Hospital Marketing Condition and Strategy -Of a General Hospital- (병원마케팅 실태와 전략방안 -지역사회의 일 종합병원을 대상으로-)

  • The purpose of this study was to investigate the real condition of hospital marketing and concrete strategy on medical consumers need for enhance the effectiveness of hospital management. The data were collected from January 27 to February 3, 2000 at a general hospital located in M city to 205 in-patients and out-patients by questionnaires. The research tool was based on literatures. The data was analysed by the use of percentage, mean, t-test and ANOVA by SAS(Statistical Analysis System) Package Program. The results of this study are summerized as follows: 1. Mean score of the marketing mix (4P 's) was 3.1 in total and each mean score was 3.6 in product, 2.5 in prices, 3.3 in place and 3.1 in promotion. The following are the items which received the highest scores in each: 'The hospital is clean' in service category; 'the prices of meals at the restaurant the patients' families use are reasonable ' in price sphere. In distribution, 'it is easy to locate the hospital' and in promotion,'we'll use this hospital again' and 'we'll recommend this hospital to others'. 2. Marketing mix factors(4P's) was significantly different on the general characteristics in terms of marriage status, occupations and the period of hospitalization. 3. The most important reason for choosing this hospital was 'This is a general hospital with good facilities' and the next were 'The hospital staffs are kind and the hospital is clean' and 'The traffic is convenient to come'. The most important factors contributing to a good image formation of the hospital were 'this is a clean hospital', 'This is a hospital with kind people' and 'this hospital is equipped with good medical facilities'. 4. The factors for making a good image concerning the nursing service were professional knowledge, good nursing skills and kindness. After grasping the marketing conditions of the hospital aided by the above-mentioned research results, the researchers concludes that it is necessary to develop an institutionalized service strategy to increase the satisfaction the patients feel about the hospital facilities and kindness of the staff and as a result, to differentiate it from other medical institutions.

A Comparative Study on Evaluation Methods of Permanent Impairment in Korea (우리나라의 신체장애평가법에 관한 비교연구 - 신체장해등급법, McBride법, 미국의학협회 (AMA)기준법을 중심으로 -)

  • In this study, literature review was done to examine and compare the current status and problems of different evaluation approaches toward permanent impairment in Korea. Alternatives and improvements in the current approaches in Korea were suggested. Series of cases were also examined to compare different approaches applied to the real cases, using 105 cases from a hospital data and another 207 cases from a insurance company data. The main findings of the literature review are as follows; 1. The current evaluation methods of permanent impairment in Korea are grouped into two categories, grading and rating. Gradings of impairments are expressly specified in 17 various statutes. 2. In Grading methods, the rigid system of 74 different grades has been adopted uniformally for the convenience of administration, which may not be, appropriate or valid from medical and scientifical aspect. 3. The adventage of McBride method is assessment of occupational disability rate. However the classified compensable occupations are only 280 and limited to manufacturing industries in 1960s'of U.S.A., which is not appropriate to current Korean circumstances. Especially, the job list does not include managerial officers or mental workers. 4. AMA Guides is the scientific and reasonable method for the assessment of physical impairment rate. However compensation and reparation of impairment case is difficult because this method cannot assess the disability rate according to occupation, age, etc. The results of cases comparative study are as follows: 5. The physical Impairment could be compared in 167 out of total 312 cases, and for the cases of complex impairment, McBride method underestimate physical impairment rate compared with AM A method. 6. When disability late was assessed, occupation was considered the compensation of only 85 cases, and age was used in only 21 cases. This was because occupation and age compensation in McBride method are unreasonable. 7. The most Ideal alternative is to assess physical impairment according to AMA method and then to develop a compensation method appropriate for the circumstances of Korea society.

