Assessment of early nutritional state in critical patients with intoxication and the effect of nutritional status on prognosis (중독 중환자에서의 초기 영양상태평가와 예후와의 관계)

  • Purpose: Nutritional status and support in critically ill patients are important factors in determining patient recovery and prognosis. The aim of this study was to analyze the early nutritional status and the methods of nutritional support in critically ill patients with acute poisoning and to evaluate the effect of nutritional status on prognosis. Methods: A retrospective study was conducted in tertiary care teaching hospital from January 2018 to December 2020. in an emergency department of university hospital, 220 patients who were stayed more than 2 days of poisoning in intensive care unit were enrolled. Results: 155 (70.5%) of patients with acute poisoning had low-risk in nutritional risk screening (NRS). Patients with malignancy had higher NRS (low risk 5.2%, moderate risk 18.5%, high risk 13.2%, p=0.024). Patients of 91.4% supplied nutrition via oral route or enteral route. Parenteral route for starting method of nutritional support were higher in patients with acute poisoning of herbicide or pesticide (medicine 3.2%, herbicide 13.8%, pesticide 22.2%, p=0.000). In multivariate logistic regression analysis, herbicide or pesticide intoxication, higher risk in NRS and sequential organ failure assessment over 4.5 were affecting factor on poor recovery at discharge. Conclusion: NRS in patients intoxicated with herbicide or pesticide were higher than that in patients intoxicated with medicine intoxication. Enteral nutrition in patients intoxicated with herbicide or pesticide was less common. Initial NRS was correlated with recovery at discharge in patient with intoxication. It is expected to be helpful in finding patients with high-risk nutritional status in acute poisoning patients and establishing a treatment plan that can actively implement nutritional support.

  • This study examines the paradigm shift in vocational training with the introduction of remote training, which has been conducted fragmentarily by project, to respond to the COVID-19 pandemic. It analyzes the operation's problems and limitations by dividing the remote training process from assessment to budget execution by subject, stage, and procedure. Additionally, it collects various stakeholders' opinions to propose improvement plans for the vocational training support system to effectively respond to the paradigm shift in vocational training in the digital and non-face-to-face era. The study assumes that the assessment of training institutions and the training process should be innovated in a way that can accommodate the direction of vocational training in the digital and non-face-to-face era instead of focusing on traditional collective training. Based on this premise, it suggests ways to enhance the pre-approval screening system and the training institution assessment system.

  • Objectives: This study attempted to evaluate the usefulness and direction of development of post-traumatic assessment and interventions based on the opinions of psychiatrics and disaster and trauma-related experts using the Delphi survey technique. Methods: In-depth individual interviews served as the pre-survey and were followed by Delphi primary and secondary surveys. Specialists in child and adolescent mental health, psychological support professionals specialized in disasters and related practitioners with experience of disasters in Korea completed a set of questionnaires and participated in focus group interviews and in-depth individual interviews on post-traumatic assessment and intervention. Results: We found that the following issues have a significant impact on the interventions after disasters: the proper time of the initial interview in the event of a disaster, assessment notices, aged assessment services, mandatory enforcement measures, scale screening and treatment intervention elements, symptoms degree classification, intervention standardization, the use of a levelled program, care unit environment, and operation plan. Conclusion: This study proposed effective mental health intervention measures and has implications for the development of evaluation treatment protocols after disasters.

Human Risk Assessment of a Contaminated Site Using Korean Risk-Based Corrective Action (K-RBCA) Software (한국형 소프트웨어를 이용한 유류.중금속 복합오염지역의 인체위해성평가 및 RBCA Tool Kit과의 비교분석)

  • By using a newly developed Korean risk-based corrective action (K-RBCA) software (K-RBCA) and the RBCA Tool Kit, risk assessment was performed on a site that was contaminated with aromatic hydrocarbons and heavy metals. Eight chemicals including benzene, ethylbenzene, xylenes, naphthalene, benz(a) anthracene, benzo(b) fluoranthene, benzo(a) pyrene, and arsenic that exceeded the US EPA Soil Screening Level were chosen as the target pollutants. A conceptual site model was constructed based on the site-specific effective exposure pathways. According to the RBCA Tool Kit the carcinogenic risk of arsenic was larger than $10^{-6}$, which is the generally acceptable carcinogenic risk level. The K-RBCA estimated the same level of carcinogenic risk for arsenic. With the RBCA Tool Kit, the carcinogenic risk of benzo(a) pyrene was estimated to be about $1.3{\times}10^{-6}$. However, with the K-RBCA benzo(a) pyrene did not exhibit any risk. The inconsistency between the softwares was attributed to the different fundamental settings (i.e., medium division) between the two softwares. While the K-RBCA divides medium into surface soil, subsurface soil, and groundwater, the RBCA Tool Kit divides medium into only soil and groundwater. These differences lead to the different exposure pathways used by the two softwares. The K-RBCA considers the exposure pathways in surface soil and subsurface soil separately to estimate risk, however, the RBCA Tool Kit considers the surface soil and subsurface soil as one and uses the integrated exposure pathways to estimate risk. Thus the resulting risk is higher when the RBCA Tool Kit is used than when the K-RBCA is used. The results from this study show that there is no significant difference in the risks estimated by the two softwares, thus, it is reasonable to use the K-RBCA we developed in risk assessment of soil and groundwater. In addition, the present study demonstrates that the assessor should be familiar with the characteristics of a contaminated site and the assumptions used by a risk assessment software when carrying out risk assessment.

