  • The application of Shinkihuan umder the combination of configuration, color, pulse and symptoms leads to the following conclusions: 1. Shinkihuan is applied to the diseases related with lung, kidney and the spine: difficult urination of old people, shortness of breath, hematuria, loss of voice, distorted vision, deafness of deficiency, hemorrhoids, thirst, carbuncle, suppurative infection, children's retardation of walking and noma, etc... 2. The effect of Shinkihuan is as follows. It nourishes the lung, replenishes the kidney fluid, relieves the phlegm, tranquilizes fever due to deficiency of blood and reinforces the liver. 3. Shinkihuan is appropriate for the persons with the following characteristics in configuration: male, tall, thin, dam-typed, inverted-triangular, prominent nosed and dry.

Massive Necrotizing Fasciitis of the Chest Wall: A Very Rare Case Report of a Closed Thoracostomy Complication

  • We present a case study of necrotizing fasciitis (NF), a very rare but dangerous complication of chest tube management. A 69-year-old man with shortness of breath underwent thoracostomy for chest tube placement and drainage with antibiotic treatment, followed by a computed tomography scan. He was diagnosed with thoracic empyema. Initially, a non-cardiovascular and thoracic surgeon managed the drainage, but the management was inappropriate. The patient developed NF at the tube site on the chest wall, requiring emergency fasciotomy and extensive surgical debridement. He was discharged without any complications after successful control of NF. A thoracic surgeon can perform both tube thoracostomy and tube management directly to avoid complications, as delayed drainage might result in severe complications.

  • Venous aneurysms are uncommon in the lower limb and are more frequently found in the neck and thoracic and visceral veins. However, they have been reported to cause thrombosis, pulmonary thromboembolism, and other related complications. Popliteal venous aneurysms are often undetected because they are usually asymptomatic, but they may cause pulmonary thromboembolic events. We experienced a case of a 44-year-old man who was referred for recurrent pulmonary thromboembolism. He showed no other symptoms or signs except shortness of breath. A popliteal venous aneurysm was diagnosed incidentally because the examinations were performed to detect a deep vein thrombosis in relationship to the patient's history of pulmonary thromboembolism. We report a case of surgical treatment for a popliteal venous aneurysm that was complicated by pulmonary thromboembolism.

  • An 80-year-old Korean woman who previously experienced penicillin allergy presented to a dental hospital for treatment of a fractured tooth. A dentist extracted the tooth and prescribed her A-cillin (amoxicillin hydrate) 250 mg orally 3 times a day for 3 days and Carol-F® (ibuprofen arginine) orally 3 times a day for 3 days. She received the prescriptions, returned home, and took one dose of each drug. One hour later, she experienced hypertension, itching, shortness of breath, and was unable to move her tongue to pronounce words. Approximately 6 hours after drug administration, the symptoms persisted, and the patient was admitted to an emergency medical center (EMC). The patient was discharged from the EMC after a one-night stay in the hospital for symptomatic treatment.

  • The purpose of this study is to report the effects of oriental demonstration treatment for Panic Disorder. Panic Disorder is an anxiety disorder, characterized by panic attacks which are recurring episodes of fear accompanied by somatic symptoms such as shortness of breath, palpitations, chest pain, choking, dizziness, trembling and/or faintness. Objectives: We experienced one case of Panic Disorder treated with Asian medical treatment, acupuncture and herbal medication. 20-year-old female patient suffered from Panic Disorder with nausea and insomnia. Methods: Shihosogantang-gami had been applied to the patient. Results: After treatment her Panic Disorder was improved. Conclusions: Shihosogantang -gami of Asian medical treatment could be safe and effective.

Two Cases of Adenoid Cystic Carcinoma of the Left Main Bronchus and Trachea (좌측 주기관지 및 기관에 발생한 선양낭포성암 2례)

