Factors Affecting Survival Time of Cholangiocarcinoma Patients: A Prospective Study in Northeast Thailand

  • Cholangiocarcinoma (CCA) is a major health problem and cause of death among people in Northeastern Thailand. In this prospective study 171 patients newly diagnosed with CCA by physicians in 5 tertiary hospitals in four provinces of northeastern of Thailand between February and July 2011 were followed up to January 2012. The outcome was survival time from diagnosis to death. A total of 758.4 person-months of follow-up were available. The mortality rate was 16.9 per 100 person-months (95%CI: 14.1-20.1). The median survival time among CCA patients was 4.3 months (95%CI: 3.3-5.1). Cox's proportional hazard model was used to study the independent effects of factors affecting survival time among patients. Statistically significant factors included advanced stage at diagnosis (HR: 2.5, 95%CI: 1.7-3.8), presentation with jaundice (HR: 1.7, 95%CI: 1.1-2.4) or ascites (HR: 2.8, 95%CI: 1.8-4.4), and positive serum carcinoembryonic antigen (HR: 2.3, 95%CI: 1.2-4.3). Patients who had received standard treatment had a better prognosis that those who did not (HR: 0.5, 95%CI: 0.3-0.7).

  • The Seoul Neuropsychological Screening Battery (SNSB) is known as a representative comprehensive neuropsychological evaluation tool in Korea since its first standardization in 2003. It was the main neuropsychological evaluation tool in the Clinical Research Center for Dementia of South Korea, a large-scale multi-center cohort study in Korea that was started in 2005. Since then, it has been widely used by dementia clinicians, and further solidified its status as a representative dementia evaluation tool in Korea. Many research results related to the SNSB have been used as a basis for the diagnosis and evaluation of patients in various clinical settings, especially, in many areas of cognitive assessment, including dementia evaluation. The SNSB version that was updated in 2012 provides psychometrically improved norms and indicators through a model-based standardization procedure based on a theoretical probability distribution in the norm's development. By providing a score for each cognitive domain, it is easier to compare cognitive abilities between domains and to identify changes in cognitive domain functions over time. Through the development of the SNSB-Core, a short form composed of core tests, which also give a composite score was provided. The SNSB is a useful test battery that provides key information on the evaluation of early cognitive decline, analysis of cognitive decline patterns, judging the severity of dementia, and differential diagnosis of dementia. This review will provide a broad understanding of the SNSB by describing the test composition, contents of individual subtests, characteristics of standardization, analysis of the changed standard score, and related studies.

Histopathological Evaluation of Urothelial Carcinomas in Transurethral Resection Urinary Bladder Tumor Specimens: Eight Years of Single Center Experience

  • Background: Urothelial carcinoma (UC) is a malignant neoplasm that most commonly occurs in the urinary bladder. The primary aim of this study was to evaluate the clinicopathologic features, recurrence and progression in patients with bladder urothelial cancer. Materials and Methods: The medical records of patients diagnosed with UC in the state pathology laboratory between January 2006 and July 2014 were retrospectively included. Carcinomas were categorized according to age, gender, histologic grade, tumor configuration, pathologic staging, recurrence status, and progression. Results: A total of 125 (113 men, 12 women) patients were examined. The mean age was 65.9 years and the male-to-female urothelial cancer incidence ratio was 9.4:1. Low-grade UCs were observed in 85 (68%) and high-grade in 40 (32%). A papillary tumor pattern was observed in 67.2% of the UCs. Cases were classified with the following pathological grades: 34 (27.2%) cases of pTa, 70 (56%) of pT1, and 21 (16.8%) of pT2. Recurrence occurred in 27 (21.6%) patients. Ten progressed to a higher stage (pT1 to pT2), and three cases to higher grade (low to high). We also analyzed the results separately for 70 (56%) patients 65 years of age and older. Conclusions: With early detection and diagnosis of precursor lesions in older patients, by methods such as standard urologic evaluation, urinary cytology, ultrasound scanning and contrast urography, and cystoscopy, in addition to coordinated efforts between pathologists and urologists, early diagnosis may reduce the morbidity and mortality of patients with urothelial carcinoma.

  • The author has taken intraoral standard films by the paralleling technique of 6414 teeth in 838 patients ranged from early twenties to early sixties and analysed the change of pulp cavity in the mandibular posterior teeth by ageing. The obtined results were as follows : 1. There was no significant difference between the right and left teeth. 2. The degree of change of pulp cavity in female was higher than that in male. 3. The change of pulp cavity was increased by ageing. 4.   • Safety diagnosis of various structural components and facilities is indispensable for preventing catastrophic failure of material by time-dependent and environment accelerating degradation. Also, this diagnosis of operating components should be done periodically for safe maintenance and economical repair. However, conventional standard methods for mechanical properties have the problems of bulky specimen, destructive procedure and complex procedure of specimen sampling. So, a non-destructive and simple mechanical testing method using small specimen is needed. Therefore, an advanced indentation technique was developed as a potential method for non-destructive testing of in-field structures. This technique measures indentation load-depth curve during indentation and analyzes the mechanical properties related to deformation such as yield strength, tensile strength and work-hardening index. In this paper, we characterized the tensile properties including yield and tensile strengths of the V-modified Cr-Mo steels in petro-chemical and thermo-electrical plants. And also, the effects of hydrogen-assisted degradation of the V-modified Cr-Mo steels were analyzed in terms of work-hardening index and yield ratio.

