Symptom Clusters in Korean Patients with Acute Myocardial Infarction (급성 심근경색 환자의 증상클러스터)

  • Purpose: Acute myocardial infarction (AMI) leads to death if the patient does not receive emergency treatment. Thus it is very important to recognize the symptoms in the early stage. The purpose of this study was to identify clusters of symptoms that represent AMI in Koreans. Methods: The study used a retrospective, descriptive design with secondary data analysis. Data were abstracted from 725 medical records of AMI patients admitted from June 1, 2006 to August 15, 2014 at a university hospital. Results: Analysis of the AMI symptoms revealed five symptom clusters; Cluster 1 (n=140): middle chest pain (100%), shortness of breath, and cold sweating, Cluster 2 (n=256): substernal pain (100%), cold sweating, and shortness of breath, Cluster 3 (n=47): substernal pain (95.7%), left arm pain, shortness of breath, cold sweating, left shoulder pain, right arm pain, and the lower neck pain, Cluster 4 (n=212): shortness of breath (28.3%), left chest pain, and upper abdominal pain, and Cluster 5 (n=70): cold sweating (100%), left chest pain, shortness of breath, left shoulder pain, and upper abdominal pain. Length of hospital stay and mortality rate were significantly different according to symptom clusters (F=2.52, p =.040; F=3.62, p =.006, respectively). Conclusion: Symptom clusters of AMI from this study can be used for AMI patients in order to recognize their symptoms at an early stage. The study findings should be considered when developing educational prevention programs for Koreans with AMI.

The cause of shortness of breath due to fluid retention is abnormal rising of water-evil and it srepregentative symptom are as follows'Cough or dyspnea, shortness of breath-sleeplessness, rapid respiration accused by having rest, edemd on the body and leg' 2. From the point of view, the rapid respiration accused by heart usually bring about imperfect left heart Its repregentetive symptom are cyspnea, acute dyspnea at night, bronchial wheezing edema on the leg, and the thing which bring about at moving is mildcase but what bring about at rest is severe case 4. We have known that the symptom of shortness of breath due to fluid retention are similiar to cardiac rapid respiration, and the symptom of dyspnea acused by fire-evil alike 'dyspnea at moving' acused at mild case of cardiac rapid respiration.'

  • 1. Objectives Pneumothorax is defined as the presence of air or gas in the pleural cavity causing chest pain and shortness of breath. Primary spontaneous pneumothoraces (PSPs) occur in people without underlying lung disease or trauma to the thorax. We have tried researching effect of herbal medicine Doksampalmulkeunja-rang(獨蔘八物君子湯) in PSPs. 2. Methods This patient with PSPs which size was more than 30% of Rt. lung, who had been experienced the recurrences of PSPs in Lt. lung for three times, was treated by Doksampalmulkeunja-tang(獨蔘八物君子湯) diagnosed by Soeumin. 3. Results Doksampalmulkeunja-tang(獨蔘八物君子湯) was effective against the chest discomfort and shortness of breath. The chest test was improved. 4. Conclusions The treatment with herb-medicine may have effect on PSPs symptom of Soeumin by raising the stagnant yang qi. Further Sasang medical study on PSPs is needed for recurrence prevention.

Left atrial myxoma: 2 cases report (좌심방 점액종 2례 보고)

  • The primary cardiac tumor was considered as a form of disease with poor prognosis in the past and it was hardly diagnosed before post mortem autopsy. But recent development of diagnostic procedure and the cardiac surgery using extracorporeal circulation increased the accuracy of diagnosis and the opportunity of successful treatment. The authors present two cases of left atrial myxoma which experienced during recent 4 years between 1979 and 1982. A 33 year old woman admitted with severe shortness of breath, generalized edema, ascites and the evidence of hepatopathy resulted from right sided heart failure. Preoperatively, the patient was treated with conservative medication to improve general condition for a few days. The tumor mass was removed successfully under the cardiopulmonary bypass. She, however, died of myocardial dysfunction showing low blood pressure. The tumor mass weighed 22gm and measured 5.2x4.5x3.6 cm in size. A 60 year old man admitted with shortness of breath on exertion and an episode of fainting. Following the removal of tumor mass under the cardiovascular bypass, the clinical course was satisfactory with no complication. The tumor mass weighed 105gm and measured 9x4x5 cm in size.

  • Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論)was written by Chao Yuanfang that was most active during the Sui Dynasty at A.D. 610. It classified clinical medicine by some departments of internal medicine, surgery, obstetrics and gynecology, pediatrics, and otorhinolaryngology and within each specific department, categorizes etiology and pathology according to characteristics of various diseases. It was the total disease classification book that based on the bibles of the Oriental medicine, (Huangdi''s) Internal Classic(黃帝內經), Classic of Difficult Issues(難經) and Treatise on Cold Damage and Miscellaneous Diseases(傷寒雜病論), A-B Classic of Acupuncture and Moxibustion(鍼灸甲乙經), Essential Prescriptions Worth a Thousand Gold for Emergencies(備急千金要方) and Medical Secrets of an Official(外臺秘要). It was arranged tussive causes, classes, diagnosis, prognosis and stretching for treatment by Volume 13, all symptoms of tussiculation. Dialectic part was divided into cough(咳嗽), cough with dyspnea(咳逆上氣), cough with pus and blood(咳嗽吐膿血), cough with duck crying sound(?嗽), sudden cough(暴氣咳嗽), cough with dyspnea(咳逆), cough with dyspnea and vomiting(咳逆上氣嘔吐). Disease situation part was divided into the new cough(新咳) and old cough(舊咳), deficiency syndrome(虛證) and excess syndrome(實證), visceral cough(藏府咳), etc. Out of these, cough with counterflow(咳嗽上氣), cough with dyspnea(咳逆上氣), cough with dyspnea and vomiting(咳逆上氣嘔吐), cough with shortness of breath(咳逆短氣) have a close connection with dyspnea(上氣), counterflow of qi(逆氣), dyspnea and vomiting(上氣嘔吐) and shortness of breath(短氣) in the Pathogenesis and Manifestations of All qi(氣病諸候) of vol 13. So two parts may be refer to each other. However, the content on the original book has been addition and subtraction on the original context along with many reprints. Therefore, this paper, with regard to the prints of former editions, tried to help in better comprehension of the original context through readings and Korean translation.

