Sugar Content and Protein Fractionation in Human Pleural Fluid (늑막액의 당 및 단백분획상)

  • Previous studies concerning the usefulness of pleural fluid glucose levels in differentiating causes of pleural effusions have been conflicting. Gelenger and Wiggers (1949), Calnan et al(1951) and Barber et al(1957) concluded that the lower the level of pleural fluid glucose, the more likely was tuberculosis, and that tuberculosis was unlikely if the pleural fluid glucose level was more than 80 mg/100 ml. Light and Ball(1973), however, reported that in the great majority of tuberculous pleural fluids the glucose concentration was high rather than low, concluded that the pleural fluid glucose levels were not useful in the differential diagnosis of pleural effusion. In this study, pleural fluid glucose was determined in 46 pleural effusions from various causes to evaluate the usefulness in the differential diagnosis of pleural effusion. In addition, the protein concentration and the electrophoretic patterns of protein and amylases in pleural fluid was compared with that of serum. And the results were as follows. 1. The mean glucose concentration of pleural fluid was 80.8 mg/100 ml in 22 tuberculous origin, 92.5 mg/100 ml in 12 cancer patient and 70.4 mg/100 ml in 10 undiagnosed cases. In 2 cases of paragonimiasis the pleural fliud glucose levels were low (mean, 32.0 mg/100 ml). The percentage of pleural fluid protein to serum is about 75% in all disease groups and the protein level of tuberculous pleural fluid was significantly correlated with that of serum. 2. The disc eletrophoretic patterns of pleural fluid were almost similar with that of serum in all disease groups but the prealbumin fraction was not observed in pleural fluid. 3. With the isoelectric focusing, 4 to 7 isoamylase was observed in serum and the isoelectric point was ranged from pH 5.8 to 7.8 and isoelectic point of main fracticn is pH 7.2. The isoelectic focusing patterns of amylase of pleural fluid were identical to that of serum in all disease group. With the above results it is concluded that the pleural fluid is exudate of serum and that the glucose levels of pleural fluid are not useful in the differential diagnosis of pieural effusions.

  • Even though clinical use DRE, PSA and TRUS for screening while diagnosing prostate cancer early, however, we cannot achieve to diagnose it accurately by one method. Thus, mutual supplements are strongly required in diagnosis. It means that we should identify relationship between prostate volume and serum PSA under different ages for better diagnosis. The subjects(405 men) visited the department of diagnostic radiology in a general university hospital from January 2008 to December 2008, who was going to take the prostatic evaluation by TRUS. 13 men, who did definite diagnosis of prostate cancer before through biopsy, were excluded in this study. All data were expressed as mean and standard deviations by using SPSS 14.0 package programs and were analyzed by using one-way ANOVA to evaluate the effect of different ages. And Scheffe Post hoc comparison is accomplished in order to make significant difference in prostate volume, PSA and PSAD according to ages. Pearson correlation was used for ages, prostate volume, PSA and PSAD. According to examination, the subjects' prostate volume and serum PSA level increase as person's age. We can see significant difference between group over 70 years and group from 40 and 50 years. Moreover, age, prostate volume and serum PSA have significant positive correlation. Even though we can get variations in serum PSA level by changing prostate volume according to ages, it is worthwhile to mention that more strong association for prostate volume is observed than ages in elevation of serum PSA. Therefore, it is strongly recommended to consider prostate volume weightly when it comes to evaluate for men with elevated PSA level. Finally, TRUS is an excellent extra equipment to diagnose prostate cancer because it can contribute to early diagnose and pertinent treatment of prostate cancer.

The Studies on the Transaminase Activities(1) (Transaminase 활성에 관한 연구(제 1보))

  • Transaminase has been a popular methods in the clinical laboratory for the diagnosis of particular diseases. The methods we are familiar with should be considered further on the serum states provided on the test to be done. (serum freshness, temperature and duration preserved) I have studied transaminase activities of the serum preserved under frozen (-10$\circ$C), refrigerated (4$\circ$C) and room temperature conditions. At a given interval those activities of serum are measured and then the results are compared with the optical density and calculated for the standard deviation and percent variation (Table 1. & Fig. 1.) G.O.T. activity is maintained decreased at a relatively constant rate from 2nd day to 14th day preservation under frozen and refrigerated but its activity variation at 25$\circ$C are far greater than the above conditions throughout the test periods. Elevated G.P.T. level of serum compared with G.O.T. are remained at any temperature and duration. Unless the determination of amino acid level study can not ruled out clearly.

