MMPI PROFILE TYPES OF MOTHERS WITH PSYCHIATRIC DISORDERED CHILDREN (정신 장애 아동들의 어머니의 MMPI 유형 분석)

  • Present study investigated the relationship between children's psychiatric disorders and their mothers' psychological problems. Ninety-one psychiatric outpatients from a children's hospital were classified into three diagnostic groups - internalizing symptom group(n=35), externalizing symptom group(n=35), and developmental disorder group(n=21) - based on their psychiatric problems. A normal control group was formed by matching on the age and education of mothers. Mothers' MMPI profiles which were classified into five types - normal, depression, anxiety disorder, somatization disorder, and personality disorder - were examined in relation with children's diagnoses. The profiles of the mothers in the normal control group were mostly normal, while depressive profiles were dominant among those of the clinical groups regardless of the children's diagnoses. Therefore, the data implied association between children's psychiatric problems and their mothers' psychological problems. A possibility of negative reciprocal interactions between children and their mothers was discussed. It was also suggested that further research need to examine the specific effects of children's psychiatric problems on the psychological adjustment of their mothers, as well as the effects of parental psychiatric problems on the adjustment of their offsprings.

Correlation of Testosterone and Pain Threshold in the Patients with Somatoform Disorder (신체형 장애에서의 Testosterone과 통각 역치의 상관관계)

  • Thirteen female patients of somatization disorder, undifferentiated somatoform disorder and somatoform disorder, NOS, diagnosed by DSM-IV were studied for their pain threshold and serum testosterone and the results were compared with the respective data of 20 control females. The results are as follows : 1) The pain threshold as measured by Variable Weight Pressure Algometer was significantly lower in the patient group(153.8${\pm}$39.5 gm/$0.05mm^2$) as compared to the control group(197.5${\pm}$66.7 gm/$0.05mm^2$)(p<0.05). 2) There was no significant difference of serum testosterone between the patient(0.175${\pm}$0.081 ng/ml) and the control(0.174${\pm}$0.108 ng/ml) groups. 3) A significant positive correlation was noted between the pain threshold and serum testosterone in the patient group(r=0.632, p<0.05, two tailed, Pearson's correlation test), but not in the normal control group(r= -0.405). From these results, it was suggested that the role of testosterone in endogenous pain control system might be more important in somatoform disorders than normals.

Functional Dyspepsia (기능성 소화불량증)

  • Functional dyspepsia is one of the most common bowel disorders as prevalent of 7.7% Korean population. The cardinal manifestations include bothersome postprandial fullness, early satiation, epigastric burning or pain. These features are chronic and should be presented recurrently with no other compatible organic disease to explain the symptoms. Even though it is not life-shortening, functional dyspepsia usually make the health-related quality of life worse especially if other functional bowel disorder coexist. The coexistence of functional bowel disorders is called as 'overlap syndrome'. Anxiety, somatization and insomnia is more prevalent in overlap syndrome compared with sole functional bowel disorder. Therefore, it is worthwhile that physician interviews and elucidates whether the dyspeptic patient had other kinds of functional bowel disorders, and manages the underlying psychotic pathology. Placebo effect is large in functional dyspepsia, and there is only four kinds of prokinetics that is proven to be superior to placebo. Adverse events relating prolonged administration of prokinetics sometimes fatal or irreversible, physician willing to describe prokinetics should be familiar to the possible adverse effects and the relating risk factors. Pathologic acid reflux is not uncommon in functional dyspepsia, and acid-suppressant is equivalent to the prokientics in most of dyspeptic patients.

