Signal to Noise Ratio of MR Spectrum by variation echo time : comparison of 1.5T and 3.0T (Echo time에 따른 MR spectrum의 SNR: 1.5T와 3.0T비교)

  • The purpose of this study is to know the differences of MR spectra, obtained from normal volunteers by variable TE value, through the quantitative analysis of brain metabolites by peak integral and SNR between 1.5T and 3.0T, together with PRESS and STEAM pulse sequence. Single-voxel MR proton spectra of the human brain obtained from normal volunteers at both 3.0T MR system (Magnetom Trio, SIEMENS, Germany) and 1.5T MR system (Signa Twinspeed, GE, USA) using the STEAM and PRESS pulse sequence. 10 healthy volunteers (3.0T:3 males, 2 females; 1.5T : 3 males, 2 females) with the range from 22 to 30 years old (mean 26 years) participated in our study. They had no personal or familial history of neurological diseases and had a normal neurological examination. Data acquisition parameters were closely matched between the two field strengths. Spectra were recorded in the white matter of the occipital lobe. Spectra were compared in terms of resolution and signal-to-noise ratio(SNR), and echo time(TE) were estimated at both field strengths. Imaging parameters was used for acquisition of the proton spectrum were as follow : TR 2000msec, TE 30ms, 40ms, 50ms, 60ms, 90ms, 144ms, 288ms, NA=96, VOI=$20{\times}20{\times}20mm3$. As the echo times were increased, the spectra obtained from 3.0T and 1.5T show decreased peak integral and SNR at both pulse sequence. PRESS pulse sequence shows higher SNR and signal intensity than those of STEAM. Especially, Spectra in normal volunteers at 3.0T demonstrated significantly improved overall SNR and spectral resolution compared to 1.5T(Fig1). The spectra acquired at short echo time, 3T MR system shows a twice improvement in SNR compared to 1.5T MR system(Table. 1). But, there was no significant difference between 3.0Tand 1.5T at long TE It is concluded that PRESS and short TE is useful for quantification of the brain metabolites at 3.0T MRS, our standardized protocol for quantification of the brain metabolites at 3.0T MRS is useful to evaluate the brain diseases by monitoring the systematic changes of biochemical metabolites concentration in vivo.

Molecular Epidemiology of Bacillus cereus in a Pediatric Cancer Center (소아 암 환자에서 발생한 Bacillus cereus 균혈증의 분자역학 분석에 관한 연구)

  • Purpose: Bacillus cereus has been reported as the cause of nosocomial infections in cancer patients. In our pediatric cancer ward, a sudden rise in the number of patients with B. cereus bacteremia was observed in 2013 to 2014. This study was performed to investigate the molecular epidemiology of increased B. cereus bacteremia cases in our center. Methods: Pediatric cancer patients who developed B. cereus bacteremia were identified from January 2001 to June 2014. The B. cereus bacteremia in this study was defined as a case in which at least one B. cereus identified in blood cultures, regardless of true bacteremia. Available isolates were further tested by multilocus sequence typing (MLST) analysis. A retrospective chart review was performed. Results: Nineteen patients developed B. cereus bacteremia during the study period. However, in 2013, a sudden increase in the number of patients with B. cereus bacteremia was observed. In addition, three patients developed B. cereus bacteremia within 1 week in July and the other three patients within 1 week in October, respectively, during emergency room renovation. However, MLST analysis revealed different sequence types without consistent patterns. Before 2013, five tested isolates were ST18, ST26, ST177, and ST147-like type, and ST219-like type. Isolates from 2013 were ST18, ST73, ST90, ST427, ST784, ST34-like type, and ST130-like type. Conclusions: MLST analyses showed variable ST distribution of B. cereus isolates. Based on this study, there was no significant evidence suggesting a true outbreak caused by a single ST among patients who developed B. cereus bacteremia.

