and those with an apnea-hypopnea index (AHI) ${\geq}5$ were included in the OSA group, while those with an AHI <5 were included in the simple snoring group. Results: OSA patients had significantly lower scores for Psychoticism (F=4.563, p=0.034) than simple snorers. There were no significant differences in Extraversion (F=3.029, p=0.083), Lie (F=0.398, p=0.529), or Neuroticism (F=3.367, p=0.068) scores between the two groups. In the correlation analysis of the OSA group, AHI was positively correlated with Extraversion score (r= 0.16, p=0.029) and negatively correlated with Lie score (B=-0.31, p<0.001). Using multiple stepwise linear regression analysis with the four EPQ parameter scores as dependent variables, Lie score was associated with older age (B=0.14, p<0.001) and lower AHI (B=-0.05, p<0.001), Psychoticism score was associated with higher Pittsburgh Sleep Quality Index (PSQI ; B=0.14, p<0.001), Neuroticism score was associated with higher PSQI (B=0.34, p=0.001) and female sex (B=3.15, p=0.003), and Extraversion score was associated with younger age (B=-0.08, p=0.020) and higher body mass index (B=0.26, p=0.023). Conclusion: This study suggests that patients with OSA are significantly less prone to psychotic personality characteristics than those with simple snoring. Among OSA patients, higher AHI was correlated with low falsehood and high extraversion tendencies.

Relationship between Sleep Disturbances and Cognitive Impairments in Older Adults with Depression (노인성 우울증 환자에서 수면 장애와 인지기능 저하의 관련성)

  • Objectives: Depression, sleep complaints and cognitive impairments are commonly observed in the elderly. Elderly subjects with depressive symptoms have been found to show both poor cognitive performances and sleep disturbances. However, the relationship between sleep complaints and cognitive dysfunction in elderly depression is not clear. The aim of this study is to identify the association between sleep disturbances and cognitive decline in late-life depression. Methods: A total of 282 elderly people who underwent nocturnal polysomnography in a sleep laboratory were enrolled in the study. The Korean version of the Neuropsychological Assessment Battery developed by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) was applied to evaluate cognitive function. Depressive symptoms were assessed with the geriatric depression scale (GDS) and subjective sleep quality was measured using the Pittsburg sleep quality index (PSQI). Results: The control group ($GDS{\leq}9$) when compared with mild ($10{\leq}GDS{\leq}16$) and severe ($17{\leq}GDS$) depression groups, had significantly different scores in the Trail making test part B (TMT-B), Benton visual retention test part A (BVRT-A), and Stroop color and word test (SCWT)(all tests p<0.05). The PSQI score, REM sleep duration, apnea-hypopnea index and oxygen desaturation index were significantly different across the three groups (all indices, p<0.05). A stepwise multiple regression model showed that educational level, age and GDS score were predictive for both TMT-B time (adjusted $R^2$=35.6%, p<0.001) and BVRT-A score (adjusted $R^2$=28.3%, p<0.001). SCWT score was predicted by educational level, age, apnea-hypopnea index (AHI) and GDS score (adjusted $R^2$=20.6%, p<0.001). Poor sleep quality and sleep structure alterations observed in depression did not have any significant effects on cognitive deterioration. Conclusion: Older adults with depressive symptoms showed mild sleep alterations and poor cognitive performances. However, we found no association between sleep disturbances (except sleep apnea) and cognitive difficulties in elderly subjects with depressive symptoms. It is possible that the impact of sleep disruptions on cognitive abilities was hindered by the confounding effect of age, education and depressive symptoms.

In Vitro AChE Inhibition Tests of Insecticides Using Electric eel and Housefly AChE (전기뱀장어 및 집파리 AChE를 이용한 살충제의 In Vitro AChE 저해 시험)

  • Experiments were conducted to establish an in vitro AChE inhibition test system to evaluate the potency of AChE inhibition of new chemical compounds. For a fixed time inhibition test, optimal inhibition (incubation) time to evaluate their AChE inhibition potency was 10 min. for AChE inhibitors such as DFP, DDVP, and paraoxon. The concentration of new chemical compounds with an ester group for evaluation of their inhibition potency was 10 $\mu$M under 10 min. preincubation conditions. However, the stepwise inhibition test with higher concentrations seemed to be needed for other chemical compounds. For a progressive inhibition test to calculate inhibition constants such as $K_d$, $K_3$ and $K_i$, extremely low $K_d(1.3\times10-^85.6\times10^{-7})$ and $K_3$(0. 21-0.27 $min^{-1}$) were observed under lagged preincubation time (0.8-13.3 min) and low in¬hibitor concentrations $(1\times10-^92\times10-^6M)$. However, this method seemed to be useful for comparison of AChE inhibition potency among inhibitors. Differences in inhibition potency among DFP, paraoxon, and KH501 were due to the differences in $K_d$, in other words, differences in affinities between inhibitors and AChEs. Therefore, AntiChE screening should consist of two steps. The first step is to evaluate the potency of AChE inhibition based on $I_50$ valuse obtained from fixed time inhibition tests. The second step is to study inhibition patterns and characteristics of chemical compounds selected in the first step.