Development and Validation of Tool for the Assessment of Employment Preparedness for People with Visual Impairments (시각장애인 고용준비도 검사지 개발 및 타당화 연구)

  • The purpose of this study is to develop tool for the assessment of employment preparedness for people with visual impairments. First, we developed a draft assessment factors. Expert opinion survey, delphi survey, pilot test were conducted to modify contents of the factors. Actual assessments were carried out to test the reliability and validity of the developed tool targeting job seeker with visual disabilities. The total number of valid samples are 253. Principal component analysis, confirmatory factors analysis, and correlation analysis were used for the validity test. Chronbach' alpha analysis was used for the reliability test. As a result of the reliability analysis, the reliability coefficient showed good level of 0.88-0.92. As a result of the factor analysis, it was confirmed that the composition concept of 9 factors were well reflected. The correlation coefficient between employment preparedness tool and job screening tool was 0.501, Which was statistically significant. Therefore, it was evaluated as having a concurrent validity. Finally, it was confirmed that the assessment of employment preparedness for people with visual impairments was valid as assessment tool.

  • In Korea, the dependence on living donations is high due to the shortage of organs available for donation compared to the number of people waiting for transplants and the number of living organ donations continues to increase. In particular, the number of living-donor transplantations is high worldwide, highlighting the importance of pre-transplant psychosocial evaluation of living kidney donors. According to previous studies, when evaluating living organ donors before transplantation, it is crucial to determine whether the donor can give informed consent and be aware of the risks after surgery. Pre-transplant evaluation tests such as ELPAT living organ donor Psychological Assessment Tool (EPAT), Live Donor Assessment Tool (LDAT), Living Donation Expectancies Questionnaire (LDEQ), Minnesota Multiphasic Personality Inventory-2 questionnaire (MMPI-2) and Temperament and Character Inventory (TCI) are conducted for donors. After reviewing the literature on these pre-transplant psychosocial assessment tools, we will also look at legal considerations for living kidney donors in Korea and suggest an effective and essential pre-transplant screening evaluation method for living kidney transplant donors.

  • Disturbances in memory are the most common problem in patients with an organic mental syndrome. Other patients with significant psychiatric disorders also often have difficulty with memory. So it is very important in the clinical practice of psychiatry to understand the biological and neurocognitive mechanisms of memory proessing, and to develop the assessment tools with which memory function can be evaluated reliably and validly. Moreover, memory researches provide an important viewpoint from which we can understand the pathophysiological mechanisms of major neuropsychiatric illnesses. This article focuses on our understanding of memory functions in clinical and neurobiological aspects. The relevant material will be presented in four parts : 1) terminologies needed in defining major stages of various types of memory processing : 2) neurochemical and neuroanatomical basis of memory processing : 3) brief bed-side screening tests and more comprehensive neuropsychological tests for the evaluation of memory function : 4) the characteristics of memory dysfunction in several major psychiatric illnesses.

  • Rat renal proximal tubule suspension was prepared from adult male Sprague Dawley rat (250-300g) by mechanical (non-enzymatical) method and evaluated as a pontential model for mechanistic studies and early screening of nephrotoxicity, using anionic antibiotics (cephaloridine). Cephaloridine (CPL) produced an increase in LDH release into media. This release results from decrease a proximal tubule cell viability and subsequently increase the permeability of cell viability and subsequently increase the permeability of cell membrane. Since loss of intracellular potassium and ATP into media is the sign of disruption of cell membrane, especially basolateral membrane (BLM), CPL induced proximal tubule cell compromise also appear be associated with BLM, maybe $Na^+-K^+$ ATPase. Also seen was significant depression in brush border membrane (BBM) ALP activity and no significantly increase in BBM GGT activities. The inhibition of typical anion, PAH accumulation (especially, CPL 5 mM) and cation, TEA (especially, 4hours incubation) were seen dose dependently. This is because of CPL accumulation in renal proximal tubule and increase of cytotoxicity.

Mutagenic Assessment of Olmesartan Cilexetil by Bacterial Mutation Assay

  • Hypertension is a serious health problem due to high frequency and concomitant other diseases including cardiovascular and renal dysfunction. Olmesartan cilexetil is a new antihypertensive drug associated with angiotensin II receptor antagonist. This study was conducted to evaluate the mutagenicity of olmesartan cilexetil by bacterial reverse mutation test using Salmonella typhimurium (TA100, TA1535, TA98, and TA1537) and Escherichia coli (WP2 uvrA). At the concentrations of 0, 62, 185, 556, 1667, and 5000 ${\mu}g$/plate, olmesartan cilexetil was negative in both Salmonella typhimurium and Escherichia coli regardless of presence or absence of metabolic activation system (S9 mix). These results demonstrate that olmesartan cilexetil does not induce bacterial reverse mutation.

  • The development of inexpensive high throughput methods to identify individual DNA sequences is important to the future growth of medical genetics. This has become increasingly apparent as psychiatric geneticists focus more attention on the molecular basis of complex multifactorial diseases at which most of psychiatric disease is estimated. Furthermore, candidate gene approaches used in identifying disease associated genes necessitate screening large sequence blocks for changes tracking with the disease state. Even after such genes are isolated, large scale mutational analysis will often be needed for risk assessment studies to define the likely medical consequences of carrying a mutated gene. This review provide basic knowledge of up-to-date technology, cDNA microarray which enables above mentioned various research themes.