Development of Bicycle Accident Prediction Model and Suggestion of Countermeasures on Bicycle Accidents (자전거 사고예측모형 개발 및 개선방안 제시에 관한 연구)

  • This thesis aims to improve the safety of bicycle traffic for activating the use of bicycle, main means of non-powered and non-carbon transportation in order to cope with worldwide crisis such as climate change and energy depletion and to implement sustainable traffic system. In this regard, I analyzed the problem of bicycle roads currently installed and operated, and developed the bicycle accident forecasting model. Following are the processes for this. First, this study presented the current status of bicycle road in Korea as well as accident data, collect the data on bicycle traffic accidents generated throughout the country for recent 3 years (2009~2011) and analyzed the features of bicycle traffic accidents based on the data. Second, this study selected the variable affecting the number of bicycle accidents through accident feature analysis of bicycle accidents at Jeollanam-do, and developed accident forecast model using the multiple regression analysis of 'SPSS Statistics 21'. At this time, the number of accidents due to extension per road types (crossing, crosswalk, other single road) was used. To verify the accident forecast model deduced, this study used the data on bicycle accident generated in Gwangju, 2011, and compared the prediction value with actual number of accidents. As a result, it was found out that reliability of accident forecast model was secured through reconciling with actual number of cases except certain data. Third, this study carried out field survey on the bicycle road as well as questionnaire on satisfaction of bicycle road and use of bicycle for analysis of bicycle road problems, and presented safety improvement measures for the problems deduced as well as bicycle activation plans. This study is considered to serve as the fundamental data for planning and reorganizing of bicycle road in the future, and expected to improve safety of bicycle users and to promote activation of bicycle use as the means of transportation.

The effect of subject awareness level of oral health on the quality of life for adult patients who visit hospitals and neighborhood clinics in Busan (부산지역 병의원을 내원하는 성인의 구강건강인식수준이 삶의 질에 미치는 영향)

  • The purpose of this study was to determine the quality of life according to the level of subjective perception as well as to analyze the influencing factors and improve the subjective perception to improve the quality of life. The results of the survey for adult patients in hospitals and neighborhood clinics in the region of Busan for nine months from December 2015 were as follows. Regarding the total symptoms, the women had a score of 1.41, and the younger respondents felt they suffered from no symptoms. An analysis of the quality of life by the general characteristics showed that the respondents who were better educated and had no systemic diseases had a higher score of 54.71 on the OHIP-14. The respondents who were better educated and had no systemic diseases had a higher score of 6.48 when a single item was used to determine their happiness index, and the respondents who were better educated and had no systemic diseases had a higher score of 3.82 when five items were used to rate their happiness index. The factor with the greatest influence on their scores on the OHIP-14 was the total symptoms, followed in order by the happiness index and self-rated oral health status. The variable that exerted the largest influence on the happiness index was the OHIP-14, followed by the self-rated health status, high-school or lower education, and the presence or absence of one or more chronic disease. Therefore, an improvement in the subjective awareness level is required to boost the quality of life, and a variety of health education programs should be prepared to raise the awareness level. In addition, there should be a chance to convey accurate information.

Minor Physical Anomalies in Patients with Schizophrenia (정신분열병 환자에서 신체미세기형에 관한 연구)

  • Object and Method:Minor physical anomalies(MPAs) are frequently seen in patients with schizophrenia. MPAs are considered to arise from the anomalous development of ectoderm-originated tissues in the developing fetus. Since the central nervous system originates from ectoderm, MPAs can be regarded as externally observable and objective indicators of the aberrant development which might have taken place in the central nervous system. To investigate whether MPAs are more frequent in schizophrenic patients, the frequencies of MPAs were compared between schizophrenic patients and normal controls. Total 245 schizophrenic patients diagnosed with DSM-IV(male : 158, female : 87), and 418 normal control subjects(male : 216, female : 202) were included in this study. The MPAs were measured using the modified Waldrop scale with fifteen items in six bodily regions; head, eye, ear, mouth, hand, and foot. Result:The total scores of Waldrop scale were $4.40{\pm}1.93$($mean{\pm}standard$ deviation) in patients and $3.43{\pm}1.68$ in controls for females, and for males, $4.58{\pm}1.75$ in patients and $4.28{\pm}1.59$ in controls. For females, the excess of MPAs in schizophrenic patients was statistically significant(t-test : p<0.001). For males, schizophrenic patients also showed more MPAs than normal controls, but this tendency did not reach statistical significance (t-test : p=0.094). When the modified Waldrop total scores excluding head circumference were compared, the total scores in schizophrenic patients were significantly higher for both male and female subjects(t-test : male p<0.001, female p=0.001). The individual anomaly items included in Waldrop scale were also investigated. The items of epicanthus, hypertelorism, malformed ears, syndactylia were significantly more frequent in schizophrenic patients. In contrast, the items of adherent ear lobes, asymmetric ears, furrowed tongue, curved fifth finger, single palmar crease and big gap between toes did not show any differences in frequency between schizophrenic patients and normal controls. Since a lot of statistical analyses showed different results between male and female subjects, it seems to be necessary to consider gender as an important controlling variable for the analysis, however only the item of head circumference showed statistically significant gender-related difference according to log-linear analysis. Conclusion:With a relatively large sample size, the frequencies of MPAs enlisted in Waldrop scale were compared between schizophrenic patients and normal controls in this study. MPAs were more frequently seen in schizophrenic patients and, especially, several specific items in the Waldrop scale showed prominent excess in schizophrenic patients. Although definite conclusions cannot be drawn due to the inherent limitation of the study using Waldrop scale, these results seem to support the possibility that aberrant neurodevelopmental process might be involved in the pathogenesis of schizophrenia in some of the patients.

