형산강 유역 포항 통합시 근로자들에 있어서 간흡충 감염상 및 초음파검사와 피내반응검사의 정확도 (Evaluation of sonography and skin test in diagnosis of clonorchiasis at the Hyongsan-gang (River) area)

  • 간흡충증의 진단에 흔히 쓰이는 초음파검사의 민감도와 특이도를 알아보기 위하여 1994년 3월부터 11월까지 유행지역인 형산강 유역 포항 통합시 근로자 609명을 대상으로 피내반응검사. 대변검사. 초음파검사 및 설문조사를 시행하였다. 초음파검사에서 진단적인 소견으로 간내 담관의 확장만을 인정할 경우의 민감도는 21%이었고, 담관주위 에코가 있는 경우까지 포함하면 민감도는 52%이었다. 피내반응검사의 민감도는 62%이었다. 피내반응검사 양성이면서 초음파 소견이 하나라도 있는 경우를 진단기준으로 삼으면 민감도는 46%이었다 본 조사에서는 간흡충 혹은 간흡충의 집합체에 의해 생성되는 것으로 알려진 초음파검사상 방추상이고 약하게 반향을 보이는 담관이나 담낭내의 비음영의 초점이 뚜렷하게 보이는 환자는 없었다. 초음파검사의 특이도는 간내 담관의 확장만을 진단적 소견으로 인정할 경우 95%. 담관주의 에코까지 진단적 소견에 포함하면 83%이었다. 피내반응검사의 특이도는 52%이었다. 피내반응검사 양성이면서 초음파 소견이 하나 라도 있는 경우를 진단기준으로 삼으면 특이도는 90%이었다.

  • C. albicans는 사람에게 병증을 유발하는 기회감염성 진균으로 효모형에서 균사형 및 위균사형으로 전환할 수 있다. 이러한 이형성의 감소나 상실은 병원성의 결여와 연관되어 있기에 C. albicans의 병원성에 가장 결정적인 요인으로 여겨진다. 선행 실험을 통하여 이중특이성 인산화 효소의 결손 균주에서 이형성의 감소를 관찰하였기에, 마우스 모델에서 병원성에 미치는 효과를 확인하고자 하였다. 이중특이성 인산화 효소의 결손 균주를 감염시킨 경우, 마우스의 생존기간이 약 15일로 야생형의 3.9일보다 증가하였다. 또한 신장조직에 침투한 C. albicans의 세포수는 야생형에 비해 10배 가량 감소하였다. 본 연구는 C. albicans에서 이형성과 병원성에 연관된 이중특이성 인산화효소에 의한 새로운 신호전달기작의 가능성을 제시한다.

Comparative Evaluation of the Risk of Malignancy Index Scoring Systems (1-4) Used in Differential Diagnosis of Adnexal Masses

  • Background: To determine the cut-off values of the preoperative risk of malignancy index (RMI) used in differentiating benign or malignant adnexal masses and to determine their significance in differential diagnosis by comparison of different systems. Materials and Methods: 191 operated women were assessed retrospectively. RMI of 1, 2, 3 and 4; cut-off values for an effective benign or malignant differentiation together with sensitivity, specificity, negative and positive predictive values were calculated. Results: Cut-off value for RMI 1 was found to be 250; there was significant (p<0.001) compatibility at this level with sensitivity of 60%, positive predictive value (PPV) of 75%, specificity of 93%, negative predictive value (NPV) of 88% and an overall compliance rate of 85%. When RMI 2 and 3 was obtained with a cut-off value of 200, there was significant (p<0.001) compatibility at this level for RMI 2 with sensitivity of 67%, PPV of 67%, specificity of 89%, NPV of 89%, histopathologic correlation of 84% while RMI 3 had significant (p<0.001) compatibility at the same level with sensitivity of 63%, PPV of 69%, specificity of 91%, NPV of 88% and a histopathologic correlation of 84%. Significant (p<0.001) compatibility for RMI 4 with a sensitivity of 67%, PPV of 73%, specificity of 92%, NPV of 89% and a histopathologic correlation of 86% was obtained at the cut-off level 400. Conclusions: RMI have a significant predictability in differentiating benign and malignant adnexal masses, thus can effectively be used in clinical practice.

Contribution of Second Metal Binding Site for Metal Specificity of D-Xylose Isomerase

  • The metal specificity of D-xylose isomerase from Streptomyces rubiginosus was examined by site-directed mutagenesis. The activation constants for metal ion ($Mg^{2+},{\;}Mn^{2+},{\;}or{\;}Co^{2+}$) of wild-type and mutant enzymes were determined by titrating the metal ion-free enzyme with $Mg^{2+},{\;}Mn^{2+},{\;}and{\;}Co^{2+}$, respectively. Substitutions of amino acids either on coordinated or around the M2 site (His-22O, Asn-185, Glu-186, and Glu-221) dramatically affected the activation constants as well as activity. A decrease of metal binding affinity was most significant in the presence of $Mg^{2+}$. When compared with the wild-type enzymes, the binding affinity of H220S and Nl85K for Mg^{2+} was decreased by 10-15-fold, while the affinity for $Mn^{2+}{\;}or{\;}Co^{2+}$ only decreased by 3-5-fold. All the mutations close to the M2 site changed their metal preference from $Mg^{2+}{\;}to{\;}Mn^{2+}{\;}or{\;}Co^{2+}$. These altered metal preferences may be caused by a relatively weak binding affinity of $Mg^{2+}$ to the enzyme. Thermal inactivation studies of mutants at the M2 site also support the importance of the M2 site geometry for metal specificity as well as the thermostability of the enzyme. Mutations of other important groups hardly affected the metal preference, although pronounced effects on the kinetic parameters were sometimes observed. This study proposes that the metal specificity of D-xylose isomerase can be altered by the perturbation of the M2 site geometry, and that the different metal preference of Group I and GroupII D-xylose isomerases may be caused by nonconserved amino acid residues around the M2 site.