The Status of Eating Habits and Nutrient Intakes of Preschool Children in Kyungjoo (경주지역 유치원 아동들의 식습관과 영양소 섭취상태)

  • The purpose of this study is to estimate the nutrient intakes and food habits among preschool children in Kyungjoo city. The subjects were 210 preschool children, aged 4 - 6 years. Measurements of the weight, height, chest circumference, and head circumference of the children were conducted. And general home environment and factors related to eating habits for preschool children were collected using a questionnaire that included information about family income, parents' education and occupations. The average weight-length index (WLI) for the subjects was $103.9\%$. Using the WLI, $20.0\%$ of the preschool children were underweight, $48.6\%$ were normal, $19.0\%$ were overweight, and $12.3\%$ were obese. On the R$\ddot{o}$hrer index, $13.8\%$ of the preschool children were underweight, $38.5\%$ were normal, and $47.7\%$ were over weight or obese. The average daily intake ($\%$ of RDA) of energy and each nutrient was 1323.5 kcal ($81.3\%$), Ca 484.3 mg ($80.7\%$), Fe 7.05 mg ($88.1\%$), vitamin A 420.0 RE ($105\%$), vitamin $B_{2}$ 0.76 mg ($95.0\%$), vitamin $B_{2}$ 0.87 mg ($87.0\%$), and vitamin C 53.1 mg ($106.2\%$) , respectively. In particular, older subjects had lower intake in RDA $%$ of calcium and iron. The energy intake ratio from snacks was much higher than the recommended level of the preschool children. With regard to frequency of regularity of breakfast, $1.9\%$ of preschool children skipped every morning and $7.6\%$ of the children skipped more than 5 per week. With regard to the intake frequency of vegetables, fruits, complex carbohydrates, and milk, $13.3\%$, $19.9\%$, $22.8\%$, and $41.8\%$ of the children ate more than 5 times per week. The eating habit score was positively correlated (r = 0.18, P < 0.05) with household income. This study suggests that nutrition education to increase fruit and vegetable consumption for preschool children should be emphasized. Also a nutrition education program is needed to enhance consuming calcium and iron intake for adequate growth.

Current Status of Ophthalmic Optometry Laboratory Personnel in Korea, Japan, and the United States (한국, 일본, 미국 안과검사인력의 현황)

  • The education and training system, the ISCO (International Standard Classification of Occupations), and the legal systems of Japan and the United States consider all ophthalmic optometry laboratory personnel as ophthalmologists. They encompass optometrists, orthoptists, optometric technicians, and ophthalmic medical technicians. Data retrieved from the KOSIS (Korean Statistical Information Service) revealed that the number of opticians associated with the department of ophthalmology in 2022 could be appraised by classifying their medical institutions; contrarily, the number of clinical laboratory technologists could not be assessed. However, the current research investigated a general tertiary hospital and determined that clinical laboratory technologists outnumber opticians. Classification in Korea is based on ophthalmic optometry laboratory personnel, ISCO, ISCED (International Standard Classification of Education), the medical service act, the act on medical service technologists, and the higher education act. These results cannot be compared to the optometrists evaluated in the United States. Ophthalmology is a suitable profession for optometric technologists and technicians who perform under the instructions of ophthalmologists and optometrists. The field of eye healthcare would be benefitted by assigning the management based on their qualification according to the requirement of the job title, such as 'Clinical Optometry Technologist' to be given to clinical laboratory technologists and opticians who work in the ophthalmic optometry laboratories after obtaining a private qualification endowed by the Korean Ophthalmological Society and the Korean Optometry Society.

A Qualitative Case Study on Conflict Transfer Factors of College Graduates with Early Careers (대졸초기경력자의 갈등 전이 요인에 대한 질적 사례 연구)

  • The purpose of this study is to identify the factors influencing conflict transition through the conflict transition process experienced by college graduates with early careers. The participants of this study were in-depth interviews with a total of 8 young people aged 25-34 who graduated from college and found their common subjects in all cases. As a result, the topics such as 'lack of recognition and compensation', 'unfairness of supervisor', 'characteristics of job (interdependence, autonomy)' were derived. 'Lack of recognition and compensation' brought about a sense of skepticism about the idea that even though they worked hard, they could not be recognized. And it caused a conflict transition. When the participants felt the 'unfairness of the supervisor', the task conflicts were further amplified and it transfer to relationship conflict. Among the characteristics of 'job characteristics', when highly interdependent occupations were carried out, conflict transition was shown and conflict levels were high. On the other hand, the work environment with job autonomy showed job satisfaction and did not show conflict transition. This study has an academic significance in that it revealed the types of conflicts and conflict transfer factors experienced in early career status of college graduates. In addition, this research have the practical implications to improves the participants' understanding of the conflict transfer through actual scene in the workplace.