Analyses of the Studies on Cancer-Related Quality of Life Published in Korea (암 환자 삶의 질에 대한 국내 연구논문 분석)

  • Purpose : The purpose of the present study was to analyze and evaluate prior studies published in Korea on the cancer-related quality of life, in order to make recommendations for further research. Materials and Methods : A total of 31 studies were selected from three different databases. The selected studies were analyzed according to 11 criteria, such as site of cancer, domain, independent variable, research design, self/proxy rating, single/battery instrument, translation/back translation, reliability, validity, scoring, and findings. Results : Of the 31 studies, approximately half of them were conducted using a mixed cancer group of patients. Many of the studies asserted that the concept of quality of life had a multidimensional attribute. Approximately 30% were longitudinal design studies giving information about the changes in quality of life. In all studies, except one, patients directly rated their level of quality of life. With respect to the questionnaires used for measuring the quality of life, most studies did not consider whether or not their reliability and validity had been established. In addition, when using questionnaires developed in other languages, no studies employed a translation/ back-translation technique. All studies used sum or total scoring methods when calculating the level of quality of life. The types of variables tested for their influence on qualify of life were quite limited. Conclusion : It is recommended that longitudinal design studies be peformed, using methods of data collection whose validity and reliability has been confirmed, and that studies be conducted to identify new variables having an influence on the quality of life.

  • The small size food organism(under the size 150${\mu}m$) is needed as food for early stage of marine fish larvae of small mouse (e.g the group of grouper). This study was investigated to develop a method for copepod Apocyclops sp. culture in combination with the rotifer B. rachionus for stable culture of copepod species and harvest of various size food organisms. The culture conditions as temperature, salinity, culture volume, photo period, culture preiod and observation interval were 25${\circ}C$, 22ppt, 40ml, all dark except to observation time, 16 days and every two day during the experimental period, respectively. The Tetraselmis suecica was used as the food for the two testing orgtanisms. After every two day counting, theses two organisms were transferred to fresh culture tanks with Tetraselmis suecica of $7{\times}10^5$cells/ml. In the mixed culture of B. rotundiformis and A. sp., growth of rotifer was suppressed by mixed culture with A. sp. whereas the growth of copepod Apocyclops was promoted in the mixed culture with rotifer B. rotundiformis (the maximum density was 22 individuals/ml through the 16 culture days). Moreover, the number of copepod nauplius were promoted about 2 times in the mixed culture compared to the numbers in single species culture. With this combination culture, the havested two food organisms of variable sizes. This size variation of food organisms was useful tools for larval rearing of small mouse marine fish larvae and next step food organism size of post hatched larvae.

Population density of potato virus vectors In the Kwanghwal Area, Kimje-gun, Cholla-Pukto, on the western coast (씨감자 생산을 위한 매개 진딧물 조사 - 전북 김제군 광활면의 진딧물 분포상 -)

