Study of oriental medical science documentory records of tinnitus and neuropsychiatric aspect of hiccup (이명(耳鳴)에 관한 정신의학적 문헌고찰(文獻考察))

  • 1. According to causes of attack and symptoms, tinnitus is divided into two categories; deficiency and excess. Causes of excess syndrome of tinnitus were wind fire in the liver and gallbladder, phlegm fire, blood stagnation, and heat in meridian system and the causes of deficiency syndrome of tinnitus were qi deficiency or blood deficiency after an illness or delivery, yin deficiency of liver and kidney, and deficiency of sea of the marrow. 2. Tinnitus was related to the vicera and bowels, especially to liver, gallbladder and urinary bladder. 3. In regard of method of treatment, tonify kidney, nourish heart, clear the liver and discharge heat are used according to visera and bowel theory. Clear phlegm and downbear fire are used for phlegm fire. Tonify spleen and kidney is used for ancestral vessel deficiency. Dispel wind and dissipate fire can be used according to theory of five elements' motion and six kinds of natural factors. 4. The basal meridian of acupuncture and moxibustion treatment were the channels of Shaoyang.. 5. Regarding neuropsychiatric aspect of tinnitus, sudden anger and depression of mind were the main mechanism of disease and liver fire was the main cause. The prescriptions for neuropsychiatric tinnitus were Dangguiyonghuehwan, and Yongdamsagantang.

Effects of Crataegi Fructus Hot-Water Extract on Hair Growth and Growth Factor Gene Expression of Human Dermal Papilla Cells (산사 열수추출물의 모발 성장과 모유두세포의 성장인자 유전자 발현에 대한 영향)

  • Objectives : This study was carried out to investigate the effects of Crataegi Fructus water extract(CFWE) on hair growth in an alopecia model of C57BL/6N mice and human dermal papilla cells(hDPCs). Methods : Six-week old mice were depilated and separated in 3 groups ; CON, MXD(2% Minoxidil), and CFWE. The treatments were applied twice a day for 18 days. The hair growth was determined photographically. The hair density, thickness and length were identified by Folliscope and the weights of body were measured. In dorsal skin tissue, the expression of hair growth-related protein was analyzed by Western blot. In hDPCs with/without $IFN-{\gamma}$, cell proliferation and the expression of hair growth-related genes were analyzed. Results : We observed that CFWE promoted hair growth compared to CON. CFWE improved the hair density, thickness and length compared to CON. CFWE increased the $Wnt/{\beta}$-catenin signaling in dorsal skin. In hDPCs, CFWE accelerated the cell proliferation and inhibited $IFN-{\gamma}$-induced hDPCs degeneration. CFWE increased the mRNA expression of ${\beta}$-catenin, Axin-2, BMP-4, FGF-7, FGF-10, and ALP compared to CON and $IFN-{\gamma}$ treated cells. Conclusions : These results suggest that CFWE has a hair regrowth activity via $Wnt/{\beta}$-catenin signaling and can be useful for the treatment of alopecia.

Clinical Usefulness of M-test on Patients with Chronic Neck Pain: A Single Arm Pre-post Comparison Study (M-test에 근거한 만성 경항통 환자의 진단 및 치료: 전후비교 임상연구)

  • Objectives The purpose of this study was to investigate the clinical usefulness of M-test (Meridian test) as an adjunctive evaluation and treatment in patients with chronic neck pain. Methods This study was a single arm pre-post comparison study. Thirty-six eligible subjects with chronic neck pain were recruited from August to September, 2015. M-test was used for evaluating the condition of meridian, which can induce the limitation of ROM and body discomfort. Subjects were offered intradermal acupuncture treatment on one-acupoint for 48 hours. Cervical numeric rating scale (NRS), cervical range of motion (ROM), neck disability index (NDI) and surface electromyography (SEMG) were measured before and after the treatment. Total NRS and the number of movement limitation of M-test were also measured before and after the treatment. Results Among the 36 participating subjects, 4 subjects were lost to follow-up or excluded in accordance with the criteria. Significant differences on Cervical NRS and NDI were found after the treatment (p<0.001). There was a significant difference in the range of left cervical rotation (p<0.05). Root mean square (RMS) of SEMG significantly decreased on the right sternocleidomastoid muscle (p<0.05), but significantly increased on the right trapezius muscle (p<0.05). Median frequency (MdF) of SEMG significantly increased on both sternocleidomastoid muscles. There appears to be significant differences after the treatment in total NRS and the number of movement limitation of M-test (p<0.05). Conclusions These results suggest that the evaluation and treatment of M-test based on the meridian and collateral theory were effective on cervical NRS and NDI, and also improved the movability of human body.

