Effects of folic acid supplementation on serum homocysteine levels, lipid profiles, and vascular parameters in post-menopausal Korean women with type 2 diabetes mellitus

  • BACKGROUND/OBJECTIVES: Complications of diabetes, such as cardiovascular disease, are associated with increased mortality among type 2 diabetes mellitus patients. Homocysteine has been recently identified as a predictor of cardiovascular disease-related complications in diabetes. We investigated whether or not supplementation with folic acid tablets can lower homocysteine levels and improve parameters related with vascular complications. SUBJECTS/METHODS: We conducted a non-randomized 8-week trial involving postmenopausal diabetic women (n = 25) supplemented with $800{\mu}g$ of folic acid ($400{\mu}g$ twice a day) daily. Subjects' serum levels of folate, homocysteine, and vitamin $B_{12}$ were measured, along with vascular function and brachial-ankle pulse wave velocity. RESULTS: Folic acid supplementation significantly increased serum folate levels (P < 0.0001), reduced homocysteine levels (P < 0.0001), and increased vitamin $B_{12}$ levels (P = 0.0063). There were significant decreases in low-density lipoprotein cholesterol levels as well as the ratios of low-density lipoprotein cholesterol to high-density lipoprotein cholesterol and total cholesterol to high-density lipoprotein cholesterol. Brachial-ankle pulse wave velocities were not altered by supplementation. Changes in serum vitamin $B_{12}$ after folic acid supplementation were negatively correlated with changes in brachial-ankle pulse wave velocity. CONCLUSIONS: In this study on postmenopausal Korean women with type 2 diabetes mellitus, folic acid supplementation reduced serum homocysteine levels, increased serum folate and vitamin $B_{12}$ levels, and lowered lipid parameters.

The Nutritional Status and Intervention Effects of Multivitamin-Mineral Supplementation in Nursing-Home Residents in Korea (노인복지시설 거주노인의 영양상태와 비타민-무기질 보충 효과에 관한 연구)

  • This study was performed to assess the nutritional status of nursing home residents and to assess the effect of nutrition intervention. The subjects were 123 people aged over 60 years from 5 different nursing homes. The nutrition intervention study was carried out by supplementing their diet with multivitamin-minerals for 2 months. The mean intakes of most nutrients did not meet the RDA, Though the nutrient content of the menus provided by the facilities were satisfactory. Nutrients of which intakes fell below 75% of the RDA were protein, Ca, Fe, vitamin A vitamin B$_1$, and vitamin B$_2$. The BMIs of male and female subjects were 22.0kg/$m^2$ and 24.6kg/$m^2$ and the WHRs were 0.92 and 0.90, respectively. The percentage of subjects with hypertention (BP$\geq$140/90mmHg) and with anemia(Hb$\leq$13mg/100$m\ell$ in men, Hb$\leq$12mg/100$m\ell$ in women) were 34.6% and 41.9%, respectively. The serum cncentrations of albumin, total protein, triglyceride, total cholesterol, HDL-cholesterol, and total lipid fell within normal ranges. However, 27.5% of the subjects showed a high serum cholesterol level of over 250mg/100$m\ell$. The concentration of C3 was 81.2mg/100$m\ell$, IgG, 1343mg/100$m\ell$, and IL-2, 0.766mg/$m\ell$. after 2 months of vitamin-mineral supplementation, the levels of blood glucose and total cholesterol were significantly decreased and triglyceride was significantly increased. The vitamin-mineral supplementation had no effect on the mean levels of vitamin A and E, IgG, IL2, and C3. However, the intervention resulted int he improvement of serum vitamin A and E levels when the subject\`s serum levels were low before the supplementation.

