Effects of a Coaching Program on Comprehensive Lifestyle Modification for Women with Gestational Diabetes Mellitus (임신성 당뇨 임부의 혈당조절을 위한 포괄적인 생활양식조절 코칭 프로그램의 효과)

  • Purpose: The purpose of this study was to investigate the effects of using a Coaching Program on Comprehensive Lifestyle Modification with pregnant women who have gestational diabetes. Methods: The research design for this study was a non-equivalent control group quasi-experimental study. Pregnant women with gestational diabetes were recruited from D women's hospital located in Gyeonggi Province from April to October, 2013. Participants in this study were 34 for the control group and 34 for the experimental group. The experimental group participated in the Coaching Program on Comprehensive Lifestyle Modification. The program consisted of education, small group coaching and telephone coaching over 4weeks. Statistical analysis was performed using the SPSS 21.0 program. Results: There were significant improvements in self-care behavior, and decreases in depression, fasting blood sugar and $HbA_1C$ in the experimental group compared to the control group. However, no significant differences were found between the two groups for knowledge of gestational diabetes mellitus. Conclusion: The Coaching Program on Comprehensive Lifestyle Modification used in this study was found to be effective in improving self-care behavior and reducing depression, fasting blood sugar and $HbA_1C$, and is recommended for use in clinical practice as an effective nursing intervention for pregnant women with gestational diabetes.

Effects of Evidence Based Practice Integrated Critical Care Clinical Practicum (근거중심실무 연계 중환자간호 실습교육의 적용 및 효과)

  • Purpose: This study examines the effects of integrating Evidence Based Practice (EBP) into a critical care clinical practicum on nursing students' access and use of information resources and EBP competency. Methods: A one-group pretest-posttest design was used. Fifty senior nursing school students from a university participated. A critical care clinical practicum combined with EBP consisted of six full days of clinical practicum in intensive care units with EBP education. Group and individual training in EBP skills, lectures, small group discussion and conferences were provided. Data were analyzed using paired t tests for 50 participants. Results: The scores of evidence based practice competency increased significantly (p<.001) showing significant improvement in searching and classifying the evidence. Nursing students' access and use of research evidence improved (p=.004). Conclusion: This study showed that the integration of EBP into a clinical practicum was effective in improving accessibility and usefulness in research evidence such as guidelines and research articles, and increasing EBP competency in undergraduate students.

Effects of Family Conflict Mitigation Programs by Watching Documentaries on Conflicts, Autonomic Nerve Activation, and Happiness of the Elderly in Long-Term Care Hospitals (다큐멘터리 시청을 활용한 가족갈등 완화프로그램이 요양병원 입원 노인의 갈등, 자율신경활성도와 우울에 미치는 효과)

  • Purpose: To determine effects of family conflict mitigation documentary programs on conflicts, autonomic nerve activation (ANA), and happiness of elderly in long-term care hospitals. Methods: This study used a non-equivalent control group with a pretest-posttest design. It was conducted on 39 elderly patients (19 in the experimental group and 20 in the control group) in long-term care hospitals. The experimental group (n=19) received family conflict mitigation documentary programs. The family conflict mitigation documentary programs consisted of four sessions (40-50 minutes per session). These programs were implemented in small groups, with each group having five elderly. Data were analyzed by sing the conflict checklist, the Oxford Happiness Questionnaire, Chi-square test with Fisher's exact test, independent t-test, paired t-test, and repeated measure ANOVA using the SPSS/WIN 21.0 program. Results: The participants in the experimental group showed significantly decreased scores of conflict (t=-2.31, p=.028) and the value of sympathetic nervous system activity (t=8.36, p=.007) compared with those of the control group. The participants in the experimental group showed significantly increased the value of parasympathetic nervous system activity (t=-2.91, p=.008) and scores of happiness (t=5.46, p<.001). Conclusion: The family conflict mitigation documentary programs on conflicts, ANA, and happiness of the elderly in long-term care hospitals are effective intervention programs for mitigating conflicts between elderlies and their families and for improving happiness of the elderly in long-term care hospitals.

An Evaluation of the Importance of Living SOC Major Indicators in Small and Medium Sized Cities Using AHP (AHP를 활용한 중소도시 생활SOC 주요지표의 중요도 평가)

  • This study evaluated the importance between the major indicators of living SOC in small and medium cities. AHP layer analysis was performed on a group of experts and their importance and priorities were evaluated. The results of the assessment are as follows: First, the village(walking) living zone was considered relatively more important than the local hub(vehicle) living zone. Second, the importance of health care in the local hub(vehicle) living area was assessed to be the most important in the comprehensive evaluation. Third, medical care, care, education and convenience in the village(walking) living area were ranked second, third, fourth and fifth, respectively. Fourth, the relative importance of physical education, learning, physical education, learning, relaxation, culture and village(walking) living areas in the local hub(vehicle) living areas were valued low. It is necessary to seek living SOC policies and directions in consideration of the special nature of urban social structure in small and medium-sized cities in Korea.