Anti-convulsant Effects of Methanol Extract of Gastrodia Elata on Kainic Acid-induced Epilepsy Mouse Model (Kainic acid로 유도된 생쥐의 간질 발작에 대한 천마 메탄올 추출물의 항경련 효과 연구)

  • Kainic acid (KA) is a excitatory agonist causing epileptic seizure and excitotoxicity in the hippocampus. Gastrodia Elata (GE) is known to have anti-convulsant and anti-oxidant effects. This study was investigated a possible role of GE in suppressing epileptic seizure using KA-induced epilepsy mouse model. Eight-week-old male C57BL/6 mice were administrated GE (50 or 500 mg/kg) once a day for 5 days, and then injected KA (30 mg/kg) intraperitoneally. Behavioral changes in mice by KA were evaluated for 90 minutes immediately after the KA administration. Six hours after the KA administration, their brains were harvested and the expressions of glutamate decarboxylase 67 (GAD-67) and K+-Cl- cotransporter 2 (KCC2) in the hippocampus of the mice were measured by immunohistochemistry.GE delayed the onset of epileptic seizure after KA administration, suppressed the severity of the seizure and decreased the number of severe seizures dose dependently. Moreover, GAD-67 and KCC2 expressions in the cornu ammonis (CA) 1 and CA3 of 500 mg/kg GE administrated mice were significantly increased compared to those in KA-treated mice.GAD-67 and KCC2 play an important role in regulating GABAergic system. Our results suggest that GE has anti-convulsant effect against KA-induced epileptic seizure through enhancing GABAergic system.

  • The study about the five viscera's "Syndromes due to Damage" and "Disease" of 『Donguibogam. Five viscera and Six Bowels』, with several existing information on the specificity of the five viscera, were discussed as the physiological characteristics and the pathological conjugation of the five viscera. The results were as follows. The five viscera keep Essence, Spirit, Qi, Blood, Ethereal soul, and Corporeal soul, and There inside the body and keeping in the center. Based on this, it serves to make the body to strengthen. So it can be understood in relation to meat and shells. Characteristics of the specificity of the five viscera is determined to see the eyes, nose, mouth, ears revealed the face and the body that protect the five viscera, and characteristics that in relation to the date or time or season, Also that determined in conjunction with the face color, skin condition. In 『Lingshu』, 『Nanjing』 "how do pathogen involvement in the disease of the five viscera" and "disease occurs spontaneously in the Main meridian of the five viscera " and "Syndromes due to Damage to the Five viscera" in 『Donguibogam. Five viscera and Six Bowels』, It was discussed about the physiology and the pathology that is caused by biological activity according to the characteristics of the five viscera specificity "Outer symptoms" of "Five viscera Disease" in 『Donguibogam. Five viscera and Six Bowels』 is the description of the characteristics of behavior and emotional characteristics of the specificity of the five viscera. Also if you use excessive functions associated with emotional that was fall in consumptive disease.

A Study on Yongyakgwon(用藥勸) in Chochangkyeul(草窓訣) (「초창결(草窓訣)」 중(中) 「용약권(用藥勸)」에 관한 연구(硏究))