Herbal Medicine for Functional Dyspepsia with Psychological Symptoms: A Systematic Review and Meta-Analysis (기능성소화불량 환자에서 나타나는 정신적 증상의 한약 치료 : 체계적 문헌고찰과 메타분석)

  • Purpose: Functional dyspepsia (FD) is a chronic syndrome accompanied by repetitive digestive symptoms that appear in the upper gastrointestinal tract and are not caused by specific diseases. Psychological symptoms like anxiety, depression, insomnia, and somatization are frequently observed in FD. The purpose of this study was to review the effect of herbal medicine on the psychological symptoms that accompany FD. Methods: Database search (PubMed, EMBASE, KISS, Kmbase, KoreanMed, NDSL, OASIS, CNKI) was performed on February 24, 2021; a total of 1825 studies were searched. After the screening, 22 studies were included. Results: The studies were assessed by Cochrane RoB 2 and sorted into a table according to psychological symptoms. Meta-analysis was performed to estimate the effect of herbal medicine. Twenty of the twenty-two studies reported that herbal medicine was significantly more effective than the control group. Only three of the studies did not have a high risk of bias. Conclusion: Herbal medicine was significantly effective with or without Western medicine and had fewer adverse effects. Severe adverse effect was not reported. Psychological symptoms in FD affect onset and duration of FD, and some FD patients want to be treated for their anxiety before other symptoms. Reliable information about treatment for the psychological symptoms of FD is lacking. We reviewed the effect of herbal medicine treatment in this study, the results of which could be selected for primary or secondary treatment for FD.

Effects of Yoga and Meditation-Focused Forest Healing Programs on Profile of Mood States (POMS) and Stress Response of Adults (요가와 명상 중심의 산림치유 프로그램이 성인의 기분상태와 스트레스 반응에 미치는 효과)

  • The purpose of this study is to verify the effects of yoga and meditation-focused forest healing programs on profiles of mood states and stress reactions of adults. We tested 17 adults who agreed to participate in the experiments that were conducted for 2 nights and 3 days between September 8 to 10, 2017 in the healing forest located at Hongcheon-gun, Gangwon-do. The test used the simplified version of the inventory for the profile of mood states (K-POMS-B: Korean version of Profile of Mood States-Brief) and the stress response inventory as the measuring tools. For data analysis, we examined the differences in the profile of mood states and stress response of subjects before and after participation in the program through the paired T-tests with the SPSS 24.0 program. The analysis of the impact of the yoga and meditation-focused forest healing program on the profile of mood states of the test subjects showed the statistically significant reduction of the total score of the profile of mood states. Although the vigor among sub-level inventories increased, it was not statistically significant. But the tension, depression, anger, fatigue, and confusion showed a statistically significant reduction. The analysis of the impact of the yoga and meditation-focused forest healing program on the stress response of the test subjects showed a statistically significant reduction. Among sub-level inventories, the tension, somatization, anger, depression, fatigue, and frustration showed a statistically significant reduction, but the aggressiveness did not. It is expected that the results of this study can be utilized in the future as reference data for clarifying the effects of yoga and meditation-focused forest healing programs.

Associations of Anxiety Symptom with Behavior and Attention in Elementary School Children with Attention-Deficit/Hyperactivity Disorder (주의력결핍-과잉행동장애 초등학생에서 불안증상이 아동의 행동 및 주의력과 가지는 관련성에 관한 연구)

  • Objective : This study aimed to investigate the associations of anxiety symptoms with behavior and attention in elementary school children with attention-deficit/hyperactivity disorder (ADHD). Methods : A total of 195 elementary school children with ADHD participated in the study. The Korean Version of ADHD Rating Scale (K-ARS) and Behavior Assessment System for Children(BASC-2) were used to measure the children's behavior. Anxiety and attention was assessed with State-Trait Anxiety Inventory for Children (STAIC) and ADHD Diagnostic System (ADS), respectively. Children with ADHD were categorized to two groups of ADHD with low anxiety (ADHD-LA) and ADHD with high anxiety (ADHD-HA) according to the total STAIC scores. Scores on K-ARS, BASC-2 and ADS were compared between two groups of ADHD-LA and ADHD-HA. Results : The Scores on K-ARS total and both subscales in ADHD-HA group were significantly higher than ADHD-LA group. Scores on the BASC-2 subscales including hyperactivity, aggression, conduct problem, anxiety, depression, somatization, withdrawal, attention problems were also significantly higher in ADHD-HA group compared to ADHD-LA group. In contrary, scores on ADS subscales were not significantly different between the both groups. Conclusion : Our study identified that the anxiety accompanied with ADHD was associated with the negative behavioral aspect in children with ADHD. However, the performances on attention task were not affected by the anxiety comorbid with ADHD. Future studies to reveal underlying mechanism are needed for further understanding the association with anxiety and ADHD.