  • To develop more reliable diagnostic measures for traumatic pericarditis in ruminant, fluctuations of serum total LDH activities and LDH isoenzymes fractions were investigated in artificially induced pericarditis of Korean native goats. Experimental group showed the tendency of increase serum LDH total activities. Effect of LDH isoenzymes on serum total LDH activities were high with decreasing order of LDH$_1$> LDH$_3$ > LDH$\sub$5/ > LDH$_2$ > LDH$_4$ and LDH$_1$ was the hightest among them. From these findings, increase of serum total LDH activities and LDH$_1$ fraction was thought to be important in traumatic pericarditis.

Evaluation of serum immunoglobulin G4 concentrations in canine pancreatitis

  • The goal of this study was to measure immunoglobulin G4 (IgG4) concentrations and to evaluate the significance of these values in the management of canine pancreatitis. The medical records of 24 dogs that visited the Veterinary Medical Teaching Hospital between December 2016 and June 2018 were retrospectively reviewed to identify dogs that had been diagnosed with pancreatitis. The serum C-reactive protein and serum IgG4 concentration in the affected dogs were highly increased compared to the healthy group. Particularly, serum IgG4 measured significantly higher in dogs with pancreatitis and concurrent immune-mediated disease (p < 0.05). In conclusion, increased serum IgG4 concentrations are a characteristic finding in dogs with pancreatitis. The results of this research indicate that an elevation in IgG4 has the potential of being used as a tool for the diagnosis of pancreatitis and concurrent immune-mediated disease.

Pre-diagnostic Clinical Presentations and Medical History Prior to the Diagnosis of Inflammatory Bowel Disease in Children

  • Purpose: The clinical presentations of inflammatory bowel disease (IBD) prior to diagnosis are so diverse or vague that many of them waste time before final diagnosis. This study was undertaken to know the medical history of the pediatric patients until the final diagnosis could be reached. Methods: The medical records of all pediatric patients who were diagnosed with IBD (Crohn's disease [CD] in 14 children, ulcerative colitis [UC] in 17) during the last 13 years were reviewed. We investigated the length of the diagnostic time lag, chief clinical presentation, and any useful laboratory predictor among the routinely performed examinations. Indeterminate colitis was not included. Results: The mean ages of children at the final diagnosis was similar in both diseases. As for the pre-clinical past history of bowel symptoms in CD patients, 5 were previously healthy, 9 had had 1-3 gastrointestinal (GI) symptoms, weight loss, bloody stool, anemia and rectal prolapse. With UC, 9 were previously healthy, 8 had had 1-3 GI symptoms, bloody stool, anorexia. The average diagnostic time lag with CD was 3.36 months, and with UC 2.2 months. Body mass index (BMI) and the initial basic laboratory data (white blood cell, hemoglobin, mean corpuscular volume, serum albumin, and serum total protein) were lower in CD, statistically significant only in BMI. Conclusion: IBD shows diverse clinical symptoms before its classical features, making the patients waste time until diagnosis. It is important to concern possibility of IBD even in the mildly sick children who do not show the characteristic symptoms of IBD.

Serodiagnosis of Echinococcosis by ELISA Using Cystic Fluid from Uzbekistan Sheep

  • According to increase of travel, the cases of imported echinococcosis have been increasing in Korea. The present study was undertaken to develop a serodiagnostic system for echinococcosis in Korea. For diagnosis of echinococcosis, the fluid of Echinococcus granulosus hydatid cysts was collected from naturally infected sheep in Uzbekistan. Also serum samples of infected patients who were surgically confirmed were collected in a hospital in Tashkent, Uzbekistan. According to the absorbance of 59 echinococcosis positive and 39 negative control serum samples, the cut-off value was determined as 0.27. The sensitivity and specificity of ELISA with hydatid fluid antigen were 91.5% and 96%, respectively. The antigen cross-reacted with the serum of some cysticercosis or clonorchiasis patients. However, immunoblot analysis on the cystic fluid recognized antigenic proteins of 7-, 16-, and 24-kDa bands in their dominant protein quantity and strong blotting reactivity. In conclusion, the present ELISA system using hydatid cyst fluid antigen from Uzbekistan sheep is sensitive and specific for diagnosis of echinococcosis cases.