Determining the optimal range of vitamin C for early red drum (Sciaenops ocellatus) juveniles

  • Vitamin C plays an important role for fish survival, growth and disease resistance. However, the optimal vitamin C for rearing red drum Sciaenops ocellatus juveniles in Vietnam is not known. To address this issue, a 70-day feeding trial was conducted to evaluate the optimal dietary vitamin C requirements for red drum juveniles. Seven isonitrogenous (55.35% protein) and isolipidic (9.07% lipid) diets were formulated to include graded vitamin C concentrations of 23.2, 124.5, 235.2, 423.8, 626.7, 824.6, and 1,027.3 mg/kg, respectively. The results showed that fish fed on 423.8 mg/kg vitamin C diet had the highest growth rate, which can be linked to the increased feed utilization. Broken-line analysis indicated that the optimal dietary vitamin C requirements of red drum juveniles were 342.92 and 405.80 mg/kg for growth parameters, feed utilization, body composition and biochemical parameters of serum. Based on these parameters the optimal vitamin C supplementation level for red drum juveniles was estimated in the range of 342.92-405.80 mg/kg vitamin C in the diets with direct applications in producing artificial feed for rearing juveniles of this species in Vietnam.

Vitamins E and C: Are They Synergistic in Protecting Liver Cells against Hepatic Ischimia and Reperfusion Injury\ulcorner (간장내 허혈 및 재관류시 Vitamin E와 C의 간세포 보호작용)

  • This study was done to determine that vitamins I and C are synergistic in protecting liver cells during hepatic ischemia and repefusion. Rats treated with vitamins I and C were subjected to 60 min of hepatic ischemia and to 1 and 5 hr of reperfusion thereafter. Serum aminotransferase level and microsomal lipid peroxidation were markedly increased by ischemia/reperfusion. These increases were significantly attenuated by vitamins E, C or its combination. Hepatic wet weight-to-dry weight ratio was increased in ischemic group, but this increase was prevented by combination of vitamin I and C. Bile flow and cholate output were markedly decreased by ischemia/reperfusion and vitamin C alone and combination of vitamin I and C restored their secretion. Cytochrome P-450 content and aminopyrine N-demethylase activity were decreased by ischemia/ reperfusion and restored by vitamin C and combination of vitamin I and C to the level of sham-operated rat. Aniline p-hydroxylase activity was increased by ischemia/reperfusion and this increase was prevented by vitamin E. Our findings suggest that ischemia/reperfusion diminishes hepatic secretory and microsomal functions by increasing lipid peroxidation and vitamins I and C synergistically ameliorates these changes.

The Effects of Vitamin E and C on Serum Cholesterol and Antioxidative enzyme in ovariectomized rat (난소 절제 흰쥐의 혈청 콜레스테롤 및 항산화효소에 미치는 비타민 E와 비타민 C의 효과)

  • In order to observe the bioactivity of ovariectomized rats, ovariectomized group (Ovx), nonovariectomized group (Sham), ovariectomized Vitamin C-treat group (Ovx+Vit C), ovariectomized Vitamin E-treat group (Ovx+Vit E) and ovariectomized Vitamin C+Vitamin E-treat group(Ovx+Vit C+E) were made. Lipidperoxides of liver and kidney, serum total cholesterol and HDL-cholesterol were investigate as follows. Lipidperoxides of liver and kidney in Ovx group were 1.78 times and 1.61 times increased compared to Sham group respectively. But, they were significantly decreased in Ovx+Vit C group, Ovx+Vit E group, Ovx+Vit C+E group compared to Ovx group. Serum total cholesterol in Ovx group was increased 2.57 times compared to Sham group. Injections of each substance such as ascorbate, tocopherol, mixture (C+E) make data of Cholesterol become low. When especially Vit C is injected, the data of cholesterol lowed by about 94%. Serum HDL-cholesterol in Ovx group decreased 36.7% compared to Sham group. And as the result of the measurement of SOD, Catalase, and GPx which are antioxidant enzyme, SOD and Catalase activities in Ovx group much higher than in Sham group. Based on the results, it is supposed that more produced free radicals increased antioxidant enzyme. And it is also thought that vitamin can inhibit aging by reducing antioxidant enzyme.