  • Objectives : Written by Yoongdongri in the 18th century Chosun, Chochangkyeul is a book that specially deals in Ungi. Yoon understood a person's body constitution by taking into factor the person's birth year, and used this information to determine his Ungi type, and understood his disease mechanism. Methods : Following the study on the first part of Chochangkyeul, named Ungiyeonron, the paper tries to study the book's second part, called Yongyakgwon. The book's core messages are summarized upon a thorough inspection. Results & Conclusions : Yoondongri was a highly-acclaimed doctor in the 18th century Chosun, and also an Ungi practitioner who utilized the theory of Ungi in his clinical practices. His practices were mainly based on the prescription of Ungi found in Chenwuze's Sanyinfang, coupled with almost all parts of Donguibogam's Husebang. He had an extensive clinical experience which helped him describe his methods of quickly responding to the side effects that sometimes occur due to misuse of medicine. Such detailed descriptions are never found in past medical books, which gives Yoon's book a great level of historical importance. Yongyakgwon divides the scripts between Gabsin, Muui, Byungye, Gyungjeong, and Yimgi in accordance with the theory of Ounhabgi, and also introduces treatments based on Yukisachun. Jangbupasuyak also introduces about six external organs and six internal organs with relation to treating wind medicine, treating heat medicine, treating cold medicine, treating dry dampness medicine, dryness-moistening medicine, pulling meridian medicine. All of these can be applied to patients easily.

A Case Report of Atopic Dermatitis treated by Soshiho-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 소시호탕(小柴胡湯)을 투여한 양명병(陽明病) 아토피 피부염 치험 1례)

  • Objective : This case report presents the effect of Soshiho-tang on Atopic Dermatitis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Shanghanlun six meridian patterns diagnostic system, we dianosed the patient as Yangmyung-byung and he had trouble getting out of bed and nighttime itch. The patient was treated with Soshiho-tang. We evaluated the results of treatment by Objective SCORAD Index and pictures. Results : After the treatment, Objective SCORAD Index decreased from 61 to 21.5. And he became to have no trouble in getting up and also nighttime itch vanished. Conclusions : This case report sowed and effectiveness of using Soshiho-tang according to Diagnostic system based on Shanghanlun provisions on Atopic Dermatitis.

  • Theory of visceral and meridian system is the most important part in traditional Korean medicine. In theory of visceral system, the aspect of function has been emphasized more than anatomical substance. In this research, the concept of visceral system would be attempted to be expanded from 'five viscera and six bowels' to whole body, and new analysis system composed of genesis, structure, function and operation. The theories of traditional Korean medicine remain at the organ system or organ level, therefore progressive research using the new analysis system is needed to approach tissue and cell levels.

Bal Mong Ja Acupuncture Treatment of Sudden Sensory Hearing Loss with Tinnitus and Dizziness (영추 발몽자를 이용한 돌발성 난청의 치험 1례)

  • Objectives : The purpose of this study is to report clinical effects of korean medical treatment based on Bal Mong Ja on a patient with sudden sensory hearing loss. Methods : In this study, we used acupuncture based on Bal Mong Ja on a patient with sudden sensory hearing loss. We also carried out treatments like herbal medication and moxibustion therapy etc. The progress of symptoms were evaluated by THI(Tinnitus Handicap Index), DHI(Dizziness Handicap Inventory), VAS(Visual Analogue Scale) once a week. Results : After 7 weeks, there were significant improvements of auditory acuity and THI, DHI, VAS. Conclusions : Acupuncture based on Bal Mong Ja is effective for relieving symptoms of sudden sensory hearing loss with tinnitus and dizziness.

A Case Report of Impulse Control Disorder treated by Daecheongryong-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대청룡탕(大靑龍湯) 투여 후 호전된 충동 조절 장애 1례)

  • Objective : This case report presents the effect of Daecheongryong-tang on Impulse Control Disorder. The patient was diagnosed with Taeyang-byung based on Shanghanlun provisions. Methods : According to Shanghanlun six meridian patterns diagnostic system, we diagnosed the patient as Taeyang-byung and she has been suffering from impulse control disorder. The patient was treated with Daecheongryong-tang. And it was provided for 60 days.The changing symptom was estimated by Global Assessment of Functioning and Visual Analogue Scale. Results : After the treatment, the patient's functioning got better from 65 to 85 on GAF. And the degree of pain was reduced from 10 to 2 on VAS. Conclusions :This case study shows an effectiveness of using Daecheongryong-tang according to Diagnostic system based on Shanghanlun provisions on Impulse Control Disorder.