The effect of agarwood inhalation using an electric incense burner on stress and brain waves (전기향로를 이용한 침향 흡입이 스트레스와 뇌파에 미치는 영향)

  • This study was conducted to investigate the effect of agarwood (Thymelaeaceae) inhalation using an electric incense burner on stress and brain waves. Sixteen participants were included in the study during the period from September 2019 to April 2020. Participant recruitment was undertaken by the 'H' center. This paper focused on stress reduction and compared the differences in stress and brain waves before and after agarwood inhalation using an electric incense burner. Electroencephalography was measured by a 19 Channel, Brainmaster Discovery, and analyzed using a NeuroGuide, LORETA (Brain Mapping). The analysis of technical statistics was carried out using SPSS/WIN 21.0 and the Wilcoxon signed-rank test. The study observed that the stress response index was reduced by a significance level of 0.01 in patients with anxiety, depression, and somatization disorders after agarwood inhalation. Secondly, alpha waves were increased by a significance level of 0.05, in 18 out of 19 regions measured, except FP 1. The difference after agarwood inhalation was the most significant in the region that affects emotion. Thirdly, a LORETA analysis found that alpha waves were increased in the brain region (BA 40) predominantly responsible for memory and emotion. This result clarifies that agarwood inhalation using an electric incense burner reduced stress and had a positive effect on brain waves and hence, has potential as an alternative therapy.

Relationship between Depression and Health Care Utilization (우울과 의료이용의 관계)

  • Background: Depressive disorders can be categorized into daily depression and clinical depression. The experience of depressive disorder can increase health care utilization due to decreased treatment compliance and somatization. On the other hand, the clinical depression group may also experience social prejudice associated with the illness, which can limit their access to health care utilization. In terms of the significance of health care utilization as a factor in individual and social issues, this study aims to compare the health care utilization of the clinical depression group with that of the non-depressed group and the daily depression group. Methods: The analysis utilized the inverse probability of treatment weighting based on the generalized propensity score. Results: As a result of the analysis, clinical depression and daily depression were higher among women, low-income groups, individuals with low education levels, and so forth. The clinical depression group was also higher among individuals who were not economically active, did not have private health insurance, or had multiple chronic diseases. The number of outpatient department visits in the depression group was significantly higher than in the non-depressed group. In addition, the number of outpatient department visits for the clinical depression group was significantly higher than that for the daily depression group. Outpatient medical expenses were higher in the depression group than in the non-depressed group, and there was no significant difference between the clinical depression group and the daily depression group. Conclusion: Health care utilization was higher in the depression group than the non-depressed group, it was also higher in the clinical depression group than the daily depression group.

The Influence of High School Students' Entrance Exam Stress on Their Mental Health (대입 준비생의 입시스트레스가 정신건강에 미치는 영향)