A Study about the Quality of Life and QALYs of Stroke Patients according to the Medical Care Utilization Behavior: Findings from the 2009-2012 Korea Health Panel Data (의료이용행태에 따른 뇌졸중 환자의 삶의 질과 QALY 비교: 2009~2012년 한국의료패널 자료를 활용한 연구)

  • Objectives: There are no studies which have investigated the health related quality of life(HRQOL) about stroke patients according to the medical care utilization behavior by longitudinal analysis. The purpose of this study is to analyze the quality of life(QOL) and quality adjusted life years(QALYs) of stroke patients of western and combined treatment group by longitudinal analysis. Methods: A retrospective cohort study was conducted among 37 new patients who initiated diagnosis with stroke in 2009 from Korea Health Panel Data. We analyzed the HRQOL of stroke patients and calculated the QALYs after medical use initiation for up to 3 years according to the medical care utilization behavior. Results: Overall, the quality of life of stroke patients was lowered somewhat from 0.8431 to 0.7864 in 2009~2012. Western treatment group was appeared slightly declined in from 0.8527 to 0.8231 and combined treatment group was shown to be falling from 0.8173 to 0.6875. The QALYs of total patients were calculated 2.3654 and western treatment group were 2.4436, combined treatment group were 2.1542 during the 4 year period. The difference of QALYs between two groups was 0.2894 QALYs. Conclusions: Although there was a small difference in QALYs of the two groups, it is not certain that the difference is come from medical care utilization behavior. Further studies should be needed to confirm the relation of the medical care utilization behavior and quality of life by considering severity of the stroke.

The Effect of Using a Nursing Care Scale Influencing on The Nursing Performance and Job Satisfaction (일개 병원에서의 환자간호 평가도구 사용이 간호사들의 업무수행정도와 직무만족에 미치는 영향)

  • This is a quasi-experimental research to test the effect of using a nursing care scale. This study identified the effects of using a nursing care scale influencing on the nursing performance and job satisfaction. The subjects consisted of 63 staff nurses in 8 medical and surgical units of one hospital affiliated to university hospital in Seoul, and assigned to experimental and control group conveniently. The data were collected in September and December, 1992. The evaluation of the nursing performance were measured by the instrument from a nursing care scale of one univerrsity hospital and job satisfaction was measured by Stamp's Scale (1978). The data were analyzed by percentage distribution, 2-test, Mann-Whitney U Wilcoxon Rank Sum W test, ANCOVA and T-test. The summarized results were as follows : 1. The nursing performance of the experimental group showed markedly increase about 13 areas of nursing care. However no significant difference in the nursing performance between the two groups was found. 2. There was a significant difference in the nursing performance between the experimental and the control group about 7 areas of nursing records(W=22.0, P<.05). 3. After using a nursing scale, there was a significant difference in the nursing performance about nursing care and nursing records between the experimental and the control group(W=25.0, P<.05). Comparing before using a nursing scale with after, there was no significant difference in the nursing performance between two groups. 4. There was no significant difference in job satisfaction between the experimental and the control group. The main reasons for there being no defference in two groups could be the small size sample and the procedure of intervention. Further comparative study is needed using more strictly controlled procedure of intervention.

Focus Group Study on Health Care Professionals' Experience of Patient Safety Education (의사와 간호사의 환자안전교육 경험에 관한 포커스 그룹 연구)

  • Purpose:This study aims to understand and explore the subjective experiences of patient safety education among health care professionals in developing a patient safety curriculum in South Korea. Methods: A qualitative descriptive study was conducted through two focus group interviews in the period October-December 2018. Eleven participants who underwent patient safety education participated in each session. All interviews were recorded and transcribed as spoken, and qualitative content analysis was used to identify categories of discussion depicting participants' subjective experience with patient safety education. Results: A total of three categories and seven themes were identified out of 77 units of analysis. Topics were identified in the dimensions of a patient safety curriculum, as follows: (1) activities for patient safety; (2) principle of patient safety (five rights, ethics, patient participation) and patient participation; (3) leadership, teamwork, and communication; and (4) reporting and learning system for patient safety events. In the dimension of methods, (5) case and evidence-based education and (6) multidisciplinary and small group teaching were identified. Finally, in the dimension of the system, (7) policies for patient safety education were identified. Conclusion: Our findings indicate that patient safety education is a significant area for health care professionals. Health care professionals suggested that a systematic patient safety curriculum would improve their knowledge and attitude toward patient safety. Moreover, it enables them to better construct a safety environment in a hospital.