Conflict resolution and political tasks on the usage of beauty care devices by beauty artists (미용업종사자의 미용기기 사용에 대한 분쟁해결과 정책적 과제)

  • In contemporary society interest in and consumption of beauty treatment are increasing, raising interest in health and beauty. However, beauty-related laws are becoming factors of hindrance of beauty development. Currently the Public Health Control Act plays a basic role in the beauty art business in Korea, However the contents are in discord with international laws and its definition is not clear. Therefore it is causing conflicts of different occupations and job associations which are similar to art business. Especially, because neither definitions nor policies on beauty care devices exist in the Public Health Control Act, beauty care devices using in foreign countries cannot be used in Korea due to classification as medical devices. Under this circumstance, therefore, beauty care device uses by beauty artists violate the law. The government has tried to solve these irrational regulations. Recently, the Small and Medium Business Administration announced 'the improvement plan of small business and young founders site regulation for public economy recovery' in a ministerial meeting on December 28, 2016. Regulations on policy preparation for skincare devices were inclusive in this announcement. It is the question whether the regulations will be executed or not. Even though beauty industrial competitiveness was presented in the 18th Presidential Council on National Competitiveness in 2009, it was not practiced. The proposal bills for beauty law improvement have been put forth several times since 2000 including an improvement plan for regulating beauty care devices. However, so far there have been no improvements. The damage on the regulation classifying beauty devices as medical devices is not only restricted to skincare. This develops beauty devices and the beauty industry which imports and exports beauty devices. When beauty devices are exported, complicated procedures are unavoidable and when beauty devices are imported, irrational problems like reregistration procedures and costs occur. The reason why an improvement plan has not gone into practice is the resistance of the dermatologists' association. Dermatologists tend to stand positively against harming public health by saying that beauty devices used by beauty artists cause people to suffer side effects. In contrast, anyone who has a licence to use beauty devices is able to use them in foreign countries. It is not only infringement of one's right as a beauty artist but also people's right to receive beauty care services. With this reason, Korean's current law under which beauty devices are ruled as medical devices should be revised with accordance to domestic surroundings. Therefore in order to advance and globalize the beauty industry, the support and cooperation of the Korean government and relevant associations is needed to legislate and revise the beauty devices laws. The relevant associations abandon regional self-centeredness and cooperate to define ranges, size and management of beauty devices for safe use. If no collaboration exists, an arbitration agency should be established to solve the problem.

A Study of Current Employment and Future Trends for Young Home Economists (가정학 전공자의 취업과 전망)

  • The current employment status among young home economists and perspectives in occupations for prospective Home Economics graduates were explored in this study which utilized information from 17 to 21 colleges in Korea and colleges in the United States during the five years of 1977∼1981. The Home Economics content areas covered in this study were Clothing and Textiles, Foods and Nutrition, Housing and Interior Design, and Child Development and Family Life. The highest percentage of Korean graduates in Clothing and Textiles was employed either as teachers at the junior high school level or as designers in the clothing and textile industries. Quite a number of the graduates were engaged in further studies at the graduate level. Korean graduates with a master's degree were teaching at the college level and some had furthered their studies at the doctorate level either here or abroad. Koreans with a bachelor's degree in Foods and Nutrition held jobs as teachers in junior high school, dieticians at mass feeding institutions and hospitals, food scientists in food industries, and researchers in institutions. Those with a master's degree were teaching at the college level. Americans with a bachelor's degree worked as dieticians, supervisors in restaurants and institutions, extension workers, researchers at various facilities, teachers and clerks. Americans with a master's or doctorate degree were engaged in teaching at colleges or supervising at research or working as extension specialists. In general, Korean graduates were found to hold positions in less varied areas than their American counter-parts. Among forty-nine graduates those working in their professional field reported less sex discrimination that those working in other fields. The major area of employment in Housing and Interior Design or Home Management graduates in Korea was teaching while in the United States it was extension work, business, governmental work and teaching. It was suggested that in the future, career development in Korea be further explored to include extension service, research, social welfare, financial planning, business, free-lancing, funeral home, home-call, and correctional education. Interviews with executives from 6 business enterprises indicated that most of them were aware of the potential contribution home economists could make for their companies but they expressed a negative attitude towards women in general due to their short stay on the job. Jobs held by Child Development and Family Life majors with a bachelor's degree in Korea were mostly teaching positions in public, junior and senior high school. However, jobs such as nursery school teaching, working in clinical setting, business, and teaching at public, junior and senior high school predominated in the United states. Most Korean graduates with a master's degree were teaching in professional colleges while in the United Stated the job variation among the graduates was rather evenly distributed among teaching at college level, public and high school, nursery school and administration areas. Reports from 7 child development majors on the job indicated that they were paid less that secretarial workers. Only half of them were working in their major area and these expressed satisfaction with their work. Two thirds of the respondents indicated no sex discrimination. It was suggested that in the future Child Development and Family Life majors pursue employment in counseling, guidance, recreation, mass media, administration and outreach work as well as education, research and parent education in services for children, teen-agers, adults and families.

