일부 지역 청각·언어장애인의 구강위생관리행태에 관한 조사 연구 (A Survey about Oral Hygiene Management Attitude among People with Hearing Impairments and Speech defect in an Area)

  • This study surveyed people with hearing impairments and speech defect living in an area of Gyeonggi-do using a self-administered questionnaire and interviewed them with a deaf interpreter's help during the period from January 21 to February 14, 2009 in order to investigate their oral health management attitude according to their general characteristics. The difference between males and females was statistically significant. The results of this study were as follows. 1. As to the number of times of tooth brushing according to gender, 62.5% of males and 34.2% of females brushed their teeth 2 times, and 25.0% and 35.4% 3 times. 2. As to experiences in scaling according to Academic qualification, those with high academic qualification had scaling more regularly 3. As to tooth brushing methods according to cohabitant, the rolling method was most common regardless of cohabitant. As to the number of times of tooth brushing according to residence type, 41.9% of those living with their parents brushed their teeth 3 times, and 69.2% of those living alone and 47.5% of married ones 2 times. 4. With regard to how to access oral hygiene management education, 81.1% of elementary school graduates used multimedia materials and sign language explanation and captions, 48.6% of middle school graduates used multimedia materials and sign language explanation and 14.3% multimedia materials and captions, 50.0% of high school graduates used multimedia materials and sign language explanation and 17.3% multimedia materials and captions. The parish for the buccal cavity hygiene managements of the hearing impairments speech defect and Development of an educational program are needed.

후두전적출환자의 Voice Rehabilitation에 대한보고 (The Report of Voice Rehabilitation after Total Laryngectomy)

  • Background and Objectives : Esophageal voice, Silicone voice Prosthesis with tracheo-esophageal (T-E) puncture have been used as vocal rehabilitation methods for postlaryngectomy. However, long-term follow-up in the voice rehabilitation in the total laryngectomees has not been reported. The purpose of this study is to analyze practice of postlaryngectomy voice rehabilitation and to find the effective voice rehabilitation. Materials and Methods : From Jan 1992 until June 2002, 75 patients underwent a total laryngectomy at Yongdong Severance Hospital. We retrospectively studied voice rehabilitation methods commonly used, acquisition levels of esophageal speech, patients satisfaction scale (5 rating scale) according to the methods in 33 of them (40 died, 2 unavailable) by using charts review, telephone interview. Results : T-E speech is most commonly used by 14 patients (42.4%) : A tracheo-esophageal procedure (primary or secondary puncture) by 21 : and 8 patients removed Provox. 1 patient had no voice rehabilitation. 7 patients (21.2%) have used esophageal speech : 4 patients of them have used it after removing Provox. Electrolarynx has been used with other voice rehabilitation methods : 4 patients have only used this method and 3 patients with T-E speech, 1 patient with esophageal speech. However, 6 patients (18%) remained without a substitute voice rehabilitation. In the satisfaction with speech and management of voice rehabilitation methods, patients using esophageal speech were most satisfied (4.1), patients with T-E speech were unsatisfied (2.3). Regarding with the acquisition level of esophageal speech in 33 patients, 22 patients (66.6%) failed without functional speech. Conclusion : To increase patients satisfaction and to achieve successful voice rehabilitation after total laryngectomy, preoperative counseling, pretesting, appropriate patient selection of each method and team decision-making and postoperative voice therapy must be considered.

Specifics of Speech Development of Children with Cerebral Palsy

  • Cerebral palsy is one of the most serious forms of disorders of the psychophysical development of children, which manifests itself in disturbances of motor functions, which are often combined with speech disorders, other complications of the formation of higher mental functions, and often with a decrease in intelligence. The article will discuss the speech disorder in children with cerebral palsy. Emphasis is placed on some important aspects, which should bear in mind, investigating the problem of specifics of speech development of children with cerebral palsy. In particular at the heart of speech disorders in the cerebral palsy is not only damage to certain structures of the brain, but also the later formation or underdevelopment of those parts of the cerebral cortex, which are of major importance in linguistic and mental activity. This is an ontogenetically young region of the cerebral cortex, which is most rapidly developing after birth (premotor, frontal, temmono-temporal). It is important to take into account, that children with cerebral palsy have disturbances of phonemic perception. Often, children do not distinguish between hearing sounds, cannot repeat component rows, allocate sounds in words. At dysarthria, there are violations of pronunciation of vowel and consonant sounds, tempo of speech, modulation of voice, breathing, phonation, as well as asynchronous breathing, alignment and articulation. As a result, we identified the main features and specifics of the speech development of children with cerebral palsy and described the conditions necessary for the full development of language. Language disturbances in children's cerebral palsy depend on the localization and severity of brain damage. Great importance in the mechanism of speech disorders has a pathology that limits the ability of movement and knowledge of the world.