  • Adenoid cystic carcinoma is an uncommon histologic variant of adenocarcinoma which usually arises from the salivary, lacrimal, or other exocrine glands. Characteristics of adenoid cystic carcinoma are its potential for extending long distance submucosally and for perineural invasion. It grows slowly and may have a prolonged course before diagnosis and after treatment. Recently, we have experienced 2 cases of adenoid cystic carcinoma arising from main bronchus and trachea. One case was 58 years old female patient. Her symptoms were productive cough with dyspnea. She has been history of shortness of breath, wheezing and cough during 4 years, which was initially diagnosed as bronchial asthma. The tumor was located on the left main stem bronchus which was obstructed the lumen nearly complete, by CT demonstration. Tissue diagnosis was confirmed as adenoid cystic carcinoma by bronchoscopic biopsy. The patient underwent radiation to relieve a bronchial stenosis caused by her tumor. The patient has remained well and is asymptomatic without evidence of clinical recurrence. The other case was 25 year old female. She complained shortness of breath and inspiratory difficulty during sleep. The tumor was located in upper trachea, which protruded from the posterior wall of the trachea and obstructed the lumen nearly complete, by CT demonstration. Tissue diagnosis was confirmed as adenocystic carcinoma by rigid bronchoscopic biopsy. The patient was underwent operation for removal of the mass and received radiotherapy. The patient has remained well are following up now.

Relationships between Symptom Experience and Quality of Life in Patients with Atrial Fibrillation (심방세동 환자의 증상경험 및 삶의 질간의 관계)

  • Purpose: In this study, relationships between symptom experience and quality of life in a cross-sectional sample of patients with Atrial Fibrillation (AF) were investigated. Methods: This descriptive study involved a convenience sample of AF patients from S university hospital, C city. One hundred and two AF patients completed psychometric validated measures of AF related symptoms and quality of life. Descriptive statistics and Pearson correlation coefficients with SPSS WIN 14.0 were used for data analysis. Results: Of 16 atrial arrhythmia-related symptoms, the patients reported 'tiredness' as the most frequent and 'shortness of breath' as the most severe. The level of overall quality of life for patients with AF was 53.92. There were significant differences in symptom frequency according to religion, New York Heart Association (NYHA) classification and left ventricular ejection fraction ; symptom severity according to monthly income and stroke ; quality of life according to age, job, alcohol intake, NYHA class and stroke. Quality of life for these patients was positively correlated with symptom frequency and symptom severity. Conclusions: This study demonstrated that patients with more frequent and severe symptoms perceive poorer quality of life than patients with less frequent and less severe symptoms. Symptom experience should be assessed early to improve quality of life for patients.

  • Coughing and breath shortness are common symptoms of nano (small) cell lung cancer. Smoking is main factor in causing such cancers. The cancer cells form on the soft tissues of lung. Deformation behavior and wave vibration of lung affected when cancer cells exist. Therefore, in the current work, phase velocity behavior of the small cell lung cancer as a main part of the body via an exact size-dependent theory is presented. Regarding this problem, displacement fields of small cell lung cancer are obtained using first-order shear deformation theory with five parameters. Besides, the size-dependent small cell lung cancer is modeled via nonlocal stress/strain gradient theory (NSGT). An analytical method is applied for solving the governing equations of the small cell lung cancer structure. The novelty of the current study is the consideration of the five-parameter of displacement for curved panel, and porosity as well as NSGT are employed and solved using the analytical method. For more verification, the outcomes of this reports are compared with the predictions of deep neural network (DNN) with adaptive optimization method. A thorough parametric investigation is conducted on the effect of NSGT parameters, porosity and geometry on the phase velocity behavior of the small cell lung cancer structure.

  • A bronchogenic cyst causing cardiac tamponade is a rare condition. We report an unusual case of a bronchogenic cyst that caused cardiac tamponade. A 49-year-old female patient presented at our emergency room with complaints of palpitations and shortness of breath that had lasted for 5 days preceding the visit. Echocardiography revealed a very large cystic mass compressing the left atrium posteriorly, and a large amount of pericardial effusion caused the diastolic collapse of the ventricles. Atrial fibrillation and aggravated dyspnea were observed, and the patient's vital signs were unstable after admission. We therefore performed an emergency operation. The bronchogenic cyst was resected by thoracotomy and the patient was discharged 12 days after the operation without any complications over 5 years of follow-up.

  • Objective: This study is performed for the purpose of observing the effect of JSD on the cough Methods: 35 patients with the cough treated with JSD herbal acupuncture at 肺兪穴(BL13) and 平喘穴, then self-conscious complain rates were studied. Results: The result as follows. 1. There are remarkable improvement in self-conscious complain rate about chronic cough, sputum, sneezing, runny nose, sore throat, shortness of breath which chronic cough patients complained. 2. There is no significance in self-conscious complain rate according to suffering and treatment period. 3. These result indicates that the effect of JSD herbal acupuncture on chronic cough si exellent.