SOFT TISSUE PROFILE CHANGE PREDICTION IN MAXILLARY INCISOR RETRACTION BASED ON CEPHALOMETRICS (두부방사선 분석에 의한 상악전치부 후방이동시 연조직 변화 예측에 대한 연구)

  • This study was carried out in order to determine soft tissue response to incisor movement and mandibular repositioning and to determine feasibility of predicting vertical and horizontal changes in soft tissue with hard tissue movement. For this study, cephalometric records of 41 orthodontically treated adult females who had Angle's Class II division 1 malocclusion were selected and stepwise multiple regression analysis was employed. Following conclusions were obtained by analysing the changes of soft tissue and hard tissue before and after treatment. 1. Hard tissue measurements that showed significant changes before and after treatment were horizontal and angular changes of maxillary incisor, horizontal,vertical and angular changes of mandibular incisor, overjet, overbite, interincisal angle, mandibular repositioning, A,B, skeletal convexity and soft tissue measurements that showed significant changes were horizontal, thickness and angular changes of upper lip, horizontal and angular changes of lower lip, interlabial angle, nasolabial angle labiomental angle, Sri, Ss, Si and soft tissue convexity(P<0.05). 2. All Soft tissue measurements changed significantly before and after treatment had between one and four hard tissue independent variables at statistically significant level, indicating that all soft tissue changes were direct relationship with hard tissue changes 3. Ova jet, horizontal change of maxillary incisor, horizontal change of maxillary root apex and horizontal change of pogonion entered into prediction equations most frequentely indicating that they were more significant variables in prediction of vertical and horizontal changes in the soft tissue with treatment, but vertical changes of mandibular incisor not entered any prediction equations, indicating that it was not considered a good predictor for soft tissue changes with maxillary incisor retraction. 4. Horizontal and vertical changes in subnasale were found to have most independent variables, significant at the 0.05 level in prediction-equations(${\Delta}$Sn(H):Ur, Is(H), Pg(H), UIA,${\Delta}$Sn(V): Is(H), Pg(H), overjet, A), indicating that subnasale changes are influenced by complex hard tissue interaction. 5. Multiple correlation coefficient($R^2$) of the soft tissue prediction equations ranges from 0.2-0.6.

Diagnostic Significance of TNF-$\alpha$ in Tuberculous and Non-Tuberculous Pleural Effusion (결핵성 및 비결핵성 흉막삼출액에서 TNF-$\alpha$ 농도의 진단적 의의)

  • Objectives : The differentiation of tuberculous effusion from the other causes of exudative pleural effusion remained difficult even with aids of biochemical analyses and pleural biopsy. As the pathophysiology of tuberculous pleural effusion is an enhanced cell mediated immunity, Adenosine deaminase(ADA) and various eytokines including Inteferon-$\gamma$, tumor necrosis factor alpha(TNF-$\alpha$) are considered as useful diagnostic tools in differentiating exudative pleural effusion. The author would like to demonstrate the diagnostic usefulness of TNF-$\alpha$ in the differentiation of exudative pleural effusion, and compared the discriminating ability of TNF-$\alpha$ with ADA. Methods : Pleural fluids obtained from 80 patients (tuberculous : 39, malignant : 31, parapneumonic : 10) with exudate pleural effusions were processed for cell counts and biochemical analysis including ADA and TNF-$\alpha$. Results : Tuberculous pleural fluid showed higher levels of ADA and TNF-$\alpha$, $48.7{\pm}32.7U/L$ and $184.1{\pm}214.2pg/mL$ than that of non-tuberculous effusion $26.0{\pm}41.3U/L$ and $44.1{\pm}114.2pg/mL$, respectively (ADA, TNF-$\alpha$, p < 0.05, p < 0.01). Receiver operating characteristics(ROC) curves were generated for ADA and TNF-$\alpha$ and the best cut-off value for adenosine deaminase and TNF-$\alpha$were considered as 30U/L and 15pg/ml, respectively. Comparing the area under the ROC curves, there was no significant difference between ADA and TNF-$\alpha$. Conclusion : For the differential diagnosis of tuberculous pleural effusion from the other causes of exudative pleural effusions, TNF-$\alpha$ as well as ADA was considered as useful diagnostic method. However adding TNF-$\alpha$ to ADA has no further diagnotic benefit than ADA alone.