  • Present system of seed potato production in Korea has several weak points and consequently has difficulties in covering annual shortage of 60,000 tons of seed potatoes. The author has an opinion that this so called 'High land system' of seed potato production adopted by the Government should be replaced by the 'Coastal area system' which is proposed by the author and has many advantages over present 'High land system'(2). In coastal areas where enormous acreage of rice paddies are spread, mostly around the villages. the primary host plants of the vectors are found. Therefore, the only source of aphid vectors are limited to the villages. The farmer's houses scattered more sparsely also have minor importance. In the previous paper(2), the author reported that the aphid vector populations were lower in the coastalareas than at Taegwanryong where the Alpine Experiment Station for the production. of seed potatoes is located. However, the number to vectors at Okku showed rather high density, where the trap was placed at the distance of 200 m from a village where peach and Hibiscus trees, the primary hosts of Myzus pesrsicae and Aphis gossypii were grown. To clarify the flight distance from the source of the aphid vectors, a trial was carried out in the Kwanghwal area, Kimje-gun, Cholla-pukto. on the western coast. 13 traps were placed at four directions and the distances between the traps were 250 m. (Fig. I) The traps 'Were operated from June 21 to October 31. The results are shown in Table 1. A total of some 70 species of aphids were found, including 5 speceis of potato virus vectors. The vectors are as follows: I. Myzus persicae (Sulzer) 2. Aphis gossypii Glover 3. Aulacorthum solani (Kaltenbach) 4. Lipaphis erysimi (Kaltenbach) 5. Macrosiphoniella sanborni (Gillette) Out of a total of 12,797 aphids, 5,187$(48\%)$ vectors were found. The trap catches at the 13 locations are shown in Fig. 2 and the numbers of the vectors at each location for each vector, except Macro-siphoniella sanborni. of which only a single individual was caught, are shown in Fig. 3-6. Number of vectors at C (3,279) (Centre of the village) is considerably higher than that at Suwon (763); however, EI. SI. WI and NI. where the distanecs from Care 250 m, showed lower numbers of vectors than that at Taegwanryong (347). The number of vectors at NI was rather than at the other 3 locations at the distance of 250m from the village. This was because C was in the southern part of the village. Consequently NI was much closer to the village than the other 3 locations of the same distance from C. Numbers of catches of the most important vector. Myzus persicae, are shown in Fig.3. The distribution pattern is typical except $S-2\;and\;W_3$, where several farmer's houses were found. If only the rice paddies were found in these locations. the numbers of the vectors would be small as the distances increase. Numbers of catches of the other 3 vectors are shown in Fig. 4-6. From these results. the author has drawn the following conclusions: 1. The aphid vector sources at the rice paddy belt in the western coast are the villages. 2. The vector densities at the locations where the distances are 250 m from the centre of the village are lower than that at Taegwanryong. 3. The vector densities become gradually lower as the distances from the centre of village increase. However. depending on the host plant situation at each location, the vector densities are variable. These minor sources of aphid vectors may be eliminated so that seed potatoes can be grown. 4. Thus. under the direction of specialists, fields suitable for seed potato production can be found in the coastal areas.

Amino Acid Biosynthesis and Gene Regulation in Seed (종자내 아미노산 합성 조절 유전자에 관한 연구)