The Relationship between Serum Vitamin D Levels and Lifestyle Risk Factors in Office Workers (사무직 근로자의 혈청 Vitamin D 수준과 생활습관위험인자와의 연관성)

  • The study investigated the relationship between vitamin D status and lifestyle risk factors in office workers. A total of 515 office workers(336 men and 179 women) aged 30 years or older were voluntarily recruited from S-city. Along with serum vitamin D levels, lifestyle risk factors including obesity index, physical fitness, metabolic risk factors, lipids profile and sedentary behavior were measured using standardized protocols. Subjects were classified as deficiency (<20ng/mL), insufficiency (20-29ng/mL), and sufficiency (≥30ng/mL) based on serum vitamin D levels. Linear contrast analysis using one-way ANOVA showed significant linear decreases in mean body fat (p=.030) and sedentary behavior (p<.001) and significant linear increases in skeletal muscle (p=.037), cardiorespiratory fitness (p<.001) and HDL-C (p=.013) across incremental serum vitamin D levels. Compared with the low cardiorespiratory fitness group, the high cardiorespiratory fitness group had a significantly higher odds ratio (men OR=2.144, p=.042, women OR=1.971, p=.045) for having vitamin D insufficiency or deficiency even adjustment after age. Compared to the shortest sitting time group, the longest sitting time group had a significantly higher odds-ratio in a group of female office workers (OR=1.262, p=.043) for having vitamin D insufficiency or deficiency even adjustment after age. The current findings of the study showed that poor physical fitness and sedentary behavior were the risk factors of low serum vitamin D levels, implying the urgent need for a healthy lifestyle modification along with vitamin D supplementation.

A Study on the Nutrients Intake and Serum Lipid Pattern in Smoking College Men (흡연 남자 대학생의 영양소 섭취 및 혈중 지질양상에 관한 연구)

  • In order to investigate the effect of smoking on serum lipid pattern and nutrient intake, the subjects, 134 smokers and 113 nonsmokers, were selected from college men student in Kwangju area. We examined the serum lipid pattern of 15 smokers and 18 non-smokers who agreed to venipunture among the subjects and also examined nutrients intake. The results obtained were as follows ; Mean concentrations of 새심 cholesterol, HDL-cholesterol and LDL-cholesterol were 187$\pm$12.1mg/dl, 41.5$\pm$1.7mg/dl, and 87.3$\pm$7.4mg/dl in smokers and 182.6$\pm$8.2mg/dl, 45.5$\pm$1.5mg/dl, and 96.3$\pm$4.3mg/dl in nonsmokers, respectively. The total energy intake was 2264kcal/day and mean constitutional rationof protein, fat and carbohydrate was 12.7 : 24.8 : 62.5 in smokes and 2287kcal/day, 14.1 :25.4 : 60.5 in nonsmokers, respctively. Mena daily intakes of calcium, iron, vitamin C and vitamin A were 612.2mg, 15.7mg, 48.2mg 2029.4 IU in smokers and 629.4mg, 15.3mg, 68.4mg 2339.1 IU in nonsmokers, respectively. Vitamin C intake was significantly different at p<0.05 between the smokers and nonsmokers. Intakes of cholesterol, SFA and MUFA were not significantly different between the two groups. Mean daily PUFA intake of smokers and nonsmokers were 5.5g and 6.9g respectively. PUFA intake was significantly different at p<0.05 between the two groups. The results of this study showed that smoking have a tendency to raise coronary heart disease occurrence in the respect of serum lipid levels and vitamin C, PUFA intake were significantly different.

Effect of Vitamin C on Hepatic Biliary and Microsomal Function in Hepatic Ischemia/reperfusion (간장 허혈 및 재관류시 Vitamin C가 간장 기능에 미치는 영향)

  • This study was done to investigate the effect of vitamin C on hepatic biliary and microsomal function during ischemia and reperfusion. Rats were treated with vitamin C(20, 100, 400, 1600 mg/kg) or with vehicle(saline) and then subjected to 60 min no-flow hepatic ischemia in vivo. Control animals were time-matched sham ischemic animals. After 1 or 5 hr of reperfusion, bile was collected, blood was obtained from the abdominal aorta, and liver microsomes were isolated. In vehicle-treated ischemic rats, serum ALT and AST levels peaked at 5 hr and were significantly attenuated by vitamin C 20 mg/kg and 100 mg/kg treatment. Similarly, hepatic wet weight-to-dry weight ratio was decreased in the vehicle-treated ischemic group. Vitamin C 20 mg/kg and 100 mg/kg treatment minimized the increase in this ratio. Lipid peroxidation was elevated in vehicle-treated ischemic group, but this elevation was also inhibited by vitamin C 20 mg/kg and 100 mg/kg treatment. Bile flow and cholate output, but not bilirubin output, were markedly decreased by ischemia/reperfuzion. Vitamin C 20 mg/kg and 100mg/kg treatment restored the secretion but vitamin C 1600 mg/kg reduced the cholate output. Cytochrome P-450 content was decreased by ischemia/reperfusion and restored by vitamin C 20 mg/kg and 100 mg/kg treatment to the level of sham operated group but decreased by vitamin C 1600 mg/kg. Aminopyrine N-demethylase activity was decreased and aniline p-hydroxylase activity was increased by ischemia/reperfusion. The changes in the activities of aminopyrine were prevented by vitamin C 20 mg/kg and 100 mg/kg treatment, but not by 400 mg/kg and 1600 mg/kg treatment. Our findings suggest that ischemia/reperfusion diminishes hepatic secretory functions as well as microsomal drug metabolizing systems, small doses(20, 100 mg/kg) of vitamin C significantly ameliorates and large doses(400, 1600 mg/kg) of vitamin C aggravated these ischemia/reperfusion-induced changes.