요추 불안정 환자에서 단순방사선 소견과 자기공명영상 소견의 비교 (Comparison of Radiography Findings and Magnetic Resonance Image Findings of Lumbar Spine Instability Patients)

  • Purpose: This study was to investigate how dose the radiography findings are to magnetic resonance (MR) image findings in the L5-S1 instability patients. The subjects of this study were comprised of eleven males and fifteen females, who had Lumbago and agreed with this research. Methods: Radiography and MR images of Lumbar spine were acquired respectively from subjects in conditions of maximum flexion and extension. The horizontal and angular displacements in lumabosacral spine radiography were used to assess the instability of lumbar spine. MR images were also used to evaluate the intervertebral disc abnormalities and change of bone marrow. Results: The results are as follows. 1. In the case of flexion transitional displacement proposed by Dupuis et al, the specificity and negative predictive value were good accuracy ($0.7{\sim}0.8$), and the negative predictive value was in average. In the case of extension displacement, the negative predictive value was about average ($0.6{\sim}0.7$), but the sensitivity, specificity and positive predictive value were below the poor (<0.6). On the other side, the specificity was about average but other things were below in the case of angular displacement. 2. In the case of flexion transitional displacement proposed by Dupuis et al., compared with the intervertebral disc abnormalities, the negative prediction value was excellent, the sensitivity good, and the specificity about average. In the case of extension, the negative prediction value was about average, but the other things were poor. On the other side the specificity and negative predictive value had good accuracy and the sensitivity and positive prediction value were below average in the case of angular displacement. Conclusion: The above results show that the radiography finding is sufficiently helpful to find the lumbar spine instability as an economic point of view.

Direct characterization of E2-dependent target specificity and processivity using an artificial p27-linker-E2 ubiquitination system

  • Little attention has been paid to the specificity between E2 and the target protein during ubiquitination, although RING-E3 induces a potential intra-molecular reaction by mediating the direct transfer of ubiquitin from E2 to the target protein. We have constructed artificial E2 fusion proteins in which a target protein (p27) is tethered to one of six E2s via a flexible linker. Interestingly, only three E2s (UbcH5b, hHR6b, and Cdc34) are able to ubiquitinate p27 via an intra-molecular reaction in this system. Although the first ubiquitination of p27 (p27-Ub) by Cdc34 is less efficient than that of UbcH5b and hHR6b, the additional ubiquitin attachment to p27-Ub by Cdc34 is highly efficient. The E2 core of Cdc34 provides specificity to p27, and the residues 184-196 are required for possessive ubiquitination by Cdc34. We demonstrate direct E2 specificity for p27 and also show that differential ubiquitin linkages can be dependent on E2 alone.

상악동의 점막비후에 대한 WATERS방사선사진과 파노라마방사선사진 및 전산화단층사진의 진단능 비교 (Comparison of Waters′ radiography, panoramic radiography, and computed tomography in the diagnosis of antral mucosal thickening)

  • With the CT findings as gold standard, the sensitivity, the specificity, and the diagnostic accuracy of Waters' radiography and Waters' radiography with panoramic radiography were compared in the diagnosis of antral mucosal thickening of 16 patients. Three oral radiologists and three non-oral radiologists interpreted the Waters' radiographs and after 4 weeks, interpreted the Waters' radiographs and panoramic radiographs simultaneously. The interpretation point was the existence or the non-existence of the mucosal thickening on the medial, the posterolateral, the floor, and the roof of maxillary sinus. The obtained results were as followed : 1. In oral radiologist group, the sensitivity, specificity and diagnostic accuracy of Waters' film were 0.7250, 0.8489 and 0.7578 respectively. 2. The sensitivity and the diagnostic accuracy in oral radiologist group were higher than those of non-oral radiologist group (P<0.05), but there was no significant difference between two groups in the specificity (P>0.05), 3. There was no significant difference of the diagnostic abilities except the specificity in oral radiologist group between Waters' radiography and Waters' radiography with panoramic radiography (P>0.05). 4. The sensitivity and the diagnostic accuracy were the highest in the case of medial wall interpretation, the specificity was the highest in the posterolateral wall. 5. In the posterolateral wall and the floor, the sensitivity and the diagnostic accuracy of oral radiologist group were higher than those of non-oral radiologist group (P<0.05).