Transient analysis of lubrication with a squeeze film effect due to the loading rate at the interface of a motor operated valve assembly in nuclear power plants

  • The valve assembly used in nuclear power plants is important safety-related equipment. In the new standard, the physical attributes are measured using a valve diagnosis test, which is used in the expansion to other non-tested valves using a quantitative test-basis methodology. With a motor-operated actuator, the state of stem's lubrication is related to physical attributes such as the stem factor and the friction coefficient. This study analyzed the numerical transient of fluid and solid lubrication with a squeeze film effect due to the loading rate on the stem and the stem nut using the experimental data. The differential equation that governs the motion mechanism of the stem and stem nut is established and analyzed. The flow rate, the fluid and the solid contact forces are calculated with the friction coefficient. Finally, we found that a change in the friction coefficient results from a change of the shear force in the solid contact mode during the interchange process between the solid contact mode and the fluid contact mode. The qualitative understanding of the squeeze film effect is expanded quantitatively for forces, thread surface distance, velocity, and acceleration, with consideration of the metal solid contact and fluid contact.

A Review on Clinical Studies on Gynecological Disease by Using a Pulse Diagnosis Device (맥진기를 활용한 부인과 질환 대상 임상연구 고찰)

  • Objectives: The purpose of this study was to survey the status of clinical use of a pulse tonometric device (PTD).Methods: We searched domestic and international articles using the PTD from online medical databases including OASIS, RISS, DBpia and PubMed. We selected articles on clinical application of PTD but excluded articles on mechanical design or software programming for developing a new PTD. Finally we found 13 articles and classified the articles according to the disease of clinical study.Results: Out of the 13 articles, 5 were focused on the dysmenorrhea. Three articles were focused on the infertility, two article were focused on the post mastectomy. According to the results, the pulse energy has been widely used in research as the primary outcome.Conclusions: We found out that until now the researches on clinical application of PTD mainly had been performed for producing a variety of PTDs. This results of this study will be used as a useful information during perform a clinical study and clinics. We suggest that the standard operating procedure for PTD will be developed, and researches for development and its application of various new contents will be performed.

Diagnosis of Managerial Ability of Farmers Practicing Environmental Friendly Agriculture (친환경농업 실천농가의 경영능력 진단)

  • This study diagnosed the managerial ability of farmers in the Gokseong area and the Gwangyang area, and analysis was conducted especially placing focus on the difference in managerial ability on whether environmental friendly agriculture has been implemented or not. According to the analysis results, the average score of all the farmers was 63.9 out of the full score of 100 which can be seen to be a medium to high standard. Also, in the diagnosis results according to whether environmental friendly agriculture has been implemented or not, the ability scores of farmers practicing environmental friendly agriculture were higher than those who were not, but large differences between the item scores for information ability and cooperative ability were shown. This is connoting a very important meaning in terms of the point that correspondence is being made to the conditions required to possess market response capabilities to the high information systemization society to unfold in the future. As a result, the important issue can be seen to be personnel cultivation in order to possess self response capabilities through the development of education programs that can improve this kind of managerial ability and revision of support systems.

Painless Jaundice Caused by Clonorchis sinensis Infection: A Case Report

  • A man with only yellowing of the skin and eye sclera was diagnosed with clonorchiasis, which rarely manifested jaundice as the initial symptom. However, because of a lack of evidence for a diagnostic gold standard, the time until definitive diagnosis was more than a week. The diagnostic process relied on inquiring about the patient's history, including the place of residence, dietary habits, and symptoms, as well as on serological findings, an imaging examination, and pathological findings. MRCP and CT results showed mild dilatation of intrahepatic ducts and increased periductal echogenicity. The eggs were ultimately found in stool by water sedimentation method after the negative report through direct smear. DNA sequencing of PCR production of the eggs demonstrated 98-100% homology with ITS2 of Clonorchis sinensis. After anti-parasite medical treatment, the patient's symptoms were gradually relieved. Throughout the diagnostic procedure, besides routine examinations, the sedimentation method or concentration method could be used as a sensitive way for both light and heavy C. sinensis infection in the definite diagnosis.

Factors influencing Video Display Terminal Syndrome in Clinical Nurses (임상간호사의 영상표시단말기 증후군 및 영향요인)

  • Purpose: The present research is a descriptive study aimed at understanding clinical nurses' Video display terminal (VDT) syndrome and identifying the factors that affect their VDT syndrome. Methods: Data were collected from 239 clinical nurses working in two metropolitan cities. Research tools included subject's VDT syndrome assessing musculoskeletal, ophthalmic, dermal, psychiatric, and whole body syndromes. The data were analyzed using frequency analysis, average and standard deviation, t-test, One-way ANOVA, and multiple regression analysis with the SPSS/WIN 20.0 program. Results: The subjects' VDT syndrome score was 1.34 out of 5. There were significant differences in participating subjects' VDT syndrome, hospital's size, working unit, health status, diagnosis of illness, having an Order Communication System (OCS), having an Electronic Medical Record (EMR) System, continuous VDT working for more than one hour, break time during VDT use, VDT use time, comfort of the chair, adjustability of the height of the chair, size of the VDT's desk, distance between the monitor and the user's eyes, resolution of the monitor, and frequency of eye exercise during VDT use. According to the research, influencing factors on VDT syndrome in clinical nurses included size of the VDT's desk, frequency of eye exercise during VDT use, having an EMR system, break time during VDT use, diagnosis illness, and having an OCS' system. Conclusion: The findings from this study can be used as a basis for future VDT syndrome prevention education and programs for clinical nurses.