구개인두부전증 환자의 한국어 음성 코퍼스 구축 방안 연구 (Research on Construction of the Korean Speech Corpus in Patient with Velopharyngeal Insufficiency)

  • Background and Objectives We aimed to develop a Korean version of the velopharyngeal insufficiency (VPI) speech corpus system. Subjects and Method After developing a 3-channel simultaneous speech recording device capable of recording nasal/oral and normal compound speech separately, voice data were collected from VPI patients aged more than 10 years with/without the history of operation or prior speech therapy. This was compared to a control group for which VPI was simulated by using a french-3 nelaton tube inserted via both nostril through nasopharynx and pulling the soft palate anteriorly in varying degrees. The study consisted of three transcriptors: a speech therapist transcribed the voice file into text, a second transcriptor graded speech intelligibility and severity and the third tagged the types and onset times of misarticulation. The database were composed of three main tables regarding (1) speaker's demographics, (2) condition of the recording system and (3) transcripts. All of these were interfaced with the Praat voice analysis program, which enables the user to extract exact transcribed phrases for analysis. Results In the simulated VPI group, the higher the severity of VPI, the higher the nasalance score was obtained. In addition, we could verify the vocal energy that characterizes hypernasality and compensation in nasal/oral and compound sounds spoken by VPI patients as opposed to that characgerizes the normal control group. Conclusion With the Korean version of VPI speech corpus system, patients' common difficulties and speech tendencies in articulation can be objectively evaluated. Comparing these data with those of the normal voice, mispronunciation and dysarticulation of patients with VPI can be corrected.

Treatment of velopharyngeal insufficiency in a patient with a submucous cleft palate using a speech aid: the more treatment options, the better the treatment results

  • Background: The submucous cleft palate (SMCP) is a type of cleft palate that may result in velopharyngeal insufficiency (VPI). Palate muscles completely separate oral and nasal cavities by closing off the velopharynx during functional processes such as speech or swallow. Also, hypernasality may arise from anatomical or neurological abnormalities in these functions. Treatments of this issue involve a combination of surgical intervention, speech aid, and speech therapy. This case report demonstrates successfully treated VPI resulted from SMCP without any surgical intervention but solely with speech aid appliance and speech therapy. Case presentation: A 13-year-old female patient with a speech disorder from velopharyngeal insufficiency that was caused by a submucous cleft palate visited to our OMFS clinic. In the intraoral examination, the patient had a short soft palate and bifid uvula. And the muscles in the palate did not contract properly during oral speech. She had no surgical history such as primary palatoplasty or pharyngoplasty except for tonsillectomy. And there were no other medical histories. Objective speech assessment using nasometer was performed. We diagnosed that the patient had a SMCP. The patient has shown a decrease in speech intelligibility, which resulted from hypernasality. We decided to treat the patient with speech aid (palatal lift) along with speech therapy. During the 7-month treatment, hypernasality measured by a nasometer decreased and speech intelligibility became normal. Conclusions: Surgery remains the first treatment option for patients with velopharyngeal insufficiencies from submucous cleft palates. However, there were few reports about objective speech evaluation pre- or post-operation. Moreover, there has been no report of non-surgical treatment in the recent studies. From this perspective, this report of objective improvement of speech intelligibility of VPI patient with SMCP by non-surgical treatment has a significant meaning. Speech aid can be considered as one of treatment options for management of SMCP.