Successful Treatment of Pleural Effusion in Small Cell Lung Cancer Patient with Gunreyngtang-gagambang

  • Objectives: We report one patient with pleural effusion and effusion-related symptoms in small cell lung cancer (SCLC) successfully treated with Gunreyngtang-gagambang. Methods: Gunreyngtang-gagambang was administered at 30 minutes after mealtime, three times a day, for two months. Except for herbal medicine, the patient did not take any treatment including pharmaceutical or non pharmaceutical for effusion. Result: Two months later, the symptoms and the pleural effusion had disappeared from chest X-ray. Conclusion: Gunreyngtang-gagambang was effective for treatment of malignant pleural effusion due to small cell lung cancer.

  • Surgical procedures on the trachea have only been undertaken within the past 50 years. Knowing the unique blood supply of the trachea and how to reduce tension on any anastomosis are key to a successful outcome. Tracheal conditions requiring surgery usually present with shortness of breath on exertion, and preoperative evaluation involves computed tomography and rigid bronchoscopy. Tracheal resection and reconstruction can be safely performed with excellent outcomes by following a well-described technique.

A Study on the Respiratory System Health Condition of Dental Laboratory Technicians in Jullabuk-do (전라북도 치과기공사들의 호흡기계 건강에 관한 조사연구 (1993년도와 2001년도 비교연구))

  • The purpose of this study was to examine the respiratory system health condition of dental technicians and to be of use for more successful health management and health promotion for them. The subjects in this study were 155 dental technicians who were working in North Jeolla province. Out of them, 70 dental mechanics were investigated in 1993, and 85 were surveyed in 2001. And the reports of the two groups on respiratory distress, including cough, sputum, phlegm, the notable sound of breathing, nasal discharge, coryza, shortness of breath and gasping, were compared. The findings of this study were as below: 1. The cough report rate was 24.3% in 1993 and 16.5% in 2001. There appeared approximately 7.8% decrease between the two years. The most powerful variables included working hours and age. 2. The complaint rate of sputum and phlegm was 47.1% in 1993 and 43.5% in 2001. The rate of 2001 reduced by 3.6%. Whether or not they smoked was identified as the most influential variable. 3. The report rate of shortness of breath and gasping was 7.4% in 1993 and 12.9% in 2001, and this rate showed about 1.5% increase in the latter year. The biggest variables were working hours and career. 4. The complaint rate of nasal discharge and coryza was 41.4% in 1993 and 44.7% in 2001, which showed about 3.3% increase. The largest variables were smoking and career. 5. The report rate of the notable sound of breathing and breathing in light little gasps was 12.0% in 1993 and 17.6% in 2001. There was approximately 5.6% increase, and the most powerful variables were working hours and career.

Superior Vena Caval Syndrome -Report of A Case- (특발성 종격동섬유화에 의한 상공정맥증후군일예)

  • This is a report of a case of superior vena caval syndrome due to idiopathic mediastinal fibrosis, which was surgically treated. The patient was 35-year-old Korean male who progressively complained shortness of breath about for 40 days prior to operation. Phlebogram of SVC showed indentation of SVC at the site of cavoatrial junction. The operation was performed under impression of bronchogenic cancer of right hilum. After thoracotomy, it was found a irregular mass in the mediastinum at the level of cavoatrial junction, which was developed to surrounding with SVC, pericardium, trachea and bronchus and they fixed together to immobile. Bypass graft between SVC and right atrial appendage was performed using a pericardial roll tube This is a report of a case of superior vena caval syndrome due to idiopathic mediastinal fibrosis, which was surgically treated. SVC pressure was markedly reduced from 32 cm $H_2O$ in preoperative to 21 cm $H_2O$in postoperative. Mediastinal fibrosis was confirmed by histopathological examination postoperatively. The postoperative course was uneventful.

Effect of Saengmaek-san on Cancer Patients with Symptoms Related to Radiation Pneumonitis after Radiotherapy : Report of 2 Cases (방사선치료 후 방사선 폐렴이 유발된 암 환자에 생맥산을 투여한 증례 보고 2례)

  • Objective : This study is purposed to report 2 cases of cancer patient whose symptoms related to radiation pneumonitis had been controlled with Saengmaek-san treatment. Methods : A 56-year-old female rectal cancer patient was prescribed with Saengmaek-san due to the symptoms which had developed 4 weeks after the completion of her radiotherapy session in both lungs. Her chief complaints were shortness of breath, dry cough and fatigue. Another case, 53-year-old male patient with hepatocellular carcinoma, had also developed symptoms of fatigue, weight loss and dry cough after his radiotherapy session in left upper lung zone. Radiological changes of both patients' chest X-ray suggested radiation pneumonitis. Results : Symptoms of the female patient were improved, especially shortness of breath, after Saengmaek-san treatment, without any aggravation in her chest X-ray result. However, infiltrative opacity in the left upper lung zone of the male patient was aggravated despite the improvement of his clinical symptoms. His remaining symptoms and radiological change were effectively controlled after steroid therapy. Conclusion : Seangmaek-san may be considered as a potential treatment for symptoms related to radiation pneumonitis with proper monitoring.