  • The purpose of this study is to provide framework for understanding children's dental utilization. In this paper Andersen-Newman's model is applied to the use of dental visits. This model consists of predisposing, enabling, and need components that describe a person's decision to use dental health services. One thousand, nine hundred seven children and their mothers were selected for the study. The children were fourth grade to sixth grade in elementary schools in Iksan city, Korea. Models are operationalized using stepwise multiple regression analysis and path analysis. The number of independent variables used in the analysis was 39 in total, ie 32 predisposing components, 6 enabling components, and 1 need component. Children's Dental utilization was measured based on the number of visits. The data collected by means of a questionnaire survey. In this study, the amount of variance by the model was 25 percent. Predisposing factors had the greatest effect on utilization. Number of restricted activity days caused by oral disease, having a regular dental care, and susceptibility on oral disease of children were found to have significant major effects on dental utilization of children. Mother's dental visits was most important factor affecting dental utilization of children.

A Case Study on High and Low Performance Areas for Family Planning (가족계획 우수.부진지역 사례연구)

  • This study was conducted to compare the characteristics of high performane areas for family planning with that of low performance areas and to find factors which strongly affected contraceptive practice behavior. For the study, eight areas were selected from 274 rural family planning canvassing areas of Korean Population Policy and Program Evaluation Study, which was an action study operated in all areas of Cheju Island from July 1, 1976 until December 31,1979. As a first step of the action study, Cheju Island was devided up 318 family planning canvasser areas Each area was consisted of 200 households in rural district and 300 households in urhan one Duriog the period of project, each canvassing area had been managed by a female family planning canvasser, selected by director of health center considering several individual conditions needed for family planning activities Basic activities of canvassers were to counsell all the eligihie couples in own charged area about family planning methods and also to distribute contraceptives such as condoms and oral pills. In case couples desire to accept sterilization including vasectomy and tubal-ligation, the canvassers played a linking role connecting potential client with family planning field workers. Canvassng areas shows significant differentce in performance for family planning, nevertheless they are supposed to have almost the same conditions regarding family planning distribution channel. Because the purpose of the Cheju project was to eliminate all the problems that existed in governmental distribution system, that is to remove geographic, economic, cognitive and administrative barriers Accumulated performances of family planning methods accepted by residents in each area were calculated by eligible women aged 14-49. And then canvassing areas were ranked according to performance score. Consequently, 4 areas in extremely high and low family planning performance areas were selected respectively. Major results were obtained by comparing characteristics of high performance area with that of low performance areas, which are as follows: 1. The mean number of living children was about the same both in high and low performance areas for family planning. But respondents' mean age (38.5) in high performance areas was higher than that (37.0) in low performance areas 2. Respondents' perception in the expectant educational level of others' children in high performance areas was higher than that in low performance areas, although respondents educational level, monthly expenditure and ratio of children in high school and above was not different. 3. Ratio of ownerships of TV and newspaper in high performance areas was highen than that in low performance areas 4. The duration of canvasser' charge in high performance areas was longer than that of low performance areas, showing the fact that canvassers didn't move cut in high performance areas 5. In high performance areas, canvassers' houses were relatively located in the center part of the village. And so villagers resided in near distances from the anvasser's house 6. 4H clubs' activities in high performance areas were more active than those in low performance areas Therefore it was assumed that cohesiveness of community in high performance areas were stronger than that in low areas. 7. Canvassers' family planning practice rate was higher than that in low performance areas, and also canvassers' human relationship was more sociable than that of canvassers in low performance areas. 8. Fourteen variables which showed relatively high significance level in $X^2$ and F test were selected as independent variables for stepwise regression analysis. According to the results of regression analysis. five of 14 variables-distributors education level ($R^2$=.4439), duration of distributor's charge ($R^2$=.6166), 4H club activities ($R^2$=.6697), canvasser's contraceptive practice ($R^2$=.7377) and location of distributions house ($R^2$=.8010) explained 80.1 percent of total variance.