Radiographic and Ultrasonographic Diagnosis of Single Intrahepatic Portosystemic Shunt in a Dog (개의 단순 간내성 간문맥전신단락증의 영상진단학 및 치료 1예)

  • A 4-month-old 5.7 kg male Golden retriever with history of seizure, depression, lethargy and anorexia was referred to Veterinary Medical Teaching Hospital, Chungbuk National University. Hematologic examination revealed microcytosis and nonregenerative anemia. Serum chemical values showed increased serum ammonia (423 $\mu$mol/L), ALP (1101 U/L), r-GTP (13.9 U/L) and CPK (1454 U/L), and decreased total protein (4.9 g/dl) and BUN (1.6 mg/dl). Microhepatia was shown in survey abdominal radiographs. Color doppler ultrasonographic examination revealed dilated tortuous vein with turbulent flow within liver parenchyma. Intraoperative jejunoportography and intraoperative ultrasonography confirmed the location and size of single intrahepatic shunt vessel in the left medial liver lobe. Also, the anomalous vessel entering the caudal vena cava was identified beneath the diaphragm. The shunting vessel was ligated with using an Ameroid constrictor. General conditions, hematologic and serum chemical values resolved gradually after surgery. One month after surgery abdominal radiograph showed normal gastric axis and it was consistent whit the normal size liver. Normal echogenecity of liver and enlargement of portal vein were shown in ultrasonography. It is assumed that survey radiography and ultrasonography are useful for diagnosis of single intrahepatic shunt in a dog and especially jejunoportography vein portography and intraoperative ultrasonography are suitable for confirmation of the anatomic location and size of the shunting vessels.

Serological evidence of West Nile viral infection in archived swine serum samples from Peninsular Malaysia

  • West Nile virus (WNV), a neurotropic arbovirus, has been detected in mosquitos, birds, wildlife, horses, and humans in Malaysia, but limited information is available on WNV infection in Malaysian pigs. We tested 80 archived swine serum samples for the presence of WNV antibody and West Nile (WN) viral RNA using ID Screen West Nile Competition Multi-species enzyme-linked immunosorbent assay kits and WNV-specific primers in reverse transcription polymerase chain reaction assays, respectively. A WNV seroprevalence of 62.5% (50/80) at 95% confidence interval (51.6%-72.3%) was recorded, with a significantly higher seroprevalence among young pigs (weaner and grower) and pigs from south Malaysia. One sample was positive for Japanese encephalitis virus antibodies; WN viral RNA was not detected in any of the serum samples.

Differential diagnosis of ovarian cysts using ultrasonogrphy and progesterone assay in slaughtered cows (초음파검사 및 혈중 progesterone 농도측정에 의한 도축우 유래 난소낭종의 감별진단)

  • To establish the differential diagnosis and treatment method in bovine ovarian cysts, specially ovarian cysts with corpus luteum, serum progesterone concentration and ulrasonography for measuring the cyclic area, thickness of cystic wall and echogenicity of corpus luteum were investigated in cystic ovaries from slaughtered cows. The incidence rates of ovarian cysts were follicular cyst 69.2% and luteal cyst 30.8%. The incidence rates of 8 various types of ovarian cysts were as follows; 2Ba 32.3%, 2Aa 25.8% and 2Bb 14.5%, respectively. The thickness of cystic wall were 2Bb 3.93mm, 2Ab 3.70mm and 1Aa 1.93mm and the serum progesterone concentrations were above 1.0ng/$m\ell$ in 2Ab, 2Bb and IAa, respectively. The cystic area of ovarian cysts with corpus luteum was 288.30mm2, but ovarian cysts without corpus luteum 542.30$\textrm{mm}^2$, and the thickness of cystic wall 2.12mm and 2.40mm, respectively. The serum progesterone concentration was 1.91ng/$m\ell$ in ovarian cysts with corpus luteum and 1.20ng/$m\ell$ ovarian cysts without corpus luteum. There was not the correlations between thickness of cystic wall and serum progesterone concentration in ovarian cysts with corpus luteum, whereas, was the correlations in ovarian cysts without corpus. These results indicated that PGF2$\alpha$ analogues can be choice for treating the ovarian cysts with corpus luteum because serum progesterone concentrations were above 1.0ng/$m\ell$ in ovarian cysts with corpus luteum. In conclusion, it is suggest that ultrasonography is useful diagnostic tool for diagnosing and choicing of treatment remedy in cystic ovaries of bovine.