구성정보와 문맥정보를 이용한 전문용어의 전문성 측정 방법 (Determining the Specificity of Terms using Compositional and Contextual Information)

  • 어떤 용어가 전문적인 개념을 많이 내포하고 있을 때 전문성이 높다고 말한다. 본 논문에서는 용어의 내부 구성정보와 외부 문맥정보를 이용하여 정보이론에 기반한 방법으로 전문용어가 내포하는 전문성을 정량적으로 계산하는 방법을 제안한다. 용어의 전문성은 용어간 상하위어 관계 설정에서 중요한 필요조건으로 사용될 수 있다. 제안한 방법은 전문용어의 내부 구성정보를 이용하는 방법, 문맥정보를 이용하는 방법 그리고 두 정보를 모두 이용하는 방법으로 나눈다. 구성정보를 이용하는 방법에서는 전문용어를 구성하는 단어의 빈도수, 가중치, 바이그램, 내부 수식구조 둥을 이용하고, 문맥정보를 이용하는 방법에서는 전문용어를 수식하는 단어들의 분포를 이용한다. 본 논문에서 제안한 방법은 분야에 독립적으로 적용될 수 있고, 전문용어 생성 절차에 대한 특정을 잘 반영할 수 있는 장점이 있다. MeSH 트리에 포함된 질병 이름의 전문성 값을 계산한 뒤 상위어의 전문성 값과 비교한 결과 82.0%의 정확률을 보였다.

Diagnostic performance of enzyme-linked immnosorbent assays for diagnosing paratuberculosis in cattle: a meta-analysis

  • To evaluate the diagnostic accuracy of two commercial ELISA tests (Allied- and CSL-ELISA) for the diagnosis of Mycobacterium paratuberculosis in cattle, Meta-analysis using English language papers published during 1990-2001 was performed. Diagnostic odds ratios (DOR) were analyzed using regression analysis together with summary receiver operating characteristic (ROC) curves. The difference in diagnostic performance between the two ELISA systems was evaluated by using linear regression. Publication bias was assessed by funnel plot and linear regression. The pooled sensitivity and specificity were 44% (95% CI, 38 to 51) and 98% (95% CI, 96 to 99) for the random-effect model. The DOR between studies was heterogeneous. The area under the fitted ROC curve (AUC) was 0.72 for the unweighted and 0.77 for the weighted model. Maximum joint sensitivity and specificity for the unweighted and weighted model from their summary ROC curve were 70% and 75%, respectively. Based on the fitted model, at a specificity of 95%, sensitivity was estimated to be 52% for the unweighted and 57% for the weighted model. From the final multivariable model study characteristic, the country was the only significant variable with an explained component variance of 13.3%. There were no significant differences in discriminatory power, sensitivity, and specificity between the two ELISA tests. The overall diagnostic accuracy of two commercial ELISA tests was moderate, as judged by the AUC, maximum joint sensitivity and specificity, and estimates from the fitted model and clinical usefulness of the tests for screening program is limited because of low sensitivity and heterogeneous of DOR. It is, therefore, recommended to use ELISA tests as a parallel testing with other diagnostic tests together to increase test sensitivity in the screening program.

The Usefulness of Immunocytochemistry of CD56 in Determining Malignancy from Indeterminate Thyroid Fine-Needle Aspiration Cytology

  • Background: Fine-needle aspiration cytology serves as a safe, economical tool in evaluating thyroid nodules. However, about 30% of the samples are categorized as indeterminate. Hence, many immunocytochemistry markers have been studied, but there has not been a single outstanding marker. We studied the efficacy of CD56 with human bone marrow endothelial cell marker-1 (HBME-1) in diagnosis in the Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) category III. Methods: We reviewed ThinPrep liquid-based cytology (LBC) samples with Papanicolaou stain from July 1 to December 31, 2016 (2,195 cases) and selected TBSRTC category III cases (n=363). Twenty-six cases were histologically confirmed as benign (six cases, 23%) or malignant (20 cases, 77%); we stained 26 LBC slides with HBME-1 and CD56 through the cell transfer method. For evaluation of reactivity of immunocytochemistry, we chose atypical follicular cell clusters. Results: CD56 was not reactive in 18 of 20 cases (90%) of malignant nodules and showed cytoplasmic positivity in five of six cases (83%) of benign nodules. CD56 showed high sensitivity (90.0%) and relatively low specificity (83.3%) in detecting malignancy (p=.004). HBME-1 was reactive in 17 of 20 cases (85%) of malignant nodules and was not reactive in five of six cases (83%) of benign nodules. HBME-1 showed slightly lower sensitivity (85.0%) than CD56. The specificity in detecting malignancy by HBME-1 was similar to that of CD56 (83.3%, p=.008). CD56 and HBME-1 tests combined showed lower sensitivity (75.0% vs 90%) and higher specificity (93.8% vs 83.3%) in detecting malignancy compared to using CD56 alone. Conclusions: Using CD56 alone showed relatively low specificity despite high sensitivity for detecting malignancy. Combining CD56 with HBME-1 could increase the specificity. Thus, we suggest that CD56 could be a useful preoperative marker for differential diagnosis of TBSRTC category III samples.