An Analysis of the Correlation between Seoul's Monthly Particulate Matter Concentrations and Surrounding Land Cover Categories (서울시 월별 미세먼지 농도와 주변 토지피복의 관계 분석)

  • The present study aims to identify the effect of land cover categories on particulate matter (PM) concentrations by analyzing the correlation between monthly PM concentrations in Seoul's air quality monitoring network and the percentages of land cover categories by buffers around air quality monitoring stations. According to a monthly correlation analysis between land cover categories and PM concentrations, in the buffer 3km, PM10 showed a better correlation than PM2.5, there was a clear negative correlation with the forest area, the grassland and the urbanized area had some positive correlation with PM10, and the barren land and the urbanized area had some positive correlation with PM2.5. According to a monthly correlation analysis of dominant land cover sub-categories and sub-sub-categories within the buffer 3km, PM10 showed a clear negative correlation with the broad-leaved forest, and some positive correlation with the road was dominant. PM2.5 showed partly negative correlation with the broad-leaved forest and partly positive correlation with the commercial area. There was a very low or no correlation with other grassland and bare land subcategories. A monthly stepwise regression analysis on noticeable land cover sub-categories and sub-sub-categories with positive or negative correlations revealed that an increasing percentage of the broad-leaved forest had a clear effect on reducing PM10 concentrations, and the road was excluded from the selected variables. Although an increasing percentage of the commercial area had some effect on increasing monthly PM2.5 concentrations and an increasing percentage of the broad-leaved forest had an effect on decreasing the PM2.5 concentrations, their effect size was smaller than that on PM10. The forest area around the city center had the largest and clearest effect on reducing PM concentrations. The urbanized area's sub-categories and sub-sub-categories were also confirmed to have some effect on increasing PM concentrations.

Analysis of Groundwater Flow Characterization in Fractured Aquifer System (파쇄대 응회암 대수층의 지하수 유동 특성화 기법)

  • On the basis of a stepwise and careful integration of various field and laboratory methods the analysis of groundwater flow characterization was performed with five boreholes (BH-1, -2, -3, -4, -5) on a pilot site of Natural Forest Park in Guemsan-gun, Chungcheongbook-do, Korea. The regional lineaments of NW-SE are primarily developed on the area, which results in the development of many fractures of NW-SE direction around boreholes made in the test site for the study. A series of surface geological survey, core logging, geophysical logging, tomography, tracer tests, and heat-pulse flowmeter logging were carried out to determine fracture characteristics and fracture connectivity between the boreholes. In the result of fracture connectivity analysis BH-1 the injection well has a poor connectivity with BH-2 and BH-3, whereas a good with BH-4 and BH-5. In order to analyse the hydraulic connectivity between BH-1 and BH-5, in particular, a conspicuous groundwater outflux in the depth of 12 m and influx in the depth of 65 m and 70 m, but partly in/outflux occurred in other depths in BH-5 were observed as pumping from BH-1. On the other hand, when pumping from BH-5 the strong outflux in the depths of 17 m and 70 m was occurred. The spatial connectivity between the boreholes was examined in the depth of 15 m, 67 m, and 71 m in BH-1 as well as in the depth of 15 m, 17 m, 22 m, 72 m, and 83 m in BH-5.

Food Habits of the Glass eel Anguilla japonica in the West Coast Estuaries of Korean Peninsula Determined by Using C and N Stable Isotopes (안정동위원소를 이용한 서해연안 실뱀장어의 먹이 습성)

  • Glass eels (Anguilla japonica) are caught in the west coast of Korea on their migratory route from the breeding grounds in the Mariana Trench along the North Equatorial Current and the Kuroshio Current. To identify the food source of natural glass eels, we analyzed the stable C and N isotopes of glass eels caught in April 2012 and investigated possible food sources in the survey area. In particular, with respect to the stable C and N isotopes of particulate organic matter, we extended the surveying area to the northern parts of East China Sea as well as the west coast of Korea. The stable C and N isotope ratios of the glass eels caught in the west coast were found to be $-20.7{\pm}0.1$‰ and $5.0{\pm}0.2$‰, respectively. The stable C and N isotope ratios of the particulate organic matter in the west coast of Korea, in which the glass eels are assumed to eat the particulate organic matter as food source, were estimated to be $-24.0{\pm}0.3$‰ and $2.8{\pm}0.4$‰, respectively. Similar data were obtained from the northern part of the East China Sea, $-24.5{\pm}0.5$‰ and $0.8{\pm}0.3$‰. The stable isotope ratios showed values differing from the stepwise increasing rates up the food web in natural aquatic ecosystem, showing that particulate organic matter in the west coast of Korea and East China Sea was not served as the glass eels food source. This result suggested that the glass eels caught in the west coast might not assimilate nutrition from the marine environment during long migration.
