Endovascular Rescue of a Narrowed Aorto-Aortic Bypass Graft in a Patient with Takayasu's Arteritis

  • We report a case of successful endovascular treatment of a pseudoaneurysm and the obstruction of an aorto-aortic bypass graft, which had been performed to treat Takayasu's arteritis fifteen years prior, at the thoracic aorta. Along with the immediate relief of proximal hypertension that had caused severe heart failure, the successful exclusion of the pseudoaneurysm and the patency of the stem graft were maintained three years after the procedure.

  • Circulatory disease is the number two cause of death next to cancer in Korea, while the cardiovascular disease alone is the number one cause of death in the US. In the present article, some background, current status and future prospects of biomedical engineering esearch on circulatory disorders are discussed in terms of the origin of atherosclerosis, computational fluid dynamics and medical imaging techniques, clinical treatments and fluid dynamics, advances in stents, hemodynamic analysis of artificial heart, and artificial blood. In particular, the importance of close collaboration of medicine and fluids engineering is emphasized.

  • Cardiac surgery in neonates with congenital heart disease has progressed dramatically in the past three decades. However, low-birth-weight neonates with congenital heart disease continue to challenge the intellectual and technical skills of healthcare professionals. We present a case of a low-birth-weight neonate with pulmonary atresia and a ventricular septal defect, in whom palliation was achieved with a right ventricular outflow tract stent using a hybrid procedure.

  • Malignant esophagorespiratory fistula is a devastating and life-threatening complication of esophageal and bronchogenic carcinomas. As a non-surgical treatment, peroral stent placement into the esophagus or airway can close-off the fistula and prevent progression of the pneumonia. Although reopening of the fistula is not uncommon despite stent placement, interventional treatment is effective for sealing off reopened ERFs. Bronchopleural fistula is a well-recognized complication of pneumonectomy. There have been several reports to occlude the fistula with use of stents and much more experience is required.

Tracheal Agenesis Reconstruction with External Esophageal Stenting: Postoperative Results and Complications

  • Tracheal agenesis is an extremely rare and typically lethal congenital disorder. Approximately 150 cases have been described since 1900, and very few cases of survival have been reported. We describe tracheal reconstruction with external esophageal stenting in a patient with Floyd's type II tracheal agenesis. Neither long-term survival nor survival without mechanical ventilation for even a single day has previously been reported in patients with Floyd's type II tracheal agenesis. The infant in the present case survived for almost a year and breathed without a ventilator for approximately 50 days after airway reconstruction using external supportive stents.

The assessment of the performance of drug-eluting stent using computational fluid dynamics

  • Numerical investigations have been conducted on the assessment of the performance of drug-eluting stent. Computational fluid dynamics is applied to investigate the flow disturbances and drug distributions released from the stent in the immediate vicinity of the given idealized stent in the protrusion into the flow domain. Our simulations have revealed the drug concentration in the flow field due to the presence of a drug-eluting stent within an arterial segment. Wall shear stress increases with Reynolds number for a given stent diameter, while it increases with stent diameter for a given Reynolds number. The drug concentration is dependent on both Reynolds number and stent geometry. In pulsatile flow, the minimum drug concentration in the zone of inter-wire spacing occurs at the maximum acceleration of the inlet flow while the maximum drug concentration gains at the maximum deceleration of the inlet flow. These results provide an understanding of the flow physics in the vicinity of drug-eluting stents and suggest strategies for optimal performance of drug-eluting stent to minimize flow disturbance.

  • 식도와 주위 장기 사이의 누공 형성은 자가 확장성 식도스텐트의 잘 알려진 합병증 중의 하나이지만, 기관지 식도 누공을 수술적으로 치료한 보고는 거의 없다. 양성 식도협착 환자에 스텐트를 사용하는 경우가 점점 늘어남에 따라 이러한 후기 합병증의 빈도도 점점 증가할 것이다. 저자들은 최근 부식성 식도협착 환자에서 식도스텐트 삽입 후 발생한 기관지 식도 누공 1예를 경험하였고, 누공절제술과 Ivor Lewis 술식으로 일차성 교정술을 시행하여 좋은 결과를 얻었기에 국내 문헌상으로는 최초로 이를 보고하는 바이다.

  • To maintain an adequate airway in a patient with tracheobronchial narrowing coming from various causes, prosthetic tracheobronchial stents provide palliative treatment for narrowed airways where surgical resection is inadvisable. After insertion, previous reported complications were granuloma formation, dysphagia, suction catheter entrapment and fatal massive hemoptysis. We report, a case of complication associated with expandible metallic stent with endobronchial stenosis due to tuberculosis.

  • Recently, many patients related to heart disease have surgical operation by expanding a blood vessel to treat the angiostenosis. So far most angioplasties have been performed using balloon-dilative stent made of stainless steel. Some researchers are studying the stent made of shape memory alloy (SMA) to operate the angioplasty more easily. and there are several papers which introduce the angioplasty using SMA. However, most of the analysis models for stents are constructed using solid elements. So much computing time is required to solve the analysis model. In this study, we suggest the SMA stent model using 1D truss element which is much faster than stent model using 3D solid element. To represent non-linear behavior of SMA, we apply 1D SMA constitutive equation of Lagoudas'. Pseudo-elastic behavior of stent structures is presented as a numerical example.

폐전절제술후 발생한 식도흉막루 -식도스텐트를 이용한 치험 1례- (Esophageal Stent in Postpneumonectomy Esophagopleural Fistula)

  • A case of esophagopleural fistula after pleuropneumonectomy is reported. A 59 years old male underwent right pleuropneumonectomy due to tuberculous empyema. The postoperative small esophagopleural fistula was confirmed by esophagogram and was initially managed by a conservative treatment. There was a persistent fistula on follow up esophagogram, therefore we planned the next treatment modality for obstruction of the fistula. For poor general conditions and arrhythmia, an esophageal stent was applied as a non-surgical method. At first, a covered-form stent was inserted, but it migrated to the stomach after 3 months. By using an uncovered-form stent, a complete obstruction of the esophagopleural fistula was achieved.