The Effect of in vitro Hemolysis on Vitamin $B_{12}$ / Folic Acid Results (용혈이 Vitamin $B_{12}$ / Folic acid 검사 결과에 미치는 영향)

  • Purpose: Vitamin $B_{12}$ and folic acid are type of water-soluble vitamin and those work as a secondary vital enzyme, but especially those are involved in the nuclear DNA synthesis. Those are mainly measured in order to diagnose megaloblastic anemia and to assess the storage of folic acid during pregnancy. It is generally known that the hemolyzed serum is useless for folic acid and vitamin $B_{12}$, but it is not easy to abide by this information because our lab (Samkwang Medical Laboratories) is reference laboratory. We tested how much the extent of hemolyzed serum had influence on the results. Materials and Method: We performed the tests of vitamin $B_{12}$, folic acid. For the hemolysis effect study, we used 40 cases. According to the level of A, B and C groups, each group had 10 samples which were mechanically hemolyzed serum. Results: Hemolysis did not affect the vitamin $B_{12}$ results. However in case of folic acid, the value increased according to the degree of hemolysis. And severe hemolyzed cases had the highest value (greater than 20 ng/ml). Conclusions: Preventing the hemolysis, it has to be informed that blood-collecting, separation and storage are performed more carefully. As you see from the above results, hemolyzed serum is not proper for folic acid test, and hemolysis does not affect the results of vitamin $B_{12}$. However, for the more accurate results, it is necessary to avoid hemolysis.

The Effects of High-dose Vitamin C Administration on the Cell-mediated Immune Response in Mice (다용량 비타민 C 투여가 생쥐 세포매개면역반응에 미치는 영향)

  • Background: Vitamin C is an essential nutrient, taken as a daily supplement by many people. Recently, high-dose vitamin C is considered as a therapeutic regimen in some clinical situations. Until now, few studies have been done with the effects of high-dose vitamin C on the immune response. Methods: In this experiment, the effects of high-dose vitamin C on cell-mediated immune response in immunologically competent mice were evaluated. After intraperitoneal injection of 2.5, 5, or 10 mg/day of vitamin C for 10 days, delayed type hypersensitivity (DTH) was provoked against DNFB in the pinnae as a model for cell-mediated immune response. Severity of DTH reaction was evaluated as the thickness of pinnae, and the vitamin C levels were measured in the serum, liver, kidney, lung, pinnae, and splenocytes. Results: After challenge, the thickness increased at its peak on the $2^{nd}$ day in all groups. On the first day, the pinnae were thicker in the injected groups than in the control. On the contrary, the increment of the pinnae thickness was attenuated and the number of cells infiltrated in the site of DTH decreased proportionately to the amount of vitamin C administered from the second day on. With vitamin C exogenously given, the serum level peaked at 30 min after injection, and returned abruptly to its basal level without accumulation. However, it accumulated in the liver, kidney, and especially in the pinnae inflamed and splenopcytes, proportionately to the amount administered. Conclusion: Based on these results, it is suggested that, in one hand, exogenously administered high-dose vitamin C accumulated in the splenocytes and presumably changed the function of them resulting in the augmented cell-mediated immune response, as was revealed in the first day of DTH reaction. On the other hand, it seems likely that the vitamin C also showed anti-inflammatory effects.