  • Human and monogastric animals can not synthesize 10 out of the 20 amino asids and therefor need to obtain these from their diet. The plant seed is a major source of dietary protein. It is particular important in their study to increase nutritional quality of the seed storage proteins. The low contents of lysine, asparagine and threonenein various cereal seeds and of cystein and methionine. In legume seeds is due to the low proportions of these amino acids in the major storage proteins, we have tried to apply the three strategies; (1) mutagenesis and selection of specific amino acid analogue resistance, (2) cloning and expression study of lysine biosynthesis related gene, (3) transfomation of lysine rich soybean glycinin gene. The 5-methyltryptophan (5MT) resistant cell lines, SAR1, SAR2 and SAR3 were selected from anther derived callus of rice (Oryza sativa L. "Sasanishiki"). Among these selected cell lines, two (SAR1 and SAR3) were able to grow stably at 200 mg/L of 5MT. Analysis of the freed amino acids in callus shows that 5MT resistant cells (SAR3) accumulated free tryptophan at least up to 50 times higher than those that of the higher than of SAS. These results indicated that the 5MT resistant cell lines are useful in studies of amino acid biosynthesis. Tr75, a rice (Oryza sativa L., var. Sasanishiki) mutant resistant to 5MT was segregated from the progenies of its initial mutant line, TR1. The 5MT resistant of TR75 was inherited in the M8 generations as a single dominant nuclear gene. The content of free amino acids in the TR75 homozygous seeds increased approximately 1.5 to 2.0 fold compared to wild-type seeds. Especially, the contents of tryptophan, phenylalanine and aspartic acid were 5.0, 5.3 and 2.7 times higher than those of wild-type seeds, respectively. The content of lysine is significantly low in rice. The lysine is synthesized by a complex pathway that is predominantly regulated by feedback inhibition of several enzymes including asparginase, aspatate kinase, dihydrodipicolinat synthase, etc. For understanding the regulation mechanism of lysine synthesis in rice, we try to clone the lysine biosynthetic metabolism related gene, DHPS and asparaginase, from rice. We have isolated a rice DHPS genomic clone which contains an ORF of 1044 nucleotides (347 amino acids, Mr. 38, 381 daltons), an intron of 587 nucleotides and 5'and 3'-flanking regions by screening of rice genomic DNA library. Deduced amino acid sequence of mature peptide domain of GDHPS clone is highly conserved in monocot and dicot plants whereas that of transit peptide domain is extremely different depending on plant specie. Southern blot analysis indicated that GDHPS is located two copy gene in rice genome. The transcripts of a rice GDHPS were expressed in leaves and roots but not detected in callus tissues. The transcription level of GDHPS is much higher in leaves indicating enormous chloroplast development than roots. Genomic DNA clones for asparaginase genes were screened from the rice genomic library by using plaque hybridization technique. Twelve different genomic clones were isolated from first and second screening, and 8 of 12 clones were analyzed by restriction patterns and identified by Southern Blotting, Restriction enzyme digestion patterns and Southern blot analysis of 8 clones show the different pattern for asparaginase gene. Genomic Southern blot analysis from rice were done. It is estimated that rice has at least 2-3 copy of asparaginase gene. One of 8 positive clones was subcloned into the pBluescript SK(+) vector, and was constructed the physical map. For transformation of lysine rich storage protein into tobacco, soybean glycinin genes are transformed into tobacco. To examine whether glycinin could be stably accumulated in endosperm tissue, the glycinin cDNA was transcriptionally fused to an endosperm-specific promotor of the rice storage protein glutelin gene and then introduced into tobacco genomic via Agrobacterium-mediated transformation. Consequently the glycinin gene was expressed in a seed-and developmentally-specific manner in transgenic tobacco seeds. Glycinin were targeted to vacuole-derived protein bodies in the endosperm tissue and highly accumulated in the matrix region of many transgenic plant (1-4% of total seed proteins). Synthesized glycinin was processed into mature form, and assembled into a hexamer in a similar manner as the glycinin in soybean seed. Modified glycinin, in which 4 contiguous methionine residues were inserted at the variable regions corresponding to the C - teminal regions of the acidic and basic polypeptides, were also found to be accumulated similarly as in the normal glycinin. There was no apparent difference in the expression level, processing and targeting to protein bodies, or accumulation level between normal and modified glycinin. glycinin.

The Value of ICAM-1 Expression and the Soluble ICAM-1(sICAM-1) Level as a Marker of Activity in Sarcoidosis: The Relationship Between the ICAM-1 Level and the Clinical Course of the Disease (유육종증의 활동성 지표로서의 ICAM-1)

  • Background: The natural course of sarcoidosis is variable from spontaneous remission to significant morbidity or death. So the assessment of disease activity is important but no single parameter was generally accepted as a good marker. Recently several studies suggested that adhesion molecules, especially ICAM-1 can be a marker, but there are some controversies. And only few data are available about the relationship of ICAM-1 with clinical follow-up course. Methods: We measured the expression of adhesion molecules on BAL cells by flow cytometry and the level of soluble ICAM-1(sICAM-1) in serum and BALF at the time of diagnosis in 12 patients with active disease and 7 inactive sarcoidosis(5 male, 14 female, mean age: $39.4{\pm}10.7$ years, mean follow-up : $20{\pm}15$ months). Follow-up clinical course were compared with the changes in serum sICAMA-1 level and the adhesion molecule on BAL cells. Results: In the patients with active disease, the ICAM-1 on AM(RMFI: $3.68{\pm}1.71$) and sICAM-1 level in serum($582{\pm}193$ng/ml) and BAL fluid($47.8{\pm}16.5$ng/ml) were all higher than those of 7 inactive disease(RMFI: $1.89{\pm}0.75$, p=0.0298, serum: $294{\pm}117$ ng/ml, p=0.0049, BALF: $20.9{\pm}8.3$ ng/ml). In the active sarcoidosis, ICAM-1 on AM(RMFI : $1.51{\pm}0.84$) and serum sICAM-1 were decreased after the therapy($250{\pm}147$ ng/ml) but no significant change was noted in inactive disease. Also we found the initial ICAM-1 on AM and serum sICAM-1 had a significant correlation with the degree of improvement in PFT after the therapy. During the follow-up, the disease relapsed in 4 patients after the discontinuation of steroid and the serum sICAM-1 level went-up again at the time of relapse. Conclusion: Our data suggest that the serum sICAM-1 level and the ICAM-1 expression on AM can be a good marker of disease activity and also a predictor of outcome in sarcoidosis.