Effect of Role Stress on Career Commitment and Turnover Intention of Hospital Workers (의료기관 종사자의 역할 스트레스가 경력몰입과 이직의도에 미치는 영향)

  • The purpose of this study is to empirically analyze the impact of role stress (role conflict, role ambiguity, and role overload) on career commitment and turnover intention in small and medium-sized hospital workers in Busan. The subjects of the study were 418 nursing and administrative workers engaged in hospitals and general hospitals in Busan. Frequency analysis, correlation analysis, and regression analysis were conducted for empirical analysis. The results of the study are as follows. First, significant differences in some research variables were identified according to gender, age, educational background, marital status, and working years. Depending on the job type, there were significant differences in all variables such as role conflict, role ambiguity, role overload, career commitment, and turnover intention. Second, role stress was found to have a significant negative effect on career commitment and a significant positive effect on turnover intention. Among the role stress, it was confirmed that the role overload had the strongest influence on career commitment and turnover intention. This study is meaningful in that it analyzed the influence of role stress, including workers in all occupations, including nursing jobs, and empirically revealed the meaning of role stress as an explanatory variable of career commitment and turnover intention.

Job Characteristics and Status of Community Occupational Therapist : Focus on OTs in Public Health Centers (지역사회 작업치료사의 업무 특성 및 실태 조사 : 보건소 근무 작업치료사를 중심으로)

  • Objective : This study was conducted to identify occupational therapists working in public health centers, the characteristics and actual conditions of occupational therapists in the community, and use them as basic data on occupational therapists in the community as of 2020. Methods : 77 questionnaires were replied by e-mail from OTs work at nationwide health public centers. Job characteristics and status were analysed by descriptive statistics and check correlation between job satisfaction and other factors. Results : Most survey respondents were female(77.9%) and 20-30(96.1%).. Some occupational therapists worked for dementia related team(72.7%) and others worked for like visiting care, health care, and rehabilitation center etc. Rate of experiences of public health center was 1-2 years(67.5%), the most common type of contract was flexible part-time worker(61%) and work intensity(94.8%) and satisfaction of work was very high(85.7%). The highest difficulty of their job was budget administrative work(26.7%) and of non-work difficulty was inequality under contracts(27.2%). They usually participated at dementia shelter, visiting OT, group OT. Difficulty of their job was high in budget administration, dementia shelters, and visiting work treatments. Goals of treatment were high in improvement of cognitive ability and, family support. Frequency of treatment was high in improvement of cognitive therapy, family support, and evaluation. Occupational therapy targets for health centers were dementia, the general elderly, and adult brain lesions, including those for ordinary people, psychiatric disorders and children. It was found that the primary occupations for evaluation were nurses (35.7%) and occupational therapists (33.7%), and that MMSE-DS, SGDS, and SMCQ were used a lot. Conclusion : This study could identify the job characteristics and status of community OTs. We hope that this result could be basic data for building expertise and role for community OTs in changing situations like community cares.