  • This study aimed at investigating the level of high school students' entrance exam stress and mental health first and also investigating if the entrance exam stress and mental health are related to gender, grade, character type, parenting style and economic status. This is expected to be used as a fundamental data for the development of health education program on high school students' stress and diagnosis of their mental health. To achieve those goals above, the questionnaire was used and the sample consisted of 600 students from general high schools in a large city, C and in a smaller city, A in Chungnam province through questionnaire and the conclusion, which was based on 582 proper questionnaires from the 600 questionnaires, through variable analysis, correlation analysis and multi-regression, is below. First, according to the information provided by respondents, the result showed the relationship between those background variables and the entrance exam stress and mental health level. As the students are more introverted and the parenting style is more authoritative, the entrance exam stress is higher and the mental health level is higher as the parenting style is more authoritative and the economic status is lower. In gender, the entrance exam stress level was high for male students in regard to parents pressure. For female students, it was due to the insufficient free time. In test performance, the good grade group showed high stress level when they don't have enough free time and the poor grade group showed high stress level when they have test tension and poor test performance. In character style, the introverted group showed high stress level in future uncertainty. In parenting style, the authoritative group showed significantly high level in all four sub-factors and there is no significant relationship with the stress level and economic statue. Female students reported higher mental health level than male students in somatization and depression. In academic achievement, the poor grade group showed high level in obsession, fear-anxiety and psychotism. In character style, the introverted group showed high level in sensitivity towards others and depression. And in parenting style, the authoritative group is higher in 9 sub-factors than the other two groups in the factor, economic status. The lower economic status group showed high mental health problem level in this order; in obsession, sensitivity towards others, depression, paranonia and psychotism. Second, the results revealed that there is a significant difference among the groups after comparing and analyzing the relationship between the mental health level according to the three groups, the first, second and third group divided by the degree of entrance exam stress. And the higher the entrance exam stress is, the higher the mental health problem level is. Verification showed that there was obvious difference among the groups. the entrance exam stress was positively correlated with the mental health level. The lack of free time, future uncertainty, test anxiety/poor test performance and parents pressure, these factors, in that order, were correlated with the mental health level. when the prediction variables which influenced on mental health are analyzed, test-anxiety/poor test performance was found to be related to mental health most. And after the factor, test-anxiety, future uncertainty and the lack of free time were listed(ranked), however results did not show any correlation with parents' pressure.

A Pilot Study of Evaluating the Reliability and Validity of Pattern Identification Tool for Insomnia and Analyzing Correlation with Psychological Tests (불면증 변증도구 신뢰도와 타당도 평가 및 심리검사와의 상관성에 대한 초기연구)

  • Objectives: The purpose of this study was to evaluate the reliability and validity of the instrument on pattern identification for insomnia (PIT-Insomnia) and verify the correlation between PIT-Insomnia and psychological tests. Methods: Two evaluators examined the pattern identification of the participants who met insomnia disorder diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorder, Fifth Edition (DSM-5) and took the Insomnia Severity Index (ISI) score over 15 once manually and twice using the PIT-Insomnia to measure the inter-rater and test-retest reliability. We also conducted the following surveys: the Pittsburgh Sleep Quality Index (PSQI), the Korean version of Beck's depression inventory (K-BDI), the Korean version of the State-Trait Anxiety Inventory (STAI-K), the Korean Symptom checklist-95 (KSCL-95), and the EuroQol-5 dimension (EQ-5D), to measure concurrent validity and correlation between the PTI-Insomnia and psychological tests. Results: 1. The test-retest reliability analysis of the pattern identification results showed moderate agreement, and test-retest reliability analysis of each pattern identification score showed agreements from poor to moderate. 2. The inter-rater reliability analysis of the pattern identification results via manual showed slight agreement, when analysis was performed with calibration, the inter-rater reliability analysis of the pattern identification results via manual showed fair agreement. 3. The concordance analysis between results via manual and the PIT-Insomnia showed poor agreement, when the analysis was performed with calibration, concordance analysis showed fair agreement. 4. The concordance analysis between the PIT-Insomnia and the PSQI showed positive linear correlation. 5. The concordance analysis between the PIT-Insomnia and the PSQI, K-BDI, STAI-K, KSCL-95, and EQ-5D showed that non-interaction between the heart and kidney have positive linear correlation with the K-BDI, anxiety item of KSCL-95, dual deficiency of the heart-spleen have positive linear correlation with somatization item of KSCL-95, paranoia item of KSCL-95, heart deficiency with timidity have positive linear correlation with stress vulnerability item of KSCL-95, parania item of KSCL-95, phlegm-fire harassing the heart have positive linear correlation with K-BDI, paranoia item of KSCL-95, depressed liver qi transforming into fire have positive linear correlation with the anxiety item of KSCL-95, parania item of KSCL-95, all pattern identification have negative linear correlation with EQ-5D. Conclusions: The PIT-Insomnia has moderate agreement of reliability and reflects the severity of insomnia since it has some concurrent validity with the PSQI. There are some correlations between the PTI-Insomnia with specific psychological tests, so we could suggest it can be used appropriately in the clinical situation.