Difference of Caring Needs According to Caring Experience of the Elderly with Dementia and Caring Situation (치매노인 부양경험 유무에 따른 부양요구의 차이와 치매노인 부앵실태)

  • The purpose of this study were to find out the difference of needs of caring between care-givers and non care-givers, and to suggest the way of lessening vigorous task of care-givers for the elderly with dementia. Data were collected from 130 nationwide respondents intentionally divided into two groups; care-givers and non care-givers in the middle aged with middle and upper income. Collected data were analyzed by frequency, percentage, t-test using SPSS package. Since the result of survey, unexpectedly, showed no difference between two groups, it could be explained as that these two groups commonly had same needs of caring for the elderly with dementia. Major findings were as follow; 1) Most Koreans stiff thought family should be the main care-giver for the elderly with dementia prior to nation or society. 2) Responsibility of caring for the elderly with dementia would be better to be shared with children instead of focusing to a child. 3) They thought ideal residential facilities for the elderly with dementia were small-scale professional dementia facility(group home) rather than home or general elderly housing. 4) Professional dementia care hospital was one of the most needed facilities for the elderly with dementia, followed by short-stay and dar-care center. 5) It was revealed care-giving task was vigorous showing that most care-givers spent 1-5 hours a day for caring, while 13% of respondents spent 11-24 hours a duty. 6) 90% of care-givers took the responsibility of main care-giver because of duty of offsprings or spouses, and wanted to be free from their current circumstances. From the result of this survey researchers would like to suggest the establishment of diverse facilities for professional dementia care to lessen the caring burden for the elderly with dementia: group home, chronic hospital, short-stay, day-care center. Financial support from the government for the housing renovation of the caring families should be considered seriously afterward. It is needed to give the opportunity to select proper paid dementia care facilities according to their income and situation of household.

Effects of Age, Breast Density and Volume on Breast Cancer Diagnosis: A Retrospective Comparison of Sensitivity of Mammography and Ultrasonography in China's Rural Areas

  • Purpose: Mammography has been confirmed as the only effective mode to improve the prognosis of patients with breast cancer in Western developed countries, but might not be a good choice in other areas of the world. One of the major challenges in China is to determine an optimal imaging modality for breast cancer screening. This study was designed to clarify the sensitivity of ultrasonography compared with that of mammography in rural China. Methods: We retrospectively studied the sensitivity of mammography and ultrasonography based on 306 breast cancer patients detected by the program of "screening for cervical cancer and breast cancer" performed in Chinese rural areas between January 2009 and December 2011, and analyzed the effects of age, breast density and volume on the sensitivity. Results: Stratified analysis showed that the sensitivity of breast ultrasonography was significantly higher than that of mammography in premenopausal patients (81.4% vs. 61.1%, p=0.02), in women ${\leq}$ 55 years of age (82.2% vs. 63.4%, p<0.01), in the high breast density group (American College of Radiology [ACR] levels 3-4) (85.9% vs. 60.6%, p<0.01) and in the small breast volume group (${\leq}$ 400 ml) (87.1% vs. 66.7%, p<0.01). Age had a significant effect on sensitivity of mammography (breast density and volume-adjusted odds ratio, 6.39; 95% confidence interval, 2.8-14.4 in age group > 55 compared to age group ${\leq}$ 45), but not that of ultrasonography. Neither breast density nor volume had significant effect on sensitivity of mammography or ultrasonography. Conclusions: Ultrasonography is more sensitive than mammography in detecting breast cancer in women under 55 year-old Chinese, especially in those with high-density and relatively small breasts.

Caregiver Burden of Patients with Dementia and Day Care Center of Small Size Model within a Zone of Life (치매환자 부양자부담과 생활권 내 소규모 주·야간 돌봄 서비스 모델)

  • The aim of this study, in order to reduce the support burden of dementia primary caregiver, within the framework of long-term care insurance system to present its founding model for the development of new services. Previous research as analytical materials and based on the CARE study between 2012 to 2013. Primary caregiver burden of patient are both physical, psychological and economic aspects as well as difficulties in many aspects and had influence on many factors such as age, care burden, economic, health, and social support. It also came high-risk primary caregiver suffer from depression. This primary caregiver to take advantage of the elderly long-term care insurance system implemented at the time of the institutional support necessary for since 2008, presented additional services in this study. Of increasing the small sizes day care center (tentative name) to open an as established in the settlements at a scale of less than nine, within 5 minutes from the residence, limited dementia by expertise in the disease home care services scale model of the current system service It was to improve the quality of existing shares, small group sizes for day care centers and community life apart.