Epidemiological Study of Air Pollution and Its Effects on Health of Urban Population (서울시(市) 대기오염(大氣汚染)이 시민보건(市民保健)에 미치는 영향(影響)에 관(關)한 조사연구(調査硏究))

  • The urban population of our country is rapidly increasing due to many factors of social structure, and sociologists are predicting that the increase rate of the urban population will be over 50% in 1980's. Above all, the population of the capital city of Seoul, is at present more than four millions. Such centring of people in cities, together with the improvement of the standards of living, caused rapid increase in the amount of fuel consumption, and this consumption of fuels became one of the primary sources of the air pollution in cities. Moreover, the heavy traffic, construction of many tall buildings, and the increasing number of new factories due to the industrial development-all these are contributing to make the matter of air pollution worse and worse in the Metropolitan, whose geographical location is quite unfavorable considered from the view point of air pollution. Most homes in Seoul use briquet as fuel, while oils are used in tall buildings. The CO, $SO_2$, and smoke that come from burning of these fuels are a great threat to the health of the urban population. With the purpose of examining the influence of air pollution upon the public health, written inquiries were made upon respiratory diseases, and the carboxyhemoglobin saturation in the blood was measured to determine whether the air pollution may affect the health of the urban population. Method of Health Examination (1) Investigation of Respiratory Diseases Patients' records were examined to figure out the monthly ambulance rate of respiratory patients to the total number of patients treated. On the other hand, by using the questionnaire form approved by the Medical Research Council's Committee on Research into Chronic Bronchitis, investigators interviewed the examinees and inquired into the respiratory symptoms. (2) Measuring of Carboxyhemoglobin Saturation From the ear lobe of the examinees, with the use of the melangeur for the white blood cell counting, blood was taken, and after diluting it ten times with 0.1% $Na{2}CO_{3}$, again diulting it 20 times with 0.5% $Na_{2}\;CO_{3}$, its absorbancy was measured. The following results are obtained from the investigation. (1) It was found out that 7.7% of the total patients under treatment were suffering from upper respiratory infection, acute or chronic bronchitis, bronchial asthma, pulmonary emphysema and bronchiectasis. Of them all, patients with upper respiratory infection numbered the greatest with 4.8% and patients with acute or chronic bronchitis the next with 2.1%, and their monthly ambulance rate was high from December to February during the winter, and from April to May and from September to October during the changeable seasons. (2) The daily ambulance rate of respiratory patients, it was revealed, had a close connection with the concentration of $SO_2$ and CO in the air. (3) It was found out that men were more subject to respiratory disease than women, and both men and women were more liable to the diseases with the advancing of age. (4) People living at Choong-ku with the heavy traffic and in the industrial zones of Yeungdungpo had high frequency of respiratory symptoms. (5) Considered from the view point of occupations, high frequency was found among those without job, with jobs unknown, merchants and intdustrial workers, whose social status was rather low and traffic policemen who were always exposed to the exhaust gas of cars. As for women, the frequency was detected in the order of those from high to low, housewives who were exposed to briquet gas, women with jobs unknown, women without jobs, whose social status was low. (6) Ex-smokers rather than smokers, of both sexes, had higher frequency. As for men, heavy smokers had high frequency, while in women light smokers had rather high frequency which was presumed to be due to their average old age. (7) Men's average of carboxyhemoglobin saturation was 9.48%, while women's was 11.3%, higher than men's. (p<0.05). Age meant no difference in the case of men, but as for women, the saturation was remarkably high between the ages from 20 to 60. (8) No regional difference was detected in the carboxyhemoglobin saturation. (9) The carboxyhemoglobin saturation was found, in the case of men, in the order of office workers, traffic policemen, students, the unemployed, merchants and industrial workers, drivers; and as for women, the order was housewives, office workers, merchants and industrial workers. (10) No significant correlation was found between the carboxyhemoglobin saturation and the concentration of CO detected in kitchens, or between the carhoxyhemoglobin saturation and the passing of time after exposure to briquet gas. No difference of carboxyhemoglobin saturation was detected between smokers and non-smokers, and the degrees of smoking; only, significant negative correlation was found between the passing of time after smoking and the carboxyhemoglobin saturation. It is ture that air pollution causes or aggravates the respiratory diseases, increases the carboxyhemoglobin saturation in the blood, but what seems to be more hazardous to the health is the air polluted by the briquet gas in the kitchens and on-dol rooms rather than the atmospheric air pollution.