바타차랴 알고리즘에서 HMM 특징 추출을 이용한 음성 인식 최적 학습 모델 (Speech Recognition Optimization Learning Model using HMM Feature Extraction In the Bhattacharyya Algorithm)

  • 음성 인식 시스템은 정확하지 않게 입력된 음성으로부터 학습 모델을 구성하고 유사한 음소 모델로 인식하기 때문에 인식률 저하를 가져온다. 따라서 본 논문에서는 바타차랴 알고리즘을 이용한 음성 인식 최적 학습 모델 구성 방법을 제안하였다. 음소가 갖는 특징을 기반으로 학습 데이터의 음소에 HMM 특징 추출 방법을 이용하였으며 유사한 학습 모델은 바타챠랴 알고리즘을 이용하여 정확한 학습 모델로 인식할 수 있도록 하였다. 바타챠랴 알고리즘을 이용하여 최적의 학습 모델을 구성하여 인식 성능을 평가하였다. 본 논문에서 제안한 시스템을 적용한 결과 음성 인식률에서 98.7%의 인식률을 나타내었다.

CHMM 어휘인식에서 채널 유사성을 이용한 선택적 음성 특징 추출 (Selective Speech Feature Extraction using Channel Similarity in CHMM Vocabulary Recognition)

  • HMM 음성 인식 시스템은 환경적 잡음과 여러 음성의 혼합으로 인하여 정확한 음성을 인지하지 못하는 단점이 있다. 따라서 본 논문은 잡음 음성으로 부터 원하는 음성만 선택하여 추출하기 위한 음성 특징 추출 기법을 CHMM을 이용하여 제안한다. 선택적 음성 추출을 위한 채널 유사성 상관 관계를 이용하여 음성 특징을 추출하는 방법을 사용하였다. 제안 기법의 실험 평가한 결과 평균 분리 왜곡도가 0.430dB 감소됨을 보임으로써 제안한 방법의 우수성을 확인하였다.

  • 음성 인식 시스템에서는 인식 성능 향상을 위한 방법으로 인간의 청취 능력을 인식 시스템에 접목하였으며 잡음 환경에서 음성 신호와 잡음을 분리하여 원하는 음성 신호만을 선택할 수 있도록 구성되었다. 하지만 실용적 측면에서 음성 인식 시스템의 성능 저하 요인으로 인식 환경 변화에 따른 잡음으로 인한 음성 검출이 정확하지 못하여 일어나는 것과 학습 모델이 일치하지 않는 것을 들 수 있다. 따라서 본 논문에서는 음성 인식 향상을 위해 감마톤을 이용하여 특징을 추출하고 음향 모델을 이용한 학습 모델을 제안하였다. 제안한 방법은 청각 장면 분석을 이용한 특징을 추출을 통해 인간의 청각 인지 능력을 반영하였으며 인식을 위한 학습 모델 과정에서 음향 모델을 이용하여 인식 성능을 향상시켰다. 성능 평가를 위해 잡음 환경의 -10dB, -5dB 신호에서 잡음 제거를 수행하여 SNR을 측정한 결과 3.12dB, 2.04dB의 성능이 향상됨을 확인하였다.

  • Total laryngectomy remains as an important treatment option in selected patients with laryngopharyngeal cancers, which inevitably sacrifices naturally produced voice. Much effort has been devoted to voice restoration for these laryngectomized patients. Several ways of voice rehabilitation after total laryngectomy have been developed and utilized thus far, including tracheoesophageal shunt speech, esophageal speech, pneumatic speech aid, and electrolarynx. Of these, tracheoesophageal shunt speech appears to be the most effective voice restoration method, while other trials might also be useful in special situations. Nevertheless, each method has its own unique mechanisms of voice production, thus has its advantages and drawbacks in clinical setting. In this review, we discuss the currently available management options for the rehabilitation of laryngectomized voice.

  • The objective evaluation of velopharyngeal closure function is the key to diagnosis and therapy control of velopharyngeal incompetency. The aim of this study is to evaluate the aerodynamic and acoustic characteristics of the velopharyngeal closure function of patients who have developed velopharyngeal incompetency after management with speech aids. The test words were composed of sustained vowels /a/, /i/, /e/, /u/, /ja/, /je/, /wi/ and polysyllabic words /p'ap'i/, /siso/, /mami/ for measuring nasalance, The data was collected before the placement of the speech aids and one to three months after. The results were as follows: The nasalance score of the velopharyngeal incompetency speakers was higher than that of the normal control group, except for nasal sounds, and was decreased after placement of the speech aids, especially in high vowels /i/ (P<.01) and /wi/ (P<.05).
